I am confident we are heading in the right direction. I wish to thank each director for their time and effort this year. In particular, I wish to recognize Debra Dawe, Judy Kosmerly and Meredith McNeil for their longstanding leadership and commitment to our Board of Directors. Debra, Judy and Meredith will be leaving our Board after many years of dedicated service. We will miss their enthusiasm and wish them the very best in their future endeavours. Over the past twelve months, so much has been done to address the problem of addiction in our downtown neighbourhood. Notably, the Harm Reduction Home Program has given hope to individuals who are homeless and alcohol dependent. Their stories have inspired us to continue to advocate on their behalf. We will do just that. Thank you to Marion Quigley, Chief Executive Officer for her unwavering determination and vision. Thank you to the leadership team and staff at CHMA-. I look forward to the year ahead. 3 CEO Report 2015 Harm Reduction Home (HRH) New to Sudbury Another year has passed and it has been filled with exciting projects, new adventures and hard work! Everyone in the agency from the individuals we work with, the staff, volunteers, our partners and Board have worked together to ensure good mental health and addiction services are available in the community. CMHA- continues to grow and we are making more community services available. Individuals and their families are being invited to participate in the planning and evaluation of services to ensure the services meet the needs of the people who require them. Quality and accountability are important to us all and we are extremely proud of the work we do! We showcased this hard work at several provincial wide conferences in the past year. Another success has been our progress with Sudbury s first Health Link. This work will continue to move forward thanks to many of our community partners. As a mental health agency, we value the social determinants of health and work towards ensuring they are met. We all have basic needs: a place to call home, friends/supports, and a source of income. Our challenge as a community is to have the right supports in place to meet these needs. As a result, we will continue to focus on more housing and supports. We hit some road blocks when we were unable to find a Harm Reduction Residence this year. But we will persevere and work with the community to ensure all citizens are proud to call the Sudbury and Manitoulin Districts their home. While new programs continue to be introduced it is just as important that our existing programs continue to help people obtain the supports and services they need. Thank you to the Intake, Justice, Housing, Case Management, Community Mobilization, Health Promotion, Finance and Administration staff for all your hard work in ensuring services are available in a timely way in our community. I look forward to the year ahead and thank everyone for all the work you do to ensure good mental health! Marion Quigley Chief Executive Officer CMHA- received funding from the North East Local Health Integration Network to establish a Harm Reduction Home (HRH) in Sudbury. The program is designed to help individuals who are homeless or at risk of homelessness and who are impacted by chronic alcohol dependence. Today, CMHA offers HRH as a day program, but by the fall of 2016, HRH will run a managed alcohol program (MAP) in a secure residential facility for 15 people. It will be staffed with qualified professional support personnel who will oversee the residents with their everyday living, primary care needs, and for the administration of alcohol on a daily basis. This program plans to address the needs of a certain group of the homeless, who are currently suffering from chronic alcohol dependence. While the abstinence model works for many, it does not work for all individuals. The participants in the HRH day program are looking forward with anticipation to the new location for the home. I am hoping this (program) will help to get my life turned around. I need to get back on my feet. How Case Managers Make a Difference This is what an individual told me. I work with an individual who s reminded me what a liar anxiety can be. Outside the program, I don t have family. I consider everyone family here. She once revealed to me that doing laundry within her apartment building was too scary, that leaving her apartment alone was too intimidating, taking the bus by herself would never be a possibility, and her intention to volunteer wouldn t come to fruition. So I went at her pace; I empowered her with coping tools; I linked her to supports; I held the hope for her when she couldn t. She s since proven anxiety wrong, on each of its claims. 4 Health Links This past year, the Northern Ontario Junior Hockey League joined the Talk Today program, allowing more than 180 Northern Ontario players to receive suicide awareness training and the opportunity to connect with their local CMHA branch, much like the OHL initiative. CMHA- has since been building a close relationship with the French River Rapids, Espanola Express and Rayside-Balfour Canadians. Health Links were introduced as a key commitment in the Ministry of Health and Long-Term Care 2012 Action Plan for Health Care. There are currently over 80 Health Links operating across Ontario their primary goal being the effective provision of coordinated care for complex patients. As lead agency, CMHA- has been collaborating with partners since early 2015 to establish the Greater Sudbury Health Link (GSHL). Ministry approval of the GSHL was received in December In November we participated in Life Vest Inside s Dance for Kindness, a worldwide flashmob/freezemob where groups around the globe dance to the same song on the same day. Its purpose is to unite the world under the banner of kindness. In Sudbury alone, 46 people participated, in addition to the volunteers and media. The population focus of all Health Links is the top five percent of complex patients. Provincially, it is estimated that these patients account for approximately two-thirds of Ontario s health care dollars. In Sudbury, local data and provider experience has identified specific opportunities to improve care coordination for those living with complex mental health conditions, addictions, and/or developmental disability. Focused on each individual s unique goals for care, GSHL plans to improve communication between providers, coordination of services and supports across sectors, and ultimately, individual well-being. GSHL is currently refining its tools and processes for care coordination and hopes to be serving individuals before the summer of Health Promotion Highlights Our Health Promotion program continues to grow in the areas of community promotion and education. We raise awareness through event management, print marketing, digital content and social media. June brought on our very first Ride Don t Hide event, in collaboration with CMHA British Columbia. Individuals were invited to take part in a community family-friendly bike ride at Camp Sudaca to raise awareness and funds for mental health. Talk Today is a partnership between the Ontario Hockey League (OHL), CMHA Ontario and local CMHA branches. For the last two seasons, CMHA- has been supporting our local Sudbury Wolves with suicide-awareness workshops, a CMHA Mental Health Coach and Champion for extra support, and an awareness game to bring the community on board with the initiative. 3 workshops Sudbury Wolves players, Billet parents, Rayside-Balfour Canadians 48 6 people trained 4 Game nights Sudbury Wolves, Rayside Balfour Canadians, French River Rapids, Espanola Express reaching 1,600 $1,345 raised 17 riders 10 volunteers people 7
5 Transitional Community Support Program New to CMHA is a program that offers short term intensive case management services to individuals with lived mental health experience. The goal is to help people find and keep appropriate and permanent housing. This can be challenging because of the mental illness and addictions of some clients, but also because it is difficult to find housing that is affordable on social assistance. The Transitional Community Support program: engage and support individuals through linking to community supports; maintaining their housing; reducing the risk of homelessness and jail; and increasing diversion from hospital. Transitional Community Support helps individuals understand the value of community and to look for ways to develop a sense of belonging while pursuing a healthy, satisfying life and stable home. Friendships, peer support and other involvements play an important role in building positive experiences. The Transitional Community Support Worker assists individuals for three to four months to sustain and maintain their housing, to reduce the risk of homelessness, and to provide necessary links to community supports and resources. Mental Health Diversion provides alternatives to criminal sanctions for people who have come into contact with the law for a minor offence. Mental Health Court Outreach Workers meet and assess individuals for acceptance into the diversion program. This includes the willingness of individuals to participate in the diversion program and their mental health at the time of the offence. CMHA s Approach to Advocacy In February, our office received a referral from Health Science North-Inpatient Psychiatry Unit. A man was charged with Fraud Under $5,000 prior to being hospitalised. At his first appearance, we obtained a screening form proposing a sentence of 18 months of probation.
