1 Summary: This document provides a summary of the patient demographic enhancements incorporated in the July 2012 Update, including changes to the entry of patient names, the charting of patient address information, and the options for searching patients. Patient Registration Prior to the July 2012 Update The Patient Name field is a single field for both first and last name allowing a maximum of 45 characters. Once complete, you click Register to finish Patient Registration. Moving to the referral involves a separate step of canceling out of registration and then searching the patient and finally selecting Patient Referral. Patient Registration Screen as of the July 2012 Update The Patient Name field is now split into Last Name/First Name with each field accommodating a maximum of 25 characters. Middle names or middle initials should be recorded in the first name field. Once you have clicked Save, you now have new options to choose: Register or Register and Refer, which will take you automatically into NDoc Patient Referral Screens Thornberry Ltd. Page 1 of 6

2 Patient Referral Demographics and Patient Summary/Visit Address Prior to the July 2012 Update The Demographics category within Patient Referral is a single screen capturing the patient address, visit address, and directions. The visit address is identified using the relationship table. When Self (Patient) is selected, the Visit Address is auto-populated with the existing patient address. Directions are then recorded for the visit address. Once charted, the visit address displays within the Demographics tab of the Patient Summary and within certain Reports Thornberry Ltd. Page 2 of 6

3 New Patient Demographics - Visit Address Enhancement as of the July 2012 Update Patient Demographic charting (within the Referral, Visit Charting and Patient Profile) is now expanded to three screens. 1. The first screen consists of general patient demographic data (e.g., gender, religion, company, etc.). 2. The second screen consists of address information. 3. The third screen consists of patient name specifics. REFRESHING ADDRESS INFORMATION Users should note that in certain cases entering patient address and phone data after the Visit Address Radio Button has been engaged will not automatically see the new or edited data within the Patient Summary or in related reports. To ensure visit address data is updated to the Patient Summary and reports, users should reactivate the Visit Address Radio Button using either of the following steps: Click the button and click CLEAR to remove the flag and then click the button again to activate. Toggle to another visit address (i.e., Temp Address 1 or 2, etc.) and then return to the actual visit address and click to activate the button again. On Screen 2, the patient address info is now separated into a new Permanent Address section and up to three new Temporary Address options. The Visit Address is now identified using a radio button to the far right of the screen. Temporary address information can now be captured to support a patient who stays or begins in a nonpermanent location and is managed by From/To date entries Thornberry Ltd. Page 3 of 6

4 Here the Permanent Address is identified as the visit address by the radio button entry and directions are charted. A new field, Address Change History, has also been added to track changes to the permanent address. Since this is the initial entry, there is no previous history. The Demographics section of the Patient Summary will continue to list whichever address is designated as the visit address. A temporary address (1, 2, or 3) may instead be designated as the visit address. In that situation, the Patient Summary will list the temporary address and directions as the visit address rather than the permanent address Thornberry Ltd. Page 4 of 6

5 Patient Visit Address Mismatch POST the July 2012 Update In certain cases, the patient s visit address may not match the permanent address. When this occurs, the July 2012 conversion will result in the following: The Patient Summary/Demographics and all associated reports will reflect the Visit Address entered prior to the update. In Visit Charting and Patient Profile, the Patient\Demographic screen will indicate in the Temporary Address/ Directions field that a mismatch was identified. See below for steps to be taken. When the July 2012 update is installed, to convert the previous visit address charting into the new fields, NDoc will compare the visit address with the patient s permanent address. If a mismatch occurs, the radio button identifying the visit address will not be automatically flagged. The visit address details will be retained in Patient Summary until the Visit Address Radio button is activated. In Visit Charting and Patient Profile, the Temporary Address 1 Directions field will identify any mismatched data between the permanent address and the visit address with the following statement: Visit Address Info entered prior to [date of update installation] has been carried forward for reference, because it did not exactly match the permanent address. Whose Residence: Visit Phone: Visit Address: Directions: Staff will need to select the Visit Address radio button that applies and the Patient Summary\Demographics display will update. In addition, the Address Change History field will show previous entries for ALL patients regardless of any mismatch Thornberry Ltd. Page 5 of 6

6 NEW Patient Search and Patient Name Options as of the July 2012 Update Patient Demographics SCREEN 3 now has options for Patient Name. Using the Preferred Name radio button, users can identify a patient s Preferred Name. At this writing, the Preferred Name does not have any associated logic, but will have additional capabilities in the future to display patient information. NEW Patient Search Enhancement as of the July 2012 Update Users can also now document, Also Known As and Prior Name(s) associated with a patient. The Patient Search now has expanded search capability, providing the ability to search for patients by their prior or AKA names or for names that start with or contain the search characters entered. For example, screen shots to the left illustrate a patient search using Starts with as the option. The second search uses the AKA Name search as an add-on and you have Patient, Bobby with the official name in parentheses. Patient with Middle Initial POST the July 2012 Update From a patient search, any entry with middle initial charted prior to the July 2012 will display as expected Thornberry Ltd. Page 6 of 6
