1 Animal Use and Care CITI Registration (New CITI Users) 1. Visit 2. Next New Users click Register Here 3. For section (1) Select your institution or organization a. Under Participating Institutions select City University of New York (CUNY) from the drop down menu b. Do not select anything under Veterans Affairs, Department of Energy, and HIV/AIDS Network Coordination 4. For section (2) Select your Username and Password a. Create a Username and Password according to the specifications given. (Write this information down somewhere, so you can access your account at any time in the future.) 5. For section (3) Create your Security Question and Answer a. Please create a security question that will be asked of you should you forget your password. 6. For section (4) Enter your name a. Enter your first name and then your last name 7. For section (5) Enter your address a. Enter your address 8. For section (6) Gender, Ethnicity and Race a. Answer A through C 9. For section (7) CME/CMU credits a. Select No 10. For section (8) Course Survey a. Select if you would like to participate in a Survey regarding the course you take. Otherwise, select No 11. Once you have completed sections 1-8 on this page, click Submit (located at the bottom of the page) a. Select your Language Preference b. Type your Institutional address (the address assigned to you by CSI) c. Select your Gender d. Select your Highest Degree Completed e. Skip Employee Number f. Enter your Banner Number if you are a CUNY Graduate Center faculty, staff and student

2 g. Type the CSI Department you are affiliated (biology, psychology, sociology, etc ) h. Select your Role in ( Assistants should choose undergraduate or graduate research assistant, Vivarium staff should choose Lab ; Principle s should choose Principle ; Co-s should choose Co- ; Veterinarians should choose Veterinarian ; members and staff should choose the appropriate designation.) i. Address information is optional j. Type your Office Phone (your primary phone) k. Additional phone number is optional l. Once you have completed the sections on this page, click Submit (located at the bottom of the page) 12. Click the circle next to Take the Working the Course. a. Do NOT Select Human Subjects Refresher Course, Responsible Conduct of, Conflicts of Interest, or Human Subjects Course. 13. Click Next Question (located at the bottom of the page) 14. Click the circle next to: a. Members ONLY if you are a member of the. b. Administrators and Facility - ONLY if you are a staff member of one of the Vivariums, an Administrator, or Institutional Official. c. Team if you are on a particular research protocols as PI, CO- PI, student, faculty, staff, or any other key personnel key personnel. 15. Click Next Question (located at the bottom of the page) 16. You are not registering another institution at this time (if you think this is wrong, please contact the Office at ). Therefore you should Click NO 17. Under Status you will see Not Started Enter or Not Passed - Re-enter, click on the word Enter or Re-enter a.
