To provide youth-friendly care to students Receive support from OSSE to engage and link services to schools Address challenges Expand success across the District Coordinate equitable service for children and youth The Future Network and Quickbase Online Referral System (Proposed) Follow-up betweenthe school, student, and provider OSSE retrieves data from both initated and completed referrals OSSE receives the initial and completed referrals DC schools access OSSE s online referral to the Network Partner School Health Provider Network Referral Program Student is encouraged to connect with the provider DC school log referral in the System An alert is sent to the provider Network Partner submits to OSSE the referral completion form 5
