2 Our Mission The mission of Grand Prairie Services is the provision of holistic emotional and behavioral health services for individuals, families and communities of the Southland region with an emphasis on recovery and optimal functioning. 4 Winner of the 2016 Best and Brightest Companies to Work for For the third year, Grand Prairie Services was recognized as one of Chicago s 101 Best and Brightest Companies to Work For. The Best and Brightest Companies to Work For competition identifies and honors organizations that display a commitment to excellence in their human resource practices and employee enrichment. Organizations are assessed based on categories such as communication, worklife balance, employee education, diversity, recognition, retention and more. GPS Board Member Honored with the Peggy A. Montes Unsung Heroine Award In recognition of Women s History Month each year, the Cook County Commission on Women s Issues presents the Unsung Heroine Award to women from across Cook County. Maurice Fitts Page, a member of Grand Prairie Services Board of Directors, was presented with the Peggy A. Montes Unsung Heroine Award for the 6 th District of Cook County on March 3, This award is designed to tell the story of the powerful impact women have had throughout history in the development of our social, cultural, economic and political institutions. In this spirit, the County honors these remarkable women as a way to bring their stories to light, to thank them and to celebrate the achievements of the many women through the history of Cook County whom they represent. 4
5 An Environment of Courtesy, Respect, Fairness & Good Faith James Williams Specialized Services Center Effective July 25th, the Lincoln Center was renamed the James Williams Specialized Clinical Services Center. James Williams has been a member of the Board of Directors for many years. His personal attributes, divinely inspired service and selfless contributions have shaped the history of Grand Prairie Services. It is with honor that we begin calling our service location the James Williams Specialized Clinical Services Center. Keith Kemp Home On October 6, 2016, Transitions Property Holding in collaboration with Grand Prairie Services dedicated its residential home located at 1236 Ashland Avenue, Chicago Heights, to Mr. Keith Kemp, a person of great character who is committed to the community and those recovering from mental illness. Grand Prairie Services is committed to providing professional and community housing for those in recovery; championing individual rights; and protecting the safety of individuals and families, in the home, and in the Southland community. 5
6 Partnerships and Collaborations Mental Health First Aid Training GPS added Mental Health First Aid training to our comprehensive array of services. Over 120 community members have been trained in Adult and Youth Mental Health First Aid during The Adult Mental Health First Aid course is appropriate for anyone 18 years and older who wants to learn how to help a person who may be experiencing a mental health related crisis or problem. Youth Mental Health First Aid is designed to teach parents, family members, caregivers, teachers, school staff, peers, neighbors, health and human services workers, and other caring citizens how to help an adolescent (age 12-18) who is experiencing a mental health or addictions challenge or is in crisis. Youth Mental Health First Aid is primarily designed for adults who regularly interact with young people. The course introduces common mental health challenges for youth, reviews typical adolescent development, and teaches a 5-step action plan for how to help young people in both crisis and non-crisis situations. Topics covered include anxiety, depression, substance use, disorders in which psychosis may occur, disruptive behavior disorders (including AD/ HD), and eating disorders. 7 Diversity First Episode Psychosis GPS provides First Episode Psychosis services through a grant with The Illinois Department of Human Services FIRST.IL. FIRST.IL is a specialized treatment program that helps individuals who are between the ages of 14 to 40 and who have had a treated or untreated psychotic illness for no more than 18 months (eligible diagnosis are schizophrenia, schizoaffective disorder, schizophreniform disorder or other specified/unspecified schizophrenia spectrum and other psychotic disorder). The goals of the program are to provide support and education for the individual and family; to improve recovery; to reduce the chances of relapse; and to set goals for the future. GPS provides Counseling, Psychiatric Care, Supported Employment/Education, Family Psychoeducation and Case Management through the FIRST.IL program. FIRST.IL at Grand Prairie Services can help As I often say, in my more than 40 years with schizophrenia, there s never been a better time to be a person with a serious mental illness. There is more hope now than ever before. Frederick J. 8 Service Excellence Back to School Fair The Child & Adolescent Services team hosted a Back To School Fair on August 25th. Children from the community were invited to celebrate the beginning of the school year. Kids enjoyed playing games, arts and crafts, face painting and a visit by the Flossmoor Fire Department. Thanks to the individual donors, school-aged children received a back pack full of school supplies. A special thank you to Bimba Manufacturing and Elite Medical Transportation for sponsoring the event. 8
9 Compassionate Care Developmental Training Grand Prairie Services operates Developmental Training Services in south suburban Cook County for residents of the Elizabeth Ludeman Developmental Center and residents of various Community Integrated Living Arrangements (CILA). Services include Pre- Vocational Skills, Vocational Skills, Medical Services, Money Management, Physical Mobility Training, Personal Care, Paid Production, Can Crushing Program, Social Clubs including Jewelry Club and Elvis Fan Club, Community Participation, Recreation and Leisure Activities and Senior Services. Bowling Outing Can Crushing Program Holiday Party Consumer Run Store Program Mission The mission of Developmental Training Services is to provide quality services with a holistic approach to enhance and promote independence in the community, personal, and interpersonal relationships and improve the quality of life for persons with intellectual disabilities. 9
10 Individual Rights and Preferences Community Reintegration GPS was invited to speak at the Mental Health Corporations of America (MHCA) Conference in Indianapolis, Indiana on Care Management Approaches to Community Reintegration of the SPMI Population. This portion of the conference provided an overview of the specific, personcentered processes employed to improve the lives of people who elect to make the life-changing transition from institutional settings to the community setting of their own homes. Dr. Sharronne Ward, President and CEO, Brady Harden, Director of Strategy and Business Development, Wendell Brown, Housing Coordinator, Adrien Teverbaugh, Upward Center Coordinator and Dr. Cynthia Thomas, Medical Director presented. Drop In The Upward Center provides consumers at Grand Prairie Services a variety of social and recreational activities which promote interpersonal role functioning. The Upward Center activities include self-help meetings, movies, arts and crafts, family and friends events, holiday parties, computer and employment trainings. The Drop-In Center is a favorite location among consumers to come and meet new friends, build new relationships and relax in a safe, social environment. The Upward Center expanded to a second location in Tinley Park, Illinois, to facilitate serving consumers in the west suburban communities. Consumers attend a Chicago Bulls Game 10
11 Hope and Healing Integrated Care Services Expanding to Indiana Grand Prairie Services has expanded its service approach to treat the whole person. GPS recognizes that wellness is a state of complete physical, mental and social well-being and not merely the absence of a disease. GPS Whole Health and Wellness Services Center, located in Lynwood, Illinois is now providing integrated healthcare services. In its commitment to supporting a healthier community, the Whole Health and Wellness Center provides opportunities for total wellness. South Suburban residents can receive primary care and behavioral care services at one location. Health assessments, Wellness groups and Health Education groups are among the many services available at the Whole Health and Wellness Services Center. By integrating mental health, substance abuse and primary care services in a convenient single location, individuals with multiple healthcare needs are offered the most effective approach to improved outcomes. GPS Whole Health and Wellness Services Center provides integrated services to address physical and mental health with the outcome of achieving balanced physical wellness. GPS is certified to provide these comprehensive services to residents of Indiana. 11
12 GPS Foundation Inspiring & Strengthening our Community Through Music, Song and Dance Grand Prairie Services Foundation Annual Fundraiser was held at the Kindig Performing Arts Center in South Holland on November 4, This year s fundraiser featured the musical talent of GPS consumers and local entertainers. GPS Foundation presented awards to key community partners. Reverend Tyrone Crider and Apostle Carl White, Jr., jointly received the Humanitarian Award. Marvin Lindsey, Community Behavioral Health Association (CBHA), received the Hope Award. Markham Courthouse, Mental Health Court, received the Partnership Award. South Suburban Hospital received the Care Coordination Award. Don Manning, South Suburban College, received the Collaboration Award. Bimba Manufacturing and Karen Brown received Certificates of Appreciation. GPS continues to thrive in the community thanks to the support of community stakeholders and the dedication of valuable staff. 14 Repeat Boutique Thrift Boutique Repeat Boutique resale shop provides financial support for Grand Prairie Services and its Residential program, which provides housing support for over 150 individuals recovering from mental illness. Since opening in 1961, Repeat Boutique volunteers are dedicated to providing a clean, well-organized shopping experience that directly benefits individuals served by Grand Prairie Services. Volunteers at Repeat Boutique are committed to providing a clean, friendly environment in which to shop. The shop is stocked with gently used merchandise including clothing for infants, youth, juniors, women and men of all ages, shoes, jewelry, toys, housewares, books, puzzles, CDs and other miscellaneous items all at prices too low to pass up. Fresh merchandise is brought in with the change of season. Every year, residents of GPS housing receive a gift card for the holiday donated from Repeat Boutique. In 2016, Repeat Boutique donated a 2015 Ford Transit Dixie Highway Homewood, Illinois (708) Hours: Tuesday Saturday 10:00 a.m. 3:00 p.m. Closed Sunday and Monday Repeat Boutique volunteers being honored at the annual Volunteer Luncheon 14
15 Employee Recognition Award for Excellence The Award for Excellence was established in 1987 by the Board of Directors to honor the memory of John and Gloria McAfee. The purpose of the award is to recognize GPS staff who provide outstanding clinical service. All full-time clinical and supervisory staff are eligible for nomination. The 2015 award recipient was Michele Watts. Spirit of Recovery The Spirit of Recovery Award is a newly established award to recognize the outstanding achievements of people and programs that provide the necessary support to help improve the lives of individuals with serious mental illnesses. The award recognizes a team that has contributed positively to fostering relationships through the development and implementation of services and activities which promote understanding and/or awareness of mental illness and associated stigma. The 2015 award recipient was SASS. Volunteer The Volunteer Award is a newly established award to recognize an individual whose generous commitment of time and energy has had a profound impact on GPS and the individuals we serve. The 2015 award recipient was Barb Creal, Repeat Boutique. Excellence in Leadership The Excellence in Leadership is a newly established award to recognize individuals in leadership roles that have truly gone the extra mile. Individuals who do more than required, and those whose efforts often go unnoticed. The 2015 award recipient was Lynn Alleruzzo. Quality Award The Quality Management Award is presented to a GPS program that displays consistent application of the highest standards within the realm of behavioral health services. Programs awarded the Quality Management Award have met and exceeded the standards of the State of Illinois and the CARF International accrediting organization, and exhibit continuous quality practice improvement. The 2015 award recipient was Developmental Training. 15
