3 MODULAR LAPAROSCOPIC INSTRUMENTS Our Modular Laparoscopic Instruments consist of three parts: the handle, the insulation sheath and the insert. Handles are available in a range of styles, including: ratcheted, non-ratcheted and multifunction stainless steel or Ultramid (polyamide) composite diameters of 5mm and 10mm and a huge range of jaw styles. Insulation sheaths come in two lengths, 33cm and 45cm and diameters of 5mm and 10mm. Inserts come in two lengths and two diameters and with a wide range of actions, including scissors, atraumatic, grasping, and biopsy forceps. We have more than 300 inserts available. 3.5mm diameter instruments are available on request. All parts are reusable, interchangeable and replaceable. All parts are interchangeable and replaceable so you never have to worry about getting them mixed up or doing without while waiting for costly repairs. 4 Laparoscopic Handles Our Ultramid modular handles are available in a range of configurations including ratcheted, non-ratcheted and with on/off or swingaway ratchet. These handles incorporate a rotating knob to support surgical accessibility and a 4mm banana plug for electrosurgical capacity. Ultramid is a lightweight durable polyamide material. Handles are also available in stainless steel. 20 Instructions for Use Assembly of modular laparoscopic instruments 1. Slide locking nut onto insulation tube. 2. Slide the insert into the insulation tube and screw in tight enough so that it doesn t come loose but can be unscrewed for reprocessing after the procedure. 3. Fully open handle to expose ball socket. Silver flange (non-removable) 4. Using an angled approach, place ball at the end of the insert into the socket on the handle. 5. Close handle and screw locking nut onto the handle tightly enough so that it doesn t come loose but can be unscrewed for reprocessing. Disassembly of modular laparoscopic instruments 1. Unscrew locking nut on insulation sheath to expose hub of sheath. 2. Fully open handle to expose ball and socket joint. 3. Remove the end of the insert (ball) from the socket by tilting the insert upwards. 4. Unscrew insert from insulation tube. 21 Instrument Care and Handling Refer to AS/NZS 4187:2014 Reprocessing of reusable medical devices in health service organizations, and your organization s processes. Always follow manufacturer instructions for use of cleaning chemicals and equipment. Universal precautions should be observed and Personal Protective Equipment (PPE) worn by all hospital personnel working with contaminated or potentially contaminated medical devices. Remove gross debris from surgical instruments with a lap sponge or x-ray detectable gauze and sterile water routinely during the laparoscopic procedure to prevent blood and body fluids from drying on the instruments. Disassemble modular laparoscopic instruments for cleaning. Metal brushes or scouring pads must not be used during manual cleaning procedures as they will damage the surface and finish of instruments. Soft-bristled, nylon brushes and/or soft non-linting cloths should be used. Do not allow contaminated devices to dry prior to reprocessing. All subsequent cleaning and sterilisation are facilitated by not allowing blood, body fluid, bone and tissue debris, saline or disinfectants to dry on used instruments. Neutral ph enzymatic and cleaning agents are recommended. Steam/moist heat is the recommended sterilisation method. Mineral oil or silicone lubricants should not be used because they: coat microorganisms prevent direct contact of the surface with steam are difficult to remove. Manual Cleaning Instructions 1. Pre-rinse: Pre-rinse as necessary with tap water. 2. Enzymatic Soak: Prepare and use the enzymatic solution per manufacturer s recommendations. Immerse fully opened and disassembled instruments in the solution. 3. Rinse: Remove the instruments from the enzymatic soak after the recommended period and rinse thoroughly with tap water. 4. Cleaning Instruments: Choose a cleaning solution appropriate for surgical instruments and follow manufacturer s instructions. Using a small, clean, hand-held brush, remove soil from all surfaces of the instruments while fully immersed in the solution. Manual cleaning should remove all visible residue. 5. Lumens: Brush lumens using clean brush of a size that will contact all surfaces and is long enough to reach from proximal to distal tip. Brush lumens while fully submerged in solution. Flush lumens with syringe or use pressure nozzle to remove loosened debris. 6. Rinse: Remove the instrument from the enzyme solution and rinse in tap water for a minimum of 3 minutes. 7. Ultrasonic Cleaning: Use an ultrasonic cleaner to remove soil from hard to reach surfaces, such as crevices and moving parts, after gross soil has been removed by previous cleaning process. Open or disassemble instruments as appropriate. Place instruments in a stainless steel instrument tray with a mesh bottom. Place the tray into the ultrasonic cleaner. Follow the recommendations of the ultrasonic manufacturer regarding cycle times, detergents, proper placement of the instrument tray, and conditioning ( degassing ) of the cleaning solution. Rinse under running water at end of cycle to remove loosened debris and to avoid re-depositing soils. 8. Decontaminate Clean Instruments: Place instruments in a suitable washer/decontaminator basket and process through a standard instrument washer/decontaminator cleaning cycle, following the manufacturer s instructions should be followed. 10. Inspection and Lubrication: Visually inspect the instrument for cleanliness and to ensure all parts are in proper working order. If contamination is noted, repeat the cleaning/decontamination process. Hinged, rotating, or articulating instruments should be lubricated with a water soluble product intended for surgical instruments that will be sterilised. 11. Drying: Before instruments are wrapped for sterilisation and storage, they must be thoroughly dry. Mechanical Decontamination Automated washer/decontaminator systems are not recommended as the sole cleaning method for surgical instruments. Follow the manufacturer s specifications when using automatic washer-sterilisers or washer/decontaminators. Instruments should be thoroughly inspected prior to sterilisation to ensure effective cleaning. Sterile Packaging Individual instruments may be wrapped in commercially available, medical grade steam sterilisation pouches (e.g., paper/plastic) to double package single instruments. Ensure that the inner pouch is large enough to contain the instrument without stressing the seals or tearing the packaging, but small enough to be placed in a secondary pouch without compromising the integrity of the outer pouch. The hospital is responsible for in-house procedures for the reassembly, inspection and packaging of the instruments after they are cleaned. Sterilisation Moist heat/steam sterilisation is the preferred and recommended method for instrument sterilisation. (Do not flash sterilise the instruments.
