2 Since 1975, the cancer program at Saint Francis Health System has provided state-of-the-art medical oncology and radiotherapy technology to residents in eastern Oklahoma and surrounding states. Committed to advancing oncology care, Saint Francis provides an array of oncology services to meet patient needs. The cancer center s treatment practices include boardcertified physicians who provide multidisciplinary care in a professional, patient- and family-focused setting. Additionally, the cancer program is accredited by the American College of Surgeons as a Comprehensive Community Cancer Program. 1
3 2016 Annual Cancer Program Update: It has been an eventful year for our oncology program and services within Saint Francis Health System. Continuing to provide excellent clinical and patient-centered care and services remains our goal. Highlights of some of the major achievements and improvements for 2016 include the following: Implementation of our oncology electronic health record module as part of Epic for both adults and pediatric cancer patients Implementation of the radiation oncology electronic health record system Pediatric oncology affiliation with St. Jude Children s Research Hospital cancer program Implementation of an oncology rehabilitation program (occupational therapy, physical therapy, speech therapy and lymphedema services) specific for cancer patients Implementation of an adult oncology patient/family advisory council to help us focus on improvement in services and processes based upon patient/family experience Expanded hours of service at the Saint Francis Breast Center for screening mammography (7:00 a.m. 6:30 p.m. Monday Thursday) Addition of an oncology certified physician assistant (PA-C) to perform survivorship visits and see adult oncology patients same day in the clinic in an effort to help reduce or prevent unnecessary visits to the emergency department and/or hospitalization Over $1.8 million in outpatient oncology drug patient assistance obtained to help deter the costs of cancer treatment through drug replacement and/or drug co-pay cards, etc. Sponsorship or hosting of multiple oncology community events for oncology patients/survivors such as the annual wig party; Saint Francis cancer survivor night at the Tulsa Drillers; Blood and Marrow Transplant survivors party; American Cancer Society Relay for Life; American Cancer Society Cattle Barron s Ball; American Cancer Society Strides Against Breast Cancer; Get Your Rear in Gear Colon Cancer Coalition Race/Walk Provided over 350 mammograms or other breast diagnostic services to patients through partnering with Oklahoma Project Woman a program designed to provide breast health screening/diagnostic services to women in Oklahoma who have no health insurance and/or limited financial resources Provision for outpatient oncology social services/ counseling Addition of an oncology nurse navigator Receipt of a grant from the Colon Cancer Coalition to promote colon cancer screening to indigent patients through the Xavier Medical Clinic Saint Francis remains the only nationally recognized Lung Cancer Alliance Screening Center of Excellence in the state of Oklahoma Warren Clinic Medical Oncology was one of 195 practices selected to participate in the CMS (Center for Medicare and Medicaid Services) Oncology Care Model (OCM) which is a five-year program designed to have practices focus on improving care coordination, efficiency in care, quality and decreased costs. The Center of Genetics implemented a new test, Colon and Lung Tumor NGS panel which covers over 1,500 gene targets spanning 22 genes that have implications in targeted therapies for colon and lung cancer Saint Francis Health System remains committed to being a leader in oncology care and services for northeastern Oklahoma. 4 Accredited by the Commission on Cancer Since 1978 The Commission on Cancer (CoC), a program of the American College of Surgeons (ACoS), recognizes cancer care programs for their commitment to providing comprehensive, high-quality and multidisciplinary patient-centered care. Saint Francis is a Comprehensive Community Cancer Program that has been continuously accredited by the Commission on Cancer since The Commission on Cancer (CoC) is the only multidisciplinary accreditation program for cancer programs in the U.S. and membership is voluntary. Saint Francis has a dedicated multidisciplinary Cancer Committee that meets quarterly to ensure the highest quality of care is being provided to our patients. This dedicated committee sets annual goals and implements improvement projects that focus on quality of care. Outstanding Achievement Award Saint Francis was surveyed by the CoC in November 2014 and received the Outstanding Achievement Award. Saint Francis is in a select group of only 75 U.S. healthcare facilities with accredited cancer programs to receive this national honor for surveys performed in The award acknowledges cancer programs that achieve excellence in providing quality care to cancer patients and is current for three years. The Saint Francis Health System cancer program was evaluated on 34 program standards categorized within one of four cancer program activity areas: cancer committee leadership, cancer data management, clinical services and quality improvement. The cancer program was further evaluated on seven commendation standards. To be eligible, all award recipients must have received commendation ratings in all seven commendation standards, in addition to receiving a compliance rating for each of the 27 other standards. The CoC conducts an on-site survey every three years to ensure compliance with current program standards. Saint Francis will be surveyed in November Ancillary and Support Services Ancillary Services Hospice The Saint Francis Hospice team provides physical, psychological, social and spiritual support to terminally ill patients and their families. The hospice team also serves as consultants for end-of-life care discussion. Pastoral Care A representative from pastoral care offers the patient and their family compassionate support and an opportunity to talk and pray about the issues they are facing. Pharmacy Registered pharmacists and qualified technicians provide a broad spectrum of services to the cancer patient, hospital staff and physician. Rehabilitative Services The goal of acute care physical therapy focuses on maintaining or maximizing a patient s functional independence. Therapy plans are individualized for each patient to ensure their personal needs are met. Social Services/Case Managers Assistance and guidance are provided to link the patient with appropriate resources they need at the time of discharge. Diagnostic Services Computerized axial tomography scan (CT) Digital radiography for mammography Magnetic resonance imaging (MRI) and breast MRI Positron emission tomography scan (PET) Nuclear medicine Support Services The following services are available throughout Saint Francis Health System: American Cancer Society resource room Community resource coordination (transportation, financial resources) Case management and patient navigation Laureate Psychiatric Clinic and Hospital Adjustment to illness counseling Caregiver and family counseling Inpatient and outpatient treatment 3
5 Prevention and Screening Programs Offered at Saint Francis Hospital Saint Francis Hospital offers a variety of annual prevention and screening programs to the community. The Saint Francis Cancer Committee is committed to developing programs that have a direct impact on health. These programs provide early detection and promote a healthy lifestyle Prevention Programs Prevention programs identify risk factors and use strategies to modify attitudes and behaviors to reduce the chance of developing cancer. Saint Francis WellNow online wellness tool Walk to Rio, a program to incentivize employees to be active regularly Annual Health Zone Fitness and Wellness Expo Hundreds of people participated in various screenings offered at the expo American Cancer Society Resource Center at the Saint Francis Cancer Center Provided assistance to over 215 patients: over 100 wigs and 260 personal health managers were provided, and the Hotel Partners Program assisted patients with hotel rooms Clear Direction* tobacco cessation classes Tobacco cessation classes are free for employees and open to the public for a $25 refundable deposit on completion of the program Point of Balance adult weight management classes More than 50 participants have lost over 250 lbs. ShapeDown* (children s weight loss program) More than 50 pediatric participants have lost over 50 lbs. *Offered several times a year at Health Zone at Saint Francis 2016 Screening Programs Screening programs detect cancers at an early stage, improving rates of survival. Over 300 people have utilized the low-dose CT lung screening for individuals who are high risk for developing lung cancer. To date four participants were diagnosed with cancer through the lung screening program. Saint Francis is the only facility in Oklahoma who has received the Screening Center of Excellence designation from the Lung Cancer Alliance for our lung cancer screening program.
