2 2 HOSPICE ANNUAL REPORT Dear Hospice supporters As I take a moment to reflect on the past year, I am filled with excitement about how far we have come. The 2016/2017 year brought a great deal of progress. As we worked to fulfil our strategic directions, we have been able to offer programs and services to more than 2,100 patients and families, reaching our hands out to our community and offering them support, education and empowerment along their journey. Since our founding in 1979, The Hospice has been community focused: through local, regional, and provincial partnerships, with the support of donors and volunteers, and under the guidance of our Boards, we have aimed to provide timely programs and services that place patients and families first. This year we were proud to celebrate the first anniversary of our state-of-the-art, ten-bed residential home, the Erie Shores Campus, in partnership with Leamington District Memorial Hospital Foundation (Erie Shores Health Foundation), which is nestled in the heart of Essex County. Serving On behalf of the entire agency, patients and families in the county, this residential home showcases our commitment to providing programs and services where the I would like to offer my sincere community is. This philosophy prompted an expansion of wellness services into Kingsville over the last year, including fitness programs, gratitude to all of the community guitar lessons, and even a coffee house. During the past year, the pilot phase began for the Sharing My Journey program, which has been instrumental in assisting patients in the community with their health care needs and goals. We are excited to see how this partnership will expand, develop, and grow, shaping health care in the region. On behalf of the entire agency, I would like to offer my sincere gratitude to all of the community partners and donors who championed The Hospice s mission this year. With your support, we have increased the sustainability of our fundraising initiatives and are helping to ensure that wellness, palliative and end-of-life care will always remain available in our community at no cost. Anyone who has visited The Hospice will know that our volunteers are at the heart of everything we do. This year, more than 750 volunteers used their considerable talents, energy and resources to ensure the best quality patient and family services. Donating over 68,000 hours annually, our volunteers are truly community heroes who help with every aspect of Hospice operations, governance, and patient care. Thank you for everything that you do! As we prepare to embark on a new fiscal year, I am excited to work with our Board of Directors, staff and volunteers to address the new strategic goals of the agency and most importantly to continue providing responsive programs and services for our patients and families. We celebrated the first anniversary of Canada s first satellite residential home, the Erie Shores Campus. Our partnership with Leamington District Memorial Hospital Foundation (Erie Shores Health Foundation) has helped us to bring care closer to home, allowing families to remain together. I am so proud of how the community has come together to fulfil a dream in such a short period of time. This service expansion will add 25% capacity to our Residential Home with two additional patient suites, two more family living spaces, a wonderful four-season sun porch, outdoor gardens and a state-of-the-art laundry facility. This project is only possible with the generous support of the Erie St. Clair LHIN which will fund nursing care for the two additional patient beds. We are most grateful to our partners at the LHIN for their vision on this project and, indeed, for all of our community partners and supporters who will help us make this $1 million capital campaign happen. You will note in our governance and quality reports that The Hospice has made substantial strides towards accreditation through the Canadian Association of Rehabilitation Facilities. We truly believe this step will demonstrate our commitment to ongoing quality improvement in our programs, services and governance. We have also dedicated significant attention to addressing our strategic plan. With objectives that address patient and family services, education, administration and governance, we continue to look to the future and the best possible methods of delivering community needs. As we enter a new fiscal year, I would like to thank departing Board Members, Dr. Mary Broga, Past Chair, and Dan McCulloch. Your expertise, energy and enthusiasm have been welcome additions to our Board and we wish you all the best in the future. We look forward to welcoming new members to our board in 2016/2017, whose skills and talents will also complement our existing membership. Finally, I would like to leave you with one last thought. Once again, we are dealing with changes in the landscape of health care for all Canadians. Founded as an alternative to assisted death and euthanasia in 1979, The Hospice of Windsor and Essex County remains committed to our mission to support, educate and empower anyone with a life-altering diagnosis. We continue to be steadfast in our dedication to providing holistic wellness and palliative care that offers a helping hand to patients, families and caregivers when they need it most. I hope you will join me in championing the role of palliative care in our health care system as well as supporting the work of Hospice in the years to come. 5 HOSPICE ANNUAL REPORT 5 Governance report This year, the Governance Committee continued with its mandate to ensure effective practices are in place for the fufillment of all responsibilities within the purview of the Hospice Board of Directors. We reviewed and recommended Board Committee and Advisory Committee Terms of Reference, and work plans in preparation for accreditation to the Board. We also successfully completed a recruitment process to replace three members of the Board. During the year in review, the orientation process for new staff and board members was revised and was piloted in the summer of Good governance is putting people at the centre of the development process. Anonymous In February, the Governance Committee facilitated a joint Hospice and Foundation Boards of Directors education session to provide ongoing education on governance and board accountability. This session was well received and helped all of our new and existing members to become familiar with emerging research and best practices. In addition, under the terms of a new by-law approved in 2015, the Governance Committee reviewed the Objects of the Corporation which were approved by the Ontario Ministry of Government Services in September In summation, the activities outlined above ensured the Board is compliant with all legislation impacting both governance and the Hospice and will meet the standards for accreditation with the Committee on Accreditation of Rehabilitative Services during our application scheduled for May 2017.
6 6 HOSPICE ANNUAL REPORT Audit & Finance report Here is a simple but powerful rule: always give people more than what they expect to get. Nelson Boswell The Hospice is committed to responsible fiscal management, fundraising, and donor stewardship. Once again, we are pleased to report that the 2016 / 2017 fiscal year yielded a balanced budget, while supporting expanded programs and services throughout the Windsor-Essex region. A joint effort between the Board Audit & Finance Committee, The Hospice team, and our many supporters, we are thankful to everyone who made this year of growth possible. Throughout the year, The Hospice expanded and renewed partnership agreements throughout the region, leveraging the significant assets present in Windsor and Essex County. We are grateful to our community partners for stepping forward to dream, plan and innovate with us - helping to improve the quality of life for all members of this vibrant community. We also owe a debt of gratitude to our funder the Erie St. Clair LHIN, and all of the individual and corporate donors who demonstrated their commitment to the hospice movement over the last year. Your donations, sponsorship and event attendance supported programs and services for more than 2,100 patients, families and caregivers facing a life-altering diagnosis. With your help, we were able to offer more than 50 wellness programs, children s programs, pain and symptom management, and palliative care in the community, and our two residential homes. In the upcoming fiscal year, we are excited to begin the expansion of our Windsor Residential Home services by adding two additional patient suites, a four season sun porch, two family rooms, in addition to state of the art laundry facilities. This project is being supported by the Erie St. Clair LHIN who will be funding the additional nursing care, as well as our private and corporate capital donors. The Residential Home service expansion project will increase our capacity by 25% and allow us to admit about 40 additional families each year. This is a significant addition to our community, that is only possible with the overwhelming community support in this region. We look forward to working with our stakeholders to ensure the continue success of Hospice programs for all of our friends and neighbours in need. 7 HOSPICE ANNUAL REPORT 7 Volunteers are the Heart of Hospice Since 1979, the Hospice has relied on volunteers to support our patient and family services. From assisting with administration, fundraising, maintenance, property management and on committees and boards, to volunteering their time caring for patients in the Hospice Residential Home and the community, volunteers are the life-blood of Hospice. Currently, we have more than 750 active volunteers who amaze us with their selflessness, generosity of spirit, kindness and compassion each and every day. On behalf of the entire agency, we would like to offer our sincere gratitude to all of the volunteers who ensure that the Hospice is able to offer programs and services at no cost. 4,273 visits with patients in the hospital 7,974 visits with Residential Home patients 2,748 hours of respite care 2,793 hours creating burden bears

9 HOSPICE ANNUAL REPORT 9 Quality Care report The Quality Committee begins each meeting with a patient story. These stories provide a framework that highlights the importance of quality and the impact services have on our patients, families, and caregivers. We continued to review in detail the Quality and Board of Directors Dashboards and the Risk Management Trend Report to ensure the effective monitoring of agency goals and targets. Key outcomes for this committee are to ensure that: quality objectives are aligned with the strategic directions for the organization; our services meet the expectations of both the people we serve and our community partners; that The Hospice achieves the accreditation standards for both the Hospice 98% of patients in the palliative medicine program at the Hospice spent their days at home supported by our holistic team of service providers Palliative Care Association and the Commission on Accreditation of Rehabilitative Services; and that Hospice has established processes to receive input from our patients/families/caregivers, volunteers and staff. Moving forward, the Committee will continue to monitor quality processes so that The Hospice maintains accreditation standards and improves its quality practices within Windsor and Essex County. 10 10 HOSPICE ANNUAL REPORT Foundation Report Established through the vision and generosity of the Toldo and O Neil families in 2011, the Foundation is committed to cultivating a model of community engagement and philanthropy, encouraging endowment building and maintaining the highest standards of donor respect, integrity, stewardship and public accountability. The Foundation contributes funds to The Hospice of Windsor & Essex County Inc., building upon more than three decades of excellence in community based hospice palliative care. The goal is to support the Hospice s ongoing commitment to provide the highest quality of care to the communities of Windsor and Essex County through programs and services, patient empowerment and education. The Foundation also assists in facilitating research and capital building projects. Throughout the 2016 / 2017 fiscal year, the Foundation continued to address three strategic directions: STRATEGIC DIRECTION 1: GROW THE ENDOWMENT TO $10 MILLION Currently, the Foundation is making progress towards increasing the endowment funding to support Hospice operations, while also building awareness in the community about the nature of the Foundation, with its separate role for operational fundraising. To date, our current Board of Directors have been excellent stewards of the Foundation s establishment and management of current endowments. As of March 31, 2016 the Foundation held $4.67 million in assets, an increase of approximately $570,000. The Foundation goal is to support the Hospice s ongoing commitment to provide the highest quality of care to the communities of Windsor and Essex County.
11 HOSPICE ANNUAL REPORT 11 During the past year, the Governance and Nominating Committee reviewed and updated the policies and procedures for the Foundation. In addition they took the lead on identifying and nominating new members of the board and subcommittees. The Foundation and all subcommittees are working closely together to meet benchmarks of achievable funding and expansion targets over the next 3-5 years. New members of subcommittees have been vital in establishing these benchmarks, bringing their expertise, knowledge and connections to help us move steadily forward in the solicitation and management of new major gifts. STRATEGIC DIRECTION 2: CREATE A CASE FOR ENDOWMENT THAT CAN BE LAUNCHED IN THE COMMUNITY Educating the public about the role of the Foundation has been a major focus of the Board and the Marketing and Communications Committee. Our strategy has been to closely align the brands and public outreach efforts between the Foundation and the Hospice. To that end, we have amalgamated the Marketing and Communications Committees for the Hospice and the Foundation, with representation from each board. This step has helped us to coordinate our community presence and ensure consistent messaging throughout the region. With the cooperation of the Marketing and Communications committee, the Foundation has created engaging cases for support that are assisting with the growth of endowment funds. Focusing on particular programs and services, the cases for support provide transparent information to current and potential donors, while also creating a sustainable future for the Hospice. To The Hospice for Life Foundation Board of Directors 2016 / 2017 Dan Amicone Chair John Blanken Treasurer Kevin Marshall Past Chair Members at Large: Tim Beaulieu date, this strategy has been very successful and will continue to be developed in the upcoming fiscal year. The Special Prospects Committee has been an important facet of our endowment growth strategy. Providing their advice and contacts, this Committee has helped to identify potential donors who may be interested in supporting the work of Hospice. Currently the priority of the Foundation is helping to develop the service expansion of the Hospice Residential Home. STRATEGIC DIRECTION 3: SUPPORTING THE WORK OF HOSPICE IN EDUCATION AND TRAINING THROUGH AN EDUCATIONAL FORUM The Foundation remains committed to ongoing educational training. In March 2017, the Hospice and Foundation boards hosted a joint retreat on board governance and best practices. This forum was very successful, building the knowledge and capacity of respective members, and empowering them to address the needs of the Hospice for the future. Rudzinski the Foundation Board is investigating opportunities for board and governance accreditation. We are eager to demonstrate our commitment to ongoing growth, education, and excellence to our community partners and donors. We are also investigating opportunities for hosting educational events through conferences locally or regionally. To achieve this goal, we have created cases for support that would provide endowment funding to offset the cost of speakers, venues, and administration. Finally, as a Board, we are proud of the achievements over the past year. The continued growth and development of the Foundation remains our top priority and we anticipate great things in the year to come. ENDOWMENT HOLDINGS +$570,000 Dan Amicone Chair, Board of Directors, The Hospice for Life Foundation During my first year as Chair of the Foundation, I was pleased to see the trend of growth and developments continue through this fiscal year. Adding endowment funding of approximately $570,000, the Foundation is steadily progressing towards its goal of $10 million in total funds. As the Past Chair of the Hospice Board of Directors, this was an excellent opportunity to see the close link between the Foundation and the Hospice. It is a testament to our donors, community partners, and supporters that this community steps forward to assist with day-to-day operations, but also has the foresight to see the benefit of long-term legacy investment, ensuring Hospice services will be available for generations to come.
12 WECCC Initiatives There s an old proverb that states: It takes a village to raise up a child. The Windsor-Essex Compassion Care Community (WECCC) is an organization dedicated to recreating that village, one that will sustain not only our children, but also our parents, our neighbours and ourselves. WECCC is working together with our network of partners, building a community of citizens, families, neighbours, organizations and leaders to provide care, growth and protection for all, starting with the most vulnerable among us. Our current goal is to ensure compassionate care to the elderly, those with long term disabilities and especially people at high risk for isolation (living alone, in poverty, newcomers and those with language barriers). To address this goal, we are creating a framework of collaboration drawn from healthcare, education, business, long-term care, human services, cultural groups, volunteers and many other committed organizations and citizens across Windsor-Essex. With funding provided by the Local Health Integration Network (LHIN), The Greenshield Foundation, and our network of volunteer support, WECCC is being graciously hosted in the Hospice Residential Home (HRH) on Empress Street in Windsor.
