A full list of non-formulary test strips is no longer provided, due to product and Drug to product and Drug Tariff changes. Prescribers are reminded to ensure that any prescribed blood glucose test strip meets the Group 1 criteria as above and is compatible with any patient specific criteria as required. Diabetic Patients who should monitor blood glucose are: Type 1 s (see below if counting carbohydrates) Type 2 s on insulin Type 2 s on SU s or Glinides or are prone to hypoglycaemia Type 2 s not on SU (only if appropriate to test) Formulary Testing Strips Group 1 - First choice formulary blood glucose testing strips Group 2 - Second choice blood glucose testing strips (less preferred due to absence of independently published evidence of ISO compliance) Expiry: May 2019
