Additionally, we plan to collect data on the prevalence of the different categories of orofacial pain as determined by the dentist performing the initial examination. We hypothesize that patients will be predominantly female, come from diverse backgrounds, have a large number of comorbidities, have high levels of disability, depression and anxiety, have a variety of orofacial pain conditions and have seen numerous other clinicians in the past for management of their condition. 3/29/2017 1

2 Description: (Include design, methodology, data collection, techniques, data analysis to be employed, evaluation and interpretation methodology for research component) Orofacial pain has been defined as pain localized to the region above the neck, in front of the ears and below the orbitomeatal line, as well as pain within the oral cavity. (Shephard 2013) In their description of orofacial pain, The American Academy of Orofacial Pain also includes symptoms related to cervical musculoskeletal pain, headaches and systemic disorders that cause pain located in the orofacial region. (AAOP website It has been estimated that fully 10% of the adult population and 50% of the geriatric population live with chronic orofacial pain. (Shinal 2007, Torisu 2010) Several studies have described the demographic and psychosocial characteristics of patients seeking medical care in a specialized orofacial pain clinic. (Esposito 2000, Blanco- Hungria 2012, MacFarlane 2002, Riley 2001, Aggarwal 2008, MacFarlane 2001, Suarez 2007) Of these studies, only one was published recently (Blanco-Hungria 2012), and this study was conducted in Spain. There is therefore a need to describe the demographic and psychosocial characteristics of patients currently seeking medical help for orofacial pain in a specialized orofacial pain clinic located in the United States. This is a cross-sectional descriptive study. Data will be collected from patients being seen for an initial visit at the Rutgers School of Dental Medicine Orofacial Clinic, 110 Bergen Street, Newark NJ Subjects aged 18 and older who are being seen for an initial visit in the Orofacial Pain Clinic and who agree to participate in the study will be included. 3/29/2017 2

3 The student intern will identify subjects being seen for an initial evaluation in the Orofacial Pain Clinic by consulting with the department secretary / scheduler. Subjects will then be approached by the student intern and asked if they might be willing to participate in the study. The student intern will obtain written informed consent from all subjects. The study will be explained to the potential subject, the consent will be read, and their questions will be answered. If the potential subject wishes to enroll, s/he will sign the consent form. The student intern will also sign and date the consent form, and a copy will be given to the subject. The student intern will then assist the subject with completing the questionnaires. Variables to be described include health, orofacial pain characteristics, function, psychosocial data, and referral patterns. Specifically, we plan to use the following questionnaires to collect data. 1. New Patient Registration form 2. Orofacial Pain History and Clinical Examination Worksheet 3. Jaw Functional Limitation Scale 4. Generalized Anxiety Disorder Scale 5. Additional questions regarding depression symptomatology Data will be entered into an excel spreadsheet and descriptive statistics will be calculated. (e.g., national or state meetings, newsletter or journal, SHP poster day) June 16, 2017 August 30, 2017 The student intern will have exposure to the interdisciplinary evaluation and treatment of patients being seen in a specialized orofacial pain clinic. Orofacial pain is a niche practice in physical therapy. Exposure to the evaluation and management of these patients will provide the student with knowledge and insight regarding the role of physical therapy in the orofacial pain setting. The Orofacial Pain Clinic is involved with educating and training dentists to evaluate and manage patients with orofacial pain. The student intern will have the opportunity to attend educational sessions and clinic rounds involving orofacial pain. I plan to submit study results to the Journal of Oral Rehabilitation for publication. The student intern will have the opportunity to be included as an author on any publications that arise from these data.
