1 Ian Adcock Imperial College London, UK Dr Adcock is Professor of Respiratory Cell and Molecular Biology at the National Heart and Lung Institute at Imperial College London. He obtained his PhD in molecular pharmacology from St Thomas s Hospital Medical School in 1987 before joining Professor Peter Barnes at NHLI in 1990 to study glucocorticoid actions in the airway. He has subsequently worked on inflammatory and oxidative stress mechanisms in severe asthma and COPD. Professor Adcock is Head of the ERS Assembly 5 (Airways Diseases) and has published over 350 publication with an H Index of 102. His research has been supported by the EU-IMI, Wellcome Trust, MRC, BBSRC, Asthma UK, British Lung Foundation and British Heart Foundation as well as several Pharmaceutical companies Peter Barnes Imperial College London, UK Peter Barnes is Margaret-Turner Warwick Professor of Medicine at the National Heart and Lung Institute, Head of Respiratory Medicine at Imperial College and Honorary Consultant Physician at Royal Brompton Hospital, London. Mario Cazzola University of Rome 'Tor Vergata', Italy Mario Cazzola is Honorary Professor of Respiratory Medicine at the University of Rome Tor Vergata and Visiting Professor at the Sackler Institute of Pulmonary Pharmacology, GKT School of Biomedical Sciences, London, UK. He is the author or a co-author of about 600 papers mainly focused on pharmacotherapy of COPD. Prof. Cazzola is serving as Editor-in-chief of Pulmonary Pharmacology & Therapeutics and of COPD Research and Practice and as an Associate Editor of Respiratory Medicine and Respiratory Research. He is a member of the Editorial Boards of Lancet Respiratory Medicine, Drugs, Respiration, COPD, International Journal of Chronic Obstructive Pulmonary Disease, and npj Primary Care Respiratory Medicine. Prof. Cazzola is a fellow of the European Respiratory Society in which he is now Internal Auditor. He has received the 2015 European Respiratory Society Lifetime Achievement Assembly Award.
2 Bart Celli Harvard Medical School, USA Bartolomé Celli is Professor of Medicine at Harvard Medical School and member of the Division of Pulmonary, Critical Care Medicine and Sleep Medicine at the Brigham and Women s Hospital, both in Boston, Massachusetts, USA. Professor Celli s primary research interest lies in the field of chronic obstructive pulmonary disease, particularly in clinical management, biomarkers and outcomes, exercise, health-related quality of life and pulmonary rehabilitation. He is the author of over 320 peer-reviewed publications and over 375 scientific abstracts. Professor Celli is past Chair of the Clinical Assembly of the American Thoracic Society (ATS), past President of the Massachusetts Thoracic Society and New England Chapter of the American College of Chest Physicians, and is currently a member of the ATS World Lung Health Committee. He is Co-chair of the ATS/European Respiratory Society (ERS) Committee that has published the ATS/ERS Standards for the Diagnosis and Treatment of COPD and is also a member of the Board of Directors of the GOLD initiative. His over 70 trainees practice throughout Boston and the rest of the United States, Europe, the Middle East, Japan and Latin America. James D. Chalmers University of Dundee, UK Dr James D Chalmers is Senior Clinical Lecturer at the University of Dundee, UK. He is chair of the European Bronchiectasis Registry (EMBARC) and runs a research group primarily focussed on neutrophilbacterial interactions in bronchiectasis and COPD. He is associate editor of the ERJ, a member of the international advisory board of the Lancet Respiratory Medicine and is secretary of the European Board for Accreditation in Pneumology. He has published more than 130 papers primary on respiratory infection, bronchiectasis and COPD. William Checkley Johns Hopkins University, USA William Checkley, MD, PhD, is Associate Professor of Medicine, International Health and Biostatistics at Johns Hopkins University. Dr. Checkley s research has focused on the epidemiology of COPD in lowand middle-income countries. He is leading observational studies on the association between biomass fuel smoke exposure and lung function, and intervention trials of improved cookstoves and clean fuel strategies to reduce biomass fuel smoke exposure and improve pulmonary outcomes. Dr. Checkley is Principal Investigator in the recently funded Global Environmental and Occupational Health Hub NIH-sponsored network and in an NIH-funded multi-country randomized intervention field trial of liquefied propane gas on health outcomes across the lifespan. Dr. Checkley has authored >150 publications in peer-reviewed journals and is a member of the editorial board of six medical journals.
3 Miguel Divo Brigham and Women s Hospital Harvard Medical School, USA Dr. Divo is a physician at Brigham and Women s Hospital (BWH) in Boston and instructor in medicine at Harvard Medical School. He received his medical degree from the Central University of Venezuela and completed a residency in internal medicine at Beth Israel in New York. He also completed fellowships in pulmonary and critical care medicine and sleep medicine at St. Elizabeth s Medical Center. Dr. Divo s clinical and research interests include COPD comorbidities, management of chronic ventilator dependent patients and lung transplantation. He has authored more than 20 peer-reviewed publications and he is recipient of the Maurizio Vignola Award for Best Publication in Pneumology from the ERS. Leonardo Fabbri University of Modena & Reggio Emilia, Italy Leonardo Fabbri is Professor of Respiratory Medicine, Director of the Section of Respiratory Diseases and of the Department of Oncology, Hematology and Respiratory Diseases of the University of Modena and Reggio Emilia. He received doctorate in Medicine and Surgery from the University of Padua on 1972 (110/110 summa cum laude), Board in Occupational Medicine from the University of Padua on 1975 (70/70 summa cum laude) and Board on Respiratory Medicine from the University of Bologna on 1978 (70/70 summa cum laude). He has been a Visiting Research Fellow at the Department of Respiratory Diseases of Tulane University and at the Cardiovascular Research Institute at the University of California, San Francisco, USA. He has served as Editor of the European Respiratory Review and of the European Respiratory Monograph and as Associate Editor of the American Journal of Respiratory and Critical Care Medicine. From 2004 he is Associate Editor of the European Respiratory Journal, from 2010 of the European Journal of Clinical Investigation and from 2012 of The Lancet Respiratory Medicine. He leads a multicenter research group carrying on clinical trials on the role of inflammation in asthma and chronic obstructive pulmonary diseases. He published more than 300 papers in peer-reviewed journals. 4 Nicholas Hopkinson Imperial College London, UK Dr Nicholas Hopkinson is a Reader in Respiratory Medicine & Honorary Consultant Physician at The National Heart and Lung Institute of Imperial College and the Royal Brompton Hospital where he runs the Advanced COPD service. His research focuses on addressing exercise and activity limitation in COPD in areas including pulmonary physiology and lung volume reduction, skeletal muscle impairment and pulmonary rehabilitation. He chairs the British Thoracic Society COPD Specialist Advisory Group and is also active in tobacco control advocacy. John Hurst University College London, UK John Hurst graduated from the University of Edinburgh Medical School and was appointed Senior Lecturer, then Reader in Respiratory Medicine at UCL in October Dr Hurst has a particular research interest in mechanisms of exacerbation susceptibility and cardiovascular risk in COPD. He was awarded the 2012 European Respiratory Society COPD Research prize. His clinical work at Royal Free London NHS Foundation Trust focuses on the Specialist COPD and Bronchiectasis services. A previous Associate Editor at Thorax, during 2012 he was appointed to the Editorial Board of the American Journal of Respiratory and Critical Care Medicine. Dr Rachel Jordan University of Birmingham, UK Dr Rachel Jordan is a Senior Lecturer in Epidemiology at the University of Birmingham, UK. Her role focuses mainly on epidemiological and health services research of COPD for which she recently held an NIHR postdoctoral research fellowship. With other investigators she is also currently in receipt of a 2m NIHR programme grant on COPD and several other related primary and secondary research grants about the identification, management and self-management of COPD. Her team have recently published a large pragmatic RCT to identify the most effective and costeffective ways to identify undiagnosed COPD in primary care, which is now proceeding to long-term follow-up.
5 William MacNee University of Edinburgh, UK William MacNee received his medical degree and MD from the University of Glasgow, Scotland, and trained thereafter at the Royal Infirmary of Edinburgh/University of Edinburgh Medical School. He was the recipient of an MRC travelling fellowship to the University of British Columbia, St Paul s Hospital, Vancouver, Canada. He was Professor of Respiratory and Environmental Medicine, University of Edinburgh, and Honorary Consultant Physician, Lothian Health. He is currently Emeritus Professor of Respiratory Medicine University of Edinburgh and Consultant Physician Lothian Health. Professor MacNee s primary research interests are in pathological mechanisms of the lung and systemic effects of COPD. Professor MacNee has published over 380 peer-reviewed papers and edited several books on COPD. Professor MacNee is a past Chair of the Scientific Committee of the European Respiratory Society (ERS) and past President of ERS. He has served on the British Thoracic Society COPD guidelines committee, was co-chair of the ERS/American Thoracic Society COPD guidelines committee and was a past member of the Global Initiative for Chronic Obstructive Lung Disease executive committee. He has been an Associate Editor of the European Respiratory Journal and the American Journal of Respiratory and Critical Care Medicine and Respiratory Research. Dr. David M. Mannino University of Kentucky, USA David Mannino, MD, is a Professor and Chair in Department of Preventive Medicine and Environmental Health at the University of Kentucky College of Public Health in Lexington, Kentucky. He also has an appointment in the Department of Epidemiology in the College of Public Health and the Division of Pulmonary and Critical Care Medicine in the Department of Medicine at the University of Kentucky College of Medicine. He is the Director of the Pulmonary Epidemiology Research Laboratory. Having obtained his medical degree from Jefferson Medical College in 1981, Dr Mannino went on to complete his internship and residency at Lankenau Hospital, both in Philadelphia, Pennsylvania. A fellowship in pulmonary medicine at West Virginia University School of Medicine/National Institute for Occupational Safety and Health in Morgantown, West Virginia, followed. Dr Mannino served as the chief science officer for the Centers for Disease Control and Prevention s Air Pollution and Respiratory Health branch following his fellowship until his retirement in Dr. Mannino has authored over 250 articles for peerreviewed journals, in addition to book chapters, editorials, and book reviews. His research interests included the epidemiology of asthma and COPD, tobacco use and treatment, and the effects of air pollutant exposure on respiratory function and lung diseases. Dr. Mannino serves on the MASAC for the Alpha-1 Foundation and is the Chief Scientific Officer for the COPD Foundation. He is also an Associate Editor for the Journal of the COPD Foundation. He is currently a Coinvestigator on three NIH-funded grants looking at COPD, including one on early detection, one on progression, and one on risk factors.

Mularski Kaiser Permanente Center for Health Research, USA Richard Mularski, MD, MSHS, MCR, FCCS leads health services research in the areas of palliative care, obstructive lung disease, and dyspnea at The Center for Health Research, Kaiser Permanente and is Adjunct Associate Professor of Medicine/Senior Scholar of Ethics at Oregon Health & Science University in Portland, Oregon. He is Senior Physician for Northwest Permanente also serving as Regional Clinical Quality Lead for COPD, Medical Director for Inpatient Respiratory Therapy, and serves the KP National Guideline Quality Program. Amongst his research experience he is a founding steering committee member of CONCERT and co-pi of the COPD Patient Powered Research Network. Jill Ohar Wake Forest University, USA Dr. Ohar earned her medical degree from Medical College of Pennsylvania in Philadelphia. She received postdoctoral training at the Medical Colleges of Pennsylvania and Virginia and has served on the faculty at Saint Louis University and Wake Forest School of Medicine where she is currently a tenured professor. Dr. Ohar is author of numerous journal articles, reviews, abstracts, and book chapters; some published in such journals as The American Journal of Respiratory and Critical Care Medicine, Journal of the American Medical Society, and Chest, among others. She currently focuses research on the diagnosis and delivery of care to COPD patients. Alberto Papi Universitá degli studi di Ferrara, Italy Alberto Papi is Professor of Respiratory Medicine and Vice President of the School of Medicine at the University of Ferrara, Italy, and Director of the Respiratory Unit of the Department of Emergency Medicine, S. Anna University Hospital, Ferrara, Italy. He trained in Respiratory Medicine at the University of Parma, before spending 4 years in the UK as a Clinical Research Fellow at the University of Southampton, where he is an Honorary Clinical Lecturer. His department is also home to the Research Centre on Inflammatory and Smoking-related Respiratory Diseases (CEMICEF). Professor Papi s main area of research is airway inflammation in asthma and COPD, its molecular mechanisms and pharmacological modulation. In particular, he has an interest in the role of respiratory viral infections in stable/exacerbated obstructive lung diseases, and the underlying molecular and immunological mechanisms. Professor Papi has previously served as a Member of the Asthma Board of the European Academy of Allergy and Clinical Immunology, and Secretary of the Respiratory Infections Group of the European Respiratory Society. He is also a member of the Editorial Board of the American Journal of Respiratory and Critical Care Medicine, Asthma Research and Practice and the Journal of COPD. He has coordinated
7 several pivotal studies in asthma and COPD, and has authored more than 200 articles in peer-reviewed journals. Dirkje Postma University Medical Center Groningen, The Netherlands Dirkje Postma Dirkje Postma is full Professor of Pulmonary Medicine at the University of Groningen and the University Medical Center of Groningen. She is both a researcher and clinician. Dirkje Postma has an impressively broad orientation able to cover the scientific palette of both asthma and COPD from basic science to clinically oriented and patient associated research, a translational researcher pur sang. Dr. Postma has developed her research in conjunction with her clinical and teaching duties and has been able to translate questions from clinical practice to basic research and vice versa. She is an active promoter of pulmonary diseases, a.o. in the NRS (Netherlands Respiratory Society), and numerous administrative and editorial positions in the Netherlands (a.o. NWO/ZonMw (Netherlands Organisation for Scientific Research), the Royal Netherlands Academy of Arts and Sciences (KNAW) and ERS and ATS. She collaborates with many leading scholars in her field. Sally Singh University Hospitals of Leicester NHS Trust, UK Sally Singh, is Professor of Pulmonary and Cardiac Rehabilitation at the University Hospitals of Leicester, and has an honorary chair at Loughborough University. She co-chaired the ATS/ERS statement on pulmonary rehabilitation and more recently the ERS /ATS systematic review and technical standards for field exercise tests. Her research interests include the measurement of exercise performance in COPD, self-management, the use of technology to support the delivery of rehabilitation and health services research focusing on the delivery of both pulmonary and cardiac rehabilitation. Joan B. Soriano Hospital La Princesa, Madrid, Spain Joan B Soriano graduated in medicine in 1993 and received his PhD in epidemiology and methodology of biomedical research in 1996, both from the Universitat Autònoma de Barcelona. He completed his education in epidemiology with a Master of Science at the Erasmus University, Rotterdam, The Netherlands, and a post-doctoral stay at the Johns Hopkins School of Public Health, in Baltimore, USA. He worked in the pharmaceutical industry in the UK from 1998, and later in the USA from Then, in 2005 he moved back to CIMERA in Mallorca, Spain. He serves in the GesEPOC COPD guidelines, has directed the SEPAR s Foreign Affairs, participates in several committees of the Ministry of Health of Spain, and collaborates extensively with both the ERS and CHEST. He spends most of his time dedicated to research on COPD and other respiratory conditions, aging and tobacco, the design of new clinical trials and translational medicine. Since 2007 he is an Associate

Daiana Stolz University Hospital Basel, Switzerland Daiana Stolz graduated in 1997 (first class honors) and completed her clinical training in Internal and Pulmonary Medicine in Brazil, Germany, Switzerland and the US. She became an Assistant Professor in Respiratory Medicine at the University Basel, Switzerland in 2007 and received a Master Degree in Public Health by the Harvard School of Public Health, US in After being honored with a Research Professorship by the Swiss National Foundation in 2011, she was appointed as Full Professor for Respiratory Medicine by the University Basel, Switzerland in Dr. Stolz contributed with several studies on pulmonary and systemic biomarkers for the characterization of COPD patients. Dr. Stolz has authored >150 publications in peer-reviewed journals and is a member of the editorial board of Chest Byron Thomashow Columbia University Medical Center, USA Dr Byron Thomashow is a Professor of Medicine Columbia University Medical Center and an Attending Physician at the New York- Presbyterian Hospital. He helped found and now serves as chairman of the board of directors of the COPD Foundation. He is the medical codirector of the Jo-Ann LeBuhn Center for Chest Disease on the Columbia campus and is medical director of the NewYork-Presbyterian Lung Volume Reduction program. He chaired the Respiratory Disease Council of the NewYork-Presbyterian Healthcare Network for the last decade. He is the co-chair for the NY State COPD Coalition, cochaired the New York State COPD Summit in 2010, co-chaired COPD7 USA 2011, COPD8USA 2013, COPD 9USA 2015 and the COPDFoundation COPD Readmission Summits in October 2013 and March He was a member of the steering committee and the co-primary investigator at the Columbia site for the National Emphysema Treatment Trial and has been and remains actively involved in multiple national clinical research projects. He was awarded the 2013 ATS Public Advisory Roundtable Excellence Award. In 2016 he received the Columbia University 2016 Leonard Tow Humanism in Medicine Award sponsored by the Arnold P. Gold Foundation.
10 George Washko Harvard Medical School, USA I am a clinical investigator with a focus in chronic lung diseases such as COPD and Pulmonary Fibrosis. My participation in therapeutic trials and observational studies has informed my interest in the use of imaging to discover biomarkers for stratification, prognostication, and intermediate study endpoints. My goal is to continue to develop and validate subjective and objective methods for the ascertainment of impactful features evident on medical imaging acquired as part of research and routine clinical care. I believe that this effort will help to maximize the potential of an underutilized resource.
