1 2009 Grants The St. Louis Affiliate of Susan G. Komen for the Cure, along with those who generously support us with their time, talent, and treasures, is working to better the lives of those facing breast cancer in the St. Louis community. As a result of the Komen St. Louis Affiliate fundraising efforts, including the annual Susan G. Komen St. Louis Race for the Cure locally presented by Wachovia Securities, over $11 million in the last 10 years has been invested in the St. Louis community for breast cancer screening, treatment, education and support programs as well as research. The Affiliate is proud to announce total grants of $3.3 million this year, a record amount for the Affiliate. Over $2.5 million will stay in the St. Louis community to fund 28 programs at local health care facilities and non-profits. These grants will go towards innovative projects in the areas of breast health and breast cancer research, education, outreach, screening, treatment and support services not otherwise available to the medically underserved populations of the following counties: In Missouri; Saint Louis City and County, Osage, Montgomery, Lincoln, Warren, Franklin, Washington, Saint Charles, Jefferson, Ste. Genevieve, Saint Francois, Perry, and Madison, and in Illinois; Clinton, Saint Clair, Madison, and Monroe. In addition to the $2.5 million in local grants, over $800,000 raised locally went to the Susan G. Komen for the Cure Grants Program to fund superior-ranked breast cancer research programs. Breast Cancer Screening and Education Program St. John s Mercy Medical Center Carol Murphy, LCSW, (314) This is the 7th year of Komen St. Louis funding for this program. The Breast Cancer Screening and Education Program provides free breast exams, mammograms and diagnostics to low income women who cannot afford a yearly breast exam and mammograms. Services are provided both at St. John s Mercy Medical Center and through its mobile mammogram van program. The program has grown significantly over the years and will serve over 720 women this grant year. Komen funding will help to provide at least 40 community breast-screening events with an average of 18 women screened per event. The goals for the program are to increase access to breast screening services as well as education, prevention and early detection of breast cancer. Follow-up services are provided at St. John s Mercy Medical Center to every woman who requires additional diagnostic services. Breast Health Initiative Outreach Project of SSM St. Joseph SSM St. Joseph in St. Charles County Terry Lohmeyer, RN, BS, (636) This is the 7th year of Komen St. Louis funding for this program. The SSM St. Joseph Breast Health Initiative Outreach Project serves women in St. Charles, Lincoln, Warren, and Montgomery counties. This program serves uninsured/underinsured women 40 to 64 and those under 40 with symptoms or at high risk of breast cancer. These women receive education on the risks of breast cancer, instruction in breast self exam, a clinical breast exam, as well as a screening or diagnostic mammograms, biopsies and follow-up care when needed. SSM St. Joseph in St. Charles County anticipates serving 390 women this year. Funding will also support a minimum of 12 community outreach programs. Breast Navigator St. John s-washington Connie Stelzer, RN, Project Director, (636) St. John s Mercy Hospital is the largest acute care hospital in Franklin County and offers health care for the residents of Franklin, St. Charles, Warren and Washington counties. Komen St. Louis funding will help support a Breast Health Care Navigator who will help to ensure that breast cancer patients from disadvantaged backgrounds get high-quality breast cancer screenings, diagnosis and treatment, regardless of their ability to pay. The navigator s duties include facilitating tests, consultations, and treatment, along with finding supportive services and maintaining contact throughout the duration of treatment. It is expected that women will receive assistance during the grant year as a result of Komen dollars in the community.

2 Bridging the Gap St. Louis University Cancer Center, Department of Internal Medicine Rebecca Dougherty, MSW, LCSW, Project Director, (314) Komen St. Louis funding will help St. Louis University to develop and evaluate a pilot program, which will provide non-medical emergency gap funding to individuals who are in active treatment for breast cancer. This financial assistance is intended for emergency, non-medical household living expenses. This may include, but not be limited to financial assistance for: rent, utilities, transportation, and payment of insurance copayments. This pilot project will include screening and needs assessment by a healthcare professional using a social service model. The desired outcome is an increase in compliance with treatment and a decrease in mortality rates from breast cancer. Approximately 250 women will be served through this program. A Comprehensive Breast Health Education & Screening Program for Women in Rural Communities Jefferson Regional Medical Center Charlotte Anderson, Women s Life Phase Coordinator, (636) This will be the 6th year of Komen St. Louis funding for this program. Jefferson Regional Medical Center intends to reach women in the rural counties of Jefferson, Ste. Genevieve, St. Francois, Washington, and northern Perry through its Breast Health Education and Screening Program. Komen funding provide 90 mammograms and 10 ultrasounds for underserved women. Jefferson Regional Medical Center will continue offering support, resources, and mentoring for breast cancer survivors who meet monthly at the hospital and rely on each other s support to cope as survivors. The Daylight Project: Pushing Back Shadow Barriers to Breast Health, Early Detection and Timely Treatment for Metro St. Louis Refugee and Immigrant Women Barnes-Jewish Hospital Refugee Health Service Eva Enoch, Daylight Project Outreach Coordinator, (314) This is the 6th year of Komen St. Louis funding for this program. The Daylight Project is a multidisciplinary effort of the Barnes-Jewish Hospital Refugee Health Services. The program attempts to reduce the many barriers to breast cancer for refugee and immigrant women, especially those with limited English proficiency. A major component of the Daylight Project is the Wise Women Program, which provides training, guidance and resources to Refugee women. Barnes-Jewish Hospital has committed to generating a minimum of an additional 350 women receiving screening mammograms through this program. Early Detection of Breast Cancer in African American Women Community Health-In-Partnership Services (CHIPS) Judy Bentley, WHNPBC, MA, President and CEO, (314) ext CHIPS operates a free health clinic in North St. Louis staffed by volunteer physicians who care for uninsured and underserved clients from all over the metropolitan area. CHIPS Early Detection of Breast Cancer in African American Women Program will address the breast cancer mortality disparity in the St. Louis metropolitan area. CHIPS seeks to improve breast cancer outcomes for uninsured African American women between the ages of 40 and 64 by promoting breast health awareness and the importance of early detection. Komen funding will support a breast health navigator who will provide one-on-one instruction and breast education materials to at-risk women. Under this program, 1,500 women will be reached through educational programs and 600 women will receive screening mammograms. The Breakfast Club s Bra and Prosthesis Program for Breast Cancer Survivors The Breakfast Club, Inc. Sherrill Jackson, BCI Founder and President, (314) This will be the 9th year of Komen St. Louis funding for this program. The Breakfast Club, Inc. is a breast cancer support group serving primarily African American women residing in North St. Louis City and North St. Louis County by providing education, awareness, resources and support. This year, Komen St. Louis will continue to partner with the Breakfast Club by funding, in full, its Bra and Prosthesis Program. Komen St. Louis funding will provide 100 mastectomy bras and 85 breast prostheses for uninsured and underserved women in the St. Louis community.

3 East St. Louis Breast Cancer Screening & Treatment Southern Illinois Healthcare Foundation Beth Zurliene, RN, Project Director, (619) This is the 2nd year of Komen St. Louis funding for this SIHF program. Southern Illinois Healthcare Foundation is the primary health care provider to poor and uninsured individuals in the Metro East area. Komen funding will support two Patient Care Navigators to work on Southern Illinois Health Care Foundation s START NOW! Breast Cancer Screening Program. As a result, over 900 patients will be navigated through the screening, diagnostic and treatment processes and will receive assistance in setting up appointments, arranging transportation for appointments when needed, and receive help to complete the paperwork to qualify for services paid for by the Illinois Breast and Cervical Cancer Program (IBCCP), Medicaid, or other insurers. Educating Women About Hereditary Risk for Breast Cancer St. Louis University, Department of Internal Medicine Suzanne Mahon, RN, DNSc, AOCN, APNG, (314) This is the 8th year of Komen St. Louis funding for this program. Through the Educating Women about Hereditary Risk for Breast Cancer Program, Komen funding will be used to support and expand the existing hereditary cancer education program at Saint Louis University by providing education and counseling to 85 new, at-risk families and continuing to provide services to an estimated 65 families already served by the program. A series of community education programs and professional education programs for healthcare providers will inform a wider sector about the role of genetics in breast cancer. Komen St. Louis is proud to support this program so that all women and at-risk families in its service area can receive quality genetics counseling for breast cancer. Empower and Engage SSM St. Mary s Health Center Mary Schurk, RNC, WHNP, Director, Women s Well, Outpatient Clinic and FETU, (314) This will be the 8th year of Komen St. Louis funding for this program. SSM St. Mary s will provide free breast health services for uninsured and underserved women ages to include mammogram screenings and diagnostics, breast healthcare education, coordination of services, and treatment. Komen funding supports a Breast Health Service and Outreach Coordinator. With Komen St. Louis funding, over 200 additional women will be served at SSM St. Mary s for breast health care services. FCHC Breast Health Screening ACCESS Project Family Care Health Centers Kathleen Garst, Chief Operations Officer, (314) ext The Family Care Health Centers Breast Health Screening ACCESS Project will provide breast health care education and access to screening mammograms for low-income women treated at Family Care. Komen St. Louis is funding a patient navigator who, through individualized case management and partnership with the Siteman Cancer Center, women found to have positive findings on screening tests will be navigated through diagnostics, treatment and recovery. Family Care plans to provide up to 750 mammograms during this current grant year. Gateway to Hope CarcinAid Foundation Cynthia Frank, Executive Director, (314) This is the 5th year of Komen St. Louis funding for this program. The CarcinAid Foundation s Gateway to Hope (GTH) program arranges donated medical and surgical care for individuals with breast cancer earning between % of the federal poverty level. Gateway to Hope expects to arrange care for 30 uninsured or underinsured low-income patients with a confirmed diagnosis of breast cancer as well as those genetically predisposed to breast cancer who are not eligible for publicly funded treatment programs. Additionally, approximately 80 referrals will be made to help individuals not eligible for GTH services. Komen funding will support a nurse manager and one outreach education nurse.

4 Iniciativa de Salud para Mujeres (Women s Health Initiative) Puentes de Esperanza Nereida Avendano, Project Director, (618) This is the 2nd year of Komen St. Louis funding for this program. Puentes de Esperanza s Women s Health Initiative seeks to increase breast health care/breast cancer knowledge and awareness and to reduce the cultural, social and economic barriers that prevent access to quality breast cancer screening and treatment for the growing Hispanic/Latina population in Metro East St. Louis. Komen St. Louis funding provides support for a Spanish-speaking Breast Health Advocate to provide education programs for women in small group and one-on-one settings, to distribute Komen breast health literature, conduct Spanish-language public awareness activities, administer risk factor screening questionnaires, and refer women to local healthcare facilities for breast cancer screening and detection services. Over 300 women will be served this year through this program with local Komen funding. Latinas Por La Cura Barnes-Jewish Hospital Refugee Health Service Barbara Bogomolov, RN, BSN, MS, Clinical Manager, Refugee Health and Interpreter Services, (314) This will be the 1st year of Komen St. Louis funding for this program as a separate applicant. For the past 2 years, small grant funding has been provided for this program through BJH s Daylight Project. Latinas Por La Cura of Barnes-Jewish Hospital has grown from a small effort to link the few Latina women willing to identify themselves as breast cancer survivors to a vibrant and active survivor support group now interested in developing a breast cancer community service arm. In this next year with Komen St. Louis funding, Latinas Por la Cura will use its energies to bring together community and health care leaders to address and eliminate system barriers that stand in the way of some women receiving breast health care screening and care. Their goal is to work with the BJH Breast Health Center and key community stakeholders to design and disseminate structured best practice models for triage and referral of vulnerable women with breast problems. Lymphedema Assistance Program St. Louis Peregrine Society, Inc. Shari Cunningham, Events Coordinator, (314) This is the 4th year of Komen St. Louis funding for this program. The St. Louis Peregrine Society manages the Lymphedema Assistance Program to help breast cancer survivors control and minimize the pain associated with lymphedema. The program provides free lymphedema garments to eligible breast cancer survivors. With Komen funding, the peregrine Society will provide 120 lymphedema garment sets for uninsured and underinsured women in the St. Louis area. The MBMC Breast HealthCare Center Reaches the Underserved Missouri Baptist Medical Center Sarah Ernsky, RN, BSN, Manager, Breast HealthCare Center, (314) Teresa Taylor, Breast HealthCare Outreach Coordinator, (314) This will be the 8th year of Komen St. Louis funding for this program. The Breast HealthCare Center at Missouri Baptist Medical Center provides screening and diagnostic mammograms to uninsured and underinsured women ages in St. Louis City and County, and eleven rural Missouri counties which include Jefferson, St. Francois, Warren, Ste. Genevieve, Perry, Lincoln, Madison, Washington, Franklin, St. Charles, and Montgomery Counties. These services are provided in the Breast HealthCare Center at Missouri Baptist and on its mobile mammography van, which is the only van in St. Louis that can accommodate the physically disabled. Additionally, educational programs provide uninsured and underinsured women knowledge about the importance of early detection of breast cancer by screening and the dynamics of breast self-exam. Women with mammography abnormalities will have a follow-up examination at the Missouri Baptist Breast HealthCare Center. With Komen funding, Missouri Baptist plans to educate and screen over 430 women in the current grant year.

5 The Navigator Project Washington University School of Medicine Vickie Knighten, Breast Cancer Patient Navigator, (314) This is the 6th year of Komen St. Louis funding for this program. The Breast Cancer Patient Navigator Program provides support to medically underserved women in the St Louis area who are treated for breast cancer at Siteman Cancer Center. The goals of the program are to decrease the time from a biopsy diagnosis of breast cancer to initiation of treatment and to increase treatment compliance by providing counseling, practical support, and removing barriers to care. The navigator assists with transportation and housing needs, scheduling, escort to tests or procedures, completing paperwork and referral to other community support agencies and services as needed. Nutritional Services for Low Income Breast Cancer Patients Food Outreach, Inc. Greg Lukeman, Executive Director, (314) ext This is the 3rd year of Komen St. Louis funding for this program. Food Outreach provides critically needed nutritional assistance for low-income women in active treatment for breast cancer. Patients receive an initial assessment by an on-staff Registered Dietitian, which allows patients to make nutritional selections from a changing menu. Choosing from the menu is significant for breast cancer patients, since appetite changes are prevalent over the course of treatment. Food is provided frozen, and also can be delivered directly to a patient s home due to inability to come to the Food Outreach facility. Nutritional supplements are also available for those who need them. Komen funding will support the nutritional needs of approximately 45 low-income breast cancer patients by providing over 25,000 in the upcoming year. Outreach, Education, and Screening in Madison County, Illinois Madison County Health Department Amy Yeager, MPH, Health Promotion Services, (618) ext This will be the 7th year of Komen St. Louis funding for this program. The Outreach, Education, and Screening Program of the Madison County Health Department helps women by addressing breast cancer awareness and screening. This Komen-funded project focuses on outreach and education to help women become more aware of the importance of screening for breast health. Media and social marketing efforts attempt to reach low-income women throughout the county. Komen funding will provide 80 screening mammograms, five diagnostic mammograms, five ultrasounds, and 30 office visits to uninsured and underserved women between the ages of in Madison County. People s Sister Connection People s Health Centers Mark Sanford, VP, Planning & Development, (314) ext This is the 7th year of Komen St. Louis funding for this program. Betty Jean Kerr People s Health Centers (PHC) will provide free mammograms, community-based in-reach, outreach, and referrals for women at risk for breast cancer. Eligible women will be referred for clinical breast exams and mammograms. This year, Peoples Health Centers will expand its internal capacity to screen more women for breast cancer. A Komen-funded navigator will act as a case manager to provide advocacy, encouragement and support services regarding breast cancer prevention, education and screening. Peoples is hoping to provide over 1000 mammograms for medically underserved and uninsured women 40 years and older, in this current grant year.

6 Reaching In A Comprehensive Program for Breast Cancer Screening St. Louis University, Department of Internal Medicine Mark Varvares, MD, Director, Saint Louis University Cancer Center, (314) Saint Louis University (SLU) Cancer Center is making direct and targeted efforts to expand its breast cancer-screening program this year. Komen funding will support a Breast Healthcare Navigator to reach underserved women. Because of its location and population served, SLU is focusing on in-reach efforts to provide breast cancer screening to underserved at-risk women who are currently receiving other tertiary care services at SLU, relatives of patients, or employees and their relatives who are uninsured or underinsured. The program targets women ages 40 to 64. The breast health navigator will work with each woman to promote annual mammograms, ensure timely and complete evaluation of any identified breast abnormalities, coordinate care if a diagnosis of breast cancer is made and provide a single, identifiable resource at SLU to serve as an entry point of contact for needed breast health care. SLU estimates that 600 women will be served through this Komen-funded partnership. The navigator will also be responsible for providing at least 15 educational programs to the community. Solutions for Women: Breast Health Education and Outreach for Underserved Women Christian Hospital Northeast-Northwest Patricia Polk, Breast Healthcare Outreach Coordinator (314) This will be the 2nd year of Komen St. Louis funding for this program. Christian Hospital Northeast-Northwest offers women of North County who are uninsured or underinsured access to screening, early diagnosis and treatment of breast cancer. Komen funding will support 200 screening mammograms for uninsured women. Christian Hospital s Komenfunded Outreach Coordinator serves as a breast health care resource, provides education programs for local organizations, churches and businesses, participates in health fairs to stress the importance of monthly self-exams and the need for yearly mammograms and recruits women for annual screenings. STEP-UP Grace Hill Grace Hill Neighborhood Health Centers, Inc. Veronica Richardson, Director, Community Health Services, (314) This is the 2nd year of Komen St. Louis funding for this program. Grace Hill Neighborhood Health Centers, Inc. aims to aggressively address the high incidence of mortality from breast cancer that disproportionately affects minority women living in the Grace Hill service areas. A Breast Health Care Coordinator focuses on in-reach and education to the women currently receiving care through Grace Hill s Chronic Disease Program. These efforts will result in over 900 women receiving clinical breast exams and screening mammography services in the current grant year. Tightening the Safety Net: Caring for Underserved People through Community Alliances and Navigation Barnes Jewish Hospital Susan Kraenzle, RN, Joanne Knight Breast Health Center, (314) This is the 10th year of Komen St. Louis funding for this program. The Joanne Knight Breast Health Center at Siteman Cancer Center has evolved from providing a few hundred mammograms in the late 1980s to one of the largest outreach programs in the entire State of Missouri. The Komen St. Louis Affiliate will fund over 1900 screening and diagnostic mammograms for low-income women in the St. Louis area through this critical breast healthcare partnership with Siteman Cancer Center. Additionally, Komen funding supports two Breast Health Navigators who will accompany 600 individuals though screening and diagnostic evaluation of a breast abnormality to diagnosis and treatment of breast cancer.

7 Wellness for Women St. Clair County Health Department Karen Pratt, Outreach & Prevention Manager, (618) This is the 8th year of Komen St. Louis funding for this program. The Wellness for Women Project, a Komen St. Louis-funded program of the St. Clair County Health Department, increases breast cancer awareness and screening to low-income women in St. Clair County, Illinois. Komen funding will provide 215 screening mammograms for low-income uninsured women, aged 40-49, and low-income women with insurance who have a high deductible and cannot afford the cost of routine mammograms. St. Clair County Health Department is the lead-agency in St. Clair County for Illinois Breast and Cervical Cancer Program. It serves as a vital resource for both Southern Illinois Healthcare Foundation and Puentes de Esperanza, Komen grantees in St. Claire County, Illinois, to assist women in accessing both state and federally health care programs. Women s Information and Screening Endeavor (WISE) SSM St. Clare Health Center Sandy Cobb, RN, BSN, RDMS, Director, Women s Imaging, (314) The Women s Information and Screening Endeavor (WISE) Program of SSM St. Clare Health Center is a patient recruitment program providing uninsured and underinsured women living along the I-44 corridor of St. Louis and Franklin Counties, in the Northwest R-I School District in Jefferson County, and in the Meacham Park area of Kirkwood with access to breast health care education, mammograms, clinical breast exams, referrals for genetic counseling, diagnostics and treatment referral when necessary. Komen funding will support 120 screening mammograms and 30 diagnostic mammograms during this current grant year CRAFT GRANT: The recipient of the CRAFT Grant award is Dr. Cynthia X. Ma, MD, PhD, Washington University School of Medicine for her Phase I/II Clinical Trial evaluating the combination of an IGF-IR antibody and an mtor inhibitor as a therapeutic strategy for patients with metastatic breast cancer. Pre-clinical studies combined targeting of IGF-IR and mtor, two of the most important molecules mediating cell growth and survival, offers great potential for the care of these patients. Agents that target growth factor receptor signaling pathways in cancer cells are among the most promising therapeutics for breast cancer, and the development of these agents is a priority in order to reduce breast cancer mortality in the next decade. As the understanding of tumor biology evolves, it has become clear that the effectiveness of a single agent may be limited because of the complex signaling networks that exist in the cancer cell. Rational design of combination regimens is often necessary to achieve maximum benefit from targeted therapeutic agents. Komen St. Louis funding will support enrollment into this clinical trial as well as research support for correlative studies for this NCI approved Phase I/II Clinical Trial evaluating IMC-A12 (IGF-IR antibody) in combination with temsirolimus (mtor inhibitor) for women with stage IV metastatic breast cancer. The correlative studies will identify essential information on which patient is likely to benefit from this treatment regimen and why.
