By being active with us, you are supporting your local community. Welcome to the 3rd quarterly report for 2015/16 for the Active Lifestyles Team. The aim of this quarterly report is to highlight the work that has taken place during October, November and December within the Active Lifestyles Team and demonstrate how Salford Community Leisure is performing Quarter 3 (October - December 2015) has been a very successful quarter, some key highlights include: Members of the team attended UK Active conference in Leeds Healthy Hips & Hearts Christmas Carol concert held at Community Centre Healthy Hips & Hearts annual Christmas Tea Dance Older People s day at which the Team delivered a Movement to Music demonstration and answered questions regarding HH&H programme Refer all monitoring system installed and all members of team trained to use the system 2 new Step Up classes started with expansion of Postural Stability Project New member of staff recruited for Postural Stability project IGR Project expansion agreed to extend position to full time and increase referral target figure from 100 to 133 per annum Consistently high attendances at classes throughout this quarter Good client retention post 8 week pass Presentation and taster session at Moving Forward Course (Breast Cancer Care UK) recovery package for women Presentation and taster session at Cancer Health & Wellbeing Course at Salford Royal Presentation and taster session at Younger Women with Secondaries Together (Breast cancer Care UK) focussing on using exercise to help with treatment side effects and manage fatigue CAN- Move Christmas Party for clients attending the programme (As seen below)
2 Exercise on referral The exercise referral scheme is for people with long term health conditions. Each individual is offered a tailored menu of activities, which can range from chair based exercises, health walks, the Get Active programme, gym access at leisure centres or Heart classes. Referral numbers have increased this quarter. Clients who are referred are now issued with an 8 week pass which entitles them to use the gym, swimming pool and Active Lifestyles classes free of charge. Number of people referred: 184 Performance target this quarter: 250 Heart classes Heart is low level circuit class, designed specifically for people who have experienced cardiac problems such as a Heart Attack or who have been diagnosed with Angina or Cardio Vascular Disease. It is also suitable for those with Cardio Vascular Disease risk factors such as high blood pressure, cholesterol or Type 2 Diabetes. Number of weekly classes delivered across the city: 15 Number of attendances recorded at classes: 2425 this quarter Health walks Health Walks are available citywide in Salford; they are short doorstep walks no more than an hour in length which are led by volunteer Health Walk Leaders. They provide the opportunity for people to become more active and healthy in a social group within their local area. Number of weekly health walks: 8 Number of attendances in quarter: 2687 Number of active health walks leaders: 22 Gym inductions People with long term health conditions require programmes of exercise to be carefully prescribed. SCL have Level 3 GP referral Gym Instructors in a number of the fitness suites who can deal with mild to moderate health conditions. Also, the Active Lifestyles Development Officers are Level 4 qualified and specially trained to work with people who have complex / multiple health conditions. In many instances these inductions will result in the customer taking up a fitness membership. Gym Inductions carried out by level 3 and 4 instructors this quarter: 70 Pulmonary rehabilitation The Active Lifestyles Team has been commissioned in partnership with the SRFT Respiratory Team to deliver the exercise element of the Pulmonary Rehabilitation service for clients with lung conditions such as Chronic Obstructive Pulmonary Disease. The Pulmonary Rehabilitation Team is monitoring and reporting on this service. Number of classes (per week): 6 Average number of clients per session: 10 Healthy Hips and Hearts (HHH s) HHH s is a project for over 50 s in the city which aims to reduce social isolation and improve health and wellbeing. There are over 80 chair based exercise sessions established across the city in residential and sheltered housing run by staff in the sites who have received the HHH s training. There are also sole volunteers, who are members of the community wishing to enhance the lives of older people by delivering community sessions on SCL s behalf. The HHH s team provides mentor training and supports the mentors and sole volunteers in establishing new sessions where appropriate. Number of HHH s sessions delivered across the city: 27 community classes weekly, average of 225 people per week Number of events/ workshops hosted: 4 Tai Chi workshop with Anne Critchton, 9 attendances (Boothstown Community Centre, 12 th October; HHH Christmas Carol Concert, 30 attendances (Beesley Green, 3 rd
3 December), HHH Tea Dance, 70 attendances (Elmbank, 7 th December), Music for Memories (Barton Hub, 17 th December). Also helped in organisation of Bingo and Boogie Event at Eccles Town Hall, 40 attendances (21 st October), HHH Christmas Party, 12 attendances at Boothstown Community Centre (15 th December), Eccles Tea Dance Christmas Party, 75 attendances (16 th December, Eccles Town Hall). Postural Stability/ Step Up Postural Stability is a 12 week programme for those who are at risk of falls. People are referred to this service from the Falls Clinic at Salford Royal Hospital. There were 3 courses running across the city in Eccles, Worsley and East Salford during quarter 3. Number of people referred: 40 Number of attendances at sessions: 375 There were two additional Step Up classes added during quarter 3, so each area had both Postural Stability and a follow on Maintenance class. The 3 classes saw an average of approximately 14 clients each week. Park Run Park Run is an international network of free, timed 5 kilometre runs. The first park run in Salford was set up in March 2012 at Worsley Woods. This popular park run starts at the Monton end of the Loop line and participants complete the 5 kilometre course at their own speed. Average number of people at each run this quarter was 207 people. Number of park runs this quarter: 14 Number of attendances this quarter: 2898 CAN-Move Programme CAN-Move is an exercise referral programme specifically for Salford residents who have been diagnosed with and are under going treatment for Breast, Bowel or Prostate cancers. CAN-Move offers one to one support with a tailored exercise programme to improve your physical activity levels and overall wellbeing. The programme is a 12 week structured exercise programme which consists of 8 weeks of specialised, supervised exercise, including instructor led circuit based classes, supervised gym sessions and Tai Chi. Following the 8 weeks the clients receive a 1 month free Aspire membership. Clients also have the opportunity to try other activities such as Tai Chi, health walks. This quarter the Team presented at the Moving Forwards Course; Younger Women with Secondaries Together (Breast cancer Care UK), and the Cancer Health and Wellbeing Course at Salford Royal. Number of referrals received: 29 Number of attendances to classes: 382 Number of attendances to supervised gym sessions this quarter: 506 Impaired Glucose Regulation The Impaired Glucose Regulation project is designed for pre-diabetic patients in Salford who have been identified by a health professional to be at risk. The programme includes a half hour consultation and 2 month supported membership to access Salford Community Leisure sites. The Project has seen an increase in the number of people referred over quarter 3 and those now taking on the extra 8 weeks therefore keeping people active for longer. Number of people referred: 30 Performance target this quarter: 25
4 Inclusive Activities Salford Community Leisure s services and activities are influenced by the demographic profile of local communities. SCL strive to provide inclusive services and activities to enable everyone to be active in their chosen leisure pursuit. The Active Lifestyles team works with adults of all ages with long term health conditions and many clients with disabilities. Several Healthy Hips and Hearts sessions take place for specific groups including classes such as Women Working Together and Salford Link for Asian ladies, Warm Hut an African Ladies Group, Gan Eden for the Jewish community, and the session at Wai Yin Centre for the Chinese community. These predominantly chair based exercise classes aim to reduce social isolation and improve health and wellbeing and All of the groups thoroughly enjoy their sessions. Members of the Active Lifestyles team are involved in monthly meetings with the Salford Black and Minority Ethnicities Forum, Celebrating Diversity Group at Ordsall, and the Community Café Asylum & Refugee seekers Group at Emmanuel Church in Ordsall. The team is also involved in various festivals and celebration events through the year, some of which are described below: 1st October (Buile Hill Park) Older People 's Day Event 12th October (St Sebastian s) Celebrating Diversity and Enhancing Equality Event with the Over 55 s Group. 13th October (Pendleton Gateway) Older people s event and The World Mental Health Day event, organised by the Celebrating Diversity Group. 26th October (Ordsall Community Café) Organised Black History Month Event with Gail Skelly and Women Working together Group which was funded by Salix s Homes. In this event all different communities got together to
5 celebrate the day with singing dancing telling stories how they came to settle in Salford. The evening also included singing,dancing, food tasting, fun and games.

9 Additional Activity In additional to the core services described above, the Active Lifestyles team also supports other areas of work, which ultimately improve opportunities and increase participation in physical activity, particularly by older people, therefore encouraging a more healthy lifestyle. Examples of additional activity this quarter include; involvement in Salford Weight Management Group working around the obesity strategy, Ongoing involvement in the Being Well Service, involvement in Health and Wellbeing task Groups the Weight Management task group and involvement in a number of relevant key conferences and workshops including: Integrated Care for Older People Collaborative An ongoing project made up of seven subgroups which aim to reduce hospital admissions in older adults, enable people to maintain independence, to die in their preferred place, and to feel socially connected with better access to services. Walking for Health National Advisory Board Planning the future and direction of Walking for Health Physical Activity Leads Meeting (Greater Manchester) All PA leads from each borough discuss updates Case Studies Name of Participant: RC Name of Session: Heart Venue: Carendon Leisure Centre The patient was referred from his GP to the Active Lifestyles Team due to being overweight, developing Type 2 diabetes and currently not taking part in any form of exercise. He was very apprehensive at first and struggled to motivate himself. He was issued with an 8 week Active Lifestyle pass, which enabled him to attend the H@H Class at Clarendon, and then got into a twice weekly routine of exercise. He was paired up with another fairly new lady and they helped and supported each other and had a good chat throughout the session. This has really helped them both to continue attending the class, and have both started to enjoy the social aspects of the class. They both now stay for the refreshments post class, and even attended the Christmas Party. Name of Participant: GH Name of Session: Healthy Hips and Hearts Venue: Kemball House GH attends the Healthy Hips and Hearts class at Kemball House, a Sheltered Housing Complex in Eccles. The class takes place every Monday morning and is open to both residents and non residents. Lillian Worthington who runs the class doesn t live on the scheme but is very happy to go there every week to deliver the exercises. She has been running the class since 2011 and it has become very popular. Around six months ago a client by the same name was on one of his regular Health Walks and started to chat to a lady on the walk about the benefits of exercise. He told her about the class at Kemball House and how much they all enjoyed it. She said that her husband was looking to attend an exercise class offering some gentle exercise following an operation at The Christie, six months earlier. Consequently her husband, GH, started to attend the class and has found it to be very beneficial.
10 At the class GH reported that around 12 months ago he had an operation to remove a malignant melanoma from his inner forearm. A very large deep piece of tissue had been removed from quite close to his elbow and there was a very large scar running all the way to his wrist. The mobility in his wrist, fingers, and thumb, had been severely affected and there was a great deal of fluid retention at the wrist due to the lymph system being compromised. He described his fingers when he first started the class as looking like pork sausages! He could not do the opposition exercises at all. However, following the mobility and strengthening exercises for the wrist and fingers which Lillian includes each week in class, GH has seen great improvement. His Physiotherapist at The Christie is absolutely delighted, as is he! He said, I come here every Monday and do these exercises. My fingers and wrist will be fine now until next week when I come again. A big Thank You is extended to Lillian for leading the class, and to clients for spreading the word. Name of Participant: Mr S Name of Session: Can- Move Venue: Eccles Leisure Centre Mr. S was referred by the Urology clinic following his prostate cancer diagnosis. His medical history also indicated that he had recently had a cardiac operation and completed Cardiac rehab at the hospital. Working closely with Mr. S the trainer used Cardiac rehab exercises for a base for his tailored programme. It was important for Mr. S to feel he has enough energy to function all day. At the beginning of the programme Mr. S tended to push himself to much and tire early due to this. The trainer worked with Mr. S to explain exercise intensities and how he should be warming up and how to progress himself safely. The trainer focused on Mr. S aerobic programme increasing the time spent on the treadmill and bike each week until he could complete a full 15 minute programme to start and finish his gym session totalling 30 minute aerobic activity. Mr. S really enjoys his programme and is really keen to keep improving and pushing himself. Experience of a participant in the IGR exercise programme and using Care Call. Story heard on I was monitoring my blood pressure for quite a while 8 years it has always been a bit too high according to my GP. He gave me medication, I m still taking tablets, and the blood pressure is still high, it did not come down. Eventually we did a blood test and he checked for sugar in my blood, he realised it was a bit too high, alarm bells started to ring, so he sent me to a diabetes specialist for advice. I was contacted after a very short time, maybe 4 days, by the diabetes team at Salford Royal Hospital (Care Call). I was surprised it was so quick. They phoned me and decided to call me on a monthly basis. Every month I had a call by phone and we discussed for half an hour things I could do. They explained the risks and how I can prevent falling into diabetes. I think there are two types of diabetes and I was at risk of type 2. They also sent me a leaflet and some information. They sent me 2 sheets about how to manage my diet size of portion, times to eat and how I should manage my meals. From 2 or 3 calls they explained in details what is carbohydrate and what is high in sugar or glucose and how that breaks down in my blood. Later they advised me to join the Salford Leisure Centre. They pushed me a lot; they encouraged me to do exercise every day. At first I said I can walk in the streets or in the park but I liked this option much more, there is a gym I can use just round the corner, it is easy to attend. To join the gym I had to go to Clarendon leisure centre and I met with a trainer called Sam Chesters (from IGR programme) and she gave me some exercises and explained how to do them. Sam agreed to be in touch every 3 or 4 weeks and I ve seen her twice. We also e mailed and she encouraged me to exercise. I got 2 months free gym membership and then
11 they offered me a reduction, just half price for the future. Yesterday was the last call and they arranged the next appointment for 3 months time this is OK, I will continue using the gym. I m not sure now if I need to change my diet to maintain my current weight or stick to the one I was given which is for weight loss. I found it very helpful. I took on a new lifestyle. I am very happy about that. I keep going (to the gym) and what is most important for me is I lost lots of kilos. They gave me a goal at the beginning I made a small error and said I was 80kg but on my scales I was 82.4kg they said I had to lose 5% in 6 months so my goal was to be 76kg. I now weigh in at 76.4kg so I ve lost about 10 pounds. On the whole the service was good; it helped me to take on a new lifestyle. I would advise others to go on this programme but I don t think everyone would choose this option I was scared when they said I could get type 2 diabetes but I don t think everyone would choose this path. I was phoned by a woman called Sue and the last call was somebody else, she told me they were in touch with Rabbi Saunders at Cheetham Hill. It s possible that passing the messages of these services that they are available and helpful though rabbinical authority could encourage other Jewish people to get involved. Generally Rabbis have an oversight of the communities so they could help, guide and advise people and eventually direct them to these services when they felt the person was ready to hear the message. I m happy I made the drastic decision to change my lifestyle and to share this experience with others. I am also happy to meet others of my age group and share their friendship. I am especially grateful to the instructors- each works very hard, single-handedly to conduct the classes. They re loving and sympathetic instructors and are very welcoming amongst the members.
