3 ATHLETIC SCREENING PROCEDURES DIRECTORY Each four-year and transfer student-athlete receives a summer time mailing that includes an Incoming Student-Athlete Letter and an Athletic Health Appraisal Form that he/she must complete and have a physician clear him or her medically, before they are able to participate. Athletic screenings for the upperclassmen are done in conjunction with Student Health Services and the Sports Medicine Staff at the end of the Spring Semester with some limited screenings in August. All Bucknell University football players are medically screened each pre-season by the Bucknell University physician and orthopedic surgeon. Each student (student-athlete) must have on file at the Student Health Services a Physical Examination that must be completed within one year prior to matriculating at Bucknell University. The examination includes immunizations, health history questions, height, weight, lungs, heart, etc. Each student-athlete that goes abroad must have an up-to-date medical screening as well as completing a short version of the physical screening form. Considering the demands of sponsoring a 26-sport athletics department, the following procedures will be utilized for student-athlete medical clearance. It is important to note that no student-athlete will be eligible for tryouts, practice, and/or competition until all of the necessary steps have been completed. The terminology of student-athlete refers to all individuals trying out for an athletic squad and/or participating in practice or any type of competition. Such cardio screenings are routinely performed either at the Student Health Office or during the initial weeks at the start of the academic year through the SMO. 2. The S/A must provide the SMO with all requested appropriate Health Insurance Information. 3. 6 CATASTROPHIC INCIDENT MANAGEMENT PLAN Definition: Catastrophic Incident 1. Sudden death of a student-athlete, coach or staff member at anytime (accidents and illness as well). 2. Disabling and/or quality of life altering injuries such as spinal cord paralysis or loss of a paired organ. Catastrophic Incident Management Team (CIMT) 1. This is a selected group of individuals that are contacted immediately when a catastrophic incident occurs. 2. They receive all the incident information and then disseminate what appropriate information needs to go to the appropriate people. 3. No other person is to speak publicly about the incident. 1. Open for designated home events only. The Stadium Training Room will be open for practices at designated times only in the Spring season. The training rooms are open Saturdays and Sundays to provide services for scheduled home events and practices only. If your team does not have a scheduled practice, treatments are by appointment only. A certified athletic trainer must be present (on site) at home contests. This complies with the Patriot League Policy. B. The training room must have a certified athletic trainer present to manage the delivery of athletic training services to those sports engaged in practice or those student-athletes who require rehabilitiation during regularly scheduled training room hours. 19 4

7 DAILY TREATMENT PROCEDURES COMMUNICATIONS Athletes coming into the Sports Medicine Center wishing to receive treatment must follow the following procedures in order to attain treatment from Bucknell University's Athletic Training Staff. Athletes must: Be showered Leave their belongings outside at the coat rack Wear the appropriate clothing (i.e., shorts, tank tops, etc.) Sign in at the computer desk Must check in with their assigned staff athletic trainer or the staff trainer on duty Sign waiver form for all new injuries SPORTS MEDICINE OFFICE AND YOUR RECRUITS Each staff certified athletic trainer has a cellular phone, which they carry with them to all practices and games. Each sport is given a walkie-talkie with their First-Aid kits for practice and/or game coverage. The Sports Medicine Center has twelve Radius SP50 Motorola multi-channel walkie-talkies. The Stadium Training Room has six walkie-talkies. If a non-traditional sport is practicing outside the training room hours, the coach is responsible for communications. There are security phones located in the vicinity of all practice/game venues. Each coach is given brief instructions on how to use the walkie-talkie. They are not to be used as a toy. Each coach is requested to communicate with the Sports Medicine Center once his or her practice has ended or in case of injury or emergency. Many of the coaches do carry cell phones with them. The Sports Medicine Center uses LINE ONE for the majority of events. If there are multiple events going on or if Administration requests the use of walkie-talkies, other channels are used. Under the NCAA Guidelines and in conjunction with our Departmental Policies and Procedures, the Office of Sports Medicine is not permitted to evaluate, treat or tape student-athletes on a recruiting visit (official or unofficial). In addition, an athletic trainer may not perform any type of medical screening unless a team physician is supervising (this includes taking heights and weights). When giving recruits and their families tours, please do not bring them directly into the Sports Medicine Facilities. Use the second floor level for observation purposes. 8 Proceduare for athletes with a suspected or confirmed eating disorder: Athlete is to be referred to Student Health Services for medical evaluation Athlete will then be referred to Psychological Services for consultation Athlete will be referred to Nutritionist Athlete will be expected to attend follow-up care as prescribed by physician Football Field Hockey Men's Lacrosse Women's Lacrosse EATING DISORDER OR DISORDERED EATING [continued] MOUTH GUARD POLICY AND PROCEDURES Mouth guards play an important role in reducing dental, oral soft tissue injuries, concussions, and injuries to the jaw. Mouth guards should be durable, resilient, resistant to tear, inexpensive, easy to fabricate, tasteless, odorless, and be clearly visible to officials (Greenberg, Springer, 1991). Athletes of the following sports MUST use a mouth guard during their sports: Each athlete of these designated teams will be issued two (2) mouth guards per season. Any given afterwards will be at a cost of $1.00. Any athlete participating without a mouth guard will not be permitted to play. TREATMENT The treatment plan will be prescribed by the University physician and/or Certified Athletic Training Staff. The treatment plan or rehabilitation program will be followed until the goals for recovery set by the University physician have been met. The final decision as to when the student-athlete may resume practice and competition medically resides with the University physician. When treatment is performed by outside Healthcare Providers other than our local consultants, it is necessary that the Medical Director and if involved, our local consultants, be notified by letter and/or telephone call promptly as to the progress and prognosis of the case. Student-athletes are expected to have their Healthcare Provider comply with this request. Injured student-athletes are expected to report for treatment and rehabilitation according to a schedule prepared by the Sports Medicine Staff. Failure of the student-athlete to keep appointments will be interpreted as unwillingness to cooperate, indicative of little interest in returning to competition as early as possible. Such behavior will be reported to the appropriate coach. PRIORITIZATION OF COVERAGE The athletic training staff will do their very best to cover practices and scheduled contests, but they need the cooperation of the coaching staff in planning their schedules and those of the student trainers. Our injury surveillance statistics, which are supported by our insurance company's statistics, and the NCAA injury statistics, show that the sports which sustain the most numerous acute injuries are: football and men's lacrosse. Because of the nature of the sport, football will have a staff athletic trainer travel with them. Men's and Women's Basketball, as well as Men's Lacrosse will have a staff athletic trainer travel with them. An "in-season" sport may have a staff athletic trainer travel with them, only if the "rules for coverage" for athletic events have been met and the staff trainer wishes to travel. This office will not cover "off-site" practices of in-season or non-traditional sports. Again, the "rules for coverage" must be satisfied before the Sports Medicine Department can release a staff trainer for travel. CHANGES IN PRACTICE AND/OR EVENTS The Sports Medicine Department MUST be informed of any chances in your regular schedule as soon as possible. Coverage will not be provided on last minute notices! 17 6

9 STUDENT TRAINERS The Sports Medicine Department will do their very best to assign a student trainer to a sport. Demand has exceeded supply with the increased formal activity of the "non-traditional sports," therefore we will not supply a student trainer to cover nontraditional practices. Please be reminded that the student trainer is an extension of the staff trainer's authority while on campus. When the sport is off-campus it is the coach who is responsible for the health care of the student-athlete; the only exception is when a certified staff athletic trainer is traveling with the team. The student athletic trainer is limited to provided the following: 1. Taping and strapping of student-athletes 2. Providing first-aid to injured student-athletes 3. Assisting and providing water to the teams 4. Documenting/communicating to staff trainers the injuries sustained by student-athletes. EATING DISORDER OR DISORDERED EATING [continued] Anorexia Nervosa [continued] Warning Signs: Wearing baggy clothes or layers of clothes Experiencing extreme fatigue and lethargy Displaying oral fixations due to hunger (chewing gum, constantly drinking diet soda) Showing high levels of anxiety, and wide mood swings Withdrawing from teammates Lanugo (soft fine hair on body) HIPAA AND WHAT IT WILL MEAN TO YOU The federal government has passed regulations under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), effective April 14, 2003, that are designed to protect the privacy of health information. Because of the HIPAA Act, the Athletic Department and Sports Medicine Staff has taken steps to keep our student-athlete's rights protected. Therefore, each student-athlete signs a standard disclaimer at the beginning of each year which is held by the Compliance Officer of the Athletic Department. If an athlete is evaluated by the Sports Medicine Staff during the year, they are also requested to sign an individual Authorization for Disclosure of Protected Health Information form. The Authorization for Disclosure form gives the Sports Medicine Staff the authority to release medical information on that specific injury to the following individuals: health care provider, coaches, student athletic trainers, parents, BU Student Insurance Program Administrator. At any time, the student-athlete may revoke or terminate the authorization by submitting in writing the termination of disclosure to the Head Athletic Trainer. Bulimia Nervosa Identifying Factors: Athlete reports regularly engaging in binge eating Athlete reports that his or her binge eating behavior is out of control Athlete also engages in self-induced vomiting Athlete may use laxatives, diuretics, severe dieting, or vigorous exercise Athlete is very focused on their body shape and weight Warning Signs: Engaging in secretive eating (or you notice food is missing) Avoiding team meals or restaurant trips Visiting the bathroom after meals Engaging in very obsessive exercise routines Talking frequently about being too fat Using diet pills or weight loss products Talking constantly about weight or food Developing dental problems or bad breath (from vomiting) Displaying dichotomous thinking about food (thinking that eating any fats and sweets is bad but all diet foods are good) 7 16

10 Types of Eating Disorders Anorexia Nervosa Bulimia Nervosa EATING DISORDER OR DISORDERED EATING In the world of intercollegiate athletics, there is a constant competitive drive for many athletes to maintain a certain look or feel about themselves and their bodies. Many athletes may take this to an extreme. As a result, many athletes suffer from an eating disorder or disordered eating. There are two kinds of eating disorders that will be discussed as well as the difference between eating disorders and disordered eating. Eating disorders can be defined as "persistent disturbance of eating or eatingrelaxed behavior that results in the altered consumption or absorption of food and that signficantly impairs physical health or psychosocial functioning." (Firburn and Walsh) Disordered eating covers a wide range of behaviors, much of which are practiced daily by many of the general population as well as athletes. Anorexia Nervosa Identifying Factors: Body weight is lower than expected by 15% or more The individual reports a distorted physical self-image The athlete reports a fear of gaining weight Evidence of a very restrictive diet Obsessive exercise is used to burn calories and control weight Irrational thinking pertaining to food intake and exerecise Warning Signs: Having experienced dramatic weight loss Falling to more than 15% below normal weight Showing excessive pre-occupation with calories, food, and weight Engaging in excessive exercise Displaying an aversion to weight training (fear of getting big) Ceasing to eat major food groups (especially meat and dairy) without adding sound nutritional replacements (beans and tofu) INSURANCE Due to the excess coverage provided by Bucknell, student-athletes must take a more active roll in their health care procedures. When dealing with athletic-related injuries, student-athlete's personal insurance is primary. Once the primary insurance company pays the claim, if there is an amount left over, that amount will be submitted to the secondary insurance. Bucknell University's Athletic Insurance is through Merkel Insurance Company. Each student has the option to purchase, through Hulse Associates, Inc., student health insurance. That insurance will cover all athletic-related injuries seen by a physician in the area. When Your Athlete Needs to be Seen by a Physician STEP 1 Is this considered an emergency? If is is an emergency, don't wait. Most insurance companies will pay for a visit to the emergency room or a large percentage of the cost of the visit. Make sure it is an emergency though! Have the athlete or their parent(s) or guardian notify their primary insurance carrier, within 24 hours, of the visit to the emergency room. STEP 2 If this is not an emergency, the athlete or their parent(s) or guardian must contact their primary insruance company to start a claim and also find out what physician the athlete may be seen by. The majority of insurance carriers have a primary care physician the student-athlete must go through for clearance or evaluation. STEP 3 Once the claim is started and the athlete has clearance to see a physician, the appointment can be made. STEP 4 If the athlete cannot see a physician in the Lewisburg area, then they must go home for the evaluation or to the closest doctor their plan allows. The Sports Medicine Staff can try to help the student-athletes with the initial contacting of the insurance company, upon request. However, it can be a long process and the main responsibility lies with the student-athlete and their parent(s) or guardian. Sometimes clearance to be seen by a physician, can take longer than 24 hours. The Sports Medicine Staff has no control over how quickly insurance companies make decisions or how quickly a student-athlete can be seen by a physician. The Sports Medicine Staff is under no obligation to transport a student-athlete to or from a doctor's appointment/mri, etc. 15 8

11 REFERRAL TO OFF-CAMPUS MEDICAL SPECIALISTS All varsity sports injuries must be seen by a Certified Athletic Trainer. Referral will be made to the appropriate physician. When it is determined that an athlete needs to be referred to an off-campus medical specialist for treatment or consultation, the procedures will be: (see steps on previous page) A Staff Athletic Trainer or Health Services Physician will make the appointment with the appropriate specialist. If a referral is made, management will be by the consultant. Health Service Physician, and Staff Athletic Trainer. It is recognized that after a consultant is seen and a recommendation is made, the injured student-athlete may desire a second opinion (in the case of surgery or change in participation status, etc.) The University's only requirement will be that the second consulation be a recognized qualified specialist in his field, as determined by the University's Medical Director. Procedures - Student-Athlete Eligibility In order for a student-athlete (S/A) to be eligible for tryouts, practice, and/or competition, the following steps must be followed. 1. S/A must complete the STUDENT-ATHLETE ELIGIBILITY CARD. [Eligibility cards are provided to head coach by the Compliance Services Office] 2. S/A presents the card to SMO for confirmation initially following medical screening. [SMO forwards all cards to the Compliance Services Office] 3. S/A must attend mandatory university program for processing of all NCAA required paperwork and introduction to the STUDENT-ATHLETE HANDBOOK. This will be part of the Annual Student-Athlete Certification Meetings. 4. S/A completes NCAA Drug Testing Consent Form (DTC) and Student- Athlete Statement (SAS) at mandatory program. [Compliance Services Office will forward completed and signed eligibility cards to head coach] 5. S/A will have his/her eligibility card returned for the purposes of securing Equipment Room services. Receipt of completed eligibility card confirms eligibility for participation in practice and/or competition. [Equipment Room forwards all eligibility cards to the Compliance Service Office to be kept on file] Other Lightning Safety Tips There should be no contact with metal objects (bleachers, fences, bats, sticks, etc.) Standing next to single trees or in a group should be avoided. If there is no other shelter, you may seek refuge in a hardtop vehicle. It is not the rubber tires that protect from lightning; it is the hard top metal roof that dissipates the lightning around the vehicle (NCAA, 1999). DO NOT LIE FLAT ON THE GROUND. Avoid using a land line telephone, taking showers and using plumbing facilties (including indoor and outdoor pools, Jacuzzis and hot tops). If someone feels his/her hair stand on end and skin tinkle, LIGHTNING IS IMMI- NENT! All persons should then assume the "lightning-safe" position described previously. Flash-To-Bang Method This method should be used in conjunction with the Strike Alert Lightning Detector as well as the Golf Course Lightning Detector. Begin counting (one, one thousand, two one thousand...) as soon as a lightning flash is seen Stop counting as soon as you hear the bang of thunder Take this number and divide it by 5 This will give you an approximation of the distance, in miles, that lightning is from the practice/game area EXAMPLE: You see a flash of lightning and begin counting. You hear the bang of thunder after counting to 45 seconds. 45 divided by 5=9. This means lightning would approximately be 9 miles away. 9 14

Golfers should drop your clubs and remove shoes. Evacuation of Stands Once officials have signaled to suspend activity, a member of the Athletic Communications Staff will make a PA announcement that fans are advised to immediately seek shelter. Resumption of Activity Activity may resume once officials give permission for home events or the head coach gives permission for practice. The decision will be based on: At least 30 minutes have passed since the last lightning flash or last sound of thunder and/or the Strike Alert Lightning Detector indicates that lightning is greater than 20 miles away. Each time lightning is observed and/or thunder is heard, the "30-minute clock" is re-set. Pre-Hospital Care of Victims of a Lightning Strike Lightning-strike victims do not remain connected to a power source and do not carry an electric charge; therefore, it is safe to touch the victim to move him/her to a safe location and to render medical attention. During an ongoing storm, lightning activity poses a deadly hazard to personnel responding to the victim. Personnel must consider his/her own safety before putting themselves into a dangerous situation. First priority is to move the victim to a safe location. It is critical to intiate CPR and AED care as soon as safely possible. Treat the most critical victims first. Lightning strike victims should be evaluated and treated for hypothermia, shock, fractures, and burns. 13 It is important to note that there are times when student-athletes who are no longer active on a team roster must be tracked for compliance purposes. For example, a student-athlete receiving athletically related aid is tracked on the CSO database for their sport, regardless of whether the student-athlete is practicing or competing. In addition, student-athletes not receiving athletically related aid might be deleted from the roster prior to the first date of competittion. After the first date of competition, NCAA rules requires that student-athletes who quit or are dismissed from a team remain identified as part of the squad. However, their change in status is noted and dated, permitting the Registrar's Office to revise the studentathlete's code in the Banner system thereby eliminating the need for future eligibility checks and academic progress reports. Procedures - Student-Athlete Eligibility [continued] A coach or Certified Athletic Trainer may, on occasion, have doubt regarding a student-athlete's medical condition and his/her acceptability for competition and practice *In such a case, the University's Medical Director may require that the studentathlete provide an examination report that confirms the student-athlete's medical capability for competition and practice. Though the student-athlete will choose the doctor for the examination and report, the Medical Director will establish the minimum level of medical credential that is acceptable to the University. *ATHLETIC ELIGIBILITY COACHING STAFF FIRST-AID AND CPR CERTIFICATION POLICY Each coach at Bucknell University, whether a graduate assistant, volunteer, full-time assistant or head coach, must have an up-to-date Adult CPR and Standard First-Aid card. If a coach is not certified the Bucknell Athletic Department holds several American Red Cross Certification Classes throughout the summer months. If a coach cannot attend any of the scheduled classes, he or she must call the American Red Cross directly and sign up with them for the class. The department will pay for the coaches to get certified through our athletic department. If the coach goes outside of the department, the cost of the class becomes their responsibility. The Sports Medicine Staff has at least one certified trainer that is American Red Cross Instructor Certified. That staff member is responsible for putting together the classes for the department and keeping up-to-date records of each coach's certification status. If a coach does not meet the certification requirements, Administration is notified. AMERICAN RED CROSS FIRST-AID - Certification every 3 years AMERICAN RED CROSS ADULT CPR - Certification Yearly 10

13 LIGHTNING POLICY EMERGENCY PROCEDURE In the event of an injury which requires immediate transportation to a medical facility, the following procedures should be used: 1. Provide appropriate first-aid 2. "Life-threatening medical emergency" - follow action plan a. Use two-way radios, have training room personnel dial 911 and give specific details b. Request that an ambulance be summoned immediately c. Summon a staff athletic trainer to the emergency immediately 3. "Non-Life threatening medical emergency" - follow action plan a. Use two-way radios or call and give location with specific directions b. Request the assistance of a staff athletic trainer immediately 4. Remain at the scene of the emergency to provide and/or direct assistance 5. Emergency phone numbers are located next to all training room phones. It is the understanding that outdoor sports pose a risk of a lightning strike with inclement weather. The Athletic Training Staff has developed a lightning safety policy to minimize the risk of injury in accordance to NCAA recommendations. To monitor lightning, the Athletic Training staff will utilize the Strike Alert Lightning Detector, The Flash-Bang method, and the Lightning Detector from the Golf Course. GENERAL POLICY: The head coach or assistant coach, if the head coach is not present, of that particular team is responsible to monitor and make the decision to suspend activity in the event of lightning. Exceptions are made during competition when officials are responsible to make the decision to suspend activity. The decision to suspend activity should be based on: A reading from the Strike Alert Lightning Detector in the 8-20 mile range. This devise will be in possession of a Staff Athletic Trainer; Hearing 3 loud blasts from the Golf Course Lightning Detector; AND/OR Utilizing the Flash-to-Bang method revealing lightning within 6 miles. Emergency While Traveling With Team Should a medical emergency occur off campus and without a Certified Athletic Trainer, the traveling coach will follow appropriate steps in acquiring emergency medical services on-site. Upon return to the Bucknell University campus, the coach must directly notify the staff athletic trainer in charge of their team of the emergency. Suspension of Event Announcement Once it is determined that there is a possible lightning strike, the officials will summon all athletes from the playing field or court (via horn, whistle, or announcement). Evacuation of the Playing Field Immediately following the announcement by officials for suspension of play, all athletes, coaches, officials, and support personnel are to evacuate to an enclosed grounded structure. If no safe structure or location is within a reasonable distance, personnel should seek a flat area and assume the "lightning-safe" position (a crouched position on the ground with the feet together, weight on the balls of the feet, head lowered, and your ears covered. DO NOT LIE FLAT! Stay in this position until the storm passes
