2 2 HOSPICE ANNUAL REPORT Dear Hospice supporters As I take a moment to reflect on the past year, I am filled with excitement about how far we have come. The 2015/2016 year brought a great deal of progress. As we worked to fulfil our strategic directions, we have been able to offer programs and services to more than 2,000 patients and families, reaching our hands out to our community and offering them support, education and empowerment along their journey. Since our founding in 1979, The Hospice has been community focused: through local, regional, and provincial partnerships, with the support of donors and volunteers, and under the guidance of our Boards, we have aimed to provide timely programs and services that place patients and families first. I am so proud to announce that we have taken a huge step forward in enhancing our ability to care for the Windsor-Essex community. On April 6, 2016 we opened the doors to a brand new, state-of-the-art, ten-bed residential home, the Erie Shores Campus, in partnership with Leamington District On behalf of the entire agency, Memorial Hospital and Foundation, which is nestled in the heart of Essex County. Already at full capacity, this residential home showcases I would like to offer my sincere our commitment to providing programs and services where the community is. This philosophy prompted an expansion of wellness gratitude to all of the community services into Kingsville this year. All of these accomplishments would not have been possible without considerable support from our donors, partners, volunteers and donors partners and volunteers thank you for making Windsor-Essex a kinder, gentler and more compassionate place to live. who championed The Hospice s The Hospice has pledged our support to the community by acting mission over the last year. as the hub for the Windsor-Essex Compassion Care Community, an initiative that is ensuring better links between service providers, volunteers and vulnerable populations. We are excited to see how this partnership will help to shape health care in the region in the year to come. On behalf of the entire agency, I would like to offer my sincere gratitude to all of the community partners and donors who championed The Hospice s mission this year. With your support, we have increased the sustainability of our fundraising initiatives and are helping to ensure that wellness, palliative and end-of-life care will always remain available in our community at no cost. Anyone who has visited The Hospice will know that our volunteers are at the heart of everything we do. This year, more than 750 volunteers used their considerable talents, energy and resources to ensure the best quality patient and family services. Donating over 60,000 hours annually, our volunteers are truly community heroes who help with every aspect of Hospice operations, governance, and patient care. Thank you for everything that you do! As we prepare to embark on a new fiscal year, I am excited to work with our Board of Directors, staff and volunteers to address the new strategic goals of the agency and most importantly to continue providing responsive programs and services for our patients and families. Chair, Board of Directors, The Hospice Dear Friends of Hospice, The past year has resulted in a great deal of change and growth for The Hospice of Windsor and Essex County. We celebrated the opening of Canada s first satellite residential home, the Erie Shores Campus. The partnership with Leamington District Memorial Hospital and Foundation has helped us to bring care closer to home, allowing families to remain together. I am so proud of how the community has come together to fulfil a dream in such a short amount of time. We are very grateful to the LHIN for their vision on this project; these new patient suites will improve access to care for all of our friends and neighbours in need. Stay tuned for updates on the project status next year. You will note in our governance and quality reports that The Hospice has made substantial strides towards accreditation through Hospice Palliative Care Ontario and the Canadian Association of Rehabilitation Facilities. I am confident this good work will continue next year. We have also dedicated significant attention to strategic planning. This was a year of transition as we completed our previous plan and worked with staff and community members to formulate new strategic goals. I would like to take a moment to remember our fellow Board Member and Hospice Champion, Brian Bildfell who died this year. Brian was a strong supporter of the Hospice movement and made special contributions to volunteer recruitment and retention through his committee work. He will be missed by every member of the Board of Directors. As we enter a new fiscal year, I would like to thank departing Board Members, Dan Amicone, Christine Davison, and Terry Shields for their service. Your expertise, energy and enthusiasm have been welcome additions to our Board and we wish you all the best in the future. The 2015/2016 year also marks the end of my term as Board Chair. Veronique Mandal will be assuming the position of Chair and I sincerely look forward to working with her next year. Finally, I would like to leave you with one final thought. Once again, we stand on the precipice of significant changes in the landscape of health care for all Canadians. Founded in opposition to assisted death and euthanasia in 1979, The Hospice of Windsor and Essex County remains committed to our mission to support, educate and empower anyone with a life-altering diagnosis. Despite recent challenges to our philosophy, we are continue to be steadfast in our dedication to providing holistic wellness and palliative care that offers a helping hand to patients, families and caregivers when they need it most. I hope that you will join me in championing the role of palliative care in our health care system as well as supporting the work of Hospice in the years to come. 5 HOSPICE ANNUAL REPORT 5 Governance report This year, the Governance Committee conducted a number of reviews and examined processes to ensure the organization continues to meet best practices in governance. The Committee developed templates for use by the Board of Directors and its Board Committees with regard to updating Terms of Reference and Committee Workplans. The Committee developed a series of Governance Policies that were adopted this past year to guide and support the activities of the Board. A process for recruitment was developed that will be put in place for future needs that ensures there is a fair and transparent process for the selection of volunteers on the Board of Directors and its Board Committees. Be a yardstick of quality. Some people aren t used to an environment where excellence is expected. Steve Jobs Following the review, revision and adoption of updated by-laws in June 2015, the Committee reviewed the organization s objects. The Committee recommended that they be updated to be consistent with pre-approved objects of the Ministry of the Attorney General as well as to reflect changes over the last 35 years in terms of current services. The Board approved this application at their November 17, 2015 meeting and seeks approval from the general membership this year. The above activities ensure that the Board of Directors meets governance standards consistent with Accreditation requirements for both the Hospice Palliative Care Association of Ontario and the Commission on Accreditation of Rehabilitation Facilities.
6 6 HOSPICE ANNUAL REPORT Audit & Finance report To give real service, you must add something that cannot be bought or measured with money, and that is sincerity and ingenuity. Douglas Adams The Hospice is committed to responsible fiscal management, fundraising, and donor stewardship. Once again, we are pleased to report that the 2015 / 2016 fiscal year yielded a balanced budget, while supporting expanded programs and services throughout the Windsor-Essex region, and the refresh and update of the Hospice Residential Home and the Children s Home. A joint effort between the Board Audit & Finance Committee, The Hospice team, and our many supporters, we are thankful to everyone who made this momentous year possible. Throughout the year, The Hospice expanded and renewed partnership agreements throughout the region, leveraging the significant assets present in Windsor and Essex County. We are grateful to our community partners for stepping forward to dream, plan and innovate with us - helping to improve the quality of life for all members of this vibrant community. We also owe a debt of gratitude to our funder the Erie St. Clair LHIN, and all of the individual and corporate donors who demonstrated their commitment to the hospice movement over the last year. Your donations, sponsorship and event attendance supported programs and services for more than 2,000 patients, families and caregivers facing a life-altering diagnosis. With the ever-changing landscape of health care, partnerships are now more important than ever. With your help, we were able to offer more than 50 wellness programs, children s programs, pain and symptom management, and palliative care in the community, and our two residential homes. In the upcoming fiscal year, we will continue to build, improve and sustain hospice care in this community.
7 HOSPICE ANNUAL REPORT 7 Volunteers are the Heart of Hospice Since 1979, the Hospice has relied on volunteers to support our patient and family services. From assisting with administration, fundraising, maintenance, property management and on committees and boards, to volunteering their time caring for patients in the Hospice Residential Home and the community, volunteers are the life-blood of Hospice. Currently, we have more than 750 active volunteers who amaze us with their selflessness, generosity of spirit, kindness and compassion each and every day. On behalf of the entire agency, we would like to offer our sincere gratitude to all of the volunteers who ensure that the Hospice is able to offer programs and services at no cost. 4,288 patients on hospital roster 8,266 visits with Residential Home patients 1,960 hours of respite care 500 burden bears made

The palliative liaison program monitors Hospice patients in the hospitals and helps to facilitate their transfer back to the community or to one of our residential homes. 9 HOSPICE ANNUAL REPORT 9 Quality Care report This year, the Quality Committee began each meeting with a patient storey. This important and new function allowed for each committee member to better connect and understand the services provided within the community and impact they have on our patients and families. We continued our review of the Quality and Board of Directors dashboards to ensure the effective monitoring of agency goals that are consistent with the strategic directions of the organization. The Committee also provided general oversight to the accreditation process for our application to the Hospice Palliative Care Association Visiting Hospice program. OUR PALLIATIVE LIAISON SAVED AN AVERAGE OF 201 HOSPITAL BED DAYS PER MONTH BY FACILITATING APPROPRIATE COMMUNITY CARE The committee continues to provide oversight while The Hospice prepares for Accreditation with the Commission on Accreditation for Rehabilitation Facilities (CARF). As part of this process, we reviewed our first report related to Risk Management trends within the organization. Our goal is to apply for accreditation with CARF in December Moving forward, the Committee will continue to monitor survey results completed by patients/families, volunteers and staff. We will also focus on ensuring that Hospice maintains and improves its quality practices within Windsor and Essex County. Quality is not an act, it is a habit. Aristotle
10 10 HOSPICE ANNUAL REPORT Foundation Report Established in 2011, the Foundation is committed to cultivating a model of community engagement and philanthropy, encouraging endowment building and maintaining the highest standards of donor respect, integrity, stewardship and public accountability. The Foundation contributes funds to The Hospice of Windsor & Essex County Inc., building upon more than three decades of excellence in community based hospice palliative care. The goal is to support the Hospice s ongoing commitment to provide the highest quality of care to the communities of Windsor and Essex County through programs and services, patient empowerment and education. The Foundation also assists in facilitating research and capital building projects. Throughout the 2015 / 2016 the Foundation has addressed three strategic directions: STRATEGIC DIRECTION 1: GROW THE ENDOWMENT TO $10 MILLION Currently, the Foundation is working towards increasing the endowment funding to support Hospice operations, while also building awareness in the community about the nature of the Foundation, with its separate role for operational fundraising. To date, our current Board of Directors have been excellent stewards of the Foundation s establishment and management of current endowments. As of March 31, 2016 the Foundation held $4.1 million in assets, an increase of approximately $500,000. The Foundation and all subcommittees will be working closely together to create benchmarks of achievable funding and expansion targets over the next 3-5 years.
11 HOSPICE ANNUAL REPORT 11 As we move forward, the Foundation has established a Governance and Nominating Committee. The role of this will be to review current and create new policies and procedures as well as appoint membership to all other committees. The Foundation and all subcommittees will be working closely together to create benchmarks of achievable funding and expansion targets over the next 3-5 years. New members of subcommittees will be vital in establishing these benchmarks, bringing their expertise, knowledge and connections to help us move steadily forward in the solicitation and management of new major gifts. STRATEGIC DIRECTION 2: CREATE A CASE FOR ENDOWMENT THAT CAN BE LAUNCHED IN THE COMMUNITY Educating the public about the role of the Foundation has been a major focus of the Board and the Marketing and Communications Committee. We have created branding for the Foundation, including a logo, brochure, giving information, website portal and integration into the fundraising efforts of the Hospice. As we move forward with creating a comprehensive case for endowment and legacy giving, we will help to decrease the number of fundraising events, instead streamlining existing events to create more revenue and investigate sources of sustainable funding. The Marketing and Communications Committee is drafting an engaging case for endowment to support our donation requests. Once created, the case for endowment will help the public to understand the role of the Foundation, the difference between operational fundraising and legacy gifts, as The Hospice for Life Foundation Board of Directors 2015 / 2016 Kevin Marshall Chair John Blanken Treasurer Dr. Donald J. Rudzinski Past Chair well the role of the Foundation in supporting Hospice programs and services. We are also pleased to announce the creation of the Special Prospects Committee. This subcommittee will take on the task of identifying opportunities and developing a plan for the solicitation of major endowment gifts. Members of the Special Prospects Committee will work closely with the Marketing and Communications Committee to promote awareness about the Foundation and giving opportunities. STRATEGIC DIRECTION 3: SUPPORTING THE WORK OF HOSPICE IN EDUCATION AND TRAINING THROUGH AN EDUCATIONAL FORUM To address this goal, the Foundation is investigating opportunities for hosting educational events through conferences locally or regionally. We are investigating sources of endowment funding that may help to achieve this target and also raise the profile of the Foundation through association with premiere educational opportunities. We are reaching the next stage of our expansion and stand on the crux of an exciting time for our community, helping to enhance the profile and sustainability of the Hospice and the Foundation who touch thousands of lives each year. In the upcoming year, we will continue to work towards our strategic goals, develop our members and create policies and procedures that support outstanding wellness, hospice palliative care and continuing education in our community. Finally, we would like to thank all of the donors, supporters and champions who have created and sustained the Foundation. We are grateful for your vision, passion and commitment to hospice palliative care - today, tomorrow and for the future. ENDOWMENT HOLDINGS +$500,000 Kevin Marshall Chair, Board of Directors, The Hospice for Life Foundation In my term as chair of the Hospice for Life Foundation Board, I was privileged to see exponential growth, not only in endowment funding but in building for future stability in order to create financial autonomy for The Hospice. Currently, the Foundation holds $4.1 million in endowment funding. Our achievements are a testament to our donors and to the vitality, dedication and expertise of the Board members who have guided us through the developmental stages. I am confident that the Board will continue to support outstanding wellness and hospice palliative care and ensure that The Hospice will be able to continue supporting our friends and neighbours when they need us most.
12 Veronique Mandal Incoming Chair, Board of Directors, The Hospice It gives me great pleasure to take on the role of chair of the Board of Directors for The Hospice. As Hospice begins to celebrate 37 years of service in the community, there is a renewed commitment from The Board, staff and volunteers to offer the best possible care to those dealing with a life-altering diagnosis, as well as their families and caregivers. It is indeed an honour to be able to work with dedicated board members, Hospice staff and exceptional volunteers as we all move forward in our focus on compassionate caring. This will be an exciting time that will see the agency take new strides in strategic planning and continue to build important relationships with community partners and service groups. I look forward to the work that lies ahead of us. Dan Amicone Incoming Chair, Board of Directors, The Hospice for Life Foundation I am extremely delighted to assume the role of chair of the Hospice for Life Foundation Board. In moving from a role on the agency Board now to the Foundation, I hope to continue the growth that has taken place since the Foundation began. It is imperative that branding of the Foundation explains why we exist and what our purpose is for the agency. With community understanding, the Foundation will flourish. Our greatest benchmark as a Board will be to grow the current endowment of $4.1 million to $10 million. Having a source of sustainable funding will allow for the continued success of Hospice now and into the future. About the WECCC There s an old proverb that states: It takes a village to raise up a child. The Windsor-Essex Compassion Care Community (WECCC) is an organization dedicated to recreating that village, one that will sustain not only our children, but also our parents, our neighbours and ourselves. WECCC is working together with our network of partners, building a community of citizens, families, neighbours, organizations and leaders to provide care, growth and protection for all, starting with the most vulnerable among us. Our current goal is to ensure compassionate care to the elderly, those with long term disabilities and especially people at high risk for isolation (living alone, in poverty, newcomers and those with language barriers). To address this goal, we are creating a framework of collaboration drawn from healthcare, education, business, long-term care, human services, cultural groups, volunteers and many other committed organizations and citizens across Windsor-Essex. With funding provided by the Local Health Integrated Network (LHIN), The Greenshield Foundation, Home Instead Senior Care, and our network of volunteer support, WECCC is planning the first pilot project in Leamington later this year. WECCC is being graciously hosted in the Hospice Residential Home (HRH) on Empress Street in Windsor. Please give us a call at ext or stop by to visit us.
