Articles must be contributed solely to The American Journal of and become the property of the publisher. Statements contained therein are the responsibility of the author(s). MANUSCRIPTS Address manuscript to the Editor-in- Chief of The American Journal of, J. Claude Bennett, M.D., Department of, University of Alabama at Birmingham, Room 621, Medical Education Building, University Station, Birmingham, Alabama Indicate, in covering letter, category of manuscript (Original Article, Review, Special Article, Case Report, Brief Clinical Observation, Letter). Requirements for manuscripts are in accord with the Uniform Requirements for Manuscripts Submitted to Biomedical Journals. For more information, refer to the following: International Committee of Medical Journal Editors: Uniform requirements for manuscripts submitted to biomedical journals. Ann Intern Med 1988; 108: Submit three copies of all elements: text, references, legends, tables, and figures. Arrange the paper in this order: (1) title page; (2) abstract; (3) text; (4) references; (5) legends; (6) tables; and (7) figures. (The total number of tables and figures should not exceed one per 500 words of text.) Number all pages in sequence, beginning with the title page as 1, abstract as 2, etc. Type all matter: (i) on 8'/~- X 11" opaque white bond paper; (2) on one side of each sheet only; (3) doublespaced; (4) with wide margins on all four sides. THE TITLE PAGE Include first name, degree, and affiliation for all authors. Indicate location of each author (city and state) at time of study and include present address if it differs. Provide a running title of three to six words. Include (as a footnote) the name and address of institution from which work originated plus information about grants, and the name and address (with zip code) of the person to whom reprint requests should be sent. THE ABSTRACT For original articles, a four-part abstract should be submitted. It should contain the following clearly labeled elements: Purpose, Patients and Methods (or Materials and Methods), Results, and Conclusion. An abstract should be submitted with reviews, special articles, and case reports for the purpose of editorial review. Note, however, that case reports will be published without an abstract. THE TEXT Do not use acronyms or abbreviations other than those for units of measurement. Cite in numerical order every reference, figure, and table. (Order of mention in text determines the number given to each.) Place acknowledgments at end of text, before references. THE REFERENCES Number in order of mention. Provide concluding page numbers for all journal references and specific page numbers for all book references. Indicate all abstracts. Follow general arrangement, abbreviations, and punctuation as given in Uniform Requirements for Manuscripts Submitted to Biomedical Journals. For periodicals, follow INDEX MEDICUS, listing all authors when six or fewer; when seven or more, list only first three and add et al. Please note that no periods are used after authors' initials. THE FIGURE LEGENDS Start at top of new page. Number pages in sequence after last page of references. Identify at the end of each legend and in alphabetical order all abbreviations in the figure described. Indicate magnification for all photomicrographs. THE TABLES Start'each table at top of new page. Give each table a number (in roman numerals; Table I, II, etc.) and a title. Number the tables in the order in which they are mentioned in the text. Do not use vertical or horizontal rules within body of tables. Provide a footnote to each table, identifying all abbreviations used. Submit only typed originals of tables; if more than one standard 8~/._, 11" sheet is required for a large table, tape sheets together carefully. Do not reduce size of table by photocopying. Limit tables to number and size necessary for clarity. The publisher allows a limited sum towards reproduction. THE FIGURES Submit three glossy prints (not originals) of each photo and drawing. Please note that artwork of published articles will not be returned. Use black ink for all charts (line drawings). Make decimals, broken lines, etc., strong enough for reproduction when reduced to column width (18 picas, 3"), two-column width (38 picas, 65/is'), or intermediate width (26 picas, 45/~d'). Do not combine very large type with very small type; all lettering should be of approximately equal size. Use arrows to designate special features. Submit high-contrast prints for roentgenographic photographs; some detail is lost in reproduction. Crop photomicrographs to show only essential field. Do not title the figure; this information should be included in the figure legend. Carefully consider proportion for each part of a multi-part figure; a vertically oriented 3 X 5" illustration cannot be combined with a horizontally oriented 5 3" illustration to produce a composite figure. Identify figures on back by number and first author's name. Number figures in order in which they are mentioned in the text. Indicate top of each figure. Submit written perm!ssion from publisher and author to reproduce any previously published figures. Limit figures to number necessary for clarity.
