4 A bit about the BC Center for Palliative Care Provides leadership to promote excellence in palliative and end of life care for people with serious, advanced lifelimiting illness Established in 2013 under the Institute for Health System Transformation and Sustainability (IHSTS) Funding from the Ministry of Health in support of the BC Provincial End of Life Action Plan Works collaboratively across all sectors (health and public) to promote education around end of life care issues and support evidence-based practice and policy development 4

5 A bit about the BC Hospice Palliative Care Association (BCHPCA) The BC Hospice Palliative Care Association (BCHPCA) was: established in 1986 by individuals and organizations committed to the advancement of hospice palliative care in British Columbia one of the earliest hospice palliative care provincial associations in Canada The BCHPCA s mission: To lead in promoting responsive, quality hospice palliative care in British Columbia; educating British Columbians on personal advance care planning and advocating for equitable access to responsive quality care 5

7 Why the Seed Grant Began Hospice Is Not A Place, But A Philosophy Of Care Now, with added support, dreams that were once only wished for communities can now become a reality. 7

8 The Benefits of the Seed Grant Program For BC communities: Support for people affected by serious illness, including the family caregiver To BC Hospice Societies: Opportunities to: Implement a new approach, or to provide a proven program offering to a new audience Gain and share new information with others, Showcase the wonderful work you are doing in your community! 8

9 The Process/Guidelines For The Seed Grant Program Timing: 3 cycles of Seed Grants to BC Hospice Societies Cycle one -February 8 th, 2016 Cycle two -Fall of 2016 and Cycle three-spring of 2017 Funding Provided by BC CPC, through a grant from the BC MoH Awards (up to $5,000) to be transferred to each successful applicant (awardee) in several payments: The first payment- after receiving the award acceptance letter from the awardee The other payments- delivered following submission of the Progress and Final Reports 9

10 Seed Grant Application Types Application Types There are two types of applications: Application A - for applicants who have a specific project idea of their own and interested to apply for a Seed Grant to fund this project. Application B - for applicants who: do not have a specific project idea and are interested to apply for a specified project designed and put forth by the BC CPC. This specified project may vary for each cycle. For this cycle (Cycle 1): Volunteer Facilitated Group Advance Care Planning Sessions 10

11 Seed Grant for Volunteer Facilitated Group Advance Care Aim of Sessions To empower the public with tools to think and talk about what matters about their quality of life in relation to changes in their health and the care that may be required. To empower volunteers within hospice societies with the education & resources needed to support public ACP info sessions. Planning Sessions 11

12 Eligibility for the Seed Grant Program Have current membership with the BC Hospice Palliative Care Association (BCHPCA). Demonstrate the ability to engage members in their projects (i.e. public, local community and regional Health Authority). For Application B: Application B awardees are able to commit at least 3 ACP workshops within the time period of their cycle. 12

13 Restrictions Awards cannot be used: To replace current funding, or as operational funding for an existing program or service. For equipment purchases or leases. For projects focusing on topics of assisted suicide, euthanasia or related matters as these topics occur outside the scope of Palliative Care. 13

14 Funding Priorities Projects must address at least one of the four following Strategic Priorities for EOL Care in BC: Promoting and facilitating public and private conversations about Advance Care Planning (ACP) and care at end of life (EOL); Promoting and facilitating citizen engagement in supporting people with serious illness and their families; Awareness and education of the public, providers, and informal caregivers about palliative care and early integration of the palliative approach to care; Initiatives that engage & empower the local community to look after terminally ill patients, provide palliative care closer to home, & improve the end of life experiences in the community. 14

15 Funding Priorities cont d Rural and Remote Communities Specialized Populations Special consideration may be required in the designing/delivering of sensitive topics: Those from diverse cultural backgrounds; Young adults living with palliative conditions; Aboriginal peoples; Individuals with dementia or other chronic mental health and substance use issues. 15

16 Application Process Deadlines for Cycle 1 To apply for funding, organizations must submit: A Letter of Intent by to seedgrant@bc-cpc.ca no later than February 15, 2016, at 5 PM PT. This letter of intent is not binding, and will be used solely for registration purposes. A Full Application Package by to seedgrant@bc-cpc.ca no later than March 04, 2016, at 5 PM PT. 16

17 The Review Process Evaluations by a three-member Review Committee (RC) will be comprised of: Community and academic health professional backgrounds who are free from conflict of interest with potential applicants. Selection Criteria Each proposal will be evaluated according to each of the following: The Opportunity The Team Plans for Program Development Potential Outcomes Successful applicants Will be notified of results by by March 24th, Letter of acceptance to acknowledge funding required 17

18 Other Requirements Required Reports Progress and Final Reports Forms will be provided by the BC CPC. Award installments will be built into the receiving of the Implementation Report and Evaluation Report. Awardees Presentations Brief presentations of the project at one or more public event held by BC CPC or its partners. 18
