2 ANNUAL REPORT 2014 CHAIR REPORT Central Sydney Health Community Network s (CSHCN) vision is a healthy Inner West. We strive to achieve this by working in unison with Inner West Sydney Medicare Local (IWSML) along with Central Sydney Allied Health Network (CSAHN) and Central Sydney GP Network (CSGPN). Our purpose is to facilitate improved health care for consumers and carers in Central Sydney. What a year! We are now two years old and CSHCN has set and achieved many goals. We wanted to double our membership so we could best advocate for consumers and carers in our region and we achieved this. Our membership is broad and culturally diverse and represents the population in the Inner West. This is also true of the CSHCN Board. Directors are not professional directors rather we each have a personal interest in ensuring the community has a strong and independent voice in our region and we each bring a different perspective. Our Board is productive with good communication and well run bimonthly meetings. Directors have participated in governance training. CSHCN aims to liaise with all stakeholders to improve the consumer s health journey. We have worked to achieve this by facilitating community participation in the IWSML Needs Assessment and the CSHCN Board playing a key role in IWSML s strategic planning. We have contributed to health planning and helped to address service gaps in the Inner West region throughout the year. This year, CSHCN member organisations contributed to improving the consumer health journey by being involved in ehealth, Practice Management and Accreditation, and the Can Get Health in Canterbury project through consumer advisor positions on IWSML project advisory committees. Member organisations and the CSHCN Board were also involved in providing feedback regarding the IWSML website which led to the inclusion of a community section on the new website, a directory of services for consumers and making the website more user friendly. our region over the past year. This has proved to be a very popular way to share information important to our members. This year we also started to look at the language used in health settings. We are encouraging the use of consumer rather than patient where possible. We continue to raise the issue of improving Health Literacy as there is a clear link between this and improved health outcomes. I would like to thank the CSHCN Board for their commitment and work this year. We have also had a good working relationship with the IWSML, CSAHN, CSGPN Boards and SLHD as we all work toward the goal of improving the consumer health journey and consumer centred care. My thanks also goes to all the IWSML staff whose support and work has allowed us to fulfil our aims and objectives over the past year. Robyn Maurice Chair MEMBERSHIP REPORT Ordinary Members of CSHCN are health community organisations which represent the interests of actual or potential recipients of health care activities and their carers within the Inner West Sydney Medicare Local region. As at 30 June 2014, CSHCN had 29 Ordinary Members reflective of a broad range of health concerns, such as alcohol and other drugs, carers, chronic disease, culturally and linguistically diverse population groups, disability, mental health and sexual health. Membership to CSHCN is free. The Central Viewpoint newsletter provided CSHCN member organisations with the opportunity to share community events, achievements and services with stakeholders in Central Sydney Health Community Network Ltd

3 ANNUAL REPORT 2014 Kay Cambourn RN BA Robyn Maurice BEd (Adult) UTS, MEd (Adult) UTS I trained as a nurse at Royal Prince Alfred Hospital in the early 80s and then went on to complete a Bachelor of Arts degree at the University of New South Wales. After becoming a mother I worked in research administration and medical research project management in the university sector for nearly 20 years. My first role in project management was coordinating the Australasian component of the Cooperative Family Registry for Colorectal Cancer Studies, an international collaboration initiated by the National Cancer Institute at the National Institutes of Health. In recent years I returned to bedside nursing at Balmain Hospital after completing the Nursing Reconnect Program. My community activities include: forming a new region of Special Olympics in Brisbane where I became an assistant athletics coach, taking an active member s role in the Fragile X Association, my interest in this is due to my having a daughter with Fragile X. Since taking a step back from paid employment, I have taught Ethics in three Inner West Primary Schools, and been a Community Advisor with CSGPN on the Practice Management Advisory Committee. I have also completed training to become a Private Patient Advocate. As a Director on the Board of Central Sydney Health Community Network, I have enjoyed being part of a new direction in delivering effective Primary Health Care. I have been a member of the CSHCN Board since its inception and have been the Chair for more than a year. I am also the Vice President of the NSW Users and AIDS Association (NUAA), a Board member of the NSW Consumer Advisory Group (CAG) and a member of several organisations including ACON, CHF and HCNSW. I have attended all Board meetings and participated in both Strategic Planning and Governance training days with IWSML. I have a sound understanding of the governance and financial issues relevant to not-for-profit organisations. As Chair, I have represented our organisation in many forums including the IWSML Website Redesign Committee, IWSML Congress Steering Committee and at an IWSML/ SLHD Integrated Primary Health Care workshop. I come with a long background of working in community based organisations. I have experience and skills in program development and delivery, health campaigns, resource development, strategic planning, evaluation, community development and peer based adult education. I am committed to improving health literacy, consumer centred health care and the health consumer journey for the full diversity of consumers and carers in our area. 4 ANNUAL REPORT 2014 Julie Millard RN, BN (Uni Syd), JP Margaret McInroy Life Governor I am passionate about mental health and work as a Consultant, with experience in the mental health and disability sectors, engaging with consumers, carers and families, government departments and community managed agencies. I work with diverse agencies to meet accreditation standards, manage projects and build service capacity. Other employment includes working in a community mental health team, as a clinical facilitator for student nurses, developing training packages on mental health/mental illness, and being a facilitator of community education programs. I have been fortunate to speak at many international, federal and state conferences, including presentations at the World Federation of Mental Health Conferences in Cape Town, South Africa and Buenos Aries in Argentina. As an Inner West resident and advocate I am actively involved in local issues, and engaged with local networks. In 1962 I ran away from home and joined the Women s Royal New Zealand Naval Service now known as RNZN, in the Transport Section. I came to Australia for a holiday, met an ex-british Army chap, re-married and went to Scotland to live. In 1971 my husband and I along with our now three children came to Australia as Assisted Migrants ( 10. Poms). Two years later we went to Fiji to live on a construction site, returning to Australia in Our family moved to Belfield in 1976 where I am still living. I was with the Police and Citizens Youth Club, PCYC, as it is known now, for 25 years and was honoured to become Life Governor of both Belmore PCYC and PCYC (NSW) Ltd. I was awarded the Centenary of Australia Medal in 2002 which I very proudly wear on all occasions that medals are worn. After being a volunteer for both the Olympic Games and Paralympics, which was an incredible time, I became heavily involved in volunteering in many different areas and organisations and am now a member of four Boards of Governance, including CSHCN. I have had and hold various positions on all these organisations including President and Treasurer. I am on ten other committees of various festivals and organisations holding positions such as Chairman of the Seniors Citizens Advisory Committee (SCAC) of Canterbury City Council, Trustee and former President of the New Zealand Sub Branch RSL (NSW) Ltd, Management Committee of the Children s Festival Organisation (CFO) Inc and Vice Chair of Canterbury Hospital Community Participation Committee to name just a few. 5 ANNUAL REPORT 2014 John Newsom BA, Assoc Dip Small Business Rob Ramjan AM, BA, BSocStuds My six years in a bank and 30 years with the ABC, BBC and SBS were uneventful apart from the last decade when I became Director of my credit union and ABC Executive supporting the development of Indigenous community media in regional and remote Australia. I spent the next 30 years in one form of community development or another. As an Australian Credit Union Foundation Trustee and working with the World Council of Credit Unions, I helped rebuild community credit unions in the South Pacific and open the Traditional Credit Union in Arnhem Land and First Nations (national) Indigenous Credit Union. I trained the leaders of several hundred community microfinance leaders in the highlands of PNG and was team leader in setting up a community microfinance bank postcivil war. Back in Australia I became a Director of Oxfam and Vice President of the Australian Council for Overseas Aid. I have just ended five years as a director of Balmain/Rozelle Community Bank. In 2005 I became a cancer patient advocate. I was Chair of Cancer Voices Australia and have held consumer representative positions with COSA, Cancer Councils, Cancer Australia and the Garvan, Lowy and Ingham Institutes. In the local district I am a member of many hospital committees and councils. I am the Chief Executive Officer of the Schizophrenia Fellowship of NSW Inc and was the inaugural Executive Director of the Schizophrenia Fellowships Council of Australia Inc. I have worked for the Fellowship for over 20 years. I have extensive experience in the provision of mental health services, especially in the non-government sector. I was also a delegate to the Mental Health Council of Australia Inc for two years, a Member of the NSW Mental Health Priority Task Force for seven years and the Guardianship Tribunal for 15 years. I am a member of the NSW Mental Health Review Tribunal, a member of the Centre for Cognitive Disorders Advisory Committee, Macquarie University, and a Trustee of the Psychosis Australia Trust. I ve also presented many papers, authored a book on mental health residential services and was the project director for The Schizophrenias: guidelines for an holistic approach to clinical practice guidelines which was commissioned by NSW Health. I was made a Member of the Order of Australia in 2007 for services to people with mental illness. Central Sydney Health Community Network Ltd

14 Independent Audit Report to the members of Central Sydney Health Community Network Ltd Report on the Financial Report We have audited the accompanying financial report of Central Sydney Health Community Network Ltd, which comprises the statement of financial position as at 30 June 2014, the statement of profit or loss and other comprehensive income, statement of changes in equity and statement of cash flows for the year then ended, notes comprising a summary of significant accounting policies and other explanatory information, and the directors' declaration. Directors' Responsibility for the Financial Report The directors of the Company are responsible for the preparation of the financial report that gives a true and fair view in accordance with Australian Accounting Standards - Reduced Disclosure Requirements and the Corporations Act 2001 and for such internal control as the directors determine is necessary to enable the preparation of the financial report that gives a true and fair view and is free from material misstatement, whether due to fraud or error. Auditor s Responsibility Our responsibility is to express an opinion on the financial report based on our audit. We conducted our audit in accordance with Australian Auditing Standards. Those standards require that we comply with relevant ethical requirements relating to audit engagements and plan and perform the audit to obtain reasonable assurance about whether the financial report is free from material misstatement. An audit involves performing procedures to obtain audit evidence about the amounts and disclosures in the financial report. The procedures selected depend on the auditor s judgement, including the assessment of the risks of material misstatement of the financial report, whether due to fraud or error. In making those risk assessments, the auditor considers internal control relevant to the Company s preparation of the financial report that gives a true and fair view in order to design audit procedures that are appropriate in the circumstances, but not for the purpose of expressing an opinion on the effectiveness of the Company s internal control. An audit also includes evaluating the appropriateness of accounting policies used and the reasonableness of accounting estimates made by the directors, as well as evaluating the overall presentation of the financial report. We believe that the audit evidence we have obtained is sufficient and appropriate to provide a basis for our audit opinion. Independence In conducting our audit, we have complied with the independence requirements of the Corporations Act We confirm that the independence declaration required by the Corporations Act 2001, which has been given to the directors of Central Sydney Health Community Network Ltd, would be in the same terms if given to the directors as at the time of this auditor s report. 7

15 Independent Audit Report to the members of Central Sydney Health Community Network Ltd Opinion In our opinion the financial report of Central Sydney Health Community Network Ltd is in accordance with the Corporations Act 2001, including: (a) giving a true and fair view of the Company s financial position as at 30 June 2014 and of its performance for the year ended on that date; and (b) complying with Australian Accounting Standards - Reduced Disclosure Requirements and the Corporations Regulations Emphasis of Matter Without modifying our opinion, we draw attention to Note 1(h) in the financial report which indicates that there is material uncertainty regarding the company s future post 30 June The company is economically dependent on the Inner West Sydney Medicare Local Limited for its revenue and provision of membership and secretariat services. In the Federal Budget delivered on 13 May 2014, it was announced that the Commonwealth Deed for Funding for Medicare Local s would cease on 30 June The impact of this announcement is that the company may cease trading on or before 30 June 2015 with the intention of winding up the company. These conditions as described in note 1(h) indicate the existence of a material uncertainty that may cast significant doubt about the company's ability to continue as a going concern and therefore, the company may be unable to realise its assets and discharge its liabilities in the normal course of business.
