3 Our service at a glance Rosebank House in Caversham near Reading is an independent hospital providing recoveryfocused 24-hour care and support for men and women with severe and enduring mental health problems, including those detained under the Mental Health Act. What do we offer? A comfortable setting, with twelve en-suite rooms (four for women and eight for men) over two floors, plus spacious communal areas, including kitchens and lounges. Rosebank House provides a step-down from acute wards; we re ideal for people who have had a period of rehabilitation and are now ready for the next step in their recovery pathway towards a less supported placement in the community. We are also a step-up for anyone in the community who is struggling or has had a placement breakdown. We support people to take part in the local community and make the most of educational, employment and social opportunities on our doorstep. We are able to accept referrals for service users from all over the UK. What sort of support can service users expect from us? We work in partnership with service users, supporting them to develop daily structures and activities to reach the goals and milestones of their recovery journey. By doing so we support them to develop or rediscover the skills and confidence to return to community living, and live more independent lives. Service users may have some physical conditions, so we support them in learning to understand these and give them guidance in staying healthy. 3

5 Individual care and support At Rosebank House, we approach everything with an open mind and see the whole person. We believe that service users all achieve at their own pace, yet whatever that pace, they can make a better life for themselves. We support and encourage them to take responsibility for their recovery. By working in collaboration with the service users, we ensure they play an active part in planning and reviewing their recovery through their care plans. These plans are focused on setting realistic goals and striking the right balance so that service users can take the positive and managed risks, fundamental to their recovery. Independence and self-respect Service users come from a range of different backgrounds and may have spent a long time in institutions. As a result, they may have lost daily living skills, like cooking and laundry. We support them to regain these, as well as economic skills while developing their confidence to look at educational and employment opportunities. Redeveloping these skills is essential to them being able to live more independently. We take a person-centred approach, tailoring activities to each person, based on their individual need. Every service user is different, and their goals and how to achieve them will be different. The sense of achievement felt by a service user when progress is reviewed improves their selfesteem enormously and encourages them towards the next milestone in their recovery journey. A joined up, professional team The staff team at Rosebank House includes registered mental health nurses, recovery support workers, a clinical psychologist, consultant psychiatrist, occupational therapists, a chef and an administrator. We also work closely with service users family and friends, where appropriate, as they are often part of the goals and aspirations of service users. 5

6 Strong community ties We re close to local amenities, like shops, churches, GP services and a Post Office, and we re only ten minutes on the bus from Reading town centre. We have strong links with local colleges, work placements and support groups in the local Reading area, giving service users access to a variety of opportunities. Some service users attend the following: Way and Means: This is a service where service users can receive training, education and work experience to support them in finding employment. New Directions: This service is run by the local borough council and provides training courses in computer skills, maths and English. People at Rosebank House always have time for me. I am able to achieve at my own pace. 6

8 How we work We work with service users to help them achieve their unique recovery goals and aspirations. Below are just some of the key tools we use to make this happen. Our service philosophy Everything we do promotes recovery. This may sound obvious but the difference with our services is that we link everything we do back to promoting the recovery of the people we support. This is why we developed our service philosophy a set of ideas and beliefs that guides how we work across all our services, including Rosebank House. The philosophy was created with the help of service users, and is made up of five areas: our mission (the core of our philosophy), our beliefs, behaviours, experience and outcomes. The philosophy is allencompassing and integrated into every aspect of how we work, including our training and individual staff objectives. 8

10 Recovery Star We use this tool to involve service users in planning every aspect of their recovery towards a fulfilled life and a positive future. The service user is central to each stage: preparing, scoring, evaluating and planning. These elements form the basis of their recovery support plans. Wellness Recovery Action Planning (WRAP) WRAP is a very effective self-management tool developed by people with mental health difficulties to help them create their own recovery action plan. Service users look at their own personal resources and use these to set their goals, describe the help they need to get there, and what might trigger their problems. WRAP is extremely helpful for emphasising an individual s sense of control over their mental health problems. 10

12 Rosebank House Our pathway Assessment An initial assessment will be carried out within two working days. We produce a report outlining how we will meet the individual s needs, which is submitted to the funding panel. Preliminary visit Once a placement offer has been made, we like to invite the service user to come to Rosebank House for a preliminary visit for them to see the service for themselves and start to get to know the team and other service users. Admission criteria: We specialise in treating people who are experiencing: Schizophrenia Obsessive compulsive disorder Autistic spectrum disorder Personality disorder Dual diagnosis Substance misuse Bipolar disorder Transition During this time they will meet other service users to start building relationships, and get to know the unit, the area, and our team. They will be introduced to their key worker who will begin to build the recovery plan with the service user, looking at what they like, what activities and interests they have and what goals they would like to work towards. Admission A service user s care plans and risk assessments are set up. The occupational therapy plan, assessment routines and recovery care plans are all based on the person s needs and aims. We look at what interests the service user has or what activities they would like to get involved in both within Rosebank House and within the local community. Service users also welcome to decorate or personalise their room if they wish: we support them where necessary to do this. Establishing an individual programme We have one-to-one recovery meetings with the service user as regularly as they need it, going through the Recovery Star and the goals they are aiming towards. We support them to complete the Recovery Star with information about where they feel they are and what they need to do to progress. We always incorporate their current interests and hobbies, as well as new interests they re discovering. The journey We undertake regular reviews with each service user and celebrate their milestones on the way to discharge. Service users have short term, intermediate term and longer term milestones to achieve. Together we work towards achieving them and preparing the service user for discharge to more independent living. We work closely with the community care teams to ensure the plans for discharge are realistic, achievable and most importantly personalised to the needs of the individual service user. Exclusions: We do not admit people with a history of sexual offending, or a recent history of repeated absconding

14 Meet the manager John Kelly became the Rosebank House manager in Here he tells us how he promotes recovery at the hospital. I have fifteen years experience in psychiatric nursing, and have worked in many different areas; from substance misuse to A&E. I ve also worked as a community psychiatric nurse in Berkshire and London. I enjoy promoting empowerment and selfworth among service users, and I aim to approach everything with an open mind. The recovery support workers support service users to set up daily plans and structure, looking at what we can offer in-house and what is available in the community. With support from the OT, recovery support workers also hold weekly group meetings on such subjects as how to use a mobile phone, shopping for and cooking healthy meals, and what to do in a crisis when they re living in the community. Occupational therapy sessions are held as often as needed to support service users to set realistic and achievable goals within their recovery plans. The whole team at Rosebank House work hard to encourage and support service users to be able to see that even with a mental illness, they still have the right to go to work, live in the community and feel part of the community. We help service users recognise that they don t need to live in a hospital or a care home for the rest of their lives and that, with support, they can live in a flat or house, pay their own bills and live an independent life. Three service users who have moved on from Rosebank House recently to supported living have achieved more than they ever thought they would. One told me: I have a front door now and my own cooker, washing machine and I can invite my friends and family around. I was given the chance to go back to college and am now working towards a maths qualification. I can also have a bank account that s not controlled by anyone else. It s difficult sometimes and I have to budget but it enables me to appreciate money and where it comes from. Thank you for giving me the chance to see there is more than just living in a hospital for the rest of my life. Rosebank House gave me hope and the staff believed in me. 15

15 Sally s story Rosebank House has helped Sally regain confidence and essential daily living and social skills, which she d all but lost after years in hospital environments. Growing up in Africa, Sally struggled with the everyday challenges of family life and had a difficult relationship with her parents. At five years old she was sent to boarding school in the UK where she showed academic potential and a gift for sports, but found it increasingly difficult to fit in. By the age of 18, Sally was living alone in a rented room and her mental health deteriorated rapidly. When she started to experience visions and voices in her head, her family admitted her to hospital. It was to be the first of many hospitalisations for Sally as she became progressively and seriously unwell over the years. A common pattern soon emerged with each admission she was nursed better, but relapsed once she returned home. The journey to recovery In 1989 Sally was admitted as a long term patient at a specialist mental health hospital where she received treatment for schizophrenia, bipolar disorder and associated suicidal tendencies. Four years on, the closure of the hospital saw Sally become one of the first service user of the brand new, purpose-built Rosebank House. Here, she enjoyed the privacy of her own en-suite room and benefited from a personalised, recovery-focused programme. With daily support, Sally followed a care pathway which worked towards giving responsibility back to her, and in turn this encouraged her to gain confidence and improve self-worth. Sally begun to regain the essential daily living and social skills, which she d all but lost after spending so many years in a hospital environment. Sally has made enormous progress and is almost ready to begin living more independently. Through our association with a local occupational therapy and training organisation, she is now enjoying paid employment. 16

17 About Care UK Rosebank House is one of a number of services run by the Mental Health division of Care UK, the UK s largest independent provider of health and social care. Care UK works in close partnership with local authorities and NHS commissioners to run a broad range of services, from urgent care services and GP practices to mental health facilities, offender health services to care homes and homecare services for older people. Our Mental Health division provides recoveryfocused services for people with diverse and complex mental health needs. We work in a range of environments, from hospitals to residential care settings and supported living facilities. We help people with a range of long-term issues including severe anxiety, depression or psychosis. We also provide specialist services for people living with eating disorders or self-harm. Whoever we work with, our focus is on recovery and empowerment in a safe and positive environment. We create care plans personalised to each individual, with the aim of building motivation, wellbeing, confidence, living skills and achieving attainable goals. With the right support, everyone can achieve their potential. 18

18 Your questions answered How long do people typically stay at Rosebank House? In general between one and three years, but it depends on the individual. How do I refer someone to the service? Referrals can be made by telephone, or in writing. The referrer is welcome to visit Rosebank House and discuss the service with the multidisciplinary team. What are some of the ways service users get involved in the local community? Service users enjoy using local services and leisure facilities and undertake study at local colleges, or even look for voluntary or paid work opportunities. What sort of environments have service users typically come from? Typically service users come from acute wards, out of area placements, locked rehabilitation or secure hospitals, and are looking to progress to the next stage of their recovery journey. Do you accept people from all over the UK, or prioritise from the local community? Most of our service users are from the Berkshire area, or are moved back from out of area placements. However, we do accept referrals from all over the UK. 19
