2 a valuable experience, enrich my exposure in knowledge and skills, and to interact with great teachers of headache area. The Short-stay scholarship 2017 consists of registration to the 18th Congress of the International Headache Society (7-10 September) and attending to ihead Headache Academy (4-5 September) in Vancouver. During attending the both scientific meeting I had a chance to meet with old colleagues, meet new friends, get the latest information on the treatment of headaches, and seen the results of original research from leading scientific centres and scientists. My special thanks to Professor Paul E. Cooper who is a Chief of Department of Clinical Neurological Sciences and Director of Dr. John H Kreeft Headache Clinic of University of Western Ontario. Prof. Paul Cooper and his secretary Joan Fleming organized a brilliant programme for my 1 month stay in London, Canada, which was the next step of scholarship. It was an amazing experience for my professional growth as a headache specialist and for the stimulating environment of my hosting department which contributed to a very fruitful observership. Every Monday we have seen the many patients with various type of headache in Dr. John H Kreeft Headache Clinic which located in Victoria Hospital. During of them I had an opportunity to see diagnostic of headache disorders and the management of all types of headache. Twice a week we had a Botox and injections clinic, where I added new knowledge of technical aspects of nerve blocks injection and botulinum toxin injection which is not common used in my country. Every Tuesday I participated in the morning rounds, where complicated neurological and neurosurgical cases were presented by neurology residents and commented by neuroradiologist and other doctors. I also had the opportunity to observe neurosurgical operations, such as decompression of the trigeminal nerve, the installation and replacement of the stimulator of the great occipital nerve and would like to thank to Dr. Andrew Parrent for sharing their experience. During the third week of my observership, the Canadian Headache Society was holding the National Neurology Residents Headache Course (13-15 October) in Toronto where I was invited to attend. Lectures and workshops were very informative. How will the experience help you in the future? 2

3 I had a really good experience in headache disorders in Canada starting from Vancouver and finished in Toronto. I met a lot of nice people, learned more about headache treatment and different ways to do diagnostics. I am very grateful that I had such an outstanding opportunity providing a positive impact on my professional development and I am happy to bring back to home new experiences and inspiration. 7 Comments Short summary of overall experience, including attendance at IHC/iHEAD It s my great pleasure to report one of the most memorable and fruitful experience in my professional life. Thanks to the award of the scholarship of the International Headache Society as part of the program IHS Short-stay Scholarship peri-ehmtic 2017 I was trained 1.5 month in following four excellent hospitals in Canada, under the supervision of distinguished specialists: 1.The Hamilton Headache Hamilton, Ontario, Canada Host: Dr Rose Giammarco 2.The Womens College Hospital: Dr Christine Lay and Dr.Marissa Lagman Bartolome 3. Toronto Headache Clinic (Dr Marek Gawel ) 4. The Toronto Headache & Pain Clinic (Procedures-Interventional Headache management of headache Injections Botox and Nerve Blocks): Dr Ian Finkelstein I also attended 18 th Congress of the International Headache Society (in Vancouver), 2017 Joint International Headache Academy (IHA)/iHEAD program, sponsored by the American Headache Society (AHS) and the International Headache Society (IHS) (in Vancouver) and the National Neurology Resident Headache Course ( in Toronto) The IHC 2017 Congress was totally fascinating, really showing new horizons in headaches, extremely well organized, with excessive symposiums, special lectures and sessions, advanced educational teaching courses, multiple clinical and preclinical sessions targeting on migraine biomarkers, emerging therapies, neuromodulation, interesting poster presentations etc. National Neurology Resident s Headache Course (NNHRC) was very informative and successful 3 day course, providing course materials and complimentary 2 year membership. Course was oriented on small group practical sessions, including excellent presentations on headaches, by local and invited speakers, case presentations etc. while attending the IHS and the residents course I was fortunate to have exposure and be able to attend lectures given by some of the world s leading headache authorities in the topic of headache, people like David Dodick, Deborah Freidman, Werner Becker, Peter J Goadsby, Zaza Katsarava etc. During my hospital stay I was warmly welcomed by all medical and non-medical staff and learned a lot. All of above mentioned Doctors where helping me to Learn as much as possible in a given time limit, In terms of the approach in evaluation and management of patients presenting with headaches, to be familiar with the different treatment strategies 2

8 (pharmacologic and non-pharmacological treatment) of migraine as well as other headache disorders (both primary and secondary). I observed the different interventional treatments of headache disorders including nerve blocks and Botox injections which is not very widely used in my country. Also became familiar with health system there, clinic set up, Pt education etc. It is a very big aid that the IHS provides funding opportunities for young doctors. This organization is making a major contribution to the advancement of medical care for Headaches within and outside Europe. I am very grateful to IHS for this generous support. Further I thank my Chef Prof. Ingorokva, my PHD supervisor, Prof. Beridze and head of the neurology department Prof. Kakabadze, without them this internship would not be possible. In my experience from daily practice and data gathered during expeditions around regions of Kyrgyz Republic, more than 80% of applied patients suffer from primary headaches. Nonetheless, currently no specialized treatment is provided to headache patients; moreover, there is an issue of systematic underestimation of headache problem by patients and doctors, including neurologists. Using the great opportunity to participate in Short stay Scholarship 2017 program from IHS, my major goal was to raise awareness about the problem of headaches in my country and establish first specialized treatment program for primary headache patients. I am very grateful to Prof. Werner Becker and Dr. Elizabeth Leroux from University of Calgary Foothills Hospital and South Health Campus for the leadership and thoughtful mentorship. I was lucky to become a part of CHAMP team, where I was able to learn treatment of headache patients step by step starting from their first attendance, following 1

10 with Botox therapy and various assessments, such as occupational therapy, physiotherapy, attending several educational lectures for patients. Furthermore, I would like to express my gratitude to IHS and CHS, for the great opportunity to participate in IHC 2017 in Vancouver, in a very well organized National Neurology Resident Headache Course in Toronto, and other courses and meetings, including Botox. I am particularly grateful to Dr. Elizabeth Leroux for her hospitality, overall support and amazing organization of my observership. Under Dr. Leroux s mentorship, I have studied relevant and useful literature, and gained invaluable experience and knowledge, which inspired me for further research of the topic. During my observership, I discovered variety of similar issues we are facing in both Kyrgyz Republic and Canada, despite the tremendous differences in advancement, which was reaffirmation of feasibility of changes in headache situation back in my country. How will the experience help you in the future? The program granted me an opportunity to obtain incredible experience and develop a plan for my further steps to improve the situation of headache, taking into account specificities of medicine in Kyrgyz Republic, in collaboration with Canadian Headache Society. Thus, I have developed a priority action plan for my return: - Raising awareness on headaches problem in Kyrgyz Republic through research work and media-resources; - Establishing headache online resource for patients and doctors, first in Central Asia; - Further development of pilot project on treatment of primary headache patients in Kyrgyz Republic for the next 2 years; Currently I am working on developing the web site and preparing a wide study on incidence of primary headaches in individuals of working age, and working on establishing management and assessment program in my country, based on experience and knowledge obtained in Canada. In addition, I would like to recommend all future applicants from developing countries to have more faith in themselves, and in real opportunity to improve treatment of headache patients, despite the shortcomings in healthcare systems. In my view, the most significant part of your training is to take a first step and determine clear objective for yourself, as only in such case you may gain maximum useful knowledge and experience in the sea of possibilities, with such limited timeframe.
