1 Annual Report 2 11 Our mission is to build relationships, resources, infrastructure and understanding so that no one in Minnesota remains homeless, and people who were homeless can prosper.

2 2 11 When the first family enrolled in 2001, Hearth Connection set out to show that people with the longest histories of homelessness and most barriers to stability could be housed. Using a collaborative and innovative approach to supportive housing, we proved exactly that. Ten years after the Pilot project began, over 2,400 adults, families, and youth across the state have accessed permanent housing and support, showing that the transition from long-term homelessness to lasting stability is well within reach. Hearth Connection designs solutions and secures funding to expand access to supportive housing in Minnesota. As we face new challenges and opportunities, Hearth Connection is moving forward with the inventive spirit of its early days. We will continue to develop best practices, evaluations, and relationships with the resolve that together, we can end long-term homelessness in Minnesota. Hearth Connection continues to share the hope--and the evidence--that the cycle of homelessness can be broken for every Minnesotan who experiences it.

3 Hearth Connection advocated throughout the 2011 legislative session for investments in permanent supportive housing. Sizeable losses in funding to the LTHSSF (Long-Term Homeless Supportive Services Fund) resulted in cuts to case management teams and the availability of intensive services. Hearth Connection is continuing efforts to diversify funding and worked to mitigate the impact of the cuts on participants and service teams. A special THANK YOU to everyone who reached out to their representatives, took part in calls to action, and shared the message that access to housing and support is essential for stable and healthy households. innovation intermediary integration Hearth Connection and its Board of Directors released a new strategic plan in Co-Pilot 2.0 was released, bringing new automated features to Hearth Connection s customized database and improving our understanding of participants needs. Hearth Connection began collaborating with counties and local nonprofits in Central Minnesota to share best practices and identify resources to address unmet needs in the area. Hospital to Home Results The Hospital to Home project with Regions Hospital and Guild Incorporated offered housing, medical care, and support services to seven people experiencing chronic medical conditions and homelessness. A one-year report on the outcomes of the participants showed a significant decrease in emergency department visits, use of clinics, and pharmacy claims after enrollment.

Start at parents 471 children 419 single adults 27 youth 1283 people received housing and supportive services in 2011 Shelly s Road Home Shelly has been earning top grades in college--even making it onto the President s List at her school. She s been a mentor for first year students and has begun pursuing scholarships. Only four years ago, she was caught in a cycle of homelessness. Shelly struggled to leave abusive relationships and had nowhere to turn. She coped with mental health issues and addiction as she stayed in shelters, treatment, and jail. When she began working with a service team in Southern Minnesota, things started turning around. She achieved sobriety and maintained it. She got her own apartment and has lived there for more than a year. Shelly continues to work on her personal goals, and with her initiative and the support of the service team, she s on her way to achieving them.
