3 Crisis System Transition Plan S1: Refine and formalize diversion program model Review the assessment tools currently being used within the system. DWG - The work group consists of LEC members, City of Chicago, All Chicago, CSH, Catholic Charities, Salvation Army, CC - HPCC and CRRC - Jesse Brown By Mid March Workgroup reviewed the current diversion assessments used by both Catholic Charities for youth and Salvation Army for families. Review the current system of diversion within the CoC Check with CRRC to confirm contact and how to refer veterans for diversion Check on language for veterans assessment and instruction on what to do Revisit the HPCC's question regarding veterans and diversion and ask if it needs to be changed Mary By March 5 It was decided that Veteran referrals will go to the CRRC during regular business hours and then to the Homeless Social Workers after hours. Meetings are held to discuss the process of how families who are placed into shelter from the field (transported by Catholic Charities) would be assessed for diversion. Invited Catholic Charities to workgroup meeting by Mid-May Ian Stephens and Luiz Barbosa from Catholic Charities attended the diversion meeting on May 14. It was determined by the discussion that Salvation Army would be the agency to do the mini diversion tool because they speak to every family on the phone prior to Catholic Charities placing them in shelter. Create a systematic approach to targeted prevention or shelter diversion (family shelter with skilled assessors) Have a sub-group meeting to discuss process flow for assessing families who are placed from the field., Tracey, Ian, Kierra, Billie Woodard (sal army) By June The group meet on May 21 to discuss the process for assessing families who have no contact with Salvation Army EHARC. 1) Catholic Charities would contact Salvation Army when they have reached the family, 2) Salvation Army would conduct a minidiversion assessment 3) if diverstion is identified Youth, Veteran or Family the family will be transported to EHARC 4) For Youth Tracey Blackburn will be contacted 5) for Veterans, the CRRC will be contacted to reach an SSVF provider who would do further assessment.6) if family can be diverted Sal Army will divert, if family cannot be diverted move to next step 7) if family cannot be diverted via, family, youth or veteran the family will be placed in shelter. Look at current modes of prevention Schedule additional training needed for Salvation Army staff on Diversion The test/pilot for family diversion will begin Find out more information about the persistence program and forward information to DWG Elder persons and ways to redirect them from entering into the shelter system Tracey, Kierra and Andrea Chatman (sal army) By end of June revised Mid July end of July/ Lynette By March 5 Salvation Army requested more time before the training to ensure staff were available. The trainings are scheduled for July 10 at 9:00 In Progress a.m. and 3:30 p.m. Andrea chatman from Salvation army stated that she will notify after the training on July 10 to confirm the pilot/test start date. S2: Think about how to incorporate diversion into CES or the front end crisis system - screening, delivery etc. Meeting scheduled for March 26 to look over supplemental that would be used for Catholic Charities mobile outreach and how the diversion questions can be included in the salvation army intake form Sub-Group to convene a meeting which includes All Chicago regarding HMIS and how it may play a role in the process. Riley Subgroup: -City of Chicago, Stephanie - CSH, Tracy, Catholic Charities Diversion, Kelly Grimes - Catholic Charities HPCC, Wendy Avila - Catholic Charities HPCC, Kierra Allen Craig - Salvation Army EHARC/Shield of Hope, Otha Gaston, LEC held meeting at Salvation Army Shield of Hope and it was decided that Sal from All Chicago would meet with and Tracy to discuss intake and diversion - Tracy and Sal to meet and information Kierra from Salvation Army Meet to look at HMIS Diversion tool used for Youth currently and see if it can be expanded to include more diversion questions Sub-Group meeting scheduled for March 29 Sal and Tracy will meet Riley, Tracy Blackburn and Sal Munoz from All Chicago Sal and Tracy Week of April 3 Sal and Tracy to meet to discuss divesion expansion workflow in HMIS. Tracey will contact Kierra about additional questions that may be needed Sal will have draft of HMIS for Tracey Sal and Tracy By April 13 Present to DWG draft of questions and possible drop down options within the diversion component of HMIS. Sal and Tracy to meet with DFSS to discuss why certain Meet with DFSS staff regarding questions need to be on intake form and if we can get it Salvation Army and Catholic charities to jive and sync with HMIS which would include adding intake form the necessary diversion questions Ongoing In progress S3: Integrate Homelessness prevention resources into the eviction court process and preceedings Sub-group to convene a meeting or call to discuss what would be needed to provide resources to the eviction court process and proceedings Schedule a call with key partners within the diversion group which includes Lynette Barnes will sub-group the forms. Lynette Barnes - All Chicago Call was held on March 23 Sub-Committee includes Lynette Barnes, All Chicago, Kelly Grimes - Catholic Charities HPCC, Wendy Avila - Catholic Charities HPCC, Mark Swartz and Jude Gonzalez, LCBH Lynette Barnes - All Chicago Review the forms that are used for the State Homeless Prevention Program (SHPF) to help with Mark Swartz from the Lawyers Comm. for Better Housing will review documents as well as the sub-group Mark Swartz - LCBH By mid April Mark received documents from Lynette to review

Drafts After Reviewing the forms convene another call to discuss content of the one-pager Call will include includes Lynette Barnes, All Chicago, Kelly Grimes - Catholic Charities HPCC, Wendy Avila - Catholic Charities HPCC, Mark Swartz, LCBH amd Riley Mark Swartz from the Lawyers Comm. for Better Mark Swartz - LCBH, Jude Gonzalez, LCBH, Housing connected Judge Wright from eviction court and Lynette Barnes, All Chicago, Kelly Grimes, he agreed to a meeting regarding the one pagers. Catholic Charities By July 1 On June 18th, Mark Swartz met with Judge Wright about providing homelessness prevention fund resources to the eviction courts, and the possibility of having a presence in the eviction court to facilitate eviction diversion. Judge Wright was very interested and would like to meet with court stakeholders about the idea and implementation. Follow-up meeting to be scheduled with Judge Wright, other court stakeholders and members of the diversion workgroup Mark Swartz - LCBH, Jude Gonzalez, LCBH, Lynette Barnes, All Chicago, Riley, DFSS, Nastasha Flowers, LCBH and Cameron Calia, LCBH by mid The meeting would tentatively be scheduled around 1, Additional members of the workgroup will also attend the meeting - The meeting was held and a presentation was given to Judge Wright, multiple Eviction Court Judges, Lawyers and client advocates. The presentation was well received and questions presented were followed up by answers. Judge Wright after the meeting stated to Mark Swartz, diversion committee member that he would give us a space to use on the 13th floor of the Daley center. The 3 asks for the presentation were 1) information be shared within the eviction court process, 2) referrals be made to our Eviction Diversion Program, and 3) that a space is provided to the group so that we can catch and engage clients as soon as they leave the court. At the meeting Cassie Lively, ED of the Center for Conflict Resolution approached asking to be a part of the workgroup Eviction Diversion Pilot Program Meeting with sub-committee to discuss HMIS tracking Mark Swartz - LCBH, Jude Gonzalez, LCBH, Lynette Barnes, All Chicago, Kelly Grimes, Catholic Charities, Riley, DFSS, Luiz Barbosa, Catholic Charities, Wendy Avila, Catholic Charities, Nastasha Flowers, LCBH and Cameron Calia, LCBH, Sal Munoz, All Chicago and Brian Rivera, All Chicago Month of The first meeting was held on Thursday, 2 at 1pm at Catholic Charities and the flow of the process was presented by Sal Munoz. The second meeting was held on Thursday, 9 at 1:00 p.m. at Catholic Charities. Attending the second meeting was Rae Kyritsi from the Center for Conflict Resolution. 6 Crisis System Transition Plan Communications Strategy Riley, Nastasha, Cameron and Jenna Temkin, All Chicago /Septe mber Ongoing Monday, 13 at 10am at LCBH. It was decided that communication will only go to the judges within the eviction courts due to wanting to control capactity. Also, it was decided that the CoC Agenda Logo will be used on all flyers and information with the following statement to explain: The Chicago Continuum of Care is a membership-based group of more than 100 organizations and individuals across the city that are dedicated to preventing and ending homelessness. The Eviction Diversion Program is a Continuum of Care initiative led by the Lawyers Committee for Better Housing. Approved Tenant Letter Lynette and Nastasha September Eviction Diversion Pilot Launch EDP Sub-Group October Work on creating an agreement for landlords to sign stating that during the time of processing they will not remove the tenant from their apartment. Communication between the Landlord and the LCBH will be open and payment will be made within 7 days. Draft was provided to the subcommittee on September 7 The pilot began on October 1, 2018 and the LCBH has an office space in room 1303 of the Daley Center. (same floor as eviction court). LCBH will be meeting individually with judges to provide the updated informational materials on the pilot as well as provide guidance on what are appropriate referrals In progress In-progress S4: Integration of Diversion into the Veterans population (National Invitation) Pilot project national kick-off call SSVF providers, government agencies and VW officials May Intended for any and all SSVF, CoC, and VA partners involved in ending Veteran homelessness and local coordination entry processes for Veterans in our community. SSVF Rapid Resolution Pilot that will take place during the summer of 2018 and have implications for national planning in the future. Community calls to answer questions and secure final commitment Pilot project planning meeting (Dallas) Spinelli, Jesse Brown VA and the CRRC and Riley Spinelli, Jesse Brown VA and the CRRC and Riley 2nd week of May June 6 and 7 Confirmed Chicago Participants and attendees for the June Dallas meeting The goal of the Pilot Project Planning Meeting was to gather information about effective rapid resolution (diversion, rapid exit, reunification, creative housing, etc.) practices including the amount and type of financial assistance that contributes to a resolution of the housing crisis. Share information with other participating cities and get additional guidance from the National level
