The predictability and longevity of dental implant are the most important part of treatment because it is installed inside of the body.

Hundreds of dental implant system have been developed and millions of dental implants have been installed in the jaw bone without any evidence.

Short-term follow-up results are not valuable because of the final destination of the implants is not known with short term recall check.

The predictability and longevity of dental implant are the most important part of treatment because it is installed inside of the body. It is natural for patient to use and maintain the dental implant until the end their life. Hundreds of dental implant system have been developed and millions of dental implants have been installed in the jaw bone without any evidence. There are thousands of articles about the advantages of each implants with very short-term follow-up such as 1 or 3 years follow-up. Short-term follow-up results are not valuable because of the final destination of the implants is not known with short term recall check. We live in the age of a 100 years old ages era which means a lot of people live longer than 100 years old. Implants in the healthy, good quality and wide bone showed good prognosis, however, implants in the poor quality with bone defects presented with poor prognosis. Repeated operations result in wound breakdown, infection, osteomyelitis, oro-antral fistula, numbness, peri-implantits, odontogenic sinusitis and pain. How can we avoid these complications? There are several factors that have an effect on the prognosis of the implants in compromised area. First factor is systemic and medical conditions of the patients. There are several diseases and medications that have adverse effect on the quality of bone such as osteoporosis, chemotherapy and bisphosphonates. Second is preconditioning of the jaw bone. It is mandatory to prepare good quality bone before implant installation. If the bone quality and quantity are not good enough to place implants, bone graft should be performed before implant installation. Third is skillful surgical techniques. Implant surgeon must be accustomed to the variety of surgical technique such as guided bone regeneration, block bone graft, sinus lifting procedure and osteotome technique. Quick and atraumatic surgery result in good prognosis. In this presentation, I would like to show the long-term follow-up of dental implants in the compromised area with guide bone regeneration, block bone graft, sinus lift and osteotome technique. And also I would like to present surgical management to overcome the complications after implants operations. Dr. Kang-Min Ahn (DDS, MSD, PhD) is Oral and Maxillofacial surgeon, Associate Professor of oral and maxillofacial surgery at Asan Medical Center, Medical College of Ulsan University. He has held positions of clinical faculty and visiting scholar in the University of Southern California (USC). His area of expertise includes Dental implant surgery, Oral cancer and microsurgical reconstruction, Oral pathology surgery, TMD and Maxillofacial trauma. He is an internationally acclaimed speaker, while he has published a signifi cant number of peer reviewed papers in the above fi elds.

In case of limited amount of bone, an extended incision, large flap and bone graft or sinus augmentation are required prior to implant placement. The most common complications associated with the standard approach include swelling, pain, flap breakdown and bone loss. Limited data concerning minimally invasive techniques have been described in the literature. Generally, such techniques involve a less-extensive incision with reduced diameter implants, or short implants and more recently, the use of computer-guided surgery for implant placement. This presentation will discuss the advantages and disadvantages of this minimally invasive implant procedure. 6 Dr. Fernando Rojas-Vizcaya, DDS, MS Adjunct Assistant Professor of Prosthodontics Department of Prosthodontics The University of North Carolina in Chapel Hill, NC Title: Integrating biology, surgery and prosthodontics to enhance esthetic and function in oral implantology; The 3A-2B biological rule For an implant restoration to be both esthetically and functionally successful, the prosthodontist must conduct a thorough treatment and complete a prosthesis design. The restoration and soft tissue are affected by the three-dimension (3D) position of the implant, as the implant s depth determines the ideal length of the crown. When determining the 3D position of the implant, the clinician must consider the biological aspects required to ensure the restoration s biological integration with the patient s hard and soft tissues. The restoration must be the first component considered in the treatment plan. In addition, the clinician must understand that the distance between the cervical contour (of the planned restoration) and the level of the bone will dictate how the surgical and prosthetic treatment plan is enacted. In this presentation, a novel 3A-2B Radiographic Biological Ruler (with biological information) will be presented and used in simple and complex cases, together with different anatomical stable points of reference to determine the ideal position of the crowns in relation with the face of the patient. Objectives 1. Understand the importance of the 3A-2B rule as a tool for implant planning and placement; 2. Integrate the concepts of biology, surgery and prosthodontics in the benefit of implant treatment protocols; 3. Be able to recognize when a bone reduction is required in order to obtain an adequate esthetic and functional restoration; and 4. Be able to use different stable anatomical points as reference to develop an adequate prosthetic treatment plan. Dr. Rojas-Vizcaya is Adjunct Assistant Professor in the Department of Prosthodontics at the University of North Carolina in Chapel Hill, NC. He is the Founder and Director of both; the Mediterranean Prosthodontic Institute and BoneModels, and maintains a private practice limited to dental implant surgery and prosthodontics in Castellon, Spain, and he lectures extensively worldwide.
