Brenes DDS, MSc One Health student The One Health concept is increasingly being adopted as a way to address complex global health problems that involve people, animals and the environment. An estimated two billion people, predominately children, are infected with soiltransmitted helminths (STH); some of the most prevalent ones belong to the Neglected Tropical Diseases group targeted by the Millennium Developmental Goals (MDG). Diagnosis of infection is predominately based on fecal examination, however, mass deworming, hygiene education and sanitation of school age children is commonly used as a prevention measure with no previous examination. Some reasons for a lack of fecal examination are cost, time and cultural sensitivities regarding fecal collection. This lack of screening means limited information on prevalence and ability to assess if prevention measures are effective. While people might be reluctant to collect their feces, many developing countries, where STH tend to be more common, have international dental aid, community and government programs where children receive free routine dental examinations. STH eggs have been found in the oral mucosa and STH antibodies have been detected in saliva. The field of salivary diagnosis is expanding, more dentists are getting involved and magnifying their competencies. In my perspective, dentistry has a key relationship with many aspects under the One Health umbrella, many zoonotic diseases, have clinical signs that manifest in the oral region (mouth, head and neck), therefore, dentists are instrumental for detecting the initial stages of these signs. As a One Health Master s student I understand the need for animal models to validate these ideas and methods. For example, animals infected with zoonotic parasites can be studied using their saliva and oral cavity examinations. Potentially, we could find the answers to the unknown questions some of diseases have. I am interested and passionate about this initiative, by broadening research perspectives to include dentistry, we could understand better the relationship between humans as sentinels to animals and vice versa. Community education should involve dentistry perspectives to improve health care using the One Health approach. For example, drivers of potential disease emergence such as shared water resources and overpopulation are related to oral health and to STH as well. The socioeconomic burden that these groups of diseases possess is immense. The numbers we can see in the disability adjusted life years (DALYs) speak for themselves. We live in an era of global community in which diversity is slowly seen as an advantage more than a problem. Different world experts are opening their walls to discuss this type of issues as one force, in a global One Health approach to challenges. It is our responsibility to provide these communities with the tools they need to fight these diseases. Information systems are great allies to disseminate powerful messages, the ability to respond directly to the underserved populations with overall less distinct boundaries is essential to accomplish One Health initiatives.
