2 1. Intrductin SA Ambulance Service staff perate in a unique and ften challenging envirnment and the demands, bth physical and mental, are many and varied. A key element in any strategy fr effectively managing fatigue is the recgnitin and understanding that effective management is a shared respnsibility which requires a cllabrative apprach by bth SAAS and its staff. SAAS is cmmitted t prviding and maintaining a safe and healthy wrk envirnment fr all staff, by actively wrking t mitigate the risks assciated with fatigue, thrugh safe systems f wrk, and fstering a culture where fatigue management is taken seriusly at all levels f the rganisatin. Due t the unpredictable nature f the 24/7 emergency peratinal envirnment, fatigue remains a challenge fr SAAS as an rganisatin and ur staff as individuals. Understand hw fatigue impacts n yur fitness fr duty and yur safety. Develp strategies t avid becming fatigued and practices t mitigate the impact f fatigue n factrs relating t fitness fr duty. 3 2. Fatigue Plicy and Prcedure Why d we care abut fatigue? Fatigue can ptentially impact an individual's perfrmance and ability t cmplete safety critical tasks e.g. driving, cmplex clinical interventins and critical decisin making. Fatigue can impact a persn's ability t perfrm wrk tasks safely and effectively and has the ptential t impact patient safety. Fatigue increases the ptential fr situatins that may result in an adverse patient safety incident. It has been identified that by increasing awareness f fatigue in the wrkplace and prviding educatin t staff n ways t evaluate and manage their fatigue, we can reduce the hazard faced by them. As fatigue management is a shared respnsibility it is imprtant fr everyne t be aware f fatigue, its causes and activities t mitigate r manage the risk. The respnsibilities f each party are discussed later in this package. Frtunately, between SAAS and ur staff, there are things we can d t manage and mitigate fatigue! Fatigue is nt new... What has changed? SAAS has been wrking in partnership with Integrated Safety Supprt (ISS) t review ur current Fatigue Management Plicy & Prcedure. Their advice is based n current science and best practice and is tested in the real wrld. SAAS' previus prcedure fcused n predicting the likelihd that a staff member may be impacted by fatigue at a future pint in the shift. As individual requirements fr sleep and susceptibility t fatigue vary greatly between peple we had n way f assessing hw significantly a staff member was impacted by fatigue at a given 'pint in time'. The new prcedure intrduces the Fatigue Self-Assessment Tl (FSAT) which is designed t assist staff in identifying relevant signs and symptms f fatigue as it relates t their current state. The FSAT is able t be used at any time f day r night, and creates impetus t emply strategies t mitigate the impact f fatigue t ensure safety f staff. 4 3. Drivers fr Fatigue Management Primary The SAAS Fatigue Risk Management System (FRMS) is designed t ensure the safety f ur staff and the patients entrusted t ur care. Secndary Imprving staff wellbeing thrugh best practice mitigatin strategies. Ensuring cmpliance with the increasing cmplexity f legislatin. Managing wrk perfrmance. What causes fatigue? Fatigue can be wrk related r nn-wrk related and can accumulate ver time. Things that can cntribute t an individual's level f fatigue may include, but nt be limited t: Prlnged mental/physical activity (time n task) Sleep lss r prlnged wakefulness High wrklad Wrk at night r in the early hurs f the mrning may require extra cnsideratin f preparatry sleep (sleep that is taken in advance f a perid that is expected t induce a degree f fatigue) and strategic napping (A shrt nap taken t increase alertness) Circadian phase (time f day) Wrklad plays a rle in the experience f fatigue. In SAAS ur wrklad is unpredictable with limited ability t influence cntrl ver every element f it. Wrklad demand can take n a number f dimensins including physical, mental r time pressure. Fatigue can be felt bth when there is t little wrklad r when there is t much wrklad. Nn wrk related factrs which may influence fatigue include sleep disrders e.g. sleep apnea, family and scial respnsibilities, sleep sacrificing due t excess scialising, emtinal stressrs, travelling lng distance prir t cmmencing wrk and als physical activity. In rder t manage fatigue in the unpredictable peratinal envirnment varied appraches are required including: Taking every pprtunity fr a break. A nap, even as shrt as 10 minutes, has been shwn t have benefits t decrease feelings f fatigue and imprve alertness. Rtating tasks when practicable can help, but is nt always feasible (e.g. sl respnders). Cpied frm SAAS elearning

5 Prlnged driving is a majr risk Fatigue is thught t be a factr in up t 20% f rad accidents wrldwide. Accidents caused by fatigue can be extremely serius and in cases where the driver falls asleep at high speed, the cnsequences can be fatal. Fatigue frm lack f sleep is an imprtant factr hwever, the length f the jurney, the type f driving (ne lng cuntry trip versus many shrt urban nes) and the time since a break frm driving are als significant. Fatigue impairment can make it mre difficult t keep the vehicle within the lane, respnd t unexpected situatins, and keep a safe buffer between yu and ther vehicles. Hw much sleep d I need? Sleep is essential fr basic human functining including maintaining alertness and perfrmance. While mst peple will feel tired n a night shift cmpared t day shift, just as there are individual differences in sleep need, there are als differences in preferred sleep timing and the ability t adapt t shift wrk. It is imprtant t nte that the same rster and lifestyle factrs may cause different levels f fatigue in different peple.the exact amunt f sleep that each individual needs t perfrm ptimally each day differs but, in general is between 7 and 9 hurs. Sleep is mst valuable when it is btained in a single blck, but a split sleep r a number f shrt sleeps is definitely better than nt getting any at all. A shrt sleep (nap) can prvide a pwerful bst t alertness. Reflectin: Apprximately hw much sleep d yu get n average per 24 hurs? What is 'Fit fr Duty' 'Fit fr Duty' is a state in which an individual presents fr wrk adequately prepared s as t be in a physical, mental and emtinal state that enables them t perfrm the essential functins f their wrk in a manner that des nt threaten the safety r health f themselves, c-wrkers, prperty, patients r the public. The respnsibility t present fit fr duty rests with the individual, as stipulated in WHS legislatin. If an emplyee is nt able t attend wrk fit fr duty fr any reasn they have a further respnsibility t reprt that t their emplyer. Emplyee's effrts t ensure fitness fr duty cmplement SAAS's respnsibility t prevent and manage wrk-related fatigue risks. This isn't always easy given ur perating envirnment, but SAAS is cmmitted t mitigating these risks where pssible. S what can I d? Managing fatigue is a shared respnsibility. One f the key elements is adequately preparing yurself fr shift. It is critical t prepare in advance f expected r pssible future demands, s that yu are nt always in recvery mde. A significant factr in the severity f fatigue symptms is lack f preparatry sleep. That is, individuals wh are nt well prepared r wh are sleep deprived befre a shift can experience mre difficulty than thse wh are well rested. If an individual is nt fully rested befre wrk and cmmences duty with a sleep debt, then generally it culd be anticipated that the sleep debt will wrsen thrughut the shift. The recmmendatin is t maximise the amunt f sleep at least ne and preferably tw days befre wrk. Cnsider yur preparatin and recvery time when deciding t undertake vertime r shift swaps. Preparatry sleep vs Recvery Sleep Preparatry sleep is sleep that is taken in advance f a perid that is expected t induce a degree f fatigue. E.g. in the afternn befre a rstered night shift. It can be taken in the frm f lnger sleeps, and als naps which may be taken during a shift in anticipatin f further call uts during the given shift. Preparatry sleep is practive in nature, t prepare fr future wake perids, rather than reactive in nature as is the case fr recvery sleep. Recvery sleep ccurs after ne r mre perids f sleep lss, which can be acute (e.g. in ne 24 hur perid) r cumulative (e.g. thrughut 7 day perid). A recvery sleep may be slightly lnger than usual but lst sleep is nt recvered hur fr hur. Cpied frm SAAS elearning

7 Recgnising Fatigue Peple ften believe that yawning, heavy eyelids and ndding ff are the first nticeable signs f fatigue but by the time yu ntice these types f signs f fatigue, yur perfrmance may already be impaired. Signs f fatigue Significant symptms f fatigue include: 1. Falling asleep r having a micr-sleep 2. Nt reacting t a safety-critical alert r infrmatin in an acceptable timeframe 3. Having lapses in attentin during safety-critical activities (e.g. driving) Other signs f bth physical and mental fatigue may include, but are nt necessarily limited t: Cmmn physical signs f fatigue Yawning Pr hand eye crdinatin Slwer reflexes and reactin times Lack f energy, increased lethargy Speech difficulties (e.g. slurred, slwed r garbled) Blurred visin r reduced visual perceptin Cmmn mental signs f fatigue Irritability, impatience Difficulty cncentrating and limited attentin span (decreased alertness) Increased risk taking Autmatin ('aut pilt' behaviur) Impaired judgment, prblem slving Difficulty making decisins Inability t be mtivated t cmplete required wrk. 9 5. Assessing Fatigue The management f fatigue is a respnsibility that is shared by management and staff. As part f the management prcess it is cnsidered essential that staff are prvided with tls t help them assess their wn ptential levels f fatigue risk expsure. Tw methds have been identified fr assessing fatigue in the peratinal envirnment: 1. The Fatigue Calculatr - which is a predictr f pssible fatigue at a later time. This shuld be cmpleted prir t yur shift t determine the ptential t becme fatigued during yur shift. 2. The Fatigue Self-Assessment Tl - as a 'pint in time' assessment f yur current fatigue level. This can be dne at any time yu feel cncerned that yu culd be impacted by fatigue. Fatigue Calculatr - predicting fatigue! In terms f predicting fatigue, the Fatigue Calculatr (FC) will be utilised t give an indicatin f a fatigue level at a future pint in time during a rstered shift. It predicts the likelihd that yu may feel fatigued during yur shift. The use f this tl allws fr practive assessment f fatigue and implementatin f mitigatin strategies fr managing fatigue prir t reaching a definitive pint f fatigue that may therwise see the staff member being determined unfit fr duty. The Fatigue Calculatr uses minimum sleep and maximum awake rules as a basic assessment tl. It is a tl t enable staff t predict their pssible risk f fatigue at a future pint in time and manage it pre-emptively either n their wn r in cnjunctin with the SDM. The Prir Sleep Wake Mdel (PSWM) is cmprised f 3 simple calculatins: 1. X Hurs sleep in the prir 24 hurs 2. Y hurs sleep in the prir 48 hurs 3. Length f wakefulness frm awakening t end f wrk Befre cmmencing a shift every staff member shuld calculate their prir wake threshld and when btained sleep desn't meet the criteria, the scre will demnstrate an increased risk f ptential fr fatigue related errr. On call staff are required t submit their fatigue calculatr scre t EOC admin by 2:30pm during the day befre their first n call shift, in rder that high fatigue scres can be cnsidered and pre-emptively managed when required. 10 EOC admin will then cllate and frward t the Cuntry On-Call Operatins Manager and SDM t allw sufficient time fr any pre-emptive strategies t be determined and implemented if required. Must be nline t access the nline fatigue calculatr - A level 2 scre (between 5-8 n the fatigue calculatr prir t cmmencement f shift) shuld be reprted t the SDM by phne. The SDM will: Make reasnable effrts t mitigate fatigue. Direct staff member t undertake FSAT t determine if they are currently fit t cmmence/cntinue shift. Dcument the incident in the shift lg. Yu need t: Increase yur selfsupervisin and mnitr wn alertness. Lk fr signs f fatigue. Ask yur clleagues t mnitr yu. Use apprpriate napping where pssible. The SDM may allcate up t a 2 hur restrative rest break, restructure wrk tasks r reallcate yu t a lwer wrk demand area if pssible. NOTE: Staff affrded the pprtunity fr a restrative rest break are still able t be tasked t emergency cases during this time if n ther apprpriate resurces are available. Arrangements will then be made t recmmence the restrative rest break as sn as is practicable after the case. A level 3 scre (9+ n the fatigue calculatr prir t cmmencement f shift) be reprted immediately t the SDM by phne t discuss ptins. The SDM will direct the staff member t undertake FSAT n cmpletin f restrative break t determine if they are fit t cmmence shift. Yu need t: undertake FSAT t determine fitness fr duty. 14 6. Fatigue Self-Assessment Tl (FSAT) Hw can I assess my current level f fatigue? Fatigue can shw itself thrugh a variety f signs and symptms, and at varius levels f severity. As we mve alng the spectrum f rested t fatigued, the effects f fatigue becme mre evident and bradly speaking shw up in the fllwing sequence: Md changes (e.g. irritability, mre easily frustrated) Cmmunicatin quality deterirates. Speed f perfrmance (physically and mentally) declines. Yu can start t make mistakes - yur ability t take in and prcess infrmatin is impaired. This affects yur ability t make decisins. The Fatigue Self-Assessment Tl (FSAT) is a means f assessing if yu are currently impacted by fatigue, and if s hw significantly yu are impacted. It is a simple tl fr assessing yurself and can be dne at any time f day r whenever yu feel that yu may be impacted by fatigue. The Fatigue Self-Assessment Tl (FSAT) The FSAT askes three questins. T determine yur verall fatigue categry - yu simply answer all three questins. The three categries f fatigue risk are Red, Amber, and Green. If ne r mre answer is red, yur fatigue categry is RED. If ne r mre answer is amber, but there is n red answer, yur fatigue categry is AMBER. If all f yur answers are green, yur fatigue categry is GREEN. 15 FSAT scres Green - Great! Yu are well rested and prepared fr wrk. Cntrls and mitigatin strategies culd include: Cntinue t self-mnitr and watch fr signs that fatigue may be setting in. Watch yur fd intake and keep hydrated. Amber Lk ut fr each ther and initiate a peer discussin. Cntrls and mitigatin strategies culd include: Any f the strategies identified fr the green categry. 16 Take pprtunities t nap strategically (10-30minutes) Restructuring f tasks and wrk breaks in cnsultatin with the SDM t, ensure the least impact n service delivery. Examples f ways that the SDM may restructure tasks includes: Relcating t lwer demand wrk area. Fllwing cnsultatin, a staff member may be std dwn fr a restrative rest break (fr up t tw hurs). The staff member must utilise this pprtunity fr sleep. Transprt may be arranged back t statin (r hme fr n call staff) t reduce the risk f driving while fatigued. Fr n call crews: The staff member shuld nt turn ff their pager during their restrative break, as the actin listed belw will mean the staff member will nt be paged during their rest break. If nly ne crew member is reprting high fatigue the SDM will discuss with the ther member their capacity t perate as a single respnder. The crew shuld advise the EOC and SDM if they are the "A r "B call sign and ensure they have the crrect pager. If the fatigue level is nt reduced at the cmpletin f the restrative rest perid it is the staff member's respnsibility t discuss further mitigatin strategies with the SDM. Red Ntify the SDM by phne immediately t discuss ptins! A red categry means yu CANNOT cmmence r cntinue a task. If the staff member cannt get in cntact with the SDM via phne they may ntify the Emergency Medical Dispatcher (EMD) via GRN r phne that they are fatigued and require urgent review by the SDM r if the SDM is unavailable the EOC TL The EMD will put the crew ut f service until the SDM advises therwise. 17 The staff member will cntinue t make attempts t cntact the SDM and prvide their wn cntact details in any messages. If a staff member reprts a red categry frm hme prir t cmmencement f duty, the SDM may allw them t remain at hme fr a restrative rest break befre cmmencing wrk. If they are unable t rest at hme, a taxi may be arranged t cllect them frm hme and transprt t a statin fr rest. When extended pst shift vertime r shift cnditins result in a red fatigue categry a taxi r suitable transprt may be arranged fr the staff members trip hme. The same may then be arranged fr the staff member t return t cllect their car when safe t d s. Any ther risk mitigatin ptin that is feasible and applicable in the circumstance will be cnsidered by the SDM fr apprval. The SDM will prvide an incident number and an IRQA must submitted with the assistance f the TL if required t HealthSAASOHSIncidents@sa.gv.au The SDM will ntify the relevant manager and dcument the incident in the shift lg. 19 7. Managing Fatigue What shuld I d if I feel fatigued? If yu are feeling fatigued, yu shuld cmplete the FSAT. The ultimate gal is t self-prepare and manage yur fatigue in rder t arrive at wrk fit fr duty. Preparatry actins fr fatigue are mre effective than mitigatin strategies and will have lnger lasting effects. If yu are rstered t wrk, arrive at wrk r ntice during yur shifts that yu are fatigued, yu will need t advise the State Duty Manager (SDM). The way in which fatigue is managed will depend n hw significantly yu are impacted by fatigue and the cause f yur fatigue. Remember that the gal is t keep yu, yur clleague, the patients and thers safe! Fatigue frm sleep lss requires sleep. Fatigue frm tasks may just require a break. Staff have a respnsibility t mnitr and reprt levels f fatigue that may impact n their ability t wrk r their safety. Managing fatigue in the uncntrlled peratinal envirnment Several strategies are recmmended r can be implemented t cmbat fatigue: 1. Preparatry sleep. Sleep taken in advance f a perid that is expected t induce a degree f fatigue e.g. a nap in the afternn befre a rstered night shift. It can be in the frm f lnger sleeps r als naps. It is practive in nature. 2. Recvery sleep. Sleep taken reactively fllwing ne r mre perids f sleep lss either acute (in ne 24 hr perid) r cumulative (thrughut a 7 day perid) It may be slightly lnger than usual, but lst sleep is nt recvered hur fr hur. 3. 20 A break t achieve a level f fatigue restratin significant t cntinue safely with a shift r task. E.g. A 2 hur restrative break may be made up f the fllwing cmpnents: a 30 minute travel / wind dwn perid, a 60 minute perid t be used fr a minute nap if pssible, and a 30 minute wake up time t eliminate sleep inertia t the maximum degree pssible. Napping even fr 10 minutes has shwn benefits t decrease fatigue in the shrt term. Sleep inertia While bth lng sleeps and shrt naps can be highly beneficial fr a wide range f reasns, yur perfrmance and alertness may be impaired briefly after yu wake up. This perid characterised by vagueness, grgginess r reduced clarity n wakening frm sleep is knwn as sleep inertia. Sleep inertia is greater if individuals wake up at a time that they wuld nrmally be asleep eg 3am. It is als greater if individuals are wken up frm sleep by an alarm. In the ambulance envirnment this alarm culd be a pager. It is highest in the first 5 minutes frm waking and reduces relatively quickly after this, hwever if an individual feels unfit fr duty due t sleep inertia then that needs t be reprted befre engaging in any safety critical task. By understanding the ptential impact f sleep inertia staff can take actin t mitigate its risks. Driving immediately after awakening SAAS, as an emplyer, has a duty f care t its staff. Each individual staff member als has a duty f care t lk after their wn welfare and the welfare f their fellw wrkmates - this is ften referred t as shared respnsibility. Staff may be required t drive shrtly after wakening. It is imprtant t be aware if yu feel impacted by sleep inertia. It may be apprpriate t change rles if yur partner is nt feeling affected. Napping Napping is the best cuntermeasure t fatigue caused by sleep lss, extended wakefulness r circadian pressure ther than a lnger sleep. Naps can deliver many f the benefits f lnger sleeps ver a shrter timeframe including; imprve alertness and perfrmance, better shrt term memry and imprved reactin time. 21 be taken practively r reactively. Generally the lnger yu nap, the greater the benefits but shrter naps have benefits t. Naps as shrt as mins can deliver measurable benefits. Naps taken practively befre a night shift can interrupt the extended perid f wakefulness and help maintain alertness. Mst f the impacts f naps are psitive, but ne pssible negative is that they can reduce yur chances f sleeping later that day r night. If yu knw that getting t sleep is difficult fr yu, be aware f nt having t lng a sleep in the hurs befre yur main sleep. Tips fr getting sleep Getting adequate sleep can smetimes be a challenge even in fairly ideal circumstances. Sleeping during the day is usually assciated with the greatest difficulty because this is a time we are prgrammed t be awake. In additin there is ften mre nise, light and distractin during the day. Sleep Quality and Gd Health It is nt just the quantity f sleep that is imprtant fr alertness and recvery frm sleep lss, but als the quality... If yu spend 8 hurs in bed, but nly sleep fr 5 hurs, then yu will nt receive r feel the same benefits f 8 hurs f sleep. General gd health can imprve sleep, s a regular exercise rutine is strngly recmmended. As a shift wrker it can be hard t find time t exercise. Mderate exercise fr 30 minutes, 3-4 times per week is ideal fr significant health benefits but anything that yu can d is better than nthing! Our exercise physilgists at SAAS can assist yu t discuss an apprpriate exercise prgramme that will fit the demands f yur rster. Befre bed, cnsider gentle exercise such as stretching, instead f a mre intense wrk ut as strenuus exercise just befre bed can make falling asleep mre difficult. As part f gd general health, a gd diet is imprtant fr daily alertness and gd quality sleep. Maintaining a stable bld sugar level can help t maintain alertness acrss hurs f wakefulness. 22 High prtein fds can als help t keep yu feeling full fr lnger during shifts withut the heavy 'sluggish' feeling that accmpanies high fat/ high sugar fds. It is a gd idea t think abut meals and snacks befre yur shifts and plan ahead t ensure yu dn't feel the need t snack n high fat r high sugar fds. If yu need help with yur diet r wuld like advice ur exercise physilgists may be able t help yu. New research suggests that keeping yur main meal during "nrmal wake times (fr example during the day) may reduce the risk f diabetes and besity. It may be beneficial t eat a dinner type meal befre starting a night shift, and nly light snacks during the shift, rather than eating yur main meal during the night.
