6 Preface The genesis of malignant tumors is currently viewed as a multistep phenomenon with regard to both molecular biology and morphology. In histology and cytology, premalignant lesions are of diagnostic relevance for the early recognition of malignancy. The present volume focuses on another definite phase in the process of malignant growth, i. e. when a lesion has passed both the potentially premalignant and the in situ stages and can just be recognized as an early invasive tumor. As in the terminology of premalignant lesions, gynecology has been the pioneer in this field with the concept of "microcarcinoma", proposed in 1947 by G. Mestwerdt. These very early stages of invasion have also been appraised as "incipient neoplasia" or "minimal neoplasia". Our preference for the last term is based on the idea that all the histologic criteria of malignant growth are present, albeit with minimal expression. These phases, visualized with increasing accuracy by modern diagnostic techniques, represent an important challenge for truly early cancer detection. Consequently, the volume has two main parts: The first meets the challenge by providing a general survey of the biology and pathology of early malignant invasion, while the second deals with the minimal forms of neoplasia in tumors in various organs. A display of histologic manifestations is followed, as far as possible, by the current spectrum of therapeutic consequences. Cryptic gliomas are also included as a very special form of minimal neoplasia. With several papers on the cytogenetics, histology and cytology of preleukemia and incipient lymphoma, the volume closes with a complex for which definition and therapeutic consequences are still somewhat controversial.

7 VI Preface Our overview of the present state of knowledge on minimal neoplasia is by no means comprehensive. We have attempted to assemble some established concepts of biology, pathology, and clinical medicine in order to provide a useful basis for diagnosis and therapy.
