One out of every seven women will be diagnosed with breast cancer in 7002 گرچه سرطان پستان هنوز يك مسئله جدي سالمتي در دنيا به شمار مي رود ولي ميزان مرگ و مير آن به علت افزايش استفاده از مامو گرافي و تشخيص زودرس كاهش يافته است. 2

12 (وضعيت هاي خوش خيم پستان) Noncancerous Conditions Fibrocystic changes :Lumpiness, thickening and swelling, often associated with a woman s period تغييرات فيبرو كيستيك شايعترين وضعيت خوش خيم پستان در زنان مي باشند اين وضعيت نتيجه نوسانات سطح هورموني است و در زنان قبل از يائسگي در سنين 20 تا 56 سالگي از همه شايعتر است. تغييرات فوق اغلب با درد و حساسيت ( ماستوديني( همراه است و در بيشتر موارد دو طرفه است. اغلب زنان عاليم را در مرحله قبل از قاعدگي احساس مي كنند. در اكثر موارد عامل خطر سازي براي سرطان نيست. تغييرات تكثيري شامل هيپرپالزي و آدنوز)تكثير آلوئول ها و مجاري پستاني( مي باشند. هيپر پالزي همرا آتيپي تنها تغيير فيبروكستيك است كه با افزايش خطر سرطان پستان به ميزان 5 برابرهمراه مي باشد. 5/03/5102 CBMS

17 (وضعيت هاي خوش خيم پستان) Noncancerous Conditions ترشح نوك پستان: در 10 تا 15 درصد از زنان مبتال به بيماري خوش خيم و 2.5 تا %3 از زنان مبتال به كارسينوم گزارش شده است ترشخ بر حسب ظاهر سبز شيري بك طرفه يا دوطرفه خوني خودبخودي يا راجعه سروزي تيره يا چركي در صورتيكه ترشح در خانم يائسه رخ دهد يا همراه توده باشد خطر سرطان بيشتر است 5/03/5102 CBMS

18 Noncancerous Conditions) ترشح بك طرفه و خودببخودي نوك پستان: شايعترين علت اتساع مجاري پستاني مي باشد دومين علت شايع ماستيت غيرنفاسي مي باشد ترشح خوني نوك پستان مستلزم ارزيابي جراحي مي باشد در طي معاينه بايد دنبال يك توده همرا در اطراف آرئول بود موارد ترشحات قابل توجه را بايستي جهت بيوپسي بافتي ارجاع داد معموال در موارد سرطاني توده ايي وجود دارد ولي در 13 درصد از سرطانها همرا با ترشح نوك پستان توده اي وجود ندارد 5/03/5102 CBMS

49 کارسينوم التهابی 2 درصد ورم منتشر پوست قهوه ای يا پرتقالی انسداد مجاری لنف به علت سرطان ممکن است با عفونت اشتباه شود ممکن است به سرعت به ساير قسمت ها منتشر شود. شيمی درمانی نقش مهمی در کنترل دارد جراحی و پرتو درمانی نيزمفيد است.

55 ژنتيک 80 درصد سابقه فاميلی وجود ندارد سرطان پستان فاميلی: در بين افراد خانواده ديده می شود و الگوی ارثی اتوزومال را دنبال نمی کند اما ريسک فاکتوری جهت ايجاد بيماری پستان با شروع زودرس می باشد و در حال حاضر تست ژنتيکی خاصی برای آن وجود ندارد داشتن خويشاوند درجه 1 خطر سرطان پستان را 3 برابر داشتن 2 خويشاوند درجه 1 خطر سرطان پستان را 5 برابر جهش ژنی: BRCA1,BRCA2 که ژن های سرکوب کننده سرطان هستند با شروع زودهنگام سرطانهای پستان و تخمدان همراه است

66 درمان جراحی: راديکال ماستکتومی تعديل شده : axillary lymph node dissection :ALND بافت و حلقه پستانی و غدد زير بغل برداشته می شود. ولی عضالت پستانی بزرگ و کوچک دست نخورده می مانند توتال ماستکتومی بافت و حلقه پستانی بدون غدد زير بغل برداشته می شود. ولی ممکن نمونه برداری از غده پيشتاز انجام شود: SLNB sentinel lymph node biopsy

70 عوارض پرتو درمانی قرمزی تورم پوست زخم پوستی خستگی افسردگی پنومونی شکستگی دنده پستان فيبروز چند هفته تا چند ماه بعد از تکميل درمان عوارض رفع می شوند

71 شيمی درمانی غالبا قبل از پرتو درمانی و بعد از جراحی انجام می شود. پلی کموتراپی در طی يک دوره 3 سيکلوفسفاميد- ماهه تا 6 متوتروکسايدو فلورويوراسيل) CMF ( جهت بيماران با مسموميت قلبی و بيماری ها ی محدود کننده مصرف می شود سيکلوفسفومايد آدريامايسين يا دوکسوروبيسين فلويوراسيل AC يا (CAF) ترکيب رژيم ها در بيماران در معرض خطر

72 تاکسان ها : پاکلی تاکسل و دوسی تاکسل در بيماران با غدد لنفاوی منفی و سرطان بزرگ استفاده می شود. شيمی درمانی متراکم بهتر اثر می کند اثرات جانبی تهوع استفراغ تغيير حس چشايی- آلوپسی- التهاب مخاط- افزايش وزن تضعيف مغز استخوان قطع قاعدگی تاکسان: نروپاتی محيطی- درد عضالنی درد مفاصل ورزش کاربرد رشد عناصر خونی در کاهش عوارض موثر است.
