2 Additional Instructions Please type EVERYTHING. No hand written applications will be accepted. Please separate each section by making title pages for each section. Paper clip each section title page to the documentation for that section (do not stable). Staple the actual blank application together. Do not fill in the point values on the application.

3 Additional Sections and Areas Your chapter will receive points for all sections or areas not discussed here by having the appropriate paper work on file with our office. If you turn in all required paper work (Registration Packets, Rosters, New Member Information, Election Report Form, Membership Intake Permission Form, etc), by the designated deadlines your chapter should receive full point values for these sections and areas.

9 Homecoming and Fountain Day Participation For the appropriate semester, please discuss your participation in Homecoming and Fountain. Indicate the names of your members who attended and how long each member stayed. Also include five photos of each event.

10 Council Meeting Attendance Letter from Council President To Whom It May Concern: Beta Beta Beta Sorority is an active member of XYZ Council. Their council representatives attended 100% of our council meetings on time. Sincerely, Karla Manning President, XYZ Council

11 Chapter Website Please include a printout of the home page of your website On the homepage, there should be a date that indicates when the site was last updated. If the site was not updated at least a month prior to your print out, you will NOT receive points. Links for the history of your chapter and organization, your chapter s upcoming events, and membership requirements must be displayed on your homepage. National or regional websites do not count Facebook groups will not count either Please see for more ideas for your chapter s website.

12 Diversity, Community Building, Gender Issues, Sexual Assault, Gay/Lesbian Issues, Mediation and Conflict Resolution Event To receive credit for this, please include the following: Pictures of your members at the event Letter from the sponsor of the event stating that your organization did attend The date of the event must be stated in the letter In the letter, please have them list the names of the individuals in attendance The letter must be signed by the president of the sponsoring organization Flier, pamphlet, or some type of literature from the event If 50% of your organization does not attend the event, you will NOT receive credit.

13 Member Participation on a Non-Greek Related Student Organization E-Board To receive participation for this, please include the following: Signed letter from the president of the student organization in which one of your members is on the e-board The letter must state the position of your organization s member and the dates of his/her term in office

14 Letter Day To receive credit for this, please include the following: Statement from your chapter president that says when your letter day occurred Three photos from your letter day Label each picture with the names of the members featured Names of each member who participated If 75% of your chapter does not participate, you will NOT receive credit.

15 Community Awareness (Non-Greek Student Organization Event) To receive credit for this, please include the following: Pictures of your members at the event Letter from the sponsor of the event stating that your organization did attend The date of the event must be stated in the letter In the letter, please have them list the names of the individuals in attendance The letter must be signed by the president of the sponsoring organization Flier, pamphlet, or some type of literature from the event If 75% of your organization does not attend the event, you will NOT receive credit.

17 Large Scale non-university Coordinated Community Service/Philanthropic Project To receive credit, include the following: Two-five photos from the event Label each photo with the names of your chapter members featured in each photo Proposal for the event stating what the event was, when it was, where it was, how many people attended, and if you raised money, how much you collected Letter from the agency you coordinated the event for Names of your organization s members who attended If 70% of your chapter does not attend, you will not receive credit.

18 Community Service Hours To receive credit for this, please include the following: A list of each of your members along with the number of community service hours each member has completed during the semester.
