8 08 Dentist Section Professor Hom-Lay Wang DDS, MSD, PhD Dr. Hom-Lay Wang, Collegiate Professor of Periodontics, Professor and Director of Graduate Periodontics at the University of Michigan. Dr. Wang received his DDS from Taipei Medical College, Taipei, Taiwan, DDS, MSD from Case Western Reserve University, Cleveland, Ohio and Ph D from Hiroshima University, Japan. He published more than 22 book chapters/invited reviews and more than 300 scientific articles. Dr. Wang is a Diplomate and a Former Co -Chair and Director of the American Board of Periodontology and is the Diplomate and Former President and Board Director of the ICOI. He serves as an Associate Editor for The International Journal of Oral & Maxillofacial Implants, ejournal of Oral & Maxillofacial Research, and International Chinese Journal of Dentistry, and Founding Editorial board member for newly introduced Clinical Advances in Periodontics Editorial and many others. Dr. Wang is the recipient of following awards/honors : AADS clinical research fellowship award (1992), Best papers in Journal of Oral Implantology (2004) and Implant Dentistry (2004), Morton L. Perel Annual Award for Dental Implant Educator (2007), and AAP special citation award (2013). Management of Implant Complications: Classification, Prevention and Management This lecture addresses common implant complications, both biologic and biomechanical aspect as well as the approaches to avoid these problems before they occur. A decision tree of how to manage these complications will be presented. The pros and cons of techniques used to treat implant diseases/complications such as chemotherapeutic agents, apically positioned flap, implant surface detoxification, guided bone regeneration, soft tissue grafts, implant removal as well as re-implantation will be discussed. 9 Dentist Section 09 Dr. Young-Ku Heo DDS, MSD, PhD Dr. Heo received his DDS from DanKook University Dental School, Korea, completed his advanced graduate study in prosthodontics, and received his degree of Master of Science at the Boston University, School of Dental Medicine. He was a full-time Professor at Catholic University, Medical School, Department of Dentistry, Seoul Korea, and received his Ph. D. in Medical Science/Dentistry, Catholic University. He was an Adjunct Assistant Clinical Professor at Center for Implantology, Boston University, and Adjunct Associate Clinical Professor at DanKook University. Dr. Heo has lectured intensively on dental implants worldwide and has developed numerous innovative devices such as sinus crestal and lateral approach kits, fixture remover, screw remover, customized titanium membrane, autogenous chip bone maker, tools for treating implant complications, etc. He maintains a private practice limited to prosthodontics, implantology and esthetic dentistry in Seoul Korea. He is also the President & CEO of Neobiotech Co., Ltd. What are The Next Neo Solutions for Stress-Free Implant Dentistry? The wave of digital technology in dentistry is drastically increasing today. Custom abutment and full zirconia Cr. & Bridge have become popular in dental implants and digital impressions taken by the intraoral scanner facilitated immediate loading. Basic principle of fabrication of precision computerized navigation surgical stent is using CT data and virtual model via either model scan or intraoral scan. Navigation surgery helps dentist to place implants where exactly he or she wants with desired angulation and depth. At the same time we understand more about loading protocol like immediate, early or anytime loading. From the CT data and virtual model, we can fabricate precision navigation surgical stent and prosthesis at the same time through CAD/CAM technology with many other materials. In this oral presentation, I d like to show cases which I treated with Precision guide system and CAD/CAM restoration and future possibility.

The quantity of keratinized gingiva for primary or secondary surgery is determined mostly by the bucco-lingual position of horizontal incision. Also, papilla involved or preserved incision, which often is replaced by long crevicular incision, may be determined by the intersection of vertical incision. Furthermore, the amount of keratinized gingiva would be changed by the size of the healing screw in non-submerged surgery. GBR case often lacks flap coverage and has the risk of postoperative side effects as a result of soft tissue dehiscence, thus requiring overall soft tissue control for flap design and strategic suture technique. In Mandibular posterior area, most flap coverage can be managed through mandibular lingual surgical flaps using a dull Prichard, Seldin, or periosteal elevator for Pikos finger dissection of mylohyoid m. up to mylohyoid ridge along with buccal releasing incision within buccal mucosa. However, high degree of caution is necessary when releasing the mental n. branch area. Since the posterior mandibular gingiva is thinner than those of the maxillary area, a horizontal mattress suture and simple interrupted suture with a relatively light tensional force may be enough. Maxillary posterior area, on the other hand, is nearly incapable of advancement of the palatal side that flap coverage should be obtained through the extensive dissection and releasing of the buccal gingiva, and often times require the removal of torus and thinning of the connective tissue in the palatal side. A strong tensional force by vertical mattress suture and simple interrupted suture is required in these thick flap area. In the maxillary anterior area, excessive releasing incision is esthetically unpleasing, and vertical incision should take the form of the scar tissue that horizontal extension of the flap through long crevicular incision is advantageous with performing vertical incision around the canine area. Connective tissue or free gingival graft is helpful when the maxillary anterior area does not have enough flap when operating GBR. I will discuss the precautions, techniques, and clinical cases based on different areas in this lecture.

As the health of the patient is always of paramount priority the ultimate goal of success is directly linked to the degree of safety measures which are in place prior, during and following the operation. To ensure this safety it is often best for the practitioner to focus on the simplicity of application when preforming the procedure. This simplicity in turn helps to create a method which can be implemented with a significant amount of ease thus reducing unnecessary or additional steps for the dentist while simultaneously decreasing the amount of inconvenience for the patient. In doing so it shortens the overall operation time, contributes to a higher success rate of the sinus graft procedures and minimizes a number of pre-operational complications that often arise. In this lecture, we will look into theoretical considerations and simple clinical approaches in order to assist practitioners in confidently conducting sinus grafts with the greatest possible ease. The topics to be covered will be The Surgical and Biomechanical Factors, Simple and Effective Treatment Planning, Anatomical Structure Around the Maxillary Sinus, Healing Procedures, and Timing. Within these main topics we will also explore graft materials which have recently been shown to be very effective in maxillary sinus operations, such as PRF. In addition we will examine the effectiveness and efficiency of instruments used in the procedure such as the Sinus Crestal Approach Kit and the Sinus Lateral Approach Kit. Along with this we will spend time focusing on several methods that can be applied to assist in decreasing potential complications that may arise throughout the process as well as introducing safeguards to aid in any unpredictable circumstances that have the potential to effect to procedure. The overall objective of this lecture is to promote methods and practices which will help both the practitioner and the patient undergo the entire sinus graft process as simply, safely and easily as possible.

In addition, the spread of PACS (Picture Archiving Communication System) standardized CT scan s filename extensions and created the DICOM (Digital Image Communication in Medicine) standard, which facilitated further development of other software programs allowing 3D imaging analysis and diagnosis in the medical world. Among medicine, dentistry is considered highly digitized and some great examples are the manufacturing of restorations using optical scanner, touch probe scanner, or CAD/CAM technology, as well as the implant planning through CT image and the guided surgery. While CT image reconstruction to 2D-3D have been used for diagnostic purpose in implant dentistry field for quite a bit, it was not until the 21st century that the guided implant surgery, which reflected preoperative diagnosis and treatment planning, actually allowed the implant surgery to be performed. Fontin and others defined the concept of Computer-Guided Surgery (CGS) in order to emphasize the importance of accurately delivering the prior treatment planning to the actual implant placement. This lecture will analyze different types of guided systems, recognize their limitations, and finally introduce the NeoGuide through various clinical reports.

Today, sinus lift operation has become one of the common practices. Various drills and devices for lifting sinus without damage(perforation) of the sinus membrane via alveolar crestal approach have been developed as well as instruments for more advanced procedure such as sinus lateral approach. This lecture will discuss methods for diagnosis, prognosis and to prevent ENT problems. Clinical cases of sinus crestal and lateral approach using Neobiotech s new product the SinusAll Kit will be introduced. Furthermore, methods for creating an optimal position of window to the sinus lateral wall, prediction and preparing a reasonable quantity of bone graft materials prior to operation, and managing acute postoperative maxillary sinusitis following sinus lift will be presented.

In comparison to the conventional implant surgery that relied on panoramic radiography, today both practitioners and patients benefit from the sufficient preoperational diagnosis and information available that leads to more predictability in implant surgery. The generally known concept of CT-guided implant surgery does not necessarily require the manufacture of surgical guides. Depending on the accuracy of data reproduction, surgical procedures are classified differently. Even without the exclusive 3D software, the generally used 2D image of CBCT allows much more accurate image compare to those from panoramic radiography. When the CT image is made three-dimensional through the 3D software, a clearer anatomical structure is reproduced. Knowledge of the anatomy and potential situation of the surgery site without flap opening and positioning of implants exactly on the planned area can minimize surgical trauma. In cases without GBR, or if minor GBR can be performed through minimal flap opening, prediction of the surgical recovery and outcome is possible. In addition, the contour of the customized abutment can be used in manufacturing not only the temporary prosthesis, but also the final prosthesis. The ultimate goal of the surgical guide is to perform minimally invasive surgery and successfully predict the surgical outcome.

The known cause of biologic complications is the bacterial infection inside the biofilm and they share similar aspects of periodontal diseases. Consequently, treatment methods for implant complications are similar to those of periodontal diseases. In fact, the treatment modalities as well as the treatment strategies for implant complications so far are not much different from the management of periodontal diseases. The critical factor for treating periodontal diseases is not the tissue regeneration, but the stop of spread of defects through controlling inflammation. The periodontal treatment objective is to remove the causative factors in the short term and to reduce the depth of periodontal pocket for plaque control in the long term. The two types of per-implant diseases are the peri-implant mucositis and the peri-implantitis. Obviously, treating peri-mucositis is much more effective than dealing with peri-implantitis associated bone loss. Appropriate management of peri-implant mucositis through its reversible process can possibly restore the site to its original condition and stop its progression into peri-implantitis. Therefore, detecting peri-implant diseases in its early stages and preventing aggravation are recommended. If the area progressed into the stage of peri-implantitis and peri-implantitis-associated bone loss has already started, surgical approach is recommended.

Clinical performance of Zirconia Prosthesis With the increasing interest in esthetics, zirconia, a high-strength ceramic, extended its range of clinical application to the posterior region and, furthermore, to implant abutments (Koutayas et al. (2009) Zirconia in Dentistry : Part 2. Evidence-based clinical breakthrough). The reliability of zirconia framework seems verifiable through inferring couple of 5-year clinical data and one 10-year clinical follow-up study (Sax et al. (2011) 10-year clinical outcomes of fixed dental prosthesis with zirconia frameworks). 2. Background of Full Zirconia Nonetheless, zirconia prosthesis showed higher percentage of veneer chipping than PFM, and the main concern in the field now focused on finding a solution to this complication, which yet remains as an on-going problem (Al-Amled et al. (2010). Review article Clinical trial in zirconia). Thus, full zirconia crown or monolithic zirconia crown without veneer may be an alternative to veneered zirconia crown. Full zirconia prosthesis is more convenient for clinical use and became popular and its application in implant prosthesis became a growing trend today. However it is necessary to think over the following two aspects of zirconia. 1) Effect of Zirconia Surface treatment on Mechanical properties and Low Temperature Degradation 2) Attrition of Opposing Teeth 3. Zirconia Abutment 1) Optical Properties (Esthetics) 2) Biological Properties (Biocompatibility) 3) Clinical Applications

If the cases are located in the esthetic zone, more precise level of surgical & restorative approaches are demanding to reach the optimal goal of implant dentistry. This presentation, a collaborative effort by a concept of interdisciplinary approaches, offers clinical guidelines as well as practical strategies for treating common types of failure especially about soft and hard tissue management in anterior implant cases. Emphasizing the need for diligent planning, precise techniques, and complete communication among all specialists of the implant procedure, I would like to advocate a sequential treatment approach and to show summarized data from the literatures to enhance patient comfort and function and to improve treatment predictability. I would also like to share strategies I have learned through experiences for reducing surgical complications in the esthetic zone. Detailed clinical cases demonstrate the most common types of failure and the steps taken to restore function and esthetics. All members of the implant procedure and clinicians who have faced more complicated cases than average will benefit from the suggested approach presented in this presentation. Lecture Contents 1. The effort to find the way that can facilitate the maintenance and trouble shooting, as well as the fabrication of implant prostheses has never ceased. As a result more dentists are expanding the procedural options which may be available to their patients. However, one of the most significant aspects to consider in determining which procedure would be in the best interest of a successful implant is the amount and quality of alveolar bone, has often been overlooked in regard to its importance. Along with this factor, is the criteria used to determine the potential benefits and shortcomings which may result from the appropriate timing or mistiming if extraction is deemed necessary. In cases where implant treatment is a viable option it is often in the best interest to opt for a quick extraction to prevent any possibility of further damage while simultaneously creating a more favorable environment. Following extraction and in order to create the enhancement of alveolar bone quality and quantity, socket preservation treatment is a recommended procedure which should be implemented in most traditional situations. The focus of this presentation will be addressing these issues and will be divided into the following three sections; methods and the prevention of reduction in the amount of lateral and vertical augmentation, discussion of the necessary materials needed to ensure success of the treatment and the exploration of the clinical effects of the treatment. The overall objective is to present the benefits which socket preservation treatment has to offer for both patient and practitioner when implemented correctly.
