4 Service Area: Healthy Living Service 1. SCDSB Kathi Wallace Director We have a liaison PHN to each school board that facilitates addressing health issues identified at the school/school board level and also facilitates inclusion of/comprehensively addressing health unit priorities at the school board. This partnership has been in place without a formal agreement for over 10 years. The liaison PHN has a key contact identified in each board usually at the superintendent level and has regularly scheduled meetings to review issues of mutual concern/benefit. Almost every program at the health unit has OPHS standards that relate to schools or the key stakeholders of parents, educators etc school are our main access points to the population we are trying to access. Both the SCDSB and the SMCDSB are also active partners in the COMPASS and Student Support Leadership Initiatives and participate on the coalition. 2. SMCDSB Brian Beal Director see above 3. TLDSB Larry Hope Director see above in place since merger. The TLDSB hosts regular meetings with community partners in the Muskoka area (including the health unit) 4. Centre for Addiction and Mental Health 5. Barrie Community Health Centre Susan Lalonde-Rankin Susan is a regional education consultant with CAMH. She has worked with health unit staff on several drug and alcohol misuse prevention projects intensively over the past 10+ years. Christine Colcy The Barrie Community Health Centre has worked with the Chronic Disease Prevention Tobacco program to increase the health center staff s capacity to provide tobacco cessation interventions and supports. 6. The South Simcoe Service Coordination Committee Michelle Bergin, Co-Chair All of the social service groups in South Simcoe meet on a regular basis through this committee so it would be a great way to connect with a whole group of service providers.
