4 IN MEMORIAM Joep Lange was the Professor of Medicine, Head of the Department of Global Health, at the Academic Medical Center (AMC), University of Amsterdam, Executive Scientific Director of the Amsterdam Institute for Global Health and Development (AIGHD), and Chair of the Steering Committee AIGHD. Joep Lange had been involved in HIV research and treatment since He was the architect and principal investigator of several pivotal trials on antiretroviral therapy and on the prevention of mother-to-child transmission of HIV in both the developed and developing world. In addition to various positions at the AMC, he was Chief of Clinical Research and Drug Development at the Global Programme on AIDS of the World Health Organization in Geneva from 1992 to From he was President of the International AIDS Society. He served on numerous advisory boards for both private and public sector organisations, including the Strategic and Technical Advisory Committee of the WHO HIV Department, the External Advisory Committee of the US HIV Vaccine Trials Network and the International Advisory Board of the Institute for Global Health of Imperial College. He also served as the Chairman of the PharmAccess Foundation (which he founded) and as Scientific Advisor to the Board of the Health Insurance Fund Foundation, which pioneers mechanisms of sustainable financing of health care in resource-poor settings. He was a member of the Supervisory Board of KNCV Tuberculosis Foundation. Joep Lange founded and was Editor-in-Chief of the journal Antiviral Therapy and served on several other editorial boards of scientific journals. He published more than 350 papers in peer-reviewed journals and guided more than 30 PhD students. In 2007 he was awarded the Eijkman Medal for his achievements in Tropical Medicine and International Health. The workshop organizers and Caribbean HIV community mourn the loss of Dr. Joep Lange who died on July 17 th 2014 in the Malaysian Airlines tragedy over Ukraine. We offer our condolences to his family and friends, while extending our gratitude to Eric van Gorp for delivering the Distinguished Lecture and Tribute to Dr. Lange and his life s work. 2

Minister of Health Cornelius de Weever Ministry of Health, St.Maarten I am extremely honored as Minister of Public Health Social Development & Labor to welcome you to our shores as you participate in the third joint CHART-CCAS-CMLF Meeting and the 11 th annual CCAS HIV Workshop; moreover I am very pleased that our ministry could have assisted in making the hosting of this event possible. Over the years St. Maarten has done much to bring awareness to the community about HIV/AIDS. We were very successful in bridging the private and public sector through our RED Campaign, which ran for four years with a different theme each year mainly targeting the private sector through business participation each month of the year. We have also been able to tackle the topic of discrimination in the workplace by enforcing an HIV/AIDS Workplace policy within government and at various private entities. I have noticed that the theme Prevention benefits of antiretroviral therapy in Caribbean HIV patients: the central role of patient engagement in the continuum of care has as focus developing a successful treatment and prevention program. The theme of the workshop has been and remains a point of attention for us here on St. Maarten. I would like to wish all attendees to the conference fruitful discussions, and remember to take advantage during your spare time of what our beautiful island has to offer. Welcome! 3

6 W E L C O M E Ms. Cheryl Larmonie CCAS Country Representative for St.Maarten It is a great honor for me to welcome all delegates and the other healthcare professionals to the 11 th CCAS Workshop and organized jointly for the 3 rd time by CHART-CCAS-CMLF. On behalf of the Government of St. Maarten and the local committee, we are pleased and proud to be hosting this annual HIV workshop in this beautiful and lovely all inclusive resort, Sonesta Maho Beach Resort & Casino. CCAS with the joint partner s organizations has played an important role in achieving success in HIV prevention, treatment counseling and care for the countries in the Caribbean. CCAS has provided the opportunity for the countries in the Caribbean to review and learn from each other s challenges and experiences. And CCAS also helps the countries to reflect on the achievements of other countries in the region. Each island provide their most recent data, showing the development of the country HIV status. The Caribbean countries demonstrate a strong commitment to share, and strive for a world free of new HIV infections, zero discrimination, and zero AIDS related deaths. This year theme is: Prevention benefits of antiretroviral therapy in the Caribbean HIV patients; the central role of patient engagement in the continuum of care. I would like to take this opportunity to express my sincere thanks to CCAS, CHART, CMLF and everybody else that I forgot to mention who was involved in organizing this workshop, I truly appreciate your dedication. Healthcare professionals come together to continue work on the success of the HIV joint meeting. The exchange of experience has become of vital importance between the Caribbean islands. We need to learn from each other. At the same time, it can be very fruitful to get to know people and experts from other countries. There will be possibilities to make closer contacts in the coffee breaks, at dinner and lunch and probably at a drink in the evening. I hope that you will use this chance. 7 W E L C O M E Professor Brendan Bain Caribbean HIV/AIDS Regional Training Network (CHART) A warm welcome from the CHART Regional Coordinating Unit at the University of the West Indies (UWI-CHART) and from the rest of the CHART Network to participants at this year s CHART-CCAS-CMLF workshop. This is the sixth successive year of the CHART-CCAS partnership and the third time in a row that we are joining with CMLF. Congratulations to CCAS on its eleventh HIV laboratory workshop! As before, this year s meeting will provide timely updates for clinicians and laboratory personnel. It will also help to coalesce the efforts of individuals and small teams that work in relative isolation throughout the year and serve to advance the effort to roll back the HIV epidemic in the Caribbean. New ideas are often generated in this annual meeting and initiated at national and regional level. Examples of new initiatives that will be reported at this year s workshop are the OECS Viral Load referral network and the Virtual HIV Research Unit (VHRU): a public gateway for Caribbean HIV reports. Progress in building the capacity of Caribbean laboratories and in the development of a Caribbean laboratory network will be demonstrated during the workshop. Thanks are due to the Caribbean Office of the CDC Division of Global HIV and AIDS and CMLF for recent advances in this area. Dialogue between clinicians and laboratorians during the meeting will help participants to gain greater appreciation of the complementarity of these two areas of work. CHART welcomes Dr. Sharon Weissman and Dr. Helmut Albrecht from the Division of Infectious Diseases at the University of South Carolina School of Medicine (USCSOM) to the clinical section of the meeting. The link with USCSOM is part of a new partnership with UWI for establishment of a post-doctoral fellowship in Clinical Infectious Diseases. Funding for the partnership comes from the US President s Emergency Plan for AIDS Relief (PEPFAR) via a Twinning Grant from the Health Resources and Services Administration (HRSA) to the American International Health Alliance (AIHA). PEPFAR is also providing support to CHART via HRSA. The planning team is grateful to the Ministry of Health of Sint Maarten for hosting the workshop. 8 W E L C O M E Professor Clive Landis Caribbean Cytometry and Analytical Society (CCAS) It gives me great pleasure to welcome participants to the third Joint CHART- CCAS-CMLF Meeting and the 11 th Annual CCAS HIV Workshop. These workshops have consistently promoted a unified training approach combining the best practices of clinical, laboratory and social care in order to extend quality HIV testing to all, encourage recruitment and retention of infected persons into care, and drive down community viral load. We are proud within our member societies to have adopted a collaborative approach that has foreshadowed the WHO Consolidated Guidelines issued in 2013 on the use of antiretroviral drugs for treating and preventing HIV infection. The Caribbean region has been praised by UNAIDS for overseeing a decline in mother to child transmission and new infections in adults, with treatment as prevention garnering most plaudits in achieving these gains. The theme Prevention benefits of antiretroviral therapy in Caribbean HIV patients: the central role of patient engagement in the continuum of care has been selected to drill down into further detail what constitutes a successful treatment and prevention program. It begins with an accessible HIV test capable of reaching our vulnerable populations, which faces the perennial challenge how to destigmatize the disease in our small island settings and to extend the necessary support to assist diagnosis and recruitment of positive persons into care. The next step in the continuum is to retain and monitor patients during care to stage antiretroviral therapy and treat coinfections. And finally, possibly the hardest step, is to provide the necessary social support to counsel effective drug adherence in our patients, thereby driving down HIV viral load and securing the full prevention benefits of antiretroviral therapy in our communities. Finally, I wish to express my sincerest gratitude to The Ministry of Health for their enthusiastic hosting of this year s training workshop. My appreciation is extended especially to Cheryl Larmonie, CCAS Regional Council Member for St. Maarten, and her local organizing committee, to our partner organizations CHART and CMLF, my CCAS workshop organizing committee, and the distinguished faculty. I am grateful to our diverse range of exhibitors & sponsors who have combined again to host a multi-vendor expo: the expo is always popular with our professional HIV caregivers to assist their informed choice of commercial options best suited to their individual country settings. 9 W E L C O M E Ms. Valerie Wilson Caribbean Med Labs Foundation (CMLF) On behalf of the Caribbean Med Labs Foundation (CMLF), I join with CCAS, the CHART Network and the Ministry of Health of St. Maarten in welcoming you to the eleventh CCAS meeting and fourth joint meeting of the CCAS, PANCAP and CMLF partnership aimed at continued development of the regional HIV/AIDS laboratory network. Our sincere thanks to the Ministry of Health of St. Maarten for warmly welcoming us and hosting this meeting. It is a notable achievement in the fight against HIV/AIDS, that the theme of this year s meeting speaks to the role of the patient in the continuum of care, as we look at the prevention benefits of ART in Caribbean patients. Such a goal has become a reality in recent years because of advances in treatment options, as well as the proof from scientific studies that have shown the positive effects of ART treatment in prevention of spread of HIV infection. In this regard, laboratory services play a critical role, both internationally and here within the Caribbean, in providing proof of the undetectable viral loads that can be achieved when HIV infected persons are effectively treated with ART. The role of the laboratory in supporting the care of HIV patients at all stages, from identification of infected persons to monitoring of treatment and care is indisputable. As CMLF continues implementation of the PANCAP Global Fund Round 9 grant, in collaboration with Ministries of Health and our regional and international laboratory and clinical colleagues, we seek to positively impact on key issues related to the strengthening of laboratory networks, including monitoring of access and quality of laboratory services. During the laboratory network meeting we will review advances made in the laboratory networks within the region, and discuss the progress of key regional initiatives including CMLF s work with countries to establish national laboratory policies, the HIV-Syphilis Elimination programme led by PAHO/WHO, and the Laboratory Quality Management Systems Stepwise Improvement Process (LQMS-SIP) being led by the CARICOM Regional Organisation for Standards and Quality (CROSQ). We will also learn from the experience of the Dutch Antilles in their initiatives to improve laboratory quality and achieve accreditation. This meeting will undoubtedly continue the CCAS tradition of providing information on cutting edge and regionally appropriate approaches and technologies. We look forward to updates shared by our international colleagues and industry experts, to learning from our collective experiences within the region and internationally, and to continuing our work together in partnership with those affected, to improve the lives of HIV/AIDS patients within our region. 7
