2 The Purpose of the is to Gather guest physicians, community, religious, health and Thought leaders together, who have large constituents. Inviting survivors, friends, Greek 1 organizational leaders along with the local church or community to a panel and discuss diseases and clinical trials. Discussion based upon Prostate and Colon Cancer, it s myths, facts and both current and future treatment options. A panel of physicians facilitate each session. The CALL TO ACTION Coalition 2 is part of a strategy to reach and educate African Americans both about disease condition and clinical trials. It will also survey the impact of clinical trials within the community. For example, there is a need to look at cost of childcare, transportation, and time off from work, time reimbursement and other factors. We know that Clinical Recruiters want to recruit minorities, but they don t want to spend extra money to do it. Whether it s because of the high cost of drug development, or because they don t know how, the fact remains, not enough financial resources go directly into outreaching to this audience. Participation in local coalitions could help establish more effective educational outreach, recruitment methods and contribute to new policies for expanding this type of outreach. NPFR has successfully modeled a prototype (specifically related to Prostate Cancer) in Detroit, MI, lead by AARP and the Detroit American Cancer Society and wish to expand to other cities and other diseases based upon the areas of interest and support in Lecture Cities. This Lecture Series will be one among others in the series that discuss diseases and medical trials which critically affect African American community, such as Cardiovascular, Diabetes, Hypertension, Cholesterol, Obesity, Prostate, Breast, Lung and Colon cancer and HIV-AIDS. Lecture series are designed to can encompass multiple diseases or just one. Fabulous 50 to Sexy at 60 Dinner Lecture UPDATE September 24, 2010 Dallas, TX Dallas Speakers, Special Guests and Participants: Speakers will include Dallas/Ft Worth and visiting physicians, African American Health Advocates, Celebrities from media and Community Leaders. Participants: physicians, community, church, Greek, health leaders, prostate and colon cancer survivors and partners. Speakers invited (unconfirmed as of August 3, 2010) include: Prostrate Cancer Segments: (speaker update early Sept.) 1. Dr. Rufus Green, MD, (Urologist) Urology Institute, Dallas 2. Dr. Carrington Mason, MD (Urologist) Methodist Hospital of Dallas Colon Cancer Segments: 1 Many African American Greek Organization have a good percentage of members who are community, health & religious leaders 2 Sample, CALL TO ACTION form at Dinner lecture Series (one of three versions).

Dr. Adnan Badr, MD (Gastroenterology) Digestive Health Associates at Texas 3. Special Guest: Dr. Elise Cook, MD. M. D. Anderson Cancer Center, Houston Special Physician Guests: We are inviting one table of physicians (MD) and health professionals tagged: Doctors in the House and will give special recognition to doctors from all hospitals, including physicians from the Martin Luther King Medical Center. There will be a Doctors In the House Speak Out special table and speaker segment following the panel presentation. This will include (3-5 minute) remarks from guest physicians. Doctors will also be able to participate in having questions directed to them during the Q&A. We are looking physicians to give brief remarks on colon cancer, enlarged prostate, erectile dysfunction, diet and exercise importance. Other special guests will include: Prostate Cancer Survivors and their partners/guests Colon Cancer Survivors and their partners/guests Update: Dallas Program, Venue Partners, Advisors, Outcomes Venue: The Venue will be the church of the world-renowned minister, Bishop T.D. Jakes. His The Potter s House Mega Church in Dallas is one of the landmark churches of South Dallas where his worldwide television ministry is taped. Advisors: Parkland Hospital, Mr. Ron Sessions; Methodist Hospital of Dallas, Mr. Tim Upshaw; Baylor Hospital and their North Dallas Prostate Cancer Coalition, Mr. Wolfram Blatner, and the VA Hospital of Dallas, Ms. Monica Smith and Mr. Curtis Jackson, UT Southwestern Medical School. The Program: This proposal outlines the program in detail. Partner: Parkland Memorial Hospital. Parkland Hospital has held a highly successful an annual Men s Health Conference over the past eight years. The Conference draws community leaders, churches, and African American and Latino communities to an all day conference and screening. The event draws physicians and professional health education speakers, and Fabulous 50 to Sexy at 60 is tying into this concept with our CALL TO ACTION.
4 The recently formed Men s Health Advisory Council, is in the planning stage of a quarterly Men s Health outreach that focuses upon MIND, BODY, SPIRIT and NUTRITION. With Dallas County divided into four quadrants for this program, they want to partner for future Fabulous 50 to Sexy at 60 Lectures Dinner Lectures. They have pledged guests for the Dinner Lecture. One of their Men s Advisors is Renowned African American Health Lecturer and Counselor, Curtis Jackson who will be among our special guests. Jackson is part of UTSW Medical School. The Men s Health Advisory, are looking at working with NPFR to develop ongoing patient education meetings utilizing partner events and resources. The information will be provided to churches, community groups, physicians and survivors who provide ongoing quarterly (or monthly) program information to be interchanged and exchanged among it s members to regularly exchange information about critical diseases, immunizations, treatment options, and healthy lifestyle education to it s members. Parkland plans to have at each meeting a Doctor In The House. physician on call to address any urgent issues or questions by participants (in Dallas County) at all meetings. Partner: Methodist Hospital Methodist Hospital of Dallas provides both prostate cancer screening, rigorous follow-up, and they also provide Mammograms utilizing a van that goes throughout Dallas County. Methodist Hospital of Dallas screens hundreds of African American men annually, and are partnerships in making contact with the extensive network of African American churches and sending invitations to all of 50 Hoops and NPFR events. Methodist has pledged guests. Methodist also will conduct prostate cancer screening. Partner: VA Hospital Many of the veterans can participate in events through the VA hospital. We work with the Recreation Director to help us schedule buses to and from events. We will also invite physicians to attend the Dinner Lecture. Partner: North Texas Prostate Cancer Coalition The North Texas Prostate Cancer Coalition (NTXPCC) has networked most of the support groups for prostate cancer survivors in the Metroplex. The individual support groups are supported by Baylor, Medical City and Texas Health hospitals across the Dallas-Fort Worth area. NTXPCC recently expanded to include a Tyler prostate cancer awareness group. NTXPCC wants to partner with NPFR to expand its education outreach and help to form a South Dallas (mostly minority) prostate cancer support and awareness group. This effort suits our CALL TO ACTION purposes perfectly, in that prostate cancer survivors who want to collaborate and set up a survivor and partner support group can be facilitated through Fabulous 50 to SEXY at 60" Dinner Lecture. We see NTXPCC as a blueprint coalition for African American men. Partner: Martin Luther King, Jr. Medical Center Over the past eight years medical professionals from the MLK Center have offered FREE health screenings for Hypertension, Cholesterol and other counseling. Physicians and
5 health professionals are invited as special dinner guests, and will offer onsite health screenings to participants. They will also take part in the Doctors in the House presentations. At the end of the program, we will allow other special guests to announce prostate or colon cancer education or events during the next three months. We will invite the Mayor and Commissioner John Wiley Price to attend. Also Tom Joyner, highly popular talk show host who will be holding his Take a Loved One to The Doctor in late September in Dallas. He has over 10 million listeners per day nation-wide, and allowing him to tie into Fabulous 50 to Sexy at 60 can be highly beneficial to our future media outreach.
