1 Mental Health Wellness Center Additional Information Course of Treatment: Within 15 days of admission to the clinic, you and your therapist will develop a comprehensive plan of treatment identifying any specific challenges you are facing in order to improve your quality of life, as well as the steps that both you and your therapist will take to aid in successful completion of these goals. Outpatient mental health and counseling sessions are generally scheduled weekly or bi-weekly with a typical session lasting for one hour. After 15 sessions or 120 days, whichever comes first, your plan of treatment is reviewed and updated. If you receive medication management, you will be required to meet at least monthly with one of our therapists to ensure you are gaining the most complete treatment possible. Convenient Hours: To conveniently accommodate the schedules of the individuals we serve, the Mental Health Wellness Center is open from Monday through Thursday from 8:00am to 7:30pm. Costs and Payment: During the initial session, you will receive detailed information regarding any eligible benefits from your insurance coverage and what financial liability you may have. If you do not have insurance coverage we do offer a private pay option we will arrange with you. The fee schedule will be explained to you as well as the options to develop a reduced pay rate if you are ineligible for insurance coverage. Accepted insurances are as follows: Aetna, Highmark/Blueshied, Cigna, CCBH, Capital Blue, UBH/Optum, Humana, Geisinger Gold, Integrated BH, Magellan, Medicare, MHNet, Tri Care, Beacon Rights and Resources: The Mental Health Wellness Center adheres to the Mental Health Laws of the State of Pennsylvania, which confer significant rights to the recipient of the mental health services. We will fully inform patients of all rights prior to initiating any plan of treatment. When members of our community require personalized support and care in an effort to achieve their very best mental, behavioral, and relational health, the Wellness Center at Access Services is here to provide the highest quality, strength-based, and holistic treatment that bridges the continuum of care. It is our goal to empower individuals in their journey to achieving their fullest potential.

2 Meet Our Team: Our highly trained, professional staff consists of a Clinical Director, Outpatient Director, multiple clinicians, as well as dedicated support staff. Our clinicians have a diverse range of expertise and include licensed clinical social workers (LCSW), licensed professional counselors (LPC), licensed psychologists, and master s level clinicians. Our Clinical Director functions as a staff psychiatrist and serves a significant number of individuals. Our Outpatient Director is a master s level clinician and manages the daily operations of the center as well as provides psychotherapy to individuals. Bahman Sholevar, MD, PhD Dr. Bahman Sholevar joined Access Services as Psychiatrist and Clinical Director in 2002, after he retired from his academic career as professor of literature & professor of psychiatry. For over 20 years he had been on the faculty of various universities such as University of Iowa, Hahnemann University, Medical College of Pennsylvania, University of Pennsylvania and Thomas Jefferson University. He holds degrees of Diploma of English Studies from University of Cambridge, B.A. from University of North Texas, M.A., M.F.A. and Ph.D. from University of Iowa and M.D. from Hahnemann University (now part of Drexel University). He is diplomat of National Board of Medical Examiners and The American Board of Psychiatry & Neurology, an elected member of The New York Academy of Sciences, and an elected Fellow of Philadelphia College of Physicians. Prior to his Academic career he served for two years as a diplomat in the International Secretariat of Central Treaty Organization in Europe. He has published 18 books in fields of psychiatry and literature in five different languages and many short pieces in many periodicals. His novel Dead Reckoning (Philadelphia: 1992) and his Collection of poetry Rooted in Volcanic Ashes (Philadelphia: 1988) were nominated for National Book Award and for a Pulitzer Prize. For many years he gave seminars on Psychiatry & the Psychology of Art around the world in California, Hawaii, Paris, Cannes, Amsterdam, Brussels, Madrid, Rome, Egypt, Kenya, Tanzania, Indonesia & Thailand. He has appeared on many TV programs such as CBS, NBC, CNN, PBS, BBC, CBC, & British Independent TV (ITV). For several years he was a regular commentator on Canada AM News Program. He has been featured in many newspapers

3 and magazines, such as New York Times, Canada s Globe & Mail, Philadelphia Inquirer, The Courier Post of New Jersey, Science magazine, MD magazine, Medical Post, Philadelphia Medicine & Pennsylvania Medicine, Publisher s Weekly & Library Journal. He has been on the Editorial Board of Arts in Psychotherapy Journal and a Book Reviewer for Philadelphia Inquirer. Carol Schank, LCSW Carol received her undergraduate degree from the Pennsylvania State University in She then earned her Master s Degree in Social Work with an emphasis in Marriage and Family Therapy from Temple University. In 2007 she became a licensed a clinical social worker and obtained credentialing to supervise other clinicians seeking to become licensed in the state of Pennsylvania. Carol s expertise as a licensed clinician is extensive as she provides counseling services that expands a broad spectrum with clients of all ages. Since 2005 Carol has been an active member with the National Association of Social Workers in Pennsylvania. Janice Price, Psychologist Janice is a licensed psychologist who received her undergraduate and graduate degrees from West Chester University. After receiving her license in 1995 she went on to complete a three-year training program through The Pennsylvania Gestalt Center For Psychotherapy and Training. Janice offers a holistic approach to therapy, incorporating a blend of present centered awareness (mindfulness), cognitive therapy, and Gestalt techniques. Her approach includes helping clients becoming aware of their habitual, conditioned responses so that they can begin to let go of this conditioning and access their own inner resources for healing and growth.

4 Tom Post, LPC Tom received his graduate degree in counseling in 2000 from Biblical Seminary and began working as both a youth pastor and a therapist. In 2009 he returned to Biblical Seminary to complete the additional graduate work required for the LPC and at that point began working for Access Services Inc. in Orwigsburg, PA as a Mobile Therapist. After graduating in 2011, he passed the National Counselor Exam from the National Board of Certified Counselors. He now serves as an Outpatient Therapist at Access Services and is a Certified Clinical Trauma Professional who works to understand the effects of trauma and helping people that have experienced trauma in their lives. In addition, he regularly meets with individuals that experience depression, anxiety, anger, or are managing difficult relationships. Tom believes that we all have inherent value, and strives to truly understand his clients and give them all respect and honesty. Bev Swiadas, LPC, NBCC Beverly Swiadas, MS. NCC. LPC obtained her Master s degree from Villanova University in Counseling and Human Relations and her undergraduate degree from Lebanon Valley College in Psychology. She currently holds a Professional Counselor license in Pennsylvania. She has over 20 years of counseling experience with individuals of all ages in industrial/organizational, career/vocational development, mental health and addictions settings. She has also supervised interns and graduates from California State University, Johns Hopkins University, University of Pittsburgh, Shippensburg University, as well as other reputable universities toward state licensure. Bev has also worked with the University of California, Motivational Training Team. She has held senior assessment positions involving the development of government programs that enhance employment, behavioral health, and juvenile probation systems. Sister Kate Bondura, LCSW Sr. Kate has an undergraduate psychology degree from Holy Family University and holds a Masters of Social Work degree from Temple University. She is a Licensed Clinical Social

5 Worker with a wide range of experience to draw from as a foster care worker, a special needs adoption worker, a social worker on a geriatric inpatient mental health unit of a hospital where she ran groups for patients, created a caregiver support/educational group and was involved in discharge planning. Further experience has been as a supervisor for a Services to Children in Their Own Home Program and as a clinical supervisor and therapist who has done direct counseling which involved child/teen, marital and family therapy. Beverly Weikel, LCSW Beverly Weikel is a licensed Family and Marital Therapist in the State of Pennsylvania with an additional certification as an International Eating Disorder Specialist. Her clinical experience spans three decades and includes extensive training through the Philadelphia Child Guidance Family Therapy Center. Her areas of specialty include treatment of the following mental health issues: ADHD, Depression, Anxiety, Phobia, Learning Disorders, Eating Disorders, OCD, PTSD, and ODD. Her work encompasses treatment of children, adolescents, and Employee Assistance Program referrals. In 2003 she was awarded the Pennsylvania State CASSP Award for child centered Therapy in Schuylkill County. Beverly has served on the Schuylkill County Board of Director for Children and Youth Development and remains a consultant for Child Development in Schuylkill County. Beverly has administered trainings in the following areas: Act 48 Guidelines (in conjunction with the Schuylkill Country Intermediate unit), Childhood Depression, Suicide, OCD, ODD, Bi-polar Disorder, Stress Management, Parenting and Child Development, Art Therapy, Play Therapy, Family Therapy, Ethics, Childhood Mental Health Depression DSM- IV, Pharmacology, ADHD, CPS Law, Charting With A Jury In Mind, Family Dynamics, The Art of Ritual, and Healthy and Mental Health. Beverly has also served on the Pottsville Hospital Panel of Professionals for Community Mental Health Awareness for the past four years. Beverly received her Advanced Clinical Social Worker License in 2004 and maintains a supervisory status serving candidates for Licensure in the field of Eating Disorders.

6 Michael R. Fox, ATR-BC, LPC Michael R. Fox is a Registered, Board-Certified Art Therapist (ATR-BC) and Licensed Professional Counselor (LPC) whom believes that therapy should be safe, fun, energizing, and individualized to the wide range of client populations he serves. He believes in meeting clients where they are at and addressing clinical issues in a cohesive, unified approach with clients as teammates, operating from a heavily humanistic and client-centered theoretical framework. Michael specializes in child, adolescent, and teen populations with a myriad of diagnoses and areas of concern, such as; bullying and issues related to poor self-esteem, LGBTQ issues, depression, anxiety, loss and bereavement, trauma, abuse, neglect, foster care placement, behavioral issues, anger management, and varied attachment disorders. Michael earned his Bachelor s degree in Psychology and Art Therapy from Arcadia University in Glenside, PA before completing his Master s degree in Creative Arts Therapy and Creativity Development at the Pratt Institute in Brooklyn, NY. Michael also serves as the Activities Director for Seton Manor Nursing and Rehabilitation Center, and devotes much of his free time as Administration Member/Program Director for Keystone Diabetic Kids Camp (KDKC), which he and his friends founded in 2013, as well as volunteering for various Loss and Bereavement camps in the area. He is passionate about art, music, dance, and all forms of creative expression. On rare occasions, you can find Michael running, visiting his friends and family, or walking his dachshunds, Rex and Troy.
