3 Heart Failure Heart Failure The only heart disease that is on the rise Prevalence: 5.7 million in US in 2006 ~550,000 new cases each year A contributing cause of 287,000 deaths in US per year

5 Psychosocial Factors & HF Depression 1 in 5 presents clinically significant depressive symptoms (Rutledge et al, 2006) Predicts higher mortality and more frequent hospitalization (Friedmann et al., 2006; De Jong et al., 2004; Jiang et al., 2001; Sherwood et al., 2007) Anxiety Estimated prevalence 18-64% (Haworth et al, 2005; Friedmann et al., 2006) Predicts higher mortality and more frequent hospitalization (Friedmann et al., 2006; De Jong et al., 2004; Tsuchihashi-Makaya et al., 2009)

10 Participants HF Patients >= 18 yo Primary Dx of HF No other life-threatening illnesses No active psychotic symptoms or severe cognitive impairments Participants 238 at baseline, 164 at 6-mo FU 9 died, 15 transplants, 50 failed to respond Dropouts did not differ in demographic, psychosocial and PHF scores except being younger, less likely to be married

11 Measures Physical Health Functioning Minnesota Living with Heart Failure Questionnaire (MLWHFQ) Physical Functioning Subscale Physical difficulties due to HF Fatigue, SOB, climbing stairs, chores, yard work, sitting, lying down to rest, sleeping at night etc. Medical Chart Reviews Cardiac disease history, other medical comorbidities, medication Blood Sample CV risk factors, fasting glucose, lipids
