2 Symposium Overview 2014 NATIONAL PRIMARY CARE SYMPOSIUM ON AGING Cornerstones for Improving Senior Health Outcomes: Outreach, Enrollment and Community Partnerships ALEXANDRIA, VA JUNE 9, 2014 Overview The National Primary Care Symposium on Aging is hosted annually by the National Center for Health and the Aging (NCHATA). The Annual symposium will take place at The Westin Alexandria in Alexandria, Virginia on June 9, All professionals, especially health center workers who serve the aging population, are invited to submit abstracts on topics pertaining to this year s theme: Cornerstones for Improving Senior Health Outcomes: Outreach, Enrollment and Community Partnerships. Presentations must address issues relevant to: The aging process Clinical health Community services for the aging The promotion of the well-being of older people through the scientific study of the social, political, and economic contexts of aging Conference Goals/Purpose This event is a great opportunity to network with others and discuss relevant senior health issues such as physical and mental health, the elder workforce, multicultural aging, and the latest research findings that improve access to quality care and health outcomes of all seniors. We encourage health center and look-a-like administrators, clinicians, senior advocates, and others who are interested in providing better quality health care to the underserved senior population to submit an abstract.

3 Symposium Workshop Tracks Workshop Tracks: This annual meeting will present subject matter experts, evidenced-based practices, resources and tools focusing on the following three (3) workshop tracks: 1. Fiscal and Program Management Collaborative Care Relationships/Delivery Financial Management Emergency Preparedness Planning Health Care Workforce 2. Performance Improvement Primary Care and Enabling Services: Replicable practices of preventive, intervention, chronic and specialty care delivery models for the aging population. Evidence-Based Practices Dementia/Alzheimer s Prevention and Wellness Chronic Disease and Pain Management

4 Symposium Workshop Tracks Operational and Administrative: Issues pertaining to the operation and management of senior programs or health providers delivering services to the senior population. Quality Improvement Innovative Models of Care Patient Centered Medical Home Development Case Management Rural Health 3. Program Development and Analysis Community Outreach and Enrollment Grant Funding Elderly-Driven Health Promotion Audits Research and Data

5 Call for Abstract Guidelines Who should submit? We encourage the following to submit abstracts for workshops: Submission Guidelines Health centers providing health care for the aging population Primary care associations All health providers Aging agencies (including state departments on aging) Non-profit community based organizations, faith based organizations Research organizations Senior centers, and other aging programs We encourage presentations that are interactive and participatory. Abstract submissions must include the following information: 1. Workshop title 2. Workshop description (not to exceed 200 words) 3. Workshop track and a brief description as to how this session is relevant to this year s conference theme 4. Three workshop learning objectives. Objectives should: a. contain measurable action words (i.e. define, list, explain, name, compare, etc.) b. include only one action per objective, and c. identify what the workshop participant will be able to do after your presentation 5. Each presenter s name, title, address, telephone number, fax number, and 6. A brief bio for each presenter please limit to half a page a. Please identify the primary presenter to be contacted. This person will receive all correspondence and will be responsible for sharing all information with their co-presenters. b. We recommend no more than three (3) presenters per workshop. 7. Topics should make an important contribution to the aging community and those who work with the aging. Workshop Objectives: Objectives to be met during the workshop are clearly Implications: Implications of findings to the field, policy theory, or areas of further research are discussed. Adaptability of innovation to other practice sites or settings is discussed. Notification Date You will be notified in February 2014 of the status of your submission. Symposium Registration and Accommodations: All presenters must register for the conference.
