3 An Integrated Regional Falls Program Success Story Margaret had been referred to the program by CCAC and attended a screening clinic in Orillia. At the clinic Margaret discussed her fear of falling and falls prevention strategies. Margaret was motivated to maintain her independence by improving her gait through exercise, balance, and muscle strength. 3

5 History The Provincial Falls Prevention Project was initiated in November 2010 as a LHIN Priority Project The project was co-led by LHINs and Public Health The objective is to improve the quality of life for Ontario Seniors aged 65 years and over and lessen the burden of falls on the health care system by reducing the number and impact of falls This collaboration between LHINs and PH was the first of its kind in Ontario and will serve as a template for future collaborations

7 Shared Accountability Both Public Health and LHINs have similar accountability indicators for falls initiatives Public Health Indicator Fall-related emergency visits in older adults aged 65+ (rate per 100,000 per year) NSM LHIN Falls Indicators Total number of falls resulting in harm (Severity of Harm Scale) Total number of clients visiting a hospital ER due to a fall in the last 90 day

12 Current State of NSM Falls Program North Simcoe Muskoka Local Health Integration Network (NSM LHIN) Staying Independent Falls Prevention Coalition Program is housed at Orillia Soldiers Memorial Hospital (OSMH) 1.35 Million dollar investment Full-time Coordinator Specialist & screening clinics throughout the region Financial Agreement between the NSM LHIN and OSMH Governance structure & accountability agreements with community partners Simcoe Muskoka District Health Unit has representation at the Coalition Government funded agencies and organizations who cover the region, some provincially and nationally Volunteer-based Not for profit charities Many members are graduates of the Canadian Fall Prevention Curriculum Contracted OT as part of funding 12

13 Future State of NSM LHIN Falls Program Clinical Community NSM LHIN and SMDHU co-chair, each bring unique resources to the program, OSMH will provide all administrative support 13
