1 Community Health West Location Access to Healthcare Services Objective A: Increase access to primary care and to a regular source of care New general pediatrics clinic opened at 180 th and Harrison 6/2016 Same Day Pediatrics implemented at Harrison providing a 3 rd location for BTNRH and community patients to receive sick visit care outside of an urgent care or ER; visits available Monday through Thursday Pediatric services directories available in English and Spanish at each service location, distributed to referring physician offices, and sent directly to patients Patient portal usage increased in 2016 Pediatric provider relocated to Harrison Street clinic fulltime to accommodate patient demand Improved availability and utilization of existing Social Work and resource services to assist patients in accessing community resources Fully complied with criteria for well child checks, child immunizations and upper respiratory infection detailed in the BCBS Primary Care Medical Home (PCMH) initiative Page 1 of 6

3 Objective C: Use the work of the Center for Neurobehavioral Research to improve clinic outcomes Promoted the Center at the Healthy Kids Carnival 8/2016 by having staff from the Center present to discuss their work and recruit research subjects Asthma/ Asthma Vision and Hearing Objective A: Improve access to pediatric allergy/asthma care Work with BTNRH pediatricians to improve care and service to pediatric patients with allergy and asthma diagnoses Objective C: Better manage allergy/asthma in children through targeted health education for children and parents Objective A: Standardize vision screening for children Recruited an additional allergist, Dr. Jill Hanson, who provides services at the Lakeside clinic 7/2016 Asthma and smoking cessation was discussed with parents and children at Well Child visits. Well Child reminders were sent to parents as per their preferred communication method Parent education available through the Knowledge Center on the hospital websites. The Center includes free access to Pediatric Advisor (available in 10 languages), health articles and Boys Town provider created education podcasts and videos Asthma education was provided at the Healthy Kids Carnival event Continuing to participate in the BCBS PCMH initiative and data generated through it to improve clinical processes and outcomes Education provided to pediatricians by the pediatric allergists regarding care and treatment for pediatric patients with a diagnosis of asthma Finalizing schedule for new pediatric allergist to provide education in 2017 at Parent Talks and community offerings Implemented routine vision screening on all children at 3yr well check Requested and received approval to purchase Page 3 of 6

4 Improve identification and management of patients with hearing loss Injury and Safety Objective A: Prevent common childhood injuries through multi-lingual education and parent/patient trainings on appropriate age based safety practices Promote use of car seats and seat belt restraints for children Objective C: Promote access to information and resources for parents and teens related to age appropriate safety topics standardized vision screening technology across all pediatric clinic locations with a planned implementation of 1 st quarter 2017 Clinic visits in ophthalmology show an increase of 38% 2016 compared to 2015 Formalized planning initiated for implementation of Hearing Loss Clinic in 2017 at 30 th Street location Increased educational materials available in ten languages through the Pediatric Advisor link on the Boys Town website Posted web ads, podcasts and videos related to child safety topics Education sessions conducted at Newborn Expos addressing safety issues with newborns 3/2016 and 9/2016 Newborn Expos held for parents at the West Hospital location 3/2016 and 9/2016; over 70 checked for placement, 9 new seats provided for families whose current seats were not safe for their children Information related to car seat and seat belt safety for children provided at the Healthy Kids Carnival event 8/12016 Promoted use of 24 hour crisis line and its multi-media and multi-lingual resources through distribution of yourlifeyourvoice cards for kids, teens and young adults at the Healthy Kids Carnival 8/2016 and through card placement in all Boys Town pediatric clinics Additional resources added to Boys Town website promoted to patients and families Nutrition, Objective A: Maintained referral links for Children s Hospital obesity program Page 4 of 6

The Center includes free access to Pediatric Advisor (available in 10 languages), health articles and Boys Town provider created education podcasts and videos Free monthly Healthy Children e-newsletter, each including an article on a general pediatric topic, behavioral topic and seasonal topic, sent to s Offered space to local community agencies, merchants and restaurants to promote healthy nutrition and physical fitness at the 8/2016 Healthy Kid Carnival; healthy snacks only offered at the event BTNRH dietician referrals for weight management for pediatric patients showed an increase in Parent Talk series offered sessions on nutrition including: breastfeeding 1/2016, Quick and Healthy dinners 3/206, Quick and Healthy snacks 6/2016, and Toddler Nutrition 10/2016 in conjunction with local Hy-Vee grocery stores in an effort to address nutrition and obesity in children Sponsored the Memorial Day run for both employees and their families and community participants; events included a 400 meter kids run, one mile walk/run and 5 mile walk/run Oral health addressed at all well child encounters and written education materials provided in 9 month, 12 month, 15 month and 24 month well check packets in English/Spanish Formalized application of fluoride varnish to all pediatric primary care patients at 9 month, 18 month and 24 month visits Collaborated with the Nebraska Dental Association to participate Collaborate with community partners to in the Newborn Expo held on 3/2016.
