24 Jefferson Center Follow Up & Monitoring Services The health care system is adapting to the ever-changing needs and demands of health care users. As the systems evolve and user engagement matures through relationship building, it creates opportunities to dramatically improve health care delivery. Many promising practices are being tested and many are yet to be discovered. One Program being tested to close the gap in Jefferson County is

25 Jefferson Center Follow Up & Monitoring Services The Child and Youth Leadership Commission (CYLC, or HB1451) in Jefferson County started a project in 2014 in the Health/Mental Health/Other Subcommittee that has created a collaborative relationship with Jefferson County School District and Jefferson Center for Mental Health to address suicide ideation in children that have been identified by the schools through a suicide risk assessment as high risk.

26 Jefferson Center Follow Up & Monitoring Services Jefferson County School District conducted 896 suicide risk assessments during school year Through the collaborative relationship and partnership of the school district and Jefferson Center for Mental Health, 37 of those assessments were identified as high risk and received specialized follow-up treatment and wrap around support from all the systems involved in that child s life. The goal is suicide prevention.

27 Jefferson Center Follow Up & Monitoring Services Closing the Gap: From micro relationship building with the individuals and families effected by suicide, to the macro relationship building with stakeholders, community members, and providers, the common thread is connectedness and partnership that heals and moves us to grow.
