At the Y, we live out our commitment to youth development, healthy living and social responsibility by providing programming based on four core values caring, honesty, respect, and responsibility. As a Y member, you have full access to the general Y facilities. We offer many classes and programs to help you find your niche at the Y! For over 110 years, the West Cook YMCA has contributed to the lives of hundreds of thousands of boys, girls, men, and women of all races, religions, and economic backgrounds. We are a non-profit, cause-driven organization with the unique opportunity to impact lives of all ages. This handbook has been designed to answer your questions and help you become more involved in the Y. 3 MEMBERSHIP INFORMATION FACILITY HOURS Monday - Friday Saturday Sunday 5:00am 10:00pm 6:00am 6:00pm 10:00am 6:00pm BABYSITTING The Y s babysitting program is a safe and supportive environment for your children so you can work your way to a healthier, happier you. Our friendly and caring staff members give your children the chance to read, play, learn, and grow while interacting with other children. You must remain in the building at all times when using our babysitting room. Please bring your own bottles, diapers, and snacks (no peanut products), and label all your belongings. Ages: 6 weeks to 9 years old. A parent or guardian age 21 or older must sign in each child. Cost: Members have complimentary access to the babysitting room for up to an hour-and-a-half. After an hour-and-a-half, the charge is $4/hour. Non-members will be charged $4/hour to use the babysitting room. Policy Reminder: Please pick up your children by the time the babysitting room closes. Failure to retrieve children by closing time will result in a charge of $1.00 per minute the child stays past the posted closing time. Room Capacity: 21. Once that capacity has been reached, additional children cannot be accepted. Morning Evening Mon & Fri 8:00am-1:00pm Mon-Thurs 4:00-8:00pm Tue-Thur, Sat 8:00am-12:00pm Fri 4:00-7:00pm The Y reserves the right to change the hours based on usage. Please check the current program guide or contact the Member Engagement Desk for current hours. MEMBERSHIP PERKS The West Cook Y features the following facilities and benefits: FREE Healthy Lifestyles Program, Initial Fitness Assessment, and Fitness Center Orientation. A wide variety of FREE and paid group exercise classes Advanced sign-up for classes Men s and Women s Spas with whirlpool, sauna, and steamroom. On-site babysitting Towel service Indoor pool for lap and family swimming Open gym time in our two gymnasiums Three handball and racquetball courts Use of our Wellness Center with cardio and strength machines Use of the Spirit Mind Body Room with free weights and functional equipment Indoor Track Access to Community Learning Center computer lab Access to over 2680 YMCAs in the USA through reciprocity right MEMBERSHIP DESCRIPTIONS Youth: Children 6 17 years of age. Young Adult: Individuals ages Adult: Individuals years of age. Senior Adult: Individuals 62+ years of age. Household: Two adults with or without dependents at the same address. Includes children up to 18 years old and children in college full time up to 23 years old. Single Household: Single parent and dependents at the same address. Includes children up to 18 years old and children in college full time up to 23 years old. Senior Household: Two adults with or without dependents at the same address. One adult must be 62+ years of age. Includes children up to 18 years old and children in college full time up to 23 years old. Program Member: Any age member of a program without full membership benefits. Access only to the class location and locker room on the day and during the time of the paid class. Use of facility by family members of Program Members is not permitted. MEMBERSHIP DUES The first month of membership dues are payable at the time of signup at a prorated amount. Your membership dues can be paid via: 1. Monthly Automatic Draft. Under this plan, an automatic charge will occur automatically on the first of the month, beginning with your first full month of membership. You may choose to draft from your checking or savings account, or a debit or credit card (Visa, MasterCard, or Discover). Your monthly dues continue as long as you are a member. You may discontinue your monthly draft by signing a membership cancellation form 10 days prior to the next automatic draft. 2. Annual Payment. Annual payments in full may be paid by using cash, check, or major credit card. Please Note: If your draft information changes (your current card expires, account number changes, card is stolen, etc.), please notify us to avoid an additional fee for a declined account. We will waive one fee per calendar year. Each additional fee is $10. Memberships may be placed on hold for up to 90 days once per calendar year. FINANCIAL ASSISTANCE Our commitment is to give every family, senior, and child access to programming that strengthens spirit, mind, and body. To make that possible, we offer financial assistance to those who are unable to afford the full membership rates. The scholarship application form is available at westcookymca.org under the Membership menu. We invite any qualified resident in our service area to apply. Assistance amounts vary by need and are awarded first-come-firstserved, based on availability of funding. 4 MEMBERSHIP FEE REFUNDS Membership Fees and Joiner s Fees are non-refundable. However, we will assist you in transferring to another Y if requested. Members who move their memberships between Ys may have a portion or all of their Joiner s Fee waived by the other Y if membership is not interrupted (but please call to verify with your new Y). MEMBER STATUS CHANGES Changes to your membership including additions or deletions of individuals, or name and address changes can only be made by the primary member.* Any changes can be processed at the Member Engagement Desk. *Primary Member: The adult on the membership account responsible for payment and all changes to a membership. There can be only one primary member per account. However, it can be changed with documentation. MEMBERSHIP CARD When you join the Y, you will be issued a membership card. Your membership card MUST be brought with you every time you come to the Y. Having your card with you grants you access to the facility and ensures your safety. Temporary pass fee for a forgotten card is $1. Replacement fee for a lost card is $5. Individuals falsifying cards or allowing others to use their card will be subject to membership suspension or termination. CORPORATE MEMBERSHIP PLAN The Y is very proud of the many companies who invest in the good health of their employees through endorsing and supporting the YMCA Corporate Membership Program. Please contact Member Engagement Director Kyle Hartje at or khartje@ westcookymca.org if your company is interested in a corporate membership. SHARE YOUR YMCA Members are always invited to bring in friends and encourage them to join. Members may bring up to two guests OR two families per visit. Each guest may use the Y four (4) times per calendar year without charge. Guests using free passes must be accompanied by a West Cook YMCA Member. After four free visits, the following guest fees will be charged: Youth Pass (ages 6-17) $6 Adult Pass (ages 18+) $11 Family Pass $14 Guests 18+ who wish to visit without a West Cook YMCA member must pay the guest fee prior to facility usage. Guest passes are non-refundable. Guest privileges and times may vary. All guests ages 15 years old and above must present a valid photo ID. To assure our members are fully accommodated, we reserve the right to restrict guest pass use. Members of other YMCAs may not bring guests. Guest passes may be denied if there are no activities available for the potential guest at the time of request. Please check current schedules. THE AWAY PROGRAM ALWAYS WELCOME AT YMCAs As a Y member, you may want to use a Y in another city or on a short-term basis. Make sure you take your Y card with you when you travel! Contact the Y you wish to visit for their specific rules regarding visiting members- or AWAY members (Always Welcome At YMCAs). Visit to find the nearest Y wherever you travel. Each Y has a different policy regarding number of visits allowed or fees charged. When visiting the West Cook YMCA, AWAY members are granted 6 visits per calendar month to our facility. Members from other Ys who wish to register for programs must sign up during Open Registration sign-up. AWAY Members will receive the Member Rate for all session-based programming. Because AWAY members are considered guests, they are not allowed to bring guests with them. When the maximum number of visits has been reached, a guest fee will be charged. Please see the Share Your YMCA section above for guest fees. Contact our Member Engagement Desk at for more information regarding our AWAY policy. THE ARMED SERVICES YMCA AND DEFENSE OUTREACH INITIATIVE The Y is proud to serve our community by offering Y memberships to eligible military families and personnel who may not have access to a nearby military facility. To determine your eligibility, please follow these steps: 1. Contact Military OneSource at militaryonesource.com or to confirm eligibility and receive an eligibility form. 2. Contact the Member Engagement Desk and request the YMCA Military Outreach Paperwork. REFER A FRIEND Are you interested in saving money? The Y can help! When you refer a friend to the Y, you will receive one month free and they will receive $50 off the Joiner s Fee! 4 The Y: For Youth Development, Healthy Living, and Social Responsibility.

5 FURTHERING OUR CAUSE VOLUNTEERING Volunteering is, and always has been, central to the YMCA movement. Our Y welcomes volunteers who can give their time, talents, and treasure to the Y and our community in exchange for immense personal gratification as they serve others. If you have a skill or a passion, chances are we can put it to good use at the Y! For more information on volunteer opportunities, please stop by the Y and fill out a volunteer application. Or, go online to www. westcookymca.org to view current volunteer opportunities and download the application. We look forward to welcoming you as a volunteer! ANNUAL SUPPORT CAMPAIGN Your Y is more than a gym it s a community of men, women and children from all walks of life. From volleyball to volunteerism, lunges to learning the alphabet, squats to successfully finding a home, West Cook Y helps our community become stronger every day. Our Annual Support Campaign is your opportunity to help friends and neighbors achieve even more. In 2014, because of gifts from members like you, we provided over $100,000 in financial assistance to more than 1,000 families, adults, seniors, and youth in the 10 communities we serve. Neighbors in need received access to childcare, preschool, fitness classes, sports, gardening, enrichment activities, and so much more! Please join us in making our community stronger by giving to the Y s Annual Support Campaign. You can add an amount to your monthly membership deduction, make a donation or complete a pledge card at the front desk, or donate online at westcookymca.org. We are also looking for Y Champions who will lend their time and support to the effort. PROGRAM INFORMATION GENERAL PROGRAM INFORMATION Registration for classes at the Y is conducted on a first-come, firstserve basis, with the exception that West Cook YMCA members are given priority over program members (or members from another Y) and are eligible to register one week prior to program members. The Y reserves the right to change schedules and classes. Classes missed due to holidays, sickness, conflicts or bad weather are not made up and no credits are issued. Full payment of class fees is required at the time of registration. You may use check, cash, debit card, MasterCard, Visa, or Discover Card. You are responsible for any payment declined by the credit card company/financial institution plus any fee they charge the West Cook YMCA. We do not accept starter checks. Checks must have a pre-printed name, address, and account number. To receive the member price on a program, a current West Cook YMCA membership is required through the entire program session. If your membership is terminated during a session, you will be required to pay the difference of program fees. Programs that are listed as Free to Members are free for West Cook YMCA members. Members from other Ys may register for classes at the member rate and may sign-up for programs during the open sign-up period. PROGRAM MATERIALS Brochures, program schedules, and flyers describing Y services and programs may be obtained at the Member Engagement Desk and online at westcookymca.org. If you would like information about a special program or about Ys in other states or countries, please visit PROGRAM/CLASS REGISTRATION West Cook YMCA members may register for programs using any of these options: Online westcookymca.org. Contact the Member Engagement Desk at to be quickly set up for this option. In Person Visit the Member Engagement Desk, pick up a class registration form, fill it out and we ll process it for you right away. Required for first-time registrants. Via Fax Fax your registration form to us at Registration forms are available at the Member Engagement Desk and also online at westcookymca.org. Via Mail Sign-up by mail. 6 BECOME YOUR HEALTHIEST YOU! Come in and see one of our health experts for a free fitness consultation and let them help you map your personal plan for success. We can help you maximize your Y experience and set your goals for a healthier you. This plan includes a free fitness consultation where you can meet with a Personal Trainer or Healthy Lifestyles Coach to determine your fitness level. You can also enjoy a Wellness Center Orientation to become familiar with all of our equipment. For more details, contact Jill Moorhead at or jmoorhead@westcookymca.org. POLICIES & PROCEDURES CODE OF CONDUCT Always act according to the Y s four core values: Caring, Honesty, Respect and Responsibility. As a member of the Y, we ask that if you see something, say something. We are all responsible for our Y. Cell phone use is not permitted in locker rooms, pool deck, or Wellness Center. Please keep your phone on silent/vibrate in fitness areas. Respect others rights to participate in shared spaces. Use proper language to avoid offending others. Avoid loud, boisterous behavior that may offend or disturb other members or classes. Music, without the use of headphones, is not allowed. Have fun, but not at the expense of others! Everyone must carry their own Y membership card for front gate and locker room access. Individuals falsifying cards or allowing others to use their card will be subject to membership suspension or termination. Children under age 6 are permitted in opposite gender locker rooms. Please dry off in the shower area before entering the locker rooms. Other than the pool area, shoes and shirts must be worn at all times. No shoes on pool deck or in shower rooms. Shoe booties are provided for use on pool deck. Criminal penalties apply to the possession, use, or sale of drugs and/ or weapons. The Y will prosecute. Theft or intentional damage to property belonging to the Y or its members will result in criminal prosecution. Smoking (including e-cigarettes) or use of any tobacco product is not permitted on Y premises or outside its doors. No alcohol permitted on Y premises. The West Cook YMCA is a neutral zone and will not tolerate gang representation. The Y assumes no responsibility for personal injuries or loss of property while using our facilities. The Y has a zero tolerance policy for sexual harassment for all of those associated with the Y (members, program members, staff, volunteers, community partners, and anyone else coming in contact with the Y). No concealed weapons allowed on premises. FACILITY USE AGE GUIDELINES Safety is our primary concern. Certain areas of the Y are restricted to use by specific age groups or restricted by specific supervision guidelines, as listed below: FACILITY: Children under age 12 must be closely supervised by an adult (18+) when in the Y, including locker rooms, pool, and gym, unless in a supervised program. Ages 12+ can be in facility without a parent/guardian or adult (18+). LOCKER ROOMS: Ages 6+ must use same gender locker room. MEN S & WOMEN S SPAS: Ages 21+. Children are not allowed in Men s or Women s Spas. SWIMMING POOL: Under age 9 must have an adult (18+) in the pool with them at all times. Ages must have an adult (18+) in the facility. Children must be able to pass a swim test in order to swim without adult supervision in the pool. WELLNESS CENTER: All children ages must complete a Wellness Center Orientation prior to using equipment. Ages must be supervised by an adult (18+). FREE WEIGHT AREA: Ages 16+. Under age 16 may not use Free Weight Area unless supervised by a Personal Trainer. SPIRIT MIND BODY ROOM and INDOOR TRACK: Ages 8 15 are permitted with adult (18+) supervision. GROUP EXERCISE CLASSES: Ages 16+. Ages with adult (18+) supervision. BABYSITTING ROOM: Ages 6 weeks 9 years old. 6 The Y: For Youth Development, Healthy Living, and Social Responsibility.

7 REFUNDS & CREDIT POLICY A full refund will be given if the Y cancels a class or personal training session. If you withdraw from a class before it begins, a 100% refund will be given either by check or system credit to be used within one calendar year. Refunds by check usually take days to process. Once the class has begun and less than 50% of the class time has passed, participants that withdraw will be issued a 50% credit voucher (no refunds once classes have begun). After 50% of the class time has passed, no credit or refund will be given. No refund will be issued if you miss a personal training session without at least 24 hours notice. LOCKER ROOMS Full-size lockers are available for FREE daily use. Please bring your own lock. Please leave valuables at home or secure them in your vehicle. Daily use lockers with locks left overnight will have locks removed. Contents will be placed in the lost and found located at the Member Engagement Desk. Children ages 6+ must use same gender locker rooms (no exceptions). Please use Family Locker Room on Pool Deck. LOCKER RENTAL Locker rental is available for $5/month or $60/year. Fees are nonrefundable. Please see the Member Engagement Desk to purchase locker rental services. ATTIRE Proper attire is to be worn at all times while using the Y facilities. Street shoes and street clothes are not allowed in the gymnasiums, racquetball courts, exercise rooms, or pool area. Cut-offs and other street clothes are not allowed in the pool, whirlpool, and sauna. Proper swim wear is required in the pool. Eye guards are strongly recommended for racquetball play. Please refrain from wearing apparel with questionable imprints. FOOD AND BEVERAGES Food and beverage services are provided during special events and are restricted to certain areas of the facility. Please do not bring food or drinks into locker rooms, the gyms, the Wellness Center (bottled water and sports drinks are accepted), or activity areas. We appreciate your help in keeping your Y safe and clean. PHOTOGRAPHY RELEASE The Y will occasionally assign a photographer to take photographs of our members and participants in various activities for promotional purposes. Some of these photos will be used in Y marketing materials such as advertisements, press releases, mailers, and brochures. If you or a family member do not wish to be photographed, please step out of the photo or notify the photographer before the photo is taken. COMMENT CARDS Should you have suggestions or comments that do not require immediate attention but require a response, please feel free to use a comment card located at several areas in the building, including at the Member Engagement Desk. LOST AND FOUND The Y is not responsible for lost or stolen property. We do retain lost and found items whenever possible. Please contact the Member Engagement Desk to inquire about lost items. Items will be kept for 30 days and then given to charities in the community that are in need. FACILITY RENTALS Y facilities may be reserved for special occasions for a fee. Reservations can be scheduled by contacting the Member Engagement Desk at and asking to speak to a Coordinator. Please inquire at least 2 weeks prior to the date of rental.
