2 Welcome Letter from Our Leadership Fiscal year 2016 was a big year for 12 & 12 clients, staff, volunteers, partners and donors. We celebrated 30 years of service to our Tulsa community and 30 years of client recoveries with a 30th Anniversary celebration on December 1, In addition, with engagement and support from our community partners and donors, we were able to offer life-saving recovery tools to more than 1,700 adults suffering with addiction or co-existing substance use disorders. To treat our clients successfully, year after year, we must continually reassess treatment options, revise systems, and renovate our extensive physical plant. In FY 2016, with support from the Warren Foundation, we implemented a new treatment option, Mindfulness Based Relapse Prevention. This new treatment tool is based on research evidence, which shows many benefits from meditation in treating addiction disorders by helping clients learn to pause, observe present experience, and bring awareness to the choices before them in each moment. Mindfulness Meditation thus helps clients learn to respond in ways that serve them well, rather than in detrimental ways. Their increased self-awareness helps clients identify healthy approaches to dealing with personal stress and relapse triggers. 12 & 12 offered life-saving recovery tools for more than 1,700 adults. 2 We are also proud to note a landmark change in our administrative systems. With the generous support of government and private donors, we completed the immense process of moving all client care records into the digital age via an Electronic Medical Records (EMR) system. Now, medical, clinical and case management staff have immediate, real-time access to information for each of our clients. As a result, as clients transition through our continuum of care staff spend more time providing them with efficient and effective care and less time on paperwork. Finally, as the owners and operators of a 365/24/7 residential treatment center, one of our essential responsibilities and challenges is ongoing building renovation, maintenance and repair. This year, we were able to successfully complete a total roof replacement project at our main 4-story, 170,000 square foot Skyline facility. Now, every time it rains and our roof stays leak-free, we reaffirm our gratitude to the local foundations and individuals that generously donated to our $650,000 fundraising campaign to complete the project. 3 What We Do At 12 & 12, Inc., we offer adults suffering from the brain disease of addiction or co-existing mental health and substance use disorders life-saving recovery tools to support their ability to achieve individualized recoveries. We serve our clients through a complete continuum of care that provides multiple levels of treatment including detoxification, intensive residential treatment, outpatient and intensive outpatient treatment, counseling, transitional living, and sober living. Our service model is based on a multi-disciplinary approach that integrates medical oversight, psychiatry, nursing, counseling and case management. Our treatment approaches reflect our commitment to providing compassion, safety, respect and dignity for all in a safe and therapeutic environment. In alignment with evidence-based practices, we re simultaneously treating co-existing mental health and substance use disorders to dramatically increase our clients likelihood for recovery. In March 2013, we attained accreditation as a Comprehensive Community Addiction Recovery Center (CCARC). We are the largest CCARC in the state, and the only CCARC that provides the full continuum of services, onsite. Our main Skyline facility is located at 6333 E. Skelly Drive. It houses our administrative offices and most of our inpatient and outpatient treatment services. We have one satellite, Bryce House, located in downtown Tulsa at 1214 S. Baltimore, which provides supportive housing for homeless veterans suffering from addiction. Daily, 12 & 12 provided outpatient, inpatient or transitional living program services for 221 individuals, including more than 60 veterans. In fiscal year 2016, more than 1,700 individuals received services from our inpatient, outpatient and transitional living programs. Fifty-one percent (51%) of our clients were simultaneously treated for co-existing mental health and substance use disorders. Ninety-six (96%) percent of our clients are uninsured or unable to pay. Therefore, they were eligible to access our services through government-based contract sources, e.g., the Oklahoma Department of Mental Health and Substance Abuse Services, the Veterans Administration, Native American Tribes, etc. Unfortunately, these sources did not cover our full costs of care. Fortunately, we were able to acquire additional resources from the Tulsa Area United Way, individuals and foundations to pay for our expenses & 12 Client Success Story I m an Army veteran. When I decided to recover from my drug addictions, I turned to 12 & 12 and the VA for assistance. 12 & 12 and the VA staff were compassionate and patient, tolerant, and empowering. 12 & 12 saved my life. 12 & 12 staff loved me until I could love myself. Because of the time I spent at 12 & 12, I ve been sober since December 12, Now, I have hope for a better future for myself and those around me, in my community and in the world. ~ Curt, Army Veteran and 12 & 12 Alumnus

The assessment is administered at admission (to establish a baseline and help determine severity of AOD addiction and attendant problems) and again at discharge. A successful residential treatment episode should yield a markedly improved set of ASI scores in most, if not all of the problem areas evaluated in the ASI assessment process.
