3 INTRODUCTION & BACKGROUND Mental Health and Addiction Services in Southland are community based, providing treatment and care across all age ranges in all parts of the sector, including the primary sector. They are delivered by a diverse range of agencies including the public and private sectors, NGO s, and community groups. To effectively deliver quality outcomes for consumers and their families requires collaboration amongst providers and stakeholders in a number of different settings. Integration of services and establishing a continuum of care for clients and families is achieved by providers and stakeholders through communication, collaboration and efficient coordination of services. The Southland Mental Health and Addiction Services Map and Directory is a resource designed to support this process by supplying providers and stakeholders with up-to-date information on who s working in the sector and what their activities include. Developing and documenting a map for mental health and addiction services is important for a number of reasons: To identify the key providers and agencies delivering services (Southern DHB contracted and non-contracted) in the District. To update the levels of service being delivered by providers and stakeholders including where they are provided. To identify and analyse the needs of the population. To inform and support future planning and funding decisions and priorities. To identify a range, and scope, of services that are configured to best meet the needs of the community. To share information and promote a better understanding of mental health and addiction providers and the services they deliver. To acknowledge the important contribution made by stakeholder agencies and not-for-profit groups to the mental health and addiction sector. The information contained in this document will be updated annually to reflect changes in the mix of providers, contact details and the services provided. Issue Date June Last Reviewed February
4 SOUTHLAND MENTAL HEALTH & ADDICTIONS NETWORK - The Southland Mental Health and Addiction Network, called Future Directions was established in 2005, and aims to deliver better services by providers for mental health and/or addiction consumers and their families. It does this by: Including Everyone: The Network has members from all of the major groups working in mental health and addictions including the Southern DHB Mental Health, Addictions & Intellectual Disability Service, NGOs (Non-Government Organisations), PHO (Primary Health Organisations), Health Promotion, Consumers and Families Sharing Ideas: The Network provides the opportunity to share information, discuss ideas and concerns and to collaborate where appropriate Taking Action: The Network develops and implements projects on behalf of all providers, consumers and their families Providing Feedback: Members of the Network take ideas and information back to their own organisations in addition to providing feedback and advice to the Southern District Health Board In Southland there are 9 sectors that represent service providers, consumers and families on the Network Representative Group (NRG). The diagram illustrates the relationship, and each of them has representatives sitting on the NRG. The NRG meet every month to discuss mental health and addiction issues, develop and implement projects and to get updates on projects already running. 5 OUR PROFILE OUR DISTRICT The Southern DHB serves a population of approximately 107,000. The gender of the population is evenly split and the average age is Like the rest of New Zealand, Southland has an aging population. The diagram below indicates the ethnicity of the Southern DHB population with 86.4% of the population identifying themselves as European (NZ 75.4%), 10.3% as Maori (NZ 14%), 1.9% as Asian (NZ 8.8%) and 1.4% as Pacific Peoples (NZ 6.6%). European Maori Pacific Peoples Asian The Southern DHB district is geographically large covering an area of 38,214 km 2. The population is evenly split between urban and rural settings. Main centres include Invercargill, Gore, Queenstown, Arrowtown, Mataura, Winton, Bluff, Riverton, Stewart Island and Te Anau.. One in five people will experience a mental illness in their lifetime. The Governments National Mental Health Strategy requires that mental health services are delivered to the 3% of the population who are most severely affected by mental illness. In the Southland District this means that approximately 3,210 individuals are receiving some form of ongoing mental health care. It is also estimated that the other 17% of the population experiences periods of mild to moderate mental health problems requiring some form of support. 11 INTERSECTORAL AGENCIES Intersectoral agencies play an important part in supporting the mental health and addictions sector. While they are not specialist providers of services, their clientele is often made up of mental Children, Adolescents & Families Education Child Youth and Family Work and Income Community Support Groups health and/or addiction consumers and their families. Consumers and their families often come into contact with many of these organisations out of necessity and sometimes desperation. Intersectoral groups provide invaluable support to the mental health and addictions sector in their statutory roles and also as a provider of Young People Education Police, Justice, Probation Work and Income Community Support Groups support and advocacy services. Intersectoral agencies are members of the Southland Mental Health and Addictions Network which represents the strategic and working relationship that the different sectors share. Adults Housing Work and Income Police, Justice, Probation Community Support Groups The diagram gives an example of the sort of intersectoral services that are available and their populations of interest. Below are some of the services they deliver that support the Mental Health and Addictions Sector. Elderly Housing Disability Work and Income Community Support Groups CHILD YOUTH & FAMILY SERVICES (CYFS) GROUP SPECIAL EDUCATION (GSE) WORK AND INCOME NZ EDUCATION SECTOR HOUSING NEW ZEALAND JUSTICE/POLICE/CORRECTIONS Contracted to provide support services to children and young people with high and complex needs. Contracted to provide assessment and early intervention services to children and young people and post-vention support to schools. Provides income and employment services including in-work support and services to invalid and sickness beneficiaries. Secondary schools and SIT provide counselling services to students. The Southland College of Education deliver mental health wellness training to teachers from decile 1 schools. Contracted to provide community group housing to NGOs delivering Mental Health Services and support to clients with health and disability needs. Contracted to provide services including victim support, prison inmate counselling, addictions treatment, social work services, at-risk Units and suicide and self-harm prevention. 12 PRIMARY HEALTH CARE SERVICES Primary sector health care is normally the first contact an individual has with a health professional, for example a visit to/from a doctor or nurse. It is professional health care in the community where people reside and work. Primary Health Organisation (PHO) Southland has one PHO delivering Primary Health Care services to a total enrolled population of 104,440. The PHO is primarily a grouping of General and Nurse Practitioners. Number of General Practitioners 88 (61.8 FTE) Primary health care covers a broad range of out-of-hospital services - not all of which are government funded. It also covers specialties such as health education and promotion, counselling and disease and injury prevention. NOTE: FTE = Full Time Equivalent. 1 FTE = 40 hours per week Southern DHB-PHO Primary Mental Health Care Team In a Mental Health setting primary health care includes GP, PHO and Primary Mental Health Care Specialist services. The Southern DHB-PHO Mental Health Brief Intervention Service is available to GP patients with a mild to moderate mental health issue. Referrals are received from GPs and the service is free. The service offers access to the Brief Intervention Team with up to five sessions of support, assessment, treatment or referral to appropriate agencies. The service includes: Strengths Assessment, Goal setting, Cognitive Behavioral Therapy, Psychosocial interventions, Crisis resolution, Stress/Anxiety management, Education around illness and illness prevention for patients and/or family members and problem solving skills. A combined team of Nurse Specialists and a Psychologist deliver the service in Southland and are funded by the Southern District Health Board and the Southern PHO. 13 PUBLIC HEALTH SERVICES Mental health promotion is a population based model that targets atrisk groups and the wider community. It attempts to influence and make positive change to the social, economic, cultural, political and physical environments that affect people s mental health. Public Health South Public Health South is a District-Wide provider whose mental health promotion priorities for are Youth Suicide Prevention, Anti-discrimination and general mental health promotion. Initiatives include school programmes, education and training for practitioners and resource and policy development. Marketing, publicity and advertising campaigns also support the promotion of positive mental health messages. The priorities and initiatives are: Mental Health Awareness Week Mentally Healthy Schools All Age Suicide Prevention Mentally Healthy Communities Like Minds Like Mine SPINZ Postvention Workshops Supporting the Mental Health Foundations Skills Bank 2 N/A Invercargill, Queenstown Southern DHB, Schools, PHO, NGOs Nga Kete Matauranga Pounamu Trust Nga Kete Matauranga Pounamu is a district-wide provider who are contracted to deliver the Kia Piki Te Ora All Age Suicide Prevention programme. 14 MENTAL HEALTH CHILD, ADOLESCENT AND FAMILY SERVICES Child, adolescent and family services are traditionally delivered to young people aged 0-17 years of age. No service is delivered to a young person in isolation and families/whanau and carers are always included in the process of diagnosis, treatment and recovery planning. In Southland the Southern DHB Provider Arm is the main provider of secondary specialist services to this age group. Their service consists of a multidisciplinary team delivering from three sites across the Southland District and also includes outreach services. The Southern DHB contracts additional specialist services from a number of community based providers. These providers deliver a range of programmes and services from different locations and to specific atrisk individuals, families and communities. Southern DHB Child, Adolescent & Family Services The Southern DHB Mental Health CAF Service is District-Wide and delivers services to young people and their families aged The service is delivered by multidisciplinary teams in Invercargill, Gore and the Wakatipu. Specialist staff also travel to outreach clinics in Gore, Queenstown and Te Anau. The service also provides educational support and resources. Referral to the service is via General Practitioners, SMHET, other Mental Health and Addiction service providers, schools and self-referral. The service consists of: * Intake Worker * Occupational Therapist * Child and Family Therapists * Nurses * Social Workers * Kai Awhina * Clinical Psychologists * Psychiatrist CAFS also supports families as a part of its role working with children and young people. Families/whanau and care givers are encouraged to be involved in most aspects of the care and treatment of their children. 17 Pact Youth South 372 Invercargill, Gore, Queenstown Pact Youth South, Adventure Development, Southern DHB Mental Health Services, Family Works Pact Youth South delivers a range of support services to young people aged 0-19 affected by a mental health problem. Except for residential services which are based in Invercargill, all other services are delivered district-wide from offices in Invercargill and Queenstown. Pact Youth South employ 10 FTE staff and provide residential services (3 beds), crisis respite care (1 bed), day programmes, community support and family/whanau support. Young people are eligible for the service via self-referral, or after referral from family/whanau, a GP, government departments, community agencies and the Southern DHB CAFS team. 15 Supporting Families Southland Child, Adolescent and Family Services Supporting Families (SF) Southland deliver support and advocacy services to children who have a parent or sibling with a mental illness. They are based in Invercargill and have a District-Wide contract. Services include advocacy, support, education and information. SF Southland employs one field worker. 16 MENTAL HEALTH CHILD, ADOLESCENT AND FAMILY SERVICES - Alcohol and Drug Many young people who experience periods of mental un-wellness, also misuse alcohol and drugs. This is sometimes as an outcome of the mental illness/problem or can be one of the contributing factors. Community based providers deliver specialized alcohol and drug services to young people. These services are available to any young person in the District. Some of the most at-risk include adolescents, Maori, and young people who experience difficulties at school or at home. Adventure Development Counselling Ltd Adventure Development delivers two programmes to young people aged 19 and under. Alcohol and Drug Counselling (3 FTE) and Adventure Development Counselling (1.5 FTE). They are based in Invercargill and have a District Wide contract. The service includes alcohol and drug assessment, intervention, treatment and referral. The programme also includes outdoor adventure activities. Anyone can refer to the service, including Child, Youth and Family, Mental Health and Alcohol and Drug services, schools, GP s and self-referral. 4.5 Mirror Counselling 130+ Invercargill, Gore, Queenstown CAFS, Southern DHB AOD Services, Mirror Counselling, Gore Counselling Centre, Nga Kete Matauranga Pounamu Trust Mirror Services is a Dunedin based counselling service for young people aged 19 years and under. They deliver the Mirror Youth Day Programme (MYDP) for year olds. The programme includes a kaupapa Maori framework, within a mini th group therapy, individual counselling, life skills, A and D education, adventure based games and activities and family/whanau involvement. For Southland clients the programme also includes planned supported accommodation. They accept referrals from Mental Health AOD providers in Southland and are contracted to provide a minimum of 10 places per annum. Contracted Volume 10 Places Per Annum 2+ p/a Dunedin Adventure Development, CYFs Nga Kete Matauranga Pounamu Trust Nga Kete Matauranga Pounamu Trust delivers Alcohol and Drug Counselling services to Rangatahi (young people aged 14-18). The service is based in Invercargill and is a District-Wide contract. One full-time clinician provides assessment, counselling, treatment planning and referral services. A Rangatahi Group Therapy eight week programme is also offered. 17 MENTAL HEALTH ADULT SERVICES Adult services are delivered in the Southland District mainly to the age group. This group makes up the majority of the client group seen by mental health services in Southland and in recent years has increased in size. A wide range of secondary specialist mental health services and resources are available to adults. The Southern DHB Provider Arm is the main provider of services and also contracts with a number of community based organisations. These providers deliver a diverse range of specialist services across the district from a number of different venues and locations. To be effective services are delivered by multi-disciplinary teams providing a wide range of clinical and non-clinical specialties. The goal of mental health services is to provide continuity of care and a seamless transition for consumers and their families between services and on their journey to recovery. Southern DHB Mental Health Emergency Team The Southern DHB Mental Health Provider Arm delivers a diverse range of services to adults aged in both community and in-patient settings. The services are delivered by multi-disciplinary teams. The Southland Mental Health Emergency Team (SMHET) provides a 24 hours a day, 7 days a week mental health crisis intervention service accessed on to 800 Invercargill Southern DHB Inpatient Mental Health Unit Southern DHB Mental Health Services, Police, Pact, SF Southland, GPs The Southern DHB Inpatient Mental Health Unit consists of a 16 bed unit. 13 Acute beds and 3 Intensive Care beds. This multi-disciplinary team provide acute services for those persons requiring treatment in an inpatient setting. The team consists of 37 FTE staff. No. Beds Southern DHB Te Korowai Hou Ora Invercargill Southern DHB Mental Health Services, Pact, Te Kotuku, SF Southland, Nga Kete Matauranga Pounamu Trust Te Korowai Hou Ora provides Kaupapa Maori services within the Southern DHB s mainstream adult, child, adolescent & family mental health services. The service employs a Kaiwhakarite (Manager), Kaumatua, Maori Mental Health Workers, Kai Awhina and Registered Maori Mental Health Nurse. Referrals are made to Te Korowai Hou Ora from all other Southern DHB Mental Health Services. 18 Southern DHB Community Mental Health Teams Adult Services (continued) The Southern DHB has three Community Mental Health Teams. They are multi-disciplinary teams providing individual and group support services from Centres in Invercargill, Gore and Wakatipu (Total of 67 FTE Staff). Mental Health Needs Assessment/Service Coordination (2 FTE Staff). Community Forensic Service: Court and Community Corrections liaison in Invercargill, Gore and Queenstown and prison liaison in Invercargill (5.3 FTE Staff). Maternal Mental Health Service: For clients who are pregnant and new mothers and their babies (2 FTE Staff). Mental Health Older Person Service (4 FTE Staff). 21 Eating Disorders Regional Specialist Services (continued) The Eating Disorders Team provide a regional consult liaison service. The service is a partnership between the Southern District Health Board and Canterbury District Health Board. The purpose of the service is to provide specialist Eating Disorders Team support and guidance to Southern DHB staff and to develop and maintain links between Eating Disorder Teams. The service includes assessment, the development and review of treatment plans, access to specialist support (eg dieticians), and provides ongoing education and training to staff. No. Beds = 0.8 Forensic The Southern DHB Mental Health, Addictions & Intellectual Disability Service has regional links with the Canterbury DHB specialist Forensic Service. Services includes clinical support and specialist oversight for the Southern DHB Forensic Team. One prison and one community clinic are held each week. 22 MENTAL HEALTH ADULT SERVICES - Alcohol and Drug The misuse of alcohol and drugs often has a compounding effect on any adult who has a mental illness. This misuse is sometimes as an outcome of the mental illness/ problem or can be one of the contributing factors. A wide range of secondary specialist mental health alcohol and drug services and resources are available to adults. The Southern DHB s Provider Arm is the main provider of alcohol and drug services and also contracts with a number of community based organizations. Southern DHB Drug and Alcohol Specialist Service The Southern DHB Mental Health, Addictions & Intellectual Disability Service delivers a diverse range of services to adults aged in both community and inpatient settings. The services are delivered by multi-disciplinary teams from Southern DHB Mental Health Centres in Invercargill and Wakatipu. The service consists of: Drug and Alcohol Services: Outpatient treatment programme including individual counseling and follow-up. They also provide a methadone programme and referral to problem gambling services. 24 MENTAL HEALTH ADULT SERVICES - Accommodation and Residential For mental health consumers who experience a long-term or acute illness intensive support is often required. Residential and accommodation services provide consumers with short and long term options for care at different levels of intensity. The Southern DHB contracts these services from community based providers. Respite care, which is normally always planned, provides consumers with short-term relief. Those who are more dependant and acute will often receive care in level three or four homes. These services are based in the community and attempt to provide many of the norms and routines that most people are used to. For those aged over 65, care is often provided by Rest Homes. These consumers also receive additional support from secondary care specialist services. Pact Southland Pact Southland is the largest provider of supported accommodation services in the Southland District for Mental Health Consumers aged 18 and over. The service is based in Invercargill and is a District Wide contract. Supported accommodation is for people who are recovering from a mental illness and who are unable to live on their own. Eligibility for these services requires a referral from a health professional or the Southern DHB Mental Health Needs Assessment Service. Pact provides the following range of services: Supported Accommodation Service: 24/7 awake support provided in a community of seven modern 3 bedroom houses (16 beds available). Care includes rehabilitation programmes as identified in individual personal plans to promote recovery and independence. Respite Services: Support is provided to consumers who live independently but who need temporary relief from caring for themselves. Planned and emergency accommodation is available. Three respite beds are available. Contracted Level 4 Beds 12 Contracted Level 3 Beds 4 Kakakura Health Services - Te Kotuku Contracted Respite Beds 3 Invercargill Southern DHB Mental Health Services, GP s, Work & Income, Te Kotuku Te Kotuku is a Kaupapa Maori 24 hour residential rehabilitation and planned respite facility (Level 4) for adults who experience mental illness (non-maori clients are also accepted). Te Kotuku employ 6.2 FTE staff and the service is delivered from two houses in Invercargill with 7 general beds available, and 3 respite beds. They also employ one full-time community support worker (Kai Awhina). Te Kotuku has a District-Wide contract and access to the service requires a psychiatric diagnosis and needs assessment. 25 MENTAL HEALTH ADULT SERVICES - Consumer Support It goes without saying that consumers are an integral part of the Mental Health and Addictions sector. As well as delivering quality services, the primary and secondary sectors have a responsibility to ensure that consumers are involved in the planning, development and delivery of mental health services in the District. Southern DHB Consumer Advisory Services The Southern DHB Mental Health, Addictions & Intellectual Disability Service employs one full-time Adult Consumer Advisor and one part time (0.5 FTE) Youth Consumer Advisor. Advisors have important internal and external roles. Internally they review and comment on in-house policies, procedures and day-to-day operational issues. Advisors sit on interview panels and are involved in planning and priority setting activities. They also advise on behalf of consumers at inhouse meetings, forums and Hui. Externally, Advisors play an important role in liaising with local Consumer groups and networks - sometimes taking a lead role. They also sit-on and attend district, regional and national forums and committees. The Southern DHB does this through the employment of Consumer Advisors and by supporting the activities of other local consumer led forums and groups. Consumer representation is 1.5 N/A Invercargill Southern DHB Mental Health & Addiction Services, Moving Forward Consumer Advisory Council also a priority for the Southland Mental Health and Addictions Network. The Southern DHB also supports consumers by contracting with consumer led providers who deliver education, advocacy and support programmes and services to other consumers and their families. 28 MENTAL HEALTH ADULT SERVICES - Activity, Rehabilitation, Education & Employment An important component of any recovery from a period of mental unwellness is rehabilitation and activity. No two people will experience the same journey to recovery. For some, the journey is made easier by receiving ongoing support in the community. For others, activity and work related programmes facilitate their recuperation and recovery. Activity programmes can, for some, also provide a stepping stone to further educational and employment opportunities. Southern DHB Activity Centres Southern DHB Mental Health, Addictions & Intellectual Disability Services run three Activity Centres in Southland. Rata House and 494D in Invercargill and Welcome In, in Gore. Welcome In is supported by the Gore and Districts Mental Health Support Group. The Activity Centres are free to attend and available to consumers referred from Mental Health and Addiction Services. The Centres provide a safe, supportive environment with an emphasis on strengths, self determination, wellbeing and recovery. Rata House is open from 9am-3pm, Monday to Friday and 10am-3pm on weekends. 494D is open from 8.30am-5pm weekdays and Welcome In from 10am-4pm weekdays.
