2 February 7, 2011 Time: Hotel is reserved from Friday 8:00 am to Saturday 5:00 pm Review Agenda and come up with learning objectives that will fulfill the content for the June 2011 Conference Minutes: Our conference will be June 3-4 o HCA conflicting conference dates has been resolved Given an agenda sample to review o Change start time to 1:00 pm on Friday Dawn and Wendy worked on the agenda with regards to the time, when going into the dinner piece of it that would mean those that started at 1:00 would already be there 8 hours, it would be even longer if we start it earlier Break out for EMS stuff EKG refresher, tough calls, etc. Get with the Guidelines for hospitals At 5:00 Networking reception Keynote speakers with Dr. Cook and Monica, RACE or Lessons from the East and West o On Saturday starting at 7:30 with registration and continental breakfast Dr. Gray and Dr. Fowler doing the welcome Are we repeating the RACE, we are not sure if we are doing North and South regarding LA county vs. Boston and seeing what they did but that more East and West After keynote start at 9:00 for the subcommittee reports Break for snacks to view exhibits and posters Baseline Dallas Data possibly to be discussed Break out to varies sections Multiple times to view exhibits and posters Panel discussion Closing remarks o This is a draft This was made for the exhibitors to sale them on the conference, and because it was cut in half on the first day the discussion was whether we need to cut the price from $2500 to $1500 for the exhibitor fee, Page 2 of 9

3 February 7, 2011 because it s not two full days Committee members stated reducing fee was reasonable Asked if it would be easier if EMS and Hospital staff could not come to the whole time they could come to a portion on Friday and a portion on Saturday For EMS it can be hard because of shifts, if they cannot come the full day on either day why pay the full price of one day Have the provision to say if a department wants to send 5 people you may take 5 slots but one of the slots may cover 2 people, registrations may be shared by staff who can t attend a full day We have been discussing a two and one day fee, which was $5.00 more if you did the one day, it was going to be easier to track, We may need to take a look at decreasing that day break Or have a comment section that says (i.e. Steve/Curtis are sharing) But how is it going to work for AHA registry and what about the badges, we want to meet these needs, but maybe it would be easier if AHA setup the registry per day We are not in it to make money, the spirit of the grant is to have as many people as possible Committee members like the registry per day suggestion and make the price $25.00 per day 3. 7:40 Registration Jennifer Exhibit 3A Sample Registration Exhibit 3B Services offered from CDS Background: Members may choose which section they want on the registration items. 5 February 7, 2011 Minutes: Dawn plans to go through the sample, will look at schedule today to set time to work on it We do not have to have it complete until the end of March, still some time to work on it Tammy has some information regarding pharmacy CE and Annabelle was check on the nursing side We need information regarding EMS, pharmacy, CCI (Jennifer stated she did receive information about that and they will accept nursing CME), CME - for physicians, PA, nurse practitioners, do not have an NP on our panel or a physician s assistant however they do accept CME and Dawn will ask Steven regarding this, we do have a physician and physician it may cover PA and nurse practitioner, With the schedule and everything we have about 9 CEs hours, do the NPs get a different CE hours they can choose CE/CMEs, the PA can use CMEs, will verify it with Steve 5. 8:20 Marketing Jennifer Exhibit 5A Sample postcard Exhibit 5B Sample Website information Background: Marketing the conference with the use of the AHA s resources. Karin Reed (Marketing director) is advising with this portion of the conference planning. Minutes: It was suggested making the website link bigger Make sure to add CE/CME will be available On the website we have the whole team, but on the post card they did not fit o Stated by marketing to make sure to add your physicians and co-leads Sent post card to internal AHA staff that will be working on it and stated will need it to be mailed out this month It was suggested by Marketing to put a picture of someone that s known in the area, but committee stated the proof is good as is Page 5 of 9

6 February 7, 2011 Are we sending out anything else other then Save the Date o o Tri-fold?, have not work on this o Will put the same information on post card onto the website o Sending it out February is good, but the closer we get to the conference date send something out When would we want to send something out as we get closer to the date of the conference (i.e. two piece or tri-fold), first of May? Yes send something out first of May and for the in-between time the data staff said they will send two to three different s and then the website piece of it Will need to fix on postcard o Section Target Audience take off parentheses from (clinicians, nurses, and pharmacists) o Also physicians are not specifically listed, add to the list o Add paramedics o Remove clinicians and add CVTs Will brochures or some type of hand out be available at the time of the event o Considering providing everything online o How about having something simple with physicians bio, will we provide materials for participants to write on The hotel will provide paper notes How about providing some type of stationary with maybe Caruth as the header, something inexpensively o However providing handouts of presentation is not cost effective We will ask speaker to provide a PDF format of their presentation to upload into our website We will encourage people to visit the website where presentations will be provided for view and printout purposes o So how many do we want to print out of postcards and when do we want to give to distribute Waiting on internal staff that are going to print out, but did state would like it to go out this month Ask to print out 1000 Will start pushing them out through mail and then each one in the committee will receive some to take back to their facility, trying to get them out on March 3 rd at the Stakeholder Committee meeting We will send it out electronically Page 6 of 9

7 February 7, 2011 Also there is a data source from AHA that this will be mailed out too 6. 8:25 Exhibitors Jennifer Background: Subcommittee will invite exhibitors from the various services with regards to the Cardiac patient List of exhibitors to participate at the conference Template to be completed once schedule/agenda is selected. Minutes: Since we adjusted Friday to have half day and Saturday to full day, we spoke about changing the vendor s price from $2500 to $1500, asked if there were any objections to the change, there were none 7. 8:30 Naming of the Conference Jennifer Background: Name of the conference Group will decide what to name the conference with regards to the Caruth Grant. Caruth should be a part of the title when discussing the ideas with regards to the naming is concerned. Minutes: Still need to name the conference, the previous suggested name was not approved o The title used on the post card can be used for the name of the conference AHA Dallas Caruth AMI Advisory Symposium o If there are any other suggestions from now until the Stakeholder Committee meeting, Dawn Website has the same information as the post card o Need to change the price o It shows the save the date o It gives some timelines Registration begins Page 7 of 9

8 February 7, 2011 Poster submission deadline Reports for Symposium o Highlights who are target audience is and we need to make the same changes as on post card o Show location of the Symposium o Do we know exactly how many hours of CE/CME in order to post this information We ll be able to update website fairly quickly once we have the information We have to have that number before we open registration, Andres stated do not have to they ve made a statement Pending Approval should not hold up registration just for that o Need to change CE/CME information to include everyone involved o If there are any questions, and phone number is provided o It shows contact person regarding posters is Dawn 8. 8:45 Review Action Steps and Next Meeting Jennifer Summarize a list of action steps from today s agenda Minutes: Dawn to lead on CE/CME, if any help is need ask committee members Next meeting will be at the Westin Meet at the restaurant it on the ground floor, same time at 7:30 and at 8:15 take tour Jennifer ask about the CPR class, since we wanted to invite the community o Still in the works, may need to wait until next year o Jennifer suggested asking those in her hospitals and maybe Andres can also ask those that teach CPR, to volunteer their time to teach at the conference and place those classes on Saturday Andres did state if AHA could provide CPR cards that would help to ask those in his hospital Maybe if they learn basic CPR where they don t get CPR cards, just learn it and take a kit which is $25 o Dawn to check with someone from ECC to come in next meeting and talk about options Page 8 of 9

9 February 7, 2011 o Either we have it both days, or one big class on Saturday Maybe have the community come out from 3:00 5:00 If we do this how long does a class take, 1 hour but most people just want to learn to do compressions Could we do something with the posters after 1:00, so that we could have that area to do a 20 to 30 minute showcase to the community of how to administer CPR o Dawn to add to agenda to start discussing CPR classes With regards to the Stakeholder meeting are there any recommendations to present to the voting members to vote on o Vendor cost o Name o Sample agenda o Something for the community o Poster, that will be great for them to see and take it back to their organization to start preparing one Also the dinner is estimated to be $45 per plate, so if someone would like to bring a guest we figured we could ask for the one day fee of $25 to cover that If there are any changes to the minutes, send to Dawn 9. 9:00 Adjourn Jennifer Adjourned 8:23 am Page 9 of 9

Review Roster and Vote and approve minutes from January 10, 2011 Minutes: Committee reviewed the minutes from last meeting. The committee did not state there were any changes needed. Jennifer s only update is to state she has not received anything from CCI. 2. 7:35 Schedule/Speakers Jon Exhibit 2A Agenda Page 1 of 13

Date include June 3-4 Location: Westin Park Central, Merit Drive, Dallas, Texas Time: Hotel is reserved from Friday 8:00 am to Saturday 5:00 pm Review Agenda and come up with learning objectives that will fulfill the content for the June 2011 Conference Minutes: We are on track, with the exception of naming the conference o How about the suggestion Dr. Hillert stated in the stakeholder s meeting Deep in the heart of Texas, o We need to put a deadline on naming the conference so that we can start marketing it o Finalize it in the next meeting on the 7 th o It s going to be at the Westin Park Central at Merit Drive Review the sample agenda o Question: Is Todd Gray doing a welcome/introduction in 30 minutes? And then another 30 minutes followed by treatment development for MI patient? Do we have a speaker or are we still looking for one? Is 30 minutes for welcome/introduction long or enough? Saturday is a follow up and rather short so there s no introduction so the 15 minutes is appropriate o This is only a sample and used only for the exhibitor piece to show and sell the conference to the exhibitors o In our first talks we were talking about Friday being the premeeting stuff and wondered if we really need a welcome/introduction section o Wendy suggest having Michael/Ray as co-chairs doing our welcome Michael will be out of town o Suggested to save that for Saturday morning and incorporate that 30 minutes into the treatment and development for MI patients, so that s now 1 hour and allows for questions and answers segment at the end, because if we lock it into 30 minutes that does not give much time for Q/A o Again this is just a sample it s always easier to edit something, Page 2 of 13

13 January 24, 2011 so we will work through all that, offline o This may be one of our homework assignments, take it and tweak it around o If we are going to take some time away let s start it at 7:30 o We can have the sessions start at 10:00 or 11:00 that day, I think we can all agree we don t need two full days o The other recommendation that came up before instead of saying Basic and Advance, having an EKG refresher or ECG refresher course do we know if we have enough people who would want to attend two or three things at that time or do we want to have an ECG refresher course instead of two different ones o The two that we can probably have more out of is Advanced and the Tough Calls What are the difference between those two o I would put those two together because the Tough Calls is Advance course, as far as Basic is just the term it s not the final version o How to optimize the use of the 12 lead ECG we can take that out, have just the two and right now the ECC group from AHA is looking for some volunteers to teach that for us o If we are looking to take one of those subjects off or combine them, will we have a room large enough to accommodate all the attendees at three different rooms For example if we had 400 attendees and 4 subjects (rooms) and estimate 100 attendees at each room, if we remove 1 subject(room) will the rest be able to accommodate the remaining attendees We should have plenty of room, in fact Dawn s asked Elysse to check if we can have one of our meeting at the hotel so you all can see where we are going and see the flow of the floor Not sure our attendance is going to be high on Friday, we may have 20 people in the EKG class If we start at noon Friday, people will start to show at 3:00, 4:00 We don t want to over extend ourselves by doing it this way and too people are working Friday o We have to come up with Learning Objectives, some of the things Dawn has come up with are Identify process to shorten door-to-balloon time How about instead of door-to-balloon time, say Page 3 of 13

14 January 24, 2011 EMS to balloon time, or symptom onset How about instead of saying shorten, say streamline Describe how to participate in a regional STEMI network State the new ECC/ACA guidelines Post AMI care and hospital complications When Dawn talked to Laura Gossett, person responsible for the exhibits, she state along with the agenda she need Learning Objectives Do we want to incorporate as one of the learning objectives that we are trying to add a CPR class to educate the public It s not necessary Do we need a specific number of Learning Objectives Three How about a subject like advance STEMI treatments and have an interventionalist come and talk about the newest/lastest interventional techniques and try to find someone outside the region We have Tim Henry for Friday night, so we have one keynote that s doing the future of AMI care and interventional cardiology and the other one do like regional STEMI aspect Put Tim Henry down just as a guess haven t talk to him, however the specific topics of that conversation has to talk about first the original STEMI process Still have not heard from Dr. Meyer s who does hypothermia, there is another person in San Antonio and one in McKinney who also do hypothermia, this would be for a breakout not keynote The two best times were Friday night dinner and Saturday morning, so if you ve got future of AMI patient care Friday and then Saturday morning is that the regional STEMI because that s where we ve got Chris we want to move Chris off there and use that as our keynote on regional STEMI Hoping Dr. Jollis can do that So have Tim Henry doing the future of AMI care Friday night and then Dr. Jollis Saturday morning doing STEMI regional Page 4 of 13

15 January 24, 2011 What about the controversy between lytics and STEMI protocols, obviously we need to touch on that I know in the metropolitan area we don t have to do it that much, but in the rural areas lot of people still think that it is okay, we can do something like rural outreach, Dr. Jollis could talk about that too maybe on the STEMI system on Saturday for the rural areas because I remember that he s touched upon that that s the whole RACE project is giving lytics too for the rural hospitals that can t make it within the 90 minutes, then we also got either Peter Moyer from Boston or William Koenig from LA on the EMS side of it and looking at again incorporating the lytics and rural transfers in that discussion too on Saturday afternoon One of the things that has just changed the criteria of ACC giving more than 4000 Units of heparin that s considered an overdose and that s new to us because we ve been giving them 5000 so there can be some controversy discussion about that, decisions that ER doctors have to make We talked about the hypothermia the Post MI or the cath lab is using it and I don t know that many folks--i don t know if there s a lot of research specifically that I thought that were doing it I know that might be a talk about possibly having people bring in somebody but that standard of paying somebody was just not somewhere we want to go there but I don t know if we got any of our other surrounding physicians that can provide information regarding that If we had the advance interventional care talk, that could be included he could probably speak of the different devices/equipment/treatments Well there s a lot of literature about high risk with the Impella, some people take it out right after other people let them rest with it 12 to 24 hours it just depends on the physician, I would hope they leave it in because of the cost that s one of the big things where a lot of physicians know how much it cost therefore they don t even put it in because of that it s got great reimbursement but the cost for a catheter it s the cost/risk ration Paul St. Laurent who is known for his work on Post Page 5 of 13

16 January 24, 2011 AMI care area has agreed that he would do that for us he s done that many times for Baylor Are we okay with having someone speak from within our market or do we have any concerns about anyone feeling like some of these programs can be highlighted over another saying someone speaks over them If it s a subject that s going to be talk to physicians I just remember what was told to me, all the interventionalist know each other and they are all pretty much partners so they are not going to go, they would rather hear someone national that they don t have exposure to all the time Physicians wouldn t necessarily listen to Paul but nurses staff would because of his reputation in the Dallas county community that s the one thing and we all have to think about not all are physicians that are going to be attending they wouldn t go see Paul because most are friends with him but the other folks would, vice-versa not everybody is going to go see Chris Bjerke talk but at the same time what she s got to say as far as the pre-activation in the field she s got a lot of research for that so the physicians would go see her and plus from our geographic area and how successful they were in the Mayo clinic that kind of speaks for itself Put them towards the end of the talk maybe last that worked, that way if anyone needs to leave no disrespect Paul s done a lot of research he just sent in a publication specifically to this it s worth a listening to Evidence base and applies to everybody is what s important we try not to have anyone present a one sided view that say at my hospital we all do this or at some hospitals they do that we want something that s evidence base with some success to it Perhaps as we continue on next year or so we can have more local physician, but for our first step out 3. 7:40 Registration Tami Exhibit 3A Sample Registration Background: Page 6 of 13

Minutes: Sample registration, we can make this whatever we want o There s a company that AHA uses called CDS They are telling us we can pick and choose whatever we want on this o Used the registration for Quality of Care as an example o It is something we have to move on soon o Member type = if you are an AHA member o Primary classification = i.e. physician, EMS o Will this confuse people for the first time registering with us Maybe there s got to be a place where if you click on it, it tells what that is o Next page, first name, prefix all that I think needs to be there o Is there a difference between the red and black is the red mandatory Red is mandatory o Hospital institution you may want to say agency as well on it for EMS o Emergency contact? You ve got to think that this has been there for every conference AHA has, if there is one we strongly feel about we can go and lobby, to remove o The promo code is probably going to be something like if we decide you re a participating Caruth agency and we want to give you a specific account than we can do something like that for promo code One of the things we talked about is encouraging someone who lives in Ft.Worth to come, but it should be less expensive to Caruth agencies like Baylor and Med City and EMS, we could get a Caruth code Group rate take like $5.00 off o Would they have the option of mailing in their registration Page 7 of 13

18 January 24, 2011 The marketing piece of it is in that and we ll be talking about that in a few minutes I think they can mail it in but most of them go online but that s a good question we ll double check to make sure o If we do a group rate we don t necessarily want to do a Caruth rate we need to decide one or the other because obviously we want to promote as many people as we can and we don t want to give a discount for Caruth and group rate that s going to kind of defeat the purpose 4. 8:00 CME/CE Jennifer Exhibit 4A Checklist Exhibit 4B Planning Document with Instructions Meeting with Steven Hurdle took place at AHA office. He went through the details of the specific paperwork that will be needed from the group. Minutes: The check list with the planning document with instructions Dawn needs some help with this o Not sure if 2 or 3 must get together and do the CME planning document That has the instructions online o Granted we got a little bit of time before we have to have it complete The last week of March Do we have a sample document we can look at This is the planning document with instructions, sent to you via This is the checklist, the actual CME document that has the instructions to complete this is We ve got to fill in the right side From a timing prospective why don t we (Wendy/Dawn) start filling some stuff in and see if someone on the group that will volunteer to work via /phone together with Dawn Some of it is going to be self explanatory per Steve Hurdle however at the same time it s a bit time consuming Page 8 of 13

Minutes: We wanted to start marketing it during heart week which starts February 12 o Karen Reed who is one of the marketing directors to AHA sent some information specific to the marketing it might take just a few hours to complete They have a couple of templates we just give them the data they can add it to which ever format We can design it any which way we want, she suggested the following Put the skyline of Dallas The save the date What the conference is Then on the back pick and choose what we want o One question I have is who are we mailing them to we ve got a database within AHA for anyone who s attended a CME program o North Texas, South Oklahoma, we can do all of Texas but one of the questions I have to you guys is how do we make sure we re getting to everybody We can send it to each participating in the subcommittees and the voting members Is there any other way we should be leveraging that do we give each hospitals and EMS agencies extra 10 or 15 to distribute That is what usually happens send it to the education department, so they can hang it up in their units Do you think if we send it to the voting members like a Page 9 of 13

Caruth should be a part of the title when discussing the ideas with regards to the naming is concerned. We are still try to come up with a name we are going have it for next meeting and stick with it Wendy offered a $10 gift card for a Starbucks for the person who names the conference We already talked about the name of the conference so we re kind of in limbo, let s brainstorm o Integrity Improving North Texas EmerGency Response an Initiative with The Communities Foundation of Texas and don t know what to do for Y, Wendy said Y all o We tried STARS that was to generic, we tried SOARS=Strengthening Our AMI Response System use as North Texas SOARS o It could also be Symptom Onset Arterial Reperfusion o The purpose of this program/initiative/conference is really to address what?, the AMI care from field to the interventionalist...so is it education, is it development what is it to educate people on it or develop cohesion teamwork, Dawn stated all of that o The whole initiative is regional strengthening the system it s Page 11 of 13

22 January 24, 2011 through the education, networking, it s through all that stuff, and it s data, protocols, so the initiative is more of the strengthening the system but the conference is more focus toward education on AMI patient care o You ve seen that commercial stand up to cancer with the arrow pointing up and in the Caruth website on the film, we can do something similar instead of up like cancer go from the EMS truck to the hospital and just have that picture and that music because it s very impactful when you watch it o We have to have Caruth, but we can have it as a subtitle o So we ve got two directions one is the big initiative, but would you guys rather for the sake of the education conference have something that just says Caring for AMI, AMI Care in North Texas in the 21 st Century, or something like that and then if we get the initiative name by the time the conference comes out we can write it in and promote it then Brand it at that point so it s really two different questions are we wanting to name the initiative or you guys for the sake of the conference want to put something that just says that this is the North Texas AMI Care Symposium o Thinking about something with Caruth Creating Advance Responses and Unified Treatment of Heart attacks Subtitle - A North Texas Collaboration The conference could then be - The CARUTH Educational Symposium o Cruising through the Continuum of AMI care o Relieving the pain through community teamwork o Collaborative Area Response 8. 8:45 Review Action Steps and Next Meeting Jon Summarize a list of action steps from today s agenda Minutes: Meet the 2 nd and 4 th Monday of each month, Meeting at the hotel on the 21 st, Feb 7 th is the next meeting By those two meetings we should probably have CME application almost done, promo materials done, and registration will be up and running, we Page 12 of 13
