2 Membership Credentialing Programs
3 Credentials The Advanced Credentials Committee of the ICOI recently voted to amend and clarify ICOI's three credentialing levels. For example, the new ICOI Diplomate requirements have once again "raised the bar" so that we may establish a "common certification program" for all implant dentists around the world who wish to participate in voluntary recognition programs. ICOI's new case documentation forms will assist candidates in presenting uniformly documented cases. Outlined below are all different levels of membership in the ICOI, the pre-requisite conditions, applicant criteria and requirements with detailed instructions, applications and documentation forms at each level. ICOI Fellowship: Pre-requisite: Active ICOI Membership Who can apply: All members who place implants, restore implants and\or fabricate implant prostheses. Applications: ICOI Fellowship Application.
4 ICOI Fellowship and Diplomate For all Diplomate candidates: The committee will choose two (2) of the submitted cases that have been in function at least three (3) years and ask the candidate to bring those two cases as well as one recently completed complex case to the oral interview. At the oral interview, these three cases will be discussed. Please bring only these three complete case files to your interview. As soon as a candidate has given written notice of their intention to take ICOI's Diplomate examination, a suggested reading list/study guide will be provided to them for review purposes. If a Diplomate candidate has any questions, please contact a member of ICOI's Advanced Credentialing Committee. All applications and documentation should arrive at least six to eight weeks prior to the ICOI symposium at which you wish your credentials to be presented. Please also send via Fed-Ex, DHL, Airborne Express, Express Mail, UPS, or similar carrier in order to expedite delivery and guarantee the safety of the records. The sender should retain receipt of same. ICOI's Advanced Credentialing
5 Fellowship Requirements Provide a listing of twenty (20) completed implant cases* all of which must be at least 12 months old. Fully document five (5) of these cases on ICOI s Case Documentation Form for Fellowship Candidates and submit with the application. (See form for instructions). *Candidates who restore and place implants: Please provide a listing of ten (10) successfully completed implant cases (surgery and restoration) all of which must be at least 12 months old. Practitioner candidates: copies of pre-operative and post-operative x- rays are the minimum requirement for case documentation. Laboratory technician candidates: photographs or slides of completed cases on master casts or intra-orally are the minimum requirement for case documentation. Provide documentation of completion of seventy-five (75) hours or more of implant education (either attending in person or completing courses on-line) in the preceding five (5) years. Provide a letter of recommendation from a current ICOI Fellow, IPS Master, ICOI Diplomate or member of ICOI's Advanced Credentials Committee. Submit a current Curriculum Vitae.

9 Fellowship Maintenance Requirements: All ICOI Fellows must maintain their membership in good standing and must attend at least one ICOI/IPS sponsored or co-sponsored meeting every three (3) years. All ICOI Fellows must also accumulate seventy-five (75) hours or more of "implant education" within five (5) years after becoming an ICOI Fellow. These hours may be fulfilled through: Attendance at implant symposia or completion of on-line courses. Implant lectures, seminars or table clinics. Each lecture, seminar or table clinic given at ICOI or IPS sponsored or co-sponsored symposia will be credited on a 4:1 ratio (i.e. a two hour lecture will generate 8 hours of CE). A 2:1 ratio will be used for all non-icoi programs. Published implant articles in recognized journals. Each article and/or case report published in an ICOI publication, Implant Dentistry and/or the International Magazine of Oral Implantology, will be credited 20 hours of CE. Each implant published article in a recognized journal will be credited 10 hours of CE.
10 Case Documentation: What is required? Many ICOI members have inquired about the required case documentation. ICOI's Advanced Recognition Committee recently established the following protocol for documentation and created ICOI Case Documentation forms for each level. All submitted cases must be at least twelve (12) months old. For the first five (5) cases for Fellowship, ten (10) cases for Mastership and twenty (20) cases of which ten (10) should be a minimum of five (5) years old for Diplomate applicants*, please describe each individual case separately in a binder and\or loose-leaf notebook. In addition, please complete the appropriate ICOI Case Documentation form, providing all of the necessary information on each case submitted. Ten cases from each Diplomate candidate should be of an advanced nature such as treatment of narrow or shallow ridges or utilizing advanced restorative procedures and techniques. Practitioner candidates: copies of pre-operative and post-operative x-rays are the minimum requirement for case documentation. Laboratory technician candidates: photographs of completed cases on master casts or intra-orally are the minimum requirement for case documentation. Additional documentation may include patient slides or photographs, CT scans, preoperative evaluation and planning forms, lab and restorative work authorization forms, and/or treatment consent forms, etc. to further detail a case. All materials may be submitted digitally. Record the remaining required cases with the requested information on the appropriate ICOI Case Documenation form
11 How Can I Apply? New ICOI Fellowship, IPS Mastership and ICOI Diplomate application forms and Case Documentation forms are available from our central office or on-line as pdf files that can be downloaded and faxed to us. Applications are evaluated and processed by the committee each month. You need not wait for an entire year to apply or receive your credentials. At every ICOI sponsored or co-sponsored symposia, members are presented with their appropriate credentials. Additionally, you must also attend one (1) ICOI sponsored or co-sponsored meeting every three years and comply with the continuing education maintenance requirement at each credentialing level. There has never been any maintenance assessment levied upon credentialed ICOI members. We have a one time processing fee that must accompany the appropriate application.
12 ICOI Diplomate: Pre-requisite: Active ICOI Fellowship or IPS Mastership Who can apply: All members who place or restore implants Applications: ICOI Diplomate Application Application Diplomate Requirements: Diplomate Provide a listing of sixty (60) completed implant cases. Candidates who place implants: Your cases must include two hundred (200) or more individual implants or ancillary procedures all of which must be at least one (1) year old. Candidates who restore implants: Your cases must include restoration of one hundred (100) or more implants all of which must be at least one (1) year old. Candidates who restore and place implants: Your cases must include seventy-five (75) or more individual implants or ancillary procedures with restorations all of which must be at least one (1) year old.
13 Please record the required cases for credentials on the ICOI Case Documentation Form for Diplomate Candidates. Document twenty (20) cases and submit with the application. The twenty cases should be detailed individually on ICOI s form as follows: Ten (10) cases should be at least five years old and show some diversity in implant selection or ancillary procedures, restorative design and\or materials. Ten (10) cases should be of an advanced nature such as treatment of narrow or shallow ridges or utilizing advanced restorative procedures and techniques. Copies of pre-operative and post-operative x-rays are the minimum requirement for case documentation. Additional documentation may include patient slides or photographs, CT scans, pre-operative evaluation and planning forms, treatment consent forms, etc. to further detail a case. 14 Provide documentation of at least one hundred and fifty (150) continuing implant education hours in the preceding five (5) years (either attending in person or completing courses on-line). These hours may also be attained by teaching courses or seminars or by giving lectures and table clinics using a 4:1 ratio at any ICOI or IPS sponsored or co-sponsored symposia. (i.e. two hour lecture equals 8 hours of CE) or a 2:1 ratio for all non-icoi symposia. Submit evidence of having completed one of the following: Authored or co-authored at least three (3) articles or case reports on implant dentistry. One (1) of which must have been submitted or published in one of ICOI s publications, Implant Dentistry or the International Magazine of Oral Implantology (IMOI). or Presented at least three (3) lectures, table clinics or poster presentations at implant meetings within the last five (5) years. One (1) of which must have been at an ICOI or IPS symposium. Provide two (2) letters of recommendation from two (2) ICOI Diplomates or members of ICOI's Advanced Credentials Committee attesting to Submit a current Curriculum Vitae. Participate in a regional ICOI Diplomate examination. These will be offered during all ICOI sponsored or co-sponsored symposia. Once your application has been received, a suggested reading list and/or study guide will be provided. Examinations will be both in written and oral interview formats with three (3) examiners from ICOI s Advanced Credentialing Committee or ICOI Diplomates.
