Including any of the following: Lifestyle risk factors: Smoking Physical inactivity Poor nutrition Alcohol use Biomedical risk factors: High cholesterol High blood pressure Impaired glucose metabolism or Excess weight Family history of a chronic disease

8 40-49yr Type 2 Diabetes Risk Assessment The Australian Type 2 Diabetes Risk Assessment Tool Review risk factors underlying a patient s high risk using the Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK) Must score at least 12 points in the Australian Type 2 Diabetes Risk Test Exclude Diabetes Random or fasting blood glucose/ogtt/hba1c Provide risk-factor advice or treatment & refer to services as appropriate Must not be performed more than once every 3 years to an eligible person

10 Promotion Strategies Challenges Busy demographic (time poor) Difficult to pin down for a long appointment Priorities outside of self (family/work) May take multiple invitation attempts (over a number of years)
11 Promotion Strategies Using a multipronged approach Trial a combination of promotion channels: Opportunistic invitation Waiting room posters/digital display SMART Recalls with compelling content Phone call Letters Practice website Social Media marketing

Prepare the practice: Poster or sign in the waiting room to promote the Health Checks Nurse times and consulting room allocated All staff members are aware of the referral process to nurse & GP Templates agreed on and finalised: Recall letter Health Assessment template Referral forms Poster/waiting room sign
13 Promotion Strategies A 12-month approach 2. Database search: List of active patients in age group Tip! If database is too large, then start with small sample (eg 48-49yr olds) Make sure there s enough time left for patient to attend appointment prior to turning 50! 3. Determine Medicare eligibility of these patients: Ring Medicare provider line Ask if the patient is eligible for item 703 today. (check up to 7 patients/call) 4. Flag patient file & recall eligible patients.
14 Promotion Strategies A 12-month approach 5. Book appointment: With nurse for 15, 30 or 45min as appropriate AND With GP for standard appointment immediately acer nurse. Tip! If unable to make nurse and GP appointment on the same day, then let paments know beforehand (during booking process) that the check will take place over 2 consults. 6. Once pagents are seen, add them to recall database for follow up as required. 7. Review progress of the 12- month plan and related pagent lists at 3, 6 and 9 months. 8. Start a new 12- month plan acer one year.
