8 Three phases of HPAI outbreak in Thailand HPAI outbreak in Thailand was first recognized in January The etiologic pathogen was H5N1. Situation of the outbreak, measures taken were summarized into 3 phases 1. Initial phase: immediately after the recognition of the disease 2. Expansion phase: the duration after stamping-out and preemptive actions were taken 3. Finalized phase: the time after preemptive culling was stopped, sporadic cases were detected based on case definition and clinical signs

9 First phase : Initial Phase! Emergency response right after HPAI was recognized! Initial surveillance! Role of laboratory! Control and Prevention measures

10 Initial Surveillance! Duration: Jan 04! Area : every village, farm in the country! Sampling: 95% CI! No.samples: 75,623! Samples: Cloacal swabs for virus isolation

11 Initial Surveillance Results:! 161 cases (village, farm) in 71 districts, 40 provinces found positive

12 Actions Taken during the initial phase! Stamping-out and disinfected of infected premises! Preemptive culling of poultry within 5 km radius from infected premises! Movement control (area km radius from infected premises) Check HPAI 10 days before catching date Only negative farms could obtain movement permit from the DLD Ante-mortem check at the slaughter houses by collecting cloacal swabs to recheck for AIV Product check: all poultry products were sampling and checked for AIV Ante-mortem and product check must be negative in order to obtain health certificate for export from the DLD

13 2 nd Phase: Expansion of Surveillance! Duration: 11 Feb - 4 March 04! Areas surrounding stamped out zone (>5-10 Km radius) 2nd surveillance zone Stamped-out zone! Suspected areas (eg. low production, cases or deaths with other disease)

14 Second Surveillance! 95 % CI! No. of samples :12,000! Samples: cloacal swaps for virus isolation! Map shows locations

15 Result of the Second Surveillance! 18 new cases found (blue dots)! 17 cases located in the previously positive provinces (40 provinces)! One new case in one new province (Rot Et)! Red dots indicate previously positive cases

18 Clinical Presentation! Different from previously reported! Daily mortality rate is lower! Infected birds show no clinical signs! Death bird doesn t shed virus in feces! Symptomatic birds shed virus in feces! Mortality and morbidity in duck (some areas)

23 Current situation! The last HPAI suspected farm was stamped-out and disinfected on 24 May 2004.! No new outbreak was detected for more than 3 weeks then one isolated case was reported in Chiangmai! Pre-movement check, movement control, antemortem and product checked are still carried out nationwide for early detection and monitoring! Sero-surveillance will be conducted soon in order to identify risk areas, evaluate and monitor the prevention and control program.
