2 SCHEDULE: Friday July 8, 2011: Provincial High School Bench Press Championships Weigh Ins: 5:00 p.m. 6:30 p.m. Technical Meeting: 7:00 p.m. (Mandatory for all high school athletes and volunteers) Lifting Begins: 7:30 p.m. Saturday July 9, 2011: Provincial Powerlifting and Bench Press Championships (All classes) Weigh Ins: 8:30 a.m. 10:00 a.m. (Venue opens at 8:00 a.m.) Technical Meeting: 9:45 a.m. (Mandatory for all first time athletes and volunteers.) Lifting Begins: 10:30 a.m. Note: The athlete lifting order is contingent on the number of entries. The exact roster and order of athletes will be determined after entry deadline (June 17). Therefore, it is important to provide one s e mail address on the entry form, as this is how updated information and schedules will be distributed. Note: Weigh Ins will not necessarily be in order of lightest to heaviest. All lifters are expected to be at the venue by 8:30 a.m. (at the latest). Weigh In order will be at admission table by the front door.

3 FEE: Powerlifting $70 Bench Press $50 Both $90 High School $50 AWARDS: Both Male and Female Best High School Bench Presser Awards Both Male and Female Best Overall Powerlifting Awards Both Male and Female Best Overall Bench Presser Awards Both Male and Female Best Raw Lifter awards Both Male and Female Best Novice Lifter (first meet) Medals for weight classes Note: All awards, from all sessions, including the High School Bench Press awards, will be given out immediately following the lifting on Saturday. RAW LIFTING/CLASSIC POWERLIFTING: Raw lifting is acceptable and encouraged; however, this is not the Provincial Raw Powerlifting Championships. As a result, raw lifters will compete at the same time as the full powerlifting meet. For the purposes of the meet, raw lifting is defined, as use of wrist wraps and belt only (no knee wraps, or any type of sleeve). Lifters must also wear a non supportive singlet and a form fitting t shirt. Lifters following these guidelines will be marked with an asterisk on the results sheet indicating that they have competed without supportive equipment; and, they will be eligible to compete for the Best Raw Lifter Award.

4 APU MEMBERSHIP INFORMATION: All lifters must be members of the CPU/APU to compete. All athletes competing for the first time must obtain this membership. Please see membership application attached to the back of this form. High school athletes are exempt from this application as the membership fee is included in the entry costs. Athletes will be asked to present the membership card at weigh in. DRUG TESTING: The pursuit of doping free sport is a matter of public interest. The CPU is committed to eradicating the use of performance enhancing drugs from powerlifting in Canada. As such, all members of the CPU, by virtue of membership, are agreeing to submit to periodic drug testing, either at a sanctioned CPU contest, or on a "short notice" or "no notice" out of competition basis. The CPU also follows the guidelines of the Canadian Anti Doping Program. The CADP is the set of rules that govern doping control in Canada. Compliant with the World Anti Doping Code and all international standards, the CADP describes how the program is carried out and details the process of results management, including penalties for infractions. Full details of the CADP and other controls that the CPU adheres to may be found on the website of the Canadian Centre for Ethics in sport: VOLUNTEERS: Volunteers are required to help run the competitions. We will need spotters, loaders, score table volunteers, and floaters.

5 Contact James Bartlett (volunteer coordinator): or Note: Be reminded that in accordance with the APU constitution, if an athlete is not an active lifter at the 2011 Provincial Powerlifting Championships, he/she must volunteer in some capacity in order to be eligible to participate in the 2012 National Championships. OUT OF PROVINCE LIFTERS: Out of province lifters are welcome and encouraged to lift. Although all attempts for out of province lifters will be sanctioned and verified; the attempts will not be eligible for medal positions as they are reserved for Alberta members only. In other words, if an out of province lifter has a winning total in a given weight class, the lifter will not receive the gold medal as it will go to the highest total Athlete from Alberta. All out of province athletes will receive medals for participation. RULES: Lifting Technique: All new lifters are expected to read the following article from the APU website regarding the competition rules and procedures for the squat, bench and deadlift: These rules will be reviewed at the technical meetings, but preparation beforehand will give novice athletes a chance to orient themselves prior to competition day.

6 Equipment: If supportive equipment is being used, athletes are encouraged to read the following article from the International Powerlifting Federation regarding approved equipment: ipf.com/rules.17.0.html Be aware that not all of the supportive equipment used in gyms are approved. Please ensure all equipment (belts, wraps, sleeves, bench shirts, squat suits, and deadlift suits) meet the approved criteria. GETTING THERE: Car: From Deerfoot Trail, proceed onto the Trans Canada HWY (16 th Ave.), head west. Exit onto University Drive (north) and proceed into the bus loop. Parking: Just off the bus loop, but visible from the street, is a three story parkade called the Art Building. For more parking, proceed on University Way from the bus loop until reaching Hotel Alma; there is an outdoor parking lot in front of the hotel. These two parking locations are within walking distance from the competition venue. Transit: The University of Calgary is accessible by train and bus. For transit routes visit The bus loop is the destination to exit if arriving via bus.

7 Getting Around Campus and Finding the Gold Gym: The Gold Gym is in the Kinesiology building. If looking on a map, the building is located directly north of Hotel Alma, and the Rosa Centre (just off University Gate). For an easy landmark, enter through the doors near the statues of nine foot tall figures; this is the east entrance. Once you enter these doors, the Gold Gym is directly to your left. Please refer to the campus map and kinesiology building map attached to the end of this form. The Kinesiology building is equipped with a full service change room, including: lockers, showers, and steam rooms. Within a very short walk from the venue, there is the MacEwan Student Centre, which is a great place to enjoy food and drinks. ACCOMODATIONS: The newly established Hotel Alma is within walking distance of the venue. Hotel Alma is located on University Gate, and parking is conveniently accessed in front of the hotel. These risks include but are not limited t o: (a) All manner of injuries resulted in muscular injuries and soft tissue injuries including bruises, scrapes, cuts, etc., from executing strenuous and demanding physical techniques, contact with other participants and failure i n proper use of equipment either by myself or other participants; (b) All manner of injuries resulting in sprains, dislocations, concussion and broken bones, heart attack/stroke, spinal injury and tendonitis; (c) All manner of head, facial, eye and/or dental injuries; (d) All manner of medical problems resulting from heat exhaustion, asthma, communicable diseases, skin rashes, cramps and chemical poisoning; (e) That my risk of injury increases as I become fatigued; (f) All manner of injuries and/or death that could result from a physical confrontation whether caused by myself or someone else or from any participation in the APU Championship Activities, including but not limited to falling weights and/or equipment used in the APU Championship Activities; (g) All manner of negligent advice regarding powerlifting, bench pressing or other APU Championship Activities.

10 RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT 1. In consideration of Alberta Powerlifting Union allowing me to participate in the 2011 Provincial Powerlifting and Bench Press Championships, I agree, for myself, heirs, executors, administrators, and assigns to waive and release any and all rights and claims for damages I may have against the International Powerlifting Federation, the Canadian Powerlifting Union, the Alberta Powerlifting Union, University of Calgary, Temple Fitness, Meet Directors, and Sponsors, their officers, directors, agents, contractors, employees, coaches/instructors, trainers, volunteers, members and representatives, and the manufacturers and distributors of the equipment used in the 2011 APU Provincial Powerlifting and Bench Press Championships (collectively the Releasees ). In particular, I agree: a. TO ASSUME AND ACCEPT ALL RISKS arising out of, associated with or related to my participating in the 2011 APU Provincial Powerlifting and Bench Press Championships; b. TO RELEASE THE RELEASEES from any and all liability for any loss, damage, injury or expense that I may suffer or that my next of kin may suffer as a result of my participation in the 2011 APU Provincial Powerlifting and Bench Press Championships, due to any cause whatsoever, including breach of contract, breach of any Statutory or Other Duty of Care, including any Duty of Care Owed under the Occupiers Liability Act, RSA 1980 C. 0 3 (as amended) on the part of the Releasees; c. TO HOLD HARMLESS AND INDEMNIFY THE RELEASEES: i. from any and all liability for any damage to the personal property of, or personal injury to, any third party resulting from my participation in the 2011 APU Provincial Powerlifting and Bench Press Championships; ii. from any and all claims, demands, actions and costs which might arise out of my participating in the 2011 APU Provincial Powerlifting and Bench Press Championships. 2. I acknowledge that Powerlifting is a high risk sport and that I could be injured. 3. I certify that I am in good physical health and that I have no serious health problems and that I assume full responsibility for such conditions. 4. I agree that I am fully responsible for my well being and safety in the warm up room, on the lifting platform and while I am competing at this event. 5. I agree that I am responsible for the actions of any coach and entourage that attend this contest in my support. 6. In the event I am disqualified from this contest by means of rule infractions or due to my conduct I acknowledge and agree that I will not be eligible for reimbursement of contest entry fees. 7.
