3 Dear Participant Over the last few decades, ultrasonography has emerged from being a niche product for specialists to a routine diagnostic tool for intensive care physicians and anesthesiologists. Today, many interventions in our discipline are no longer performed without the use of ultrasonography. The reasons for this enormous development are obvious: Why grope in the dark with our needles when we actually can see? The method is strictly non-invasive, and the machines improve and become cheaper each year. It is a fancy gadget, and you might hope that your next Tesla is equipped with ultrasound just to know that you heart is still beating while your car s acceleration exceeds 10 m/s 2 (1 G). But as always, it is not all roses, and there are at least two sides to everything in medicine. Two of the four ethical principles in medicine are beneficence and non-maleficence. There is a danger that ultrasound allows us to diagnose even the smallest pathologies that are ultimately irrelevant for the clinical course of the disease. As ultrasound-based interventions become more and more safe, some unrelated interventions are performed regardless that there will be no benefit for the patient. At the bottom line, what remains is the risk for complications. Other important topics are education and competence. It is easy to plug in the ultrasound machine and display something on the screen. But, is the user trained to be able to visualize something that is real and significant? Our program covers a wide range of topics from diagnostics to teaching and limitations. We end by talking about ultrasound for interventions. We are convinced that we have created an attractive program and are grateful to the members of our renowned faculty who will certainly deliver with their outstanding presentations. We thank you all for coming to Basel to listen, learn, and discuss. Ultrasound is an integral part of Perioperative Medicine. We have to implement and refine the method and adapt our competencies. Perhaps the pendulum swings back once and its usage will be challenged more than it is today. However, ultrasound is here to stay. Hans Pargger
