2 What is the Paediatric Continence Service? The Paediatric Continence Service is a community based service offering advice, support, assessment, treatment and management of bladder and bowel dysfunction (bedwetting, daytime wetting, constipation and soiling) for children. The service will see children who have found that interventions by health visitors and/or school nursing did not suit their needs, or require further support. Following a referral to the service, children will be assessed at home, in school/preschool or in a clinic. Depending on the outcome of the assessment the child will be cared for in the community, in partnership with their GP or if needed, referred to secondary care. Who can access the service? This is a service for all children and young people aged between 0 and 19-years-old who live within Milton Keynes. The nurse-led Continence Advisory Service will assist and support parents/carers with the following: Nocturnal Enuresis (bedwetting) Starting fluid programme, toileting strategies, management plans, provision of enuresis alarm Prescribing of medication, or a combination of treatments Daytime wetting Starting fluid programme, toileting strategies. Plan of treatment - completion of diary/charts and bladder training.
3 Constipation and/or soiling 0-19 years Full assessment, toileting strategies, toilet, programme, disimpaction regime with medication, support for parents/carers. Children with special needs with delayed toileting skills - including Autism. Work with school nurses and families to put individual plan in place. Children with delayed toileting skills. Advice and support, strategies and management plans put in place. Who s in the team? Specialist Health Visitor Paediatric Continence How to access the service (what happens after referral)? Referrals will be accepted from: Health visitors School nurses Social workers GPs Complex Needs Team Milton Keynes Paediatrics Team Physiotherapists Occupational Therapists Child and Adolescent Mental Health Services (CAMHS) After the referral has been received, an appointment will be offered by letter or telephone for an initial assessment. The appointment will aim to be within eight to twelve weeks of referral. The appointment may be in your own home or other setting such as a health centre or GP surgery.
4 What do I do if I am unable to attend the appointment? It is important to ring and rearrange as soon as possible if you are unable to attend the appointment. How you can prepare for the first appointment? You will be asked questions about the issues that are affecting your child. To help us with the assessment you could start to think about any patterns or particular issues before your appointment, for example when and where they happen. What happens at the first appointment? An assessment will be completed in order to gain information and understanding of the problem and concerns you have about your child. This will involve asking questions about what is happening currently? What has been tried previously? What has been found to be successful? What has not worked? And what are you hoping from the appointment for your child? Following the assessment an individual management plan will be given to you to help and support you and your child. This could involve keeping diaries and charts to record information about toileting behaviour. There are different ways in which working with you may be carried out. If needed, your child s school, or nursery may be contacted to discuss a joint management plan. These decisions will be based on individual needs and agreed with you and your child (if appropriate) What happens next? The management plan agreed will be reviewed at each appointment you have in order to assess progress/improvement. If there is no significant improvement you may be referred to another specialist service.
