Bovine enmel specimens were rndomly llocted (n = 15) to: experimentl liquid dentifrices with 550 g F/g (ph 4.5); 550 g F/g (ph 7.0); 1,100 g F/g (ph 4.5); 1,100 g F/g (ph 7.0); 5,000 g F/g (ph 4.5); 5,000 g F/g (ph 7.0); plcebo (ph 4.5); plcebo (ph 7.0); nd commercil toothpstes with 550 g F/g (Colgte Bby Brney, ph 7.0); 1,100 g F/g (Crest, ph 7.0); nd 5,000 g F/g (Prevident, ph 7.0). The specimens were subjected to ph-cycles (deminerlistion 6 h/reminerlistion-18h dy) nd treted with one of the dentifrices 2! 15 s/dy for 7 dys. The deminerlistion ws nlysed by surfce nd cross-sectionl hrdness. Additionlly, different bovine enmel specimens (n = 9) were deminerlised for 6 h nd then treted with one of the experimentl dentifrices for 1 min. The deposition of lkli-soluble fluoride ws determined. Dt were tested using ANOVA/Tukey, Kruskll-Wllis/Dunn nd two-wy ANOVA/Bonferroni s test (p! 0.05). All F dentifrices significntly reduced the surfce enmel deminerlistion except the experimentl dentifrice with 550 g F/g (ph 7.0). Regrding the subsurfce deminerlistion, ll F dentifrices were ble to reduce the hrdness loss up to 90 m; however, in deep lyers only the experimentl dentifrices with 1,100 nd 5,000 g F/g were different from plcebo. All experimentl F dentifrices promoted fluoride deposition compred to plcebo. Higher fluoride deposition ws found for 5,000 g F/g compred to the others regrdless of the ph. Considering the experimentl protocol, the effect of fluoridted cidic or neutrl liquid dentifrices to protect ginst enmel deminerlistion in vitro seems to be modulted minly by the fluoride concentrtion. Smples from the enmel mtrix in the erly stges of secretion nd mturtion were obtined from the incisors of both mice strins fter scrifice. All detectble trces of mtrix protein were removed from the smples by sequentil extrction procedure. The protein-free crystls (n = 13 per strin) were nlysed qulittively in the AFM. Crystl surfce roughness (R) nd width were mesured. The men ( 8SD) R of the crystls of A/J strin ( nm) ws lower thn the one found for the 129P3/J strin ( nm) but the difference did not rech sttisticl significnce (t = 1.187, p = 0.247). Crystl width of the 129P3/J strin ( nm) ws found to be significntly smller (t = 4.013, p = ) thn the sme prmeter mesured for the A/J strin ( nm). The smller width of enmel crystls observed for the 129P3/J strin might help to explin the resistnce of this strin to the development of dentl fluorosis. Forty-seven nd fifty-nine 2-to-4-yer-old children living in fluoridted nd in non-fluoridted community, respectively, were rndomly llocted into 3 groups in ech community, ccording to the dentifrice they hd been using for six months: A, liquid dentifrice (LD), 550 ppm F, ph 4.5 (n = 15, non-fluridted/20, fluoridted); B, LD, 550 ppm F, ph 7.0 (n = 16/16); C, LD, 1,100 ppm F, ph 7.0 (n = 16/23). The LDs were loded onto the toothbrush using the drop technique. Plque smples were collected 5 nd 60 minutes fter the lst use of the dentifrices. Plque [F] ws nlyzed with the electrode, fter HMDS-fcilitted diffusion. Dt were tested by 2-wy repeted-mesures ANOVA nd Bonferroni s tests (p! 0.05). Men ( 8 SE, mmol/kg dry weight) plque [F]s were , , for A, B nd C, respectively, 5 min fter the dentifrices use for the fluoridted community. Corresponding vlues for the non-fluoridted community were , , nd , respectively. Sixty minutes fter toothbrushing, corresponding figures were , nd for the fluoridted community nd , nd , for the non-fluoridted community. For the fluoridted community, C ws significntly different from B regrdless the time, while A did not significntly differ from B nd C. For the non-fluoridted community, the differences mong the dentifrices or times did not rech sttisticl significnce. The results suggest tht the reduction of dentifrice ph tends to increse plque F uptke. Additionlly, in the nonfluoridted community, plque F levels did not decrese with time, suggesting tht F from dentifrice occupied more stble binding sites tht were not previously filled by F originted from the diet. Therefore, we evluted if chemiclly totl soluble fluoride (TSF) in toothpstes would be better indictor of biovilble F thn TF. One NF/ silic-bsed (1,100 ppm F) nd one MFP/CCO 3 -bsed (1,450 ppm F) toothpste were tested, either fresh or ged, simulting the decrese of TSF tht occurs with the ltter fter one yer of mnufcturing. In blind, crossover study, 20 dults volunteers ingested both toothpstes in n mount sufficient to provide 49.5 g of TF/kg body weight. Sliv collected for up to 3 h fter ingestion nd 24-h urine were used to indicte F biovilbility. Spermn correltion between the re under the curve of slivry F concentrtion versus time (AUC) nd mount of TF nd TSF ingested by volunteers ws determined. Also, the percentge of F excreted in urine ws clculted in reltion to TF or TSF ingested. TF concentrtions in the fresh nd ged toothpstes were in ccordnce to the expected vlues. All F in the NF/silic formultion, either fresh or ged, ws soluble, but in the MFP/CCO 3 formultion TSF concentrtions were 1, nd , respectively. Dt showed significnt correltion (r = 0.57, p! 0.001, Spermn) between TSF ingested nd AUC but not for TF (r = 0.14, p = 0.20). The expected percentge of F excretion, round 40 50% considering the studied popultion, ws only chieved when TSF ingested ws tken into ccount. In conclusion, the TSF frction nd not TF in toothpstes should be considered in the estimtion of potentil fluorosis risk. Since the cpcity to mintin incresed fluoride levels in the orl environment is n indictor of the nticries effect of fluoride toothpstes, the im of this study ws to evlute the vilbility of fluoride in biofilm fluid from these formultions used by children. 4 pstes: non-fluoride (negtive control), 500 g F/g or 1,100 g F/g toothpstes (NF/silic). On the lst 3 dys, they were instructed to brush only the occlusl surfces, to llow biofilm ccumultion. On the 7th dy, dentl biofilms were collected from buccl surfces of posterior teeth before nd 30 min fter toothbrushing. The biofilm fluid ws seprted from the solids nd fluoride concentrtion ws determined using n inverted ion-specific electrode. Before toothbrushing, no sttisticl difference in fluoride concentrtion in biofilm fluid ws observed mong tretment groups. Although significntly higher fluoride concentrtions were observed in the fluid 30 min fter brushing with both fluoride toothpstes when compred to the bseline vlues (p! 0.05), fluoride concentrtion ( M, men 8 SD, n = 24) in dentl biofilm fluid fter brushing ws significntly different from the control group ( ) only for the 1100 g F/g toothpste ( ), but not for the low-fluoride toothpste ( ). The findings my suggest rtionle for the conclusions of the systemtic review tht toothpste contining t lest 1,000 g F/g hs greter nticries effect thn those contining g F/g. In the present study, fluoride excretion in urine of children living in two communities with distinct levels of fluoride concentrtion in drinking wter ws determined s n indictor of fluoride biovilbility. Twenty-four h urine nd ll diet ingested by 51schoolchildren, ged two to five yers, living in communities with (n = 27) nd without (n = 24) fluoridted wter ( nd ppm F, respectively) were collected for determintion of fluoride concentrtion. Fluoride ws extrcted from dietry products (liquids nd solids) by HMDS-fcilitted diffusion nd its concentrtion wsdetermined using n ion-specific electrode s n indictor of totl fluoride ingested. Fluoride concentrtion in urine ws determined directly using the electrode, fter pproprite buffering with TISAB II. Dily mount of fluoride intke from diet ws nd g F/dy, for communities with nd without fluoridted wter, respectively (p! ). Fluoride excreted in urine ( g F/dy) ws nd , respectively (p = 0.01). The fluoride intke from diet ws 93% lower in the non-fluoridted community but the difference in fluoride excreted ws only 33% smller. Although the results show tht diet hs n importnt contribution to the totl mount of fluoride ingested dily by young children, the findings of urinry fluoride excretion suggest tht only prt of fluoride is biovilble to contribute with dentl fluorosis. Vrnishes remin on tooth surfces for 24-hours or more nd ct s slow-relesing reservoir of fluoride. Aim: The im of this study ws to evlute the rte of fluoride relese from three vrnishes: A = 3M TM ESPE TM Clinpro TM WhiteVrnish, B = Premier TM EnmelPro TM Vrnish, C = Colgte TM Durpht TM Vrnish, more thn 3 hours fter ppliction. Experimentl Approch: Vrnish ( g) ws coted on plstic slide (n = 3) over 2-inches2 nd submerged into n gitted TISAB/wter solution contining n ion-specific fluoride electrode. The fluoride in the solution ws mesured every 10 seconds for 15 minutes, nd therefter every minute using Mettler Titrtor. MilliVolts were converted to ppm using stndrdized curve, nd percent fluoride relesed ws clculted bsed on product lbel (5%NF, 22, ppm) nd initil smple weight. Fluoride relesed is reported in percent of totl fluoride content. Results: More fluoride ws relesed in the first minute thn in ny other time intervl: A = 0.27%, B = 8.62%, C = 8.41%. After 3 h, ll vrnishes relesed fluoride ccording to liner model, slope(r 2 ): A = 3.0%/h (0.993), B = 1.1%/h (0.955), C = 0.3%/h (0.995). At 24-hours, most of the contined fluoride ws relesed from vrnishes A nd B, but not C. (A = 83.32%, B = 71.82%, C = 20.22%). Conclusions: This model ssumes ll of the vrnish remins on the tooth surfce. After the 3rd hour, Vrnish A nd B continue to deliver fluoride, vrnish A delivers 11.5% more. In the mouth where sliv continuously bths the vrnish, the bility of vrnish to continuously relese fluoride is beneficil. The vrnish tht relesed low levels of fluoride initilly, exhibited the highest sustined rte of fluoride relese for the long term. This is likely relted to the vrnish s unique prolonged setting. In the present study, the nticries potentil of products formed nd retined on enmel fter APF gel (DFL, 1.23% F, ph 3.5) or FV (Durpht, 2.26% F) ppliction ws evluted using highly-controlled in situ model. 5 pplied on enmel blocks ccording to the clinicl recommendtions: APF gel ws pplied for 4 min nd FV ws kept pplied for 24 h before being removed with cetone. Hlf of the blocks were immersed in rtificil sliv for 7 dys to simulte the dissolution of CF 2 -like products ( CF 2 ) tht occurs in vivo. This procedure cused 96% reduction of CF 2 on enmel treted with APF ( to g F/cm2), wheres for the FV the reduction ws 79% ( to g F/cm2). In double-blind, crossover, short-term in situ design, 12 volunteers wore pltl pplinces contining blocks freshly-treted or exposed to sliv, kept in contct with S. mutns test plque nd cid chllenged by sucrose rinse. Untreted blocks were used s control. CF 2 products formed nd retined on enmel by both gels were ble to reduce minerl loss when compred to the control (p! 0.05). Although no significnt difference on enmel deminerliztion mong blocks freshly-treted with both F products ws observed (p ), fter ging the FV-treted blocks presented lower minerl loss thn the APF-treted blocks (p! 0.05). The results suggest tht the nticries effect of slow-recting topicl fluorides is mintined for longer periods of time. Food nd beverges hve been identified s significnt sources of fluoride intke for children. The current study ssessed fluoride intke from infnt foods nd bottled wter for one yer old children in the US. FDA totl diet study informtion ws used to determine most frequently consumed mounts nd types of infnt foods nd wter used to reconstitute formul. Fluoride content ws mesured directly (for wter) or using modifiction of the HMDS microdiffusion method (for foods). Men nd stndrd devitions were clculted. Rnges of fluoride intke for different dietry combintions bsed on the typicl US diet nd 50th percentile weight for one yer olds were lso clculted. 458 wter smples from 20 brnds nd 360 infnt food smples from three brnds were collected nd nlyzed for fluoride content. Fruit-bsed foods hd men g F/g; vegetble-bsed hd g F/g; non-chicken bsed hd g F/g; turkey dinners hd g F/g; nd chicken-bsed hd g F/g. Chicken-bsed products hd sttisticlly significnt higher men fluoride concentrtion. For wter smples, fluoride concentrtion rnged from g/ml to g/ml. Vribility between production lots ws not sttisticlly significntly different for foods or wter. Clculted fluoride intke from infnt foods nd bottled wter used to reconstitute formul rnged from 0.07 mgf/kg for dietry combintions using wter nd foods with the highest fluoride content to mgf/kg for clcultions using the lowest fluoride content foods nd wter. Significnt vrition in fluoride intke ws observed with some intke clcultions being higher thn the recommended optiml fluoride intke. For ech tretment the volunteers used pltl pplinces contining 6 fixed sterilized bovine enmel smples during dy nd night except mels. For erosive deminerliztion the mouth pplinces were immersed extrorlly in 80 ml of 0.05 M citric cid (ph 2.3) for 20 min twice dily. Individul orl hygiene ws performed with fluoride-free products nd without the pplince in situ. Before nd between the tretment periods, 1 week wsh-out period ws included. Two smples were collected from the pplinces for nlysis on dys 1, 3 nd 5 (n = 20/dy/tretment). Acid-extrcted fluoride ws nlyzed using Orion ion-selective electrode nd Orion EA-940 ion nlyzer. Mesurements were performed in triplicte, nd the fluoride concentrtion in the enmel ws determined in g F/cm2. Dt were nlyzed by mens of repeted mesure ANOVA nd post-hoc comprisons (two-sided nd t 5% significnce level). On dy 1 both groups FL ( ) nd L ( ) presented sttisticlly higher fluoride mens thn the control ( ), p! On dy 3 only F group ( ) presented significntly higher mens thn control ( , p = ), nd on dy 5 none of them (p ). CO 2 lser irrdition t 0.3 J/cm 2 combined with previous fluoride ppliction significntly incresed fluoride retention s compred to non-treted nd solely lser-treted enmel up to one dy in n in situ erosion model. Sixty bovine enmel blocks were rndomly divided into 5 groups (n = 12): RC-regulr col, RCpH ddition of bse to increse regulr col ph, RCAS ddition of sprtme to regulr col, LC-light col, nd LCpH ddition of cid to decrese light col ph. Two-thirds of the blocks surfce ws coted with nil vrnish for reference. The smples were subjected to erosive chllenges (30 ml/smple) for 2 min, 4! / dy. Between the erosive chllenges (2h) nd overnight the smples were immersed in rtificil sliv (30 ml/smple). After 1st experimentl dy, surfce hrdness test ws performed in order to provide the percentge surfce hrdness chnge (%SHC). At the 5th dy, enmel surfce loss ws mesured by profilometry ( m). Dt were tested using ANOVA nd Tukey s test (p! 0.05). Independently of the col modifictions, ll tretments promoted similr hrdness chnge of enmel surfce. RC promoted higher enmel loss ( m) thn LC ( m). The cid ddition to the light col (LCpH: m) significntly enhnced its erosive potentil, which ws comprble to RC. The bse ddition to regulr col (RCpH: m) resulted in similr wer to ER nd EL, respectively. The ddition of sprtme to the regulr col (ERA m) resulted in wer similr to LC, but lso similr to RC. In conclusion, the present in vitro study showed tht the ph modifiction of col drink interferes with the erosive potentil of prolonged enmel erosion, but not with initil erosion. Four different mterils were tested: Vertise TM Flow (Kerr), Fusio TM Liquid Dentin (Pentron Clinicl), Sel & Protect (Dentsply DeTrey) nd X-flow using Xeno V s dhesive (Dentsply DeTrey). The dentin surfce of ech specimen ws covered with lyer of the respective mteril (dimeter 5 mm, thickness: 1 mm). Then hlf of the surfce of ech specimen ws covered with tpe nd served s reference. Subsequently, the specimens were exposed to toothbrushing brsion representing six-month clinicl sitution [8,900 movements, lod: 2N, in solution of humn sliv mixed with Elmex Sensitive Professionl TM (GABA) (3: 1)]. Surfce roughness nd substnce loss were mesured using lser profilometry ( m). The smples covered with Sel & Protect did not survive toothbrushing. The X-Flow smples showed higher surfce roughness compred to the other two flowbles. Among the three composite mterils, X-flow showed the lest substnce loss ( m) followed by Vertise Flow ( m) nd Fusio Liquid Dentin ( m). Tukey s Test showed no significnt difference (p ) mong the three tested mterils with respect to substnce loss. The two tested self-dhesive flowbles showed similr performnce compred to the conventionl flowble composite. Ech morning the specimens were brushed ex situ with tooth brushes nd wter for thirty seconds nd the fluoride solutions (0.4% SnF 2 ph 2.5, 0.15% TiF 4 ph 2.1, 0.2% NF ph 6.5, ll 0.05 MF)) were pplied by pipette (one drop per second) for two minutes. Mterils nd Methods: 16 humn molrs were ech divided into four specimens, mounted on crylic mouth pplinces nd worn by 8 volunteers for 9 dys. The specimens were brushed every dy with fluoride-free toothpste. Tretments; group 1: no tretment (control), group 2: SnF 2 gel 2,500 ppm (5 min) every third dy, group 3: NF toothpste 5,000 ppm 5 min every third dy nd 2 min the other dys, group 4: NF toothpste 1,450 ppm (2 min) dily. In order to mimic gstric reflux, the specimens were etched for 2 min twice dy (0.01 M HCl). Results. The men etch depth in m for the control specimens were 32.9 SD 6.8. The men vlues for the other groups were 22.2 SD 8.4 (group 2), 30.8 SD 7.8 (group 3) nd 31.4 SD 7.7 (group 4). Compred with the control, the SnF2 treted enmel specimens showed significntly lower etch depths. The NF toothpstes (5,000 nd 1,450 ppm F) gve no significnt protective effect, p = 0.2 nd p = 0.4, respectively. The response rte ws 62% (n = 783, men 43 yers, 65% femle). Nerly ll dentists registered erosive lesions, 51% used specific scoring system: 31% two-grded system (enmel-dentine) nd 14% more detiled system. Erosive lesions were registered on the surfce-level by 54% of respondents nd on the tooth-level by 14%, while 26% registered on the ptient-level. Lesions were reported locted most often on the pltl surfces in upper nterior teeth (79%), nd occlusl surfces of first mndibulr molrs (74%) or first mxillry molrs (32%). Hlf the dentists used clinicl photogrphs for documenttion, 40% never used photos, 60% mde study models nd 34% never mde models. While no gender differences were reported by 36% of dentists, 40% hd the impression tht there were more erosive lesions in mles. Most respondents (77%) usully found probble cuse: crbonted beverges (97%), cidic juices (72%), citric fruits (46%), sports drinks (24%), cidic diet (20%) nd reflux/eting disorders with vomiting (8%). Only 21% of the dentists recorded dietry history, nd 73% reported tht they never mesured sliv secretion in ptients with erosion. The findings should be relted to behviourl dt. 1,580 schoolchildren ged 6 to 15 yers were exmined for dentl erosions by two clibrted investigtors. Erosions were recorded nd rted on scle of 1 to 3, using the index proposed by Lussi. Dt bout orl hygiene nd diet were collected using questionnires for prents nd children. Sttisticl evlution included prevlence, men number of ffected teeth nd the distribution of scores. The prevlence of erosions ws 17.8% nd strongly relted to the childrens ge with rnges from 5.0% in 6-yer-olds to 29.9% in 15-yer-olds. Boys (19.7%) showed higher prevlence thn girls (15.9%, p = 0.051, 2-test). The men number of ffected teeth ws in ll children but in children with erosions which indictes polriztion of the erosion distribution. In totl, 2488 tooth surfces exhibited erosions. To the bsic toothpste formultion (3,500 ppm Sn, 1,400 ppm F), with nd without Chitosn, vrying mounts (wt%) of brsives were dded (5, 10, 15, 20% silic or 20% polyethylene, rnge of RDA: 56 81). One formultion contined ctive ingredients only, but no brsives. Plcebo toothpste contined 20% silic, but no ctive ingredients. Tin-uptke (wt%) ws mesured by Energy-Dispersive-X-ry-spectroscopy on surfces. All specimens treted with Chitosn formultions showed significntly higher tin-uptkes (rnge in slurry groups: ; rnge in slurry/brushing groups: ), thn specimens treted without Chitosn (rnge in slurry groups , rnge in slurry/brushing groups: ). Within the tretment time with the slurries, the specimens on one side were introrlly brushed with powered toothbrush for five seconds (2.5 N). Pilot study (n = 10): Four toothpstes were tested (plcebo, two experimentl toothpstes (TP1: AmF/NF/SnCl 2, TP2: AmF/NF/SnCl 2 plus Chitosn; both 1,400 ppm F, 3,500 ppm Sn), nd SnF 2 -contining gel (3,000 ppm Sn, 1,000 ppm F)). In the min study (n = 27) experimentl TP2 ws tested ginst plcebo nd n experimentl NFtoothpste (TP3, 1,400 ppm F). Zero iupui.edu Indin University School of Dentistry, Indinpolis, Ind., USA Stnnous-contining products hve shown n bility to reduce the development of dentl erosion; however, their interction with slivry pellicle proteins is uncler nd ws therefore investigted. Ninety-six bovine enmel nd root dentin specimens (n = 8) were submitted to episodes of deminerliztion (0.3% citric cid, ph 2.6, 5 min, 4! /dy), reminerliztion (in between tretments) nd toothbrushing (2! /dy), for totl of 5 dys. The reminerlizing solutions (S1-S5) contined 1.45 m M C, 5.4 m M PO4, 0.1 M Tris buffer nd sodium zide (S1), with the ddition of 2.2 g/l of Mucin (S2), 2.2 g/l of Csein (S3), 1.1 g/l of Mucin g/l of Csein (S4) or 2.2 g/l of Albumin (S5). Humn sliv (HS) ws included s reference. Toothbrushing (2 min, 45 stk, 150 g) ws performed using slurries of 1,100 ppm F (NF) silic-bsed toothpste suspended in deionized wter (1: 3 w/w), with (TP+Sn) nd without (TP) the ddition of 3,500 ppm Sn (SnCl2). The ph of both slurries ws djusted to 4.5. Surfce loss ws determined using opticl profilometry. Dt were nlyzed by ANOVA nd Fishers LSD tests, t 5% significnce level. When ssocited to TP, HS showed the highest loss, followed by S2. The other protein-contining solutions (S3 S5) did not differ from ech other or from S1. Similr results were observed for dentin. In the presence of TP+Sn, significnt surfce loss reduction ws observed for both substrtes. HS still showed the highest dentl loss, lthough no differences were found mong groups S1 S4. S5 resulted in the lowest enmel loss, not differing only from S2. Similr trend ws observed for S5 on dentin, lthough not s pronounced. Conclusion: The stnnous contining slurry provided strong protective effect ginst dentl erosion-brsion, which seemed to be enhnced by its ssocition to lbumin (S5), especilly on enmel.
