10 Dr. Massimo BUDA, Italy Sustainable Computer Aided Implantology International Meeting Massimo Buda, founding member of the CAI, Graduated in Medicine, Specialist in Dentistry, Master in Digital Dentistry is a Drummer with a preference for Latin Jazz. Since 2000, he is interested in digital applications for dentistry. He teaches at the Istituto Stomatologico Toscano and the G. Marconi University in Rome. Today, we will talk about how important it is to prepare dentists to successfully introduce digital procedures in everyday clinical practice. The future will be dominated by more accurate and safer methods in implantology and prosthetics. However, computer aided implantology is still lacking significant penetration in the dental clinics worldwide. Today, an almost negligible amount of surgeries are performed through a guided approach. 11 Dr. Arturo D ARIENZO, Italy Standardization of the workflow to achieve success in implantology The speaker graduated in Medicine and Surgery in Florence and specialized in Odontostomatology at the University of Pisa. He completed a master ;s degree in implantology at the University of Florence. He has worked as a scientific referent with several prestigious companies that deal with medical software and dental implants. He annually holds several training courses dedicated to guided surgery and digital prosthesis. He has freelanced in Florence since The topic under discussion emphasizes the importance of standardized treatment protocols supported by the literature starting from the first approach with the patient up to the conclusion of the clinical case. The clinical and prosthetic experience is combined with the most advanced technology for minimally invasive patient rehabilitation. The wax-up performed on the intra-oral optical scan or on the patient's model is integrated with the CBCT exam for a virtual planning of the "prosthetically guided" implants. The implant design is developed both for the digital realization of a surgical guide and a working model, and for the design of the temporary prosthesis using a laboratory modelling software. The processed files are created using CAD-CAM or RP (rapid prototyping) technology, in a complete digital workflow. The surgery is made predictable and safe thanks to the great precision of the templates produced by latest generation laser printers and thanks to the use of a computerized surgical tool that allows to diagnose the bone density. It uses a probe that, inserted into the implant tunnel made by the first pilot drills, generates a friction on the bone walls that the computer of the tool translates into a chart that gives us objective indications on bone quality. Finally, it provides the operator with a densitometric value that can be related to the qualitative values of the MISCH scale. All this information collected intraoperatively, allows us to develop the best surgical strategy in real time and to choose the most suitable implant design according to the characteristics of the patient s bone and to evaluate the feasibility of carrying out rehabilitation immediately following the instructions of the literature. With this in mind, all clinical cases, from the most simple to the most complex, are programmed and completed with protocols of treatment that minimize the variables that every rehabilitative intervention necessarily reserves. The digital technology is highly involved in the modern workflow of Dentists- and Dental Technicians. The aim of this presentation is to describe clinical trial case reports based on the combination of facial analysis software, CT cone-beam, Computer Aided Implantology, CAD-CAM technology, 3D printing. The protocol is called "Digital Oriented Treatment (DOT)". The final goal is to optimise the treatment for the best patient satisfaction. A great satisfaction was recorded for patients by Likert response scale. 14 Dr. Firas HAMZEH, France The role of digital orthodontics in diagnostics and treatment of the occlusal interferences and temporomandibular joint disorders International Meeting Dr. Firas Hamzeh had advanced knowledge in Orthodontic digital solutions. He has been trained in France, in Nantes University. Dr. Firas Hamzeh is recognized as an early adopter and advocate of innovative orthodontic systems including aligners and digital custom brackets INSIGNIA BRACES, in addition to being Key Opinion Leader for ORMCO and an international Ormco speaker. He is working in private practice in Dubai. In diagnosing patients with possible TMD it is necessary to aknowledge the great variety of causes of orofacial pain and dysfunction. The treatment of the temporomandibular joint disorders can vary from simple self - care techniques to complex multidisciplinary procedures including the splint therapy which is one of the first stops in TMJ disorder treatment. It is very important to have an idea about the functional anatomy of the TMJ, understand the musculo-skeletal stable position of the condyle in the fossa, The understanding of the CPI and the interpretation of the CO/ CR discrepancy after mounting on the articulator, and the map of pain of ROCABADO, in addition to these diagnostic tools we need to understand the radiological diagnosis of the TMJ, the different stages of degenerative joint disease and disc pathology progression. The good understanding of the TMJ and the relations between the seated condyle and the stable occlusion lead us to the right way to treat TMD patients with occlusal interferences, and we have to understand the role of the orthodontist with the use of digital orthodontics in analyzing and resolving the occlusal issue. 15 Dr. Nabel MARTANI, Iraq Role of Computer Guided Implantology in Pterygomaxillary Implant Placement. Dr. Martani, an expert in computer driven implantology and aesthetic dentistry is the inventor of the San technique in computer guided implantology using AccuGuide. Dr. Martani, who is a lecturer in the Department of Conservative Dentistry at Hawler Medical University- Iraq. In 1996, he received B.D.S. degree (Dental & Oral Surgery) from University of Mosul-Iraq. After that M.Sc. degree in Conservative Dentistry from University of Salahaddin-Iraq in Followed by Ph.D. degree in 2015 from the Department of Oral Rehabilitation Science at Beirut Arab University-Lebanon. 16 Dr. Maurizio MARTINI, Dubai (UAE) OPEN SURGICAL GUIDES AND POST EXTRACTIVE IMPLANTS International Meeting PRESENTATION Following his honour degree in dentistry at the University of Ancona, and a Master II livello in IMPLANTOPROTHESIS at University of Roma, Dr Maurizio Martini attended several courses to develop his knowledge of Prosthetic Dentistry, Oral surgery, Implant Surgery and Periodontology. In particular he attended a two-year course in Parodontology by Dr P.Cortellini and Dr M.Tonetti who are two of the most important and accomplished practitioners of GTR and, a one-year course in advanced implant surgery, given by Prof M.Simion, one of the pioneers of GBR. For the last 10 years, he has almost exclusively dedicated his practice to implantology with a particular focus on immediate loading and bone regeneration, a subject on which he has lectured widely. 17 Prof. Silvio MELONI, Italy Different approaches to Sinus lift. How to manage regular and challenging cases. Prof. Silvio Meloni is adjunct professor of Periodontology at Sassari Dental School, Italy. His clinical practice is focused on Guided Bone Regeneration and soft tissue management when treating challenging cases. Prof Meloni is author of several papers on impacted Journals. In the posterior sextant of the maxilla, teeth extraction and sinus pneumatisation often requires bone augmentation procedures to place conventional implants. Maxillary sinus floor elevation is widely described in literature as a predictable surgical procedure to increase bone height and to allow dental implant placement. Different graft materials and surgical techniques have been successfully used to augment the maxillary sinus. Autogenous bone is still considered the gold standard, primarily due to its osteogenic potential, nevertheless, limited availability from intraoral donor sites could result in a need to harvest bone from extraoral sites, increasing morbidity and patient discomfort as well as the lateral approach is considered the most predictable and invasive approach. 18 Dr. Mohamed MINSHAD, Dubai (UAE) OPEN SURGICAL GUIDES AND POST EXTRACTIVE IMPLANTS International Meeting PRESENTATION Dr. Mohamed Minshad is a licensed dental practitioner, with active surgeries in India and UAE. Having a special interest into cosmetic dentistry and full mouth rehabilitation, he has attained his advanced level trainings in various dental disciplines from reputed European and international institutes. He is also the Clinical Specialist for Sinterex a Dubai based company, specializing in digital dentistry and guided implant surgeries. With the recent advancements in CBCT imaging techniques, digital planning aids and 3d printing systems, implant placement has become more precise, swift and controlled. However there remains challenges in cases with limited accessibility and compromised target sites. 19 Dr. Costa NICOLOPOULOS, UAE Cad/Cam assisted immediate implant function Dr. Costa Nikolopoulos BDS cum laude, FFD (SA) MFOS qualified as a dentist in 1984 receiving his dental degree cum laude from Wits University, Johannesburg, South Africa. He received numerous awards including the Gold Medal of the Dental Association of South Africa for the most outstanding graduate. In 1990 he completed his 4-year full time postgraduate Maxillo-Facial & Oral Surgery training at Wits University and was awarded FFD (SA) MFOS from the College of Medicine of South Africa. Since 1991 he is in full time specialist Maxillo-Facial & Oral Surgery private practice concentrating on immediate loading of dental implants. To date he has placed over dental implants. He has also presented as a key lecturer at numerous international implant congresses. 20 Dr. Petros YUVANOGLU, UAE Cad/Cam assisted immediate implant function International Meeting Dr. Petros Yuvanoglu is co-director of the Branemark Osseointegration Center Dubai. He qualified as a dentist in 1995, receiving his dental degree Summa Cum Laude from Semmelweis University in Budapest, graduating with exceptional high grade, top of his class as valedictorian. In 1998 he completed his 3 year full time post graduate Specialist Prosthodontist training at Tufts University, Boston, U.S.A. and was awarded his Certificate in the Dental Specialty of Prosthodontics. Since 1999 he is in full time private practice as a Specialist Prosthodontist working almost exclusively on the prosthodontic rehabilitation of dental implants. In 2011 together with Dr. Costa Nikolopoulos, he established the SameDay Dental Implants Clinic in Dubai, U.A.E. The use of titanium as the ideal surgical implant material for osseointegration has been extensively & well documented since the work of P.I. Branemark. More recently the clinical use of zirconia as the most biologically acceptable restorative material has come to light. Together titanium & zirconia appear to be the ultimate surgical & restorative synergy in implant dentistry. Extensive clinical cases will be shown in order to demonstrate this synergy. Current evidence-based literature supports the concept of immediate loading of dental implants showing that an initial unloaded period of 3-6 months is not necessary. An important factor for success is primary implant stability, which can be achieved by using a surface enhanced tapered implant, designed to enhance lateral compression of bone. In soft bone cases the osseodesification technique can be used to densify the spongy trabecular bone around the osteotomy site. In patients with suboptimal bone volume, bone grafts can be avoided by placing, angled and wider implants in available bone. Short and narrow implants can also be used in atrophic bone ridges. With good primary stability, these site specific implants can be loaded immediately with CAD/CAM Zirconia teeth. Zirconia is an alternative material for the fabrication of implant supported, screw retained prostheses. It is indicated in highly aesthetic cases and in patients that are consider to be bruxers and/or clenchers or suffer from sleep apnea. From a patient s point of view the reduction in treatment time between implant placement & installation of a functional prosthesis leads to increased patient satisfaction & treatment acceptance. 21 Gianni PERTICI PHD, Italy Dental and Maxillo-Facial reconstructions adopting innovative customized biomaterial Chief Executive Officer & Founder of IBI-SA (Switzerland). Gianni Pertici was born in Pontedera (Pisa, Italy) in 1976, got his Master s Degree in Chemical Engineering at the University of Pisa and from 2005 to 2009 he performed his PhD in Biomaterials at the Dept. of Chemistry (University of Pisa) and at the King s College of London. His studies were focused on processing engineering and material properties especially for biomedical field, but in recent years he acquired good knowledge also in the biological field, particularly in cell culture methods, in fact for several years he was R&D manager for the Swiss Stem Cell Bank. He is author in more than 15 publications, 2 books and single inventor of 1 patent. Currently he is also Reader at the SUPSI (University of Applied Sciences and Arts Southern Switzerland). Biomaterials for bone tissue engineering should ensure both adequate strength and volumetric maintenance. Moreover, their intimate structure should have an interconnected porous network for cell proliferation, while also providing specific signals for bone remodelling and regeneration [1, 2]. An innovative composite solution, bearing cues from both mineral components and polymeric ones, was here followed to develop a new three-dimensional bone scaffold, SmartBone (SB): a bovine derived mineral matrix is used to provide adequate solid structure and porosity, while resorbable polymers are used to reinforce it. RGD-exposing collagen fragments are finally added to promote cell colonization. Previously published results indicate that SB is osteoconductive and osteoinductive, promoting remodelling to mature bone formation in about 8-12 months [3]. High performances of this biomaterial allowed developing custom-made products (SmartBone On Demand, SBoD), solving single specific cases of bone reconstruction: starting from CT scan, personalized grafts can be provided for every kind of defects. SmartBone technology was successfully applied for more than cases. CT scans after 8 months showed impressive osteointegration and massive volume stability (>95%). SBoD custom made bone grafting technique allows complete restoration of wide defects. Histological analysis indicates that SmartBone is osteoconductive, promotes fast bone regeneration, leading to mature bone formation in about 8 months. gianni@ibi-sa.com 21

22 Dr. Marco RINALDI, Italy 3DZyGo : Guided Approach for Zygomatic Implants International Meeting Dr Marco Rinaldi MD, DMD, works as Oral and Implant Surgeon in Bologna, Italy. He has developed a clinical experiences using the 3D technologies in the field of Reconstructive Surgery. He has developed a series of surgical protocols and special Surgi Guides for sinus augmentation, bone grafting and zygomatic implants. Dr. Rinaldi published some books and recently: Marco Rinaldi, Scott D Ganz, Angelo Mottola: Computer-Guided Applications for Dental Implants, Bone Grafting, and Reconstructive Surgery, Elsevier USA, This book was also translated into Chinese and Spanish. Present a new protocol to study and plan of the position of the zygomatic implants that allows all surgical and prosthetic evaluations to be performed before surgery and transfer the treatment planning to the surgical phase. 29 Dr. Jacques VERMEULEN, France The accuracy of implant placement by experienced surgeons: guided versus freehand approach in a simulated plastic model Jacques Vermeulen, convinced for 18 years that guided implantology is superior to free-hand surgery, was often confronted during his presentations to the critics of dentists who continue to think that guided surgery is only for beginners! He realized this pilot study which comes to definitively close the debate! PURPOSE: To investigate the difference in accuracy between freehand and guided single-implant placement in situations with one or more missing teeth as performed by experienced surgeons. A total of 80 implants were placed by 10 experienced clinicians in the anterior site of maxillary models, made of polyamide by selective laser sintering and mounted in a dummy head. Each clinician performed the same four single-implant cases via freehand surgery and then with a three-dimensional fabricated SIMPLANT Guide. Two of the four cases had a single anterior tooth missing and the other two models represented a partially edentulous situation with several missing anterior teeth. For all 80 implants the average vertical, lateral, and angular deviations between the virtually planned and the achieved implant positions were measured based on a cone beam computed tomography (CBCT) scan. Regarding the whole sample, angular deviation was 7.63 degrees for the freehand method and 2.19 degrees for guided surgery. The mean difference in angular deviation differed significantly between groups and was more than three times larger for the freehand method. Lateral deviation at the coronal level of the implants was 0.42 mm and 1.27 mm for the guided and freehand methods, respectively, and at the apical level was 0.52 mm and 1.28 mm for the guided and freehand methods, respectively; the deviation at the coronal and apical levels was significantly smaller for guided surgery than for the freehand method (P =.001). Differences in the depth deviation at the apical and coronal levels were smaller (guided vs freehand surgery at the coronal level: 0.54 mm vs 0.78 mm; apical level: 0.54 mm vs 0.73 mm) but also of statistical significance (P =.05). Differences in angular, global, and lateral deviations between the clinical situations (single vs multiple missing teeth) were also significantly smaller for guided surgery, whereas the deviations in depth did not reveal any statistically significant difference between both methods for the singlespaced units. dr.vermeulen@wanadoo.fr 29
